Medical Tourism
Medical tourism (also called medical travel, health tourism or global healthcare) is a term
initially coined by travel agencies and the mass media to describe the rapidly-growing practice of
traveling across international borders to obtain health care. It also refers pejoratively to the
practice of healthcare providers traveling internationally to deliver healthcare. Services typically
sought by travelers include elective procedures as well as complex specialized surgeries such
as Aesthetic procedure, cosmetic surgery, dentistry, dermatology, eye surgery & ophthalmology,
fertility treatment, general surgery, hair transplantation, weight loss surgery and stem cell
therapy.
Over 50 countries have identified medical tourism as a national industry.
However, accreditation and other measures of quality vary widely across the globe, and there
are risks and ethical issues that make this method of accessing medical care controversial Also,
some destinations may become hazardous or even dangerous for medical tourists.

History of Medical tourism
Medical Tourism is also known as travel medicine or health tourism. This term is called
for the first time by the media and travel agencies to describe the rapid development and
the practice of large travel to different parts of the world to receive specialized treatment
health care. As a developing country, India also has the trend of medical tourism.
Medical Tourism in India provides countless options for all tourists to explore the
facilities of medical sciences in India.
People have been traveling to other countries for health purposes. There were the ancient
Greeks and Egyptians who went to hot springs and baths to improve their health.
18th and 19th century- Europeans and Americans who flocked to health spas and
sanitariums and even in remote places in the hope that they will get treatment for diseases
such as tuberculosis, gout, bronchitis or liver diseases.
Epidauria- a small territory in the Saronic Gulf. It was said that this small territory was
the sanctuary of Asklepios, known as the healing God. Thus, it was recorded in medical
tourism history that Epidauria is the original destination for medical tourism.
Asclepia Temples- which were erected to honor the Greek god of medicine, were some of
the world's first health centers.
Mansuri Hospital- the largest and most advanced hospital the world had ever seen. The
hospital's promise to serve anyone, regardless of race, religion, or wealth, drew health
travelers from far and wide.

However, dental surgery, cosmetic surgery and other more serious kinds of surgery
abroad is relatively a new phenomenon. It started in the 80s and 90s when patients began
looking for more affordable options other than what was offered in their homeland or
country.
What was referred to as "tooth tourism" grew quickly. Americans were then traveling to
Costa Rica or other Central American countries for dental bridges and other dental
procedures that were not usually covered by their insurance.
In the late 1980s, Cuba started programs to attract foreigners from countries like India,
Latin America and Europe for more affordable eye surgeries, heart surgery and cosmetic
procedures. In 1990 alone, it was reported that the Cuban government had around 2,000
medical tourists from various parts of the world.
Thailand also became a hot destination for plastic surgery and other routine medical
procedures in the late 1990s. On the other hand, New Zealand is getting more popular in
hip and knee replacements surgery.
Now, medical tourism is already a global phenomenon. More and more countries will
become destinations of medical tourism. But while there are advantages, there are also
risks or disadvantages. Medical tourism is a trade-off. You might be able to save a lot of
money at the same time have your much-needed vacation abroad but expect some
disadvantages along the way. Nonetheless, medical tourism is here to stay thanks to the
mass media, modern technology and hassle-free travel to other countries.
Popular medical travel worldwide destinations include: Argentina, Brunei, Cuba,
Colombia, Costa Rica, Hong Kong, Hungary, India, Jordan, Lithuania, Malaysia, The
Philippines, Singapore, South Africa, Thailand, and recently, Saudi Arabia, UAE, South
Korea, Tunisia and New Zealand.

History of Medical Tourism in the Philippines
- Philippine government only started promoting medical tourism in 2006.
- Philippine hospitals have been receiving foreign patients for over three decades now.
- Philippine hospitals were considered the best in the region, with people from as far as the US
seeking medical treatment here.
-Imelda Marcos put up the Philippine Heart Center in 1975
- it became an important center for cardiac care in the Asia-Pacific region because of its foreign-
trained medical personnel, state-of-the-art facilities, and advanced medical care and research.
-The Heart Center was at the forefront of developing bio-prosthetic valves and prototype medical
equipment. It was a trailblazer in coronary angioplasty in the region.

Medical Tourism in the Philippines
- Foreign Nationals have been coming to the Philippines for medical treatment as early as the
1970s.
- Medical Tourism as a government program only took off in 2006 when the Departments of
Health, Tourism, Foreign Affairs, and Trade and Industry identified the Health and Wellness
Services Program as one of their flagship programs.
-The Philippine Medical Tourism Program has four domains: full hospital care and treatment;
specialty clinics (e.g., eye, dental, and cosmetic services); wellness and spa centers; and
retirement and long-term care for the elderly.
-According to the Philippine Institute for Development Studies, cataract surgery costs, $1,424 in
the Philippines, Liposuction in the Philippines costs $1,400; Total knee replacement surgery is
more expensive in the Philippines at $5,639.
-Philippine medical professionals are also fluent in English, which reduces the possibility of
error and misunderstanding between doctor and foreign patient. Medical professionals in the
country, particularly the nurses, are known for their hospitality, skills, and excellent patient care,
which make them in demand in foreign hospitals abroad.

The Medical City (TMC) is a private, tertiary care hospital now on its fourth decade of existence.
It is located on a 1.5 hectare property along Ortigas Avenue in Pasig City. This massive structure
is composed of two towers joined by a Podium and bridge ways. The 15-storey Nursing Tower is
currently fitted for 500 beds and a future second Nursing Tower provides capacity for an
additional 300 beds. The 18 floors of the Medical Arts Tower house 280 doctor's clinics and
selected commercial spaces. Within the 6-floor Podium are located diagnostic and intervention
facilities, as well as support and administrative offices. The 3-basement level parking
accommodates over a thousand vehicles for clients and staff.
TMC offers diagnostic and treat services that address a broad range of diseases utilizing state-of-
the-art technology such as computed tomography, magnetic resonance imaging and cardiac
catheterization and interventional services among others. This is made possible through the
Clinical Departments (including their various subspecialties) of Medicine, Surgery,
Anesthesiology, Pediatrics, Obstetrics-Gynecology, Ophthalmology, Otolaryngology, Psychiatry
and Rehabilitation Medicine, as well as diagnostic and intervention units such as Clinical

Orthopedics
TRAUMA - Orthopedic trauma accounts for the majority of emergency room cases seen at The
Medical City and majority of patients treated by the department. As the "bread and butter" of
orthopedic practice, trauma care has been organized as a subspecialty to further improve the
quality of orthopedic management. The Orthopedic Trauma Unit handles mainly fractures,
dislocations and soft-tissue injuries of the extremities. Common treatment modalities consist of
surgical fracture fixation, joint reduction and non-surgical manipulation and immobilization.
ARTHRITIS & JOINT REPLACEMENT - The Arthritis and Joint Replacement Unit provides
the highest-quality medical and surgical care for patients with chronic joint disorders. The Unit
has on staff some of the best joint reconstructive specialists in the country today. Working
closely with other specialty fields, the Unit empowers patients and their caregivers with
knowledge and understanding of their disease. It provides competent, state-of-the-art surgical
care for chronic joint disorders, mainly hip and knee replacement, which is the current mainstay
of joint reconstruction.
SPORTS MEDICINE & ARTHROSCOPY - Sports-related injuries account for a significant
portion of emergency room and outpatient consultations in The Medical City. The Sports
Medicine and Arthroscopy Unit aims to address problems pertaining to these injuries, most
commonly involving the knees, ankle, shoulder, elbow, and foot. The Unit is defined by its
expertise in the utilization of arthroscopy in the diagnosis and treatment of such condition. It is
secondarily involved in the management of the degenerative conditions that may be treated in a
minimally invasive manner. It also employs a multidisciplinary approach in the holistic care of
athletes, working closely with allied specialties like rehabilitation medicine, nutrition, and sports
physiology.

PEDIATRIC ORTHOPEDIC - The Unit oversees the diagnosis and management of the common
orthopedic congenital and developmental problems of the extremities, including clubfoot
deformities, developmental hip dysplasia, bone and joint infections and neglected traumatic
injuries. The most common surgical procedures are: correction of limb deformities and limb
length discrepancies, clubfoot surgery, tendon transfers and other soft tissue procedures from
paralytic deformities, non-arthroplastic joint reconstruction, and treatment of bone and joint
infections in children. The pediatric Unit will also establish its own database for pediatric
orthopedic condition and conduct research
on important pediatric orthopedic problems. Patient and family education programs as well as
cooperation with allied specialties, such as general pediatrics, will also be instituted.
ORTHOPEDIC TUMOR - The Unit aims to make The Medical City the premiere referral center
for the treatment of musculoskeletal tumors. It is part of both the Department of Orthopedics and
The Medical City Cancer Center. Since musculoskeletal oncology is multidisciplinary in nature,
the Unit works closely with medical oncology, pathology, and radiation oncology, as well as
different support units and specialists to provide comprehensive care to our patients. The target
populations of the Unit are patients with primary bone and soft tissue tumors and those with
metastatic bone disease. Procedures done include limb salvage surgeries, all varieties of
amputations, adjuvant and neoadjuvant chemotherapy and radiotherapy, and bone and soft tissue
reconstructions. The Unit is likewise a rich source of research in the field of musculoskeletal
tumors.
HAND & MICROSURGERY - Traumatic hand injuries account for a significant number of
emergency room consults, and their frequently industrial nature lends even more applicability to
The Medical City. The Hand Unit oversees the treatment of all common hand conditions aside
from those of a traumatic nature, including infections, acquired or congenital deformities,
degenerative conditions and peripheral nerve disorders. It also offers micro-vascular replacement
and reconstruction for severed digits and hand and following limb salvage procedures, having at
its disposal the necessary equipment to deliver this specialized level of orthopedic care.

SPINE SURGERY - Because of the often-debilitating nature of spinal cord injuries, a specialized
level of surgical expertise and care is prerequisite in dealing with spine conditions. The Unit
oversees the treatment of common spine conditions including traumatic injuries, congenital
anomalies, deformities (such as scoliosis), infections and tumors. The Unit foresees a working
relationship with its colleagues in the Neurosciences from intra-operative monitoring to working
alongside neurosurgeons in complex spine cases. It will also establish its own databse for
research purposes.
TMC also serves as a hub of a network of satellite clinical facilities delivering a full range of
diagnostic and therapeutic services to ambulatory patients. Currently, Satellite Clinics are
operational in Antipolo City, Pasig City, Fairview, Cainta, Marikina and Congressional Road in
Quezon City and Lipa City. More satellite will be added as the hospital pursues its commitment
to expand its network and bring its brand of health services into the very communities of its
patient for their easy access. Additionally, Homecare services are offered whenever appropriate,
particularly to patients after discharge from confinement.

Opthalmology
The Medical City Eye Center is the eight-room-out-patient diagnostic and Laser treatment
facility that houses all the state-of-the-art ophthalmic equipment. The center is staffed by well-
trained, dedicated and pleasant ophthalmic personnel (manager, receptionist, nurses and
technicians) who work closely with the residents and consultants to deliver the highest quality
eye care.

The Medical City Operating Suites have 10 operating rooms, 2 of which are presently being used
for eye surgery, In addition, the Ophthalmology service maintains a team of 8 surgical nurses
who have all undergone specialized training for Ophthalmic surgery assisting. These nurses are
assigned exclusively to eye surgeries only to ensure specialized, appropriate and high quality
care of our patients.
Otolaryngology
At present, the Department of Otolaryngology - Head and Neck Surgery is located at the 2nd
floor of the hospital complex Podium. Its Ear, Nose, Throat - Head and Neck Surgery Center
integrates the Outpatient Clinic, Hearing and Dizziness Unit, Voice and Swallowing Laboratory
and Speech Therapy. The ENT-HNS Outpatient Clinic has expanded to include a consultation
room and three separate examination rooms for patients. The Hearing and Dizziness Unit headed
by Dr. Rina Reyes-Quintos is fully operational with modern facilities for evaluating hearing and
balance disorders including pure tone audiometry, speech testing, tympanometry, auditory
brainstem response, otoacoustic emissions and videonystagmography among others. The section
is now in the process of acquiring upgraded facilities for Pediatric Audiometry. The Voice and
Swallowing Laboratory, the first laboratory for voice and swallowing in the country, continues to
be the premier referral center for diagnosing voice and swallowing disorders in the Philippines.To date, it has the largest number patients seen on file. Included in equipment upgrade is the
installation of the new digital strobe, DIGISTROBE, and acquisition of updated swallowing
evaluation devices for its Swallowing Clinic. The Center for Snoring and Sleep Disorders is a
multidisciplinary center where the Department of ORL-HNS is also involved in. Established in
2002, it is headed by Program Director Dr. Daniel M. Alonzo and Unit Head Dr. Michael
Alexius A. Sarte. The Sleep Center gathers authorities from the departments of Medicine,
Pediatrics, Otolaryngology, Psychiatry, Anesthesiology as well as Dentists and Sleep
Technologists in diagnosing and managing sleep disorders. The Center is continuing its
expansion by involving more specialists concerned in the comprehensive management of these
patients. In order to facilitate temporal bone dissection of the residents, a Micro-Surgery Skills
Laboratory has been especially integrated in the center built with the necessary instruments such
its own operating room lights and microscope. Work is now underway to putting up two new

sections, th Nose Clinic and the Prosthetics Laboratory.
In March of 2005, two of the department's residents were able to participate in a 5-day surgical
mission conducted in Semirara Island in Caluya, Antique. Surgical cases involved cleft lip and
palate repair, septoplasties, thyroid surgeries and other neck operations. The department has
successfully been coordinating with its consultant staff and private organizations with regard to
involvement in surgical missions. in June 2005, the department again volunteered its services in
CFC's Medical-Surgical Mission in San Pablo, Laguna.
The department welcomed 2005 by re-establishing relationships with various government and
private institutions as well as establishing new ones, the latest of which is the department's tie-up
with the Philippine Band of Mercy. The Philippine Band of Mercy (PBM) is the lead provider of
comprehensive craniofacial health services for the less privileged.
Also through the help of the Philippine Foundation for Health and Development, Inc. and the
Philippine Charity Sweepstakes Office, the department continues to offer free professional
services to its charity patients through the Divine Mercy Program. Together with the PBM and
the Divine Mercy Program, our department will hopefully be able to develop a more extensivecharity program so that more health services could be rendered to the indigents for less cost. The
most recent endeavor of the department with PBM is the surgical mission in Valenzuela early
March 2006.

Admission Procedures
y In the absence of the doctor's admission order sheet or a doctor known to them, a
walk-in patient may be admitted through the Emergency Room.

While your admission is being processed and your room is being prepared, you will be asked to

wrist for the duration of their stay at the medical center.
Emergency Admissions
Patients are referred to the Emergency Room (ER) if they do not have an admission order sheetfrom their attending physician. In such cases, the ER consultant will ask the patient for the name
of their physician. The ER consultant may only assign an attending physician to their case if
they do not have a personal physician of choice.

The Admission Department accepts room reservations. However, if the patient fails to arrive

The cut-off time for discharge is 11:00 a.m.. An additional half (1/2) day rate will be charged for
room and board after 11:00 a.m. and one (1) day will be charged for room and board after 5:00
p.m..
A written discharge order must be secured from your attending physician. You must secure
discharge orders from ALL doctors consulted during your hospitalization.
Once discharge orders are secured, your nurse on duty will issue a Discharge Notice Slip with an
initial print-out of your itemized Statement of Account. Please check the items in your bill before
paying at the Cashier. Kindly inform your nurse on duty about any concerns you may have
regarding your bill.
When you are ready to pay your bill, you may then proceed to the Main Cashier or at Annex II
2nd floor Cashier. Suite room patients may request for a cashier to collect payment in their room.
Once your account is settled, the cashier will give your Official Statement of Account, Official

Legal Issues:
1. Receiving medical care abroad may subject medical tourist to unfamiliar legal issues. The
limited nature of litigation in various countries is one reason for the lower cost of care
overseas. Should problems arise, patients might not be covered by adequate personal
insurance or might be unable to seek compensation via malpractice lawsuits. Hospitals
and/or doctors in some countries may be unable to pay the financial damages awarded by
a court to a patient who has sued them, owing to the hospital and/or the doctor not
possessing appropriate insurance cover and/or medical loss. However new insurance
products are available that protect the patient should an alleged medical malpractice
occur overseas. The down-side is, in most cases, these polices are not cheap.
2. A patient who is injured through negligent care has very limited options in obtaining
justice and compensation. With different laws in other nations, the legal alternatives for
patients are few. There is a minimal legal recourse. Many international hospitals require
a medical tourism patient to sign a legal waiver stating that if they do bring a lawsuit over
the surgery the lawsuit must be brought in the country where the medical procedure was
performed. This means that there is very limited legal recourse when you travel for

medical tourism as legal judgments internationally are significantly less than in the US,
Canada or the United Kingdom.
GOOD NEWS: A new insurance product available is intended to address medical negligence
affecting patients who travel abroad for elective surgery. The ³Patient Medical Malpractice
Insurance´ is considered ³first-person´ insurance because the patient purchases the policy before
traveling. Claims are handled without attorneys in accordance with U.S. laws and customs and
are paid in U.S. dollars. This coverage may benefit medical tourists who are concerned about
their options if a bad outcome occurs due to medical negligence
3. Issues on Rights if there is a complication happened: Most hospitals, wherever they are
located, will not cover the cost of medical complications. So its better to ask questions
and get everything in writing first ± before your procedure.
4. Money Issues: What if the patient decides not to have my surgery, will he/she be able to
get his/her money back?
It depend on the policy of the hospital you have chosen, as well as when you cancel your
procedure. Make sure to have the answer to this question in writing. However, most
international hospitals I have spoken to will refund a majority if not all of the money you
have paid.

1. Potential Impact on Patients Engaged in Medical Tourism:
* Patients usually have no idea about the potential for infectious and noninfectious
complications from medical tourism, and there is little reliable data available regarding
destination countries.
* Patients rarely understand the tradeoff between saving money on medical expenses
and obtaining potentially life-prolonging medical care that the patient could otherwise not
afford, and sacrificing potential legal remedies should medical negligence occur. Currently,
³there are no reliable, comprehensive sources for patients to learn about legal recourse for
malpractice committed in [foreign] jurisdictions.´

3. Possible political risks in terms of sovereign (foreign policy) relationships:
For example, of medical tourism involving US patients traveling to other countries was to
³severely impact the comparative advantage for the US in the hospital business. . . .It can
cause a loss of reputation and prestige for the US health care system.
4. Potential Impact on Healthcare in Destination Countries
These countries sacrificing their public health sectors, particularly in terms of access by
rural/provincial and indigent populations, in favor of attracting foreign medical tourists
with attractive currencies while using public funds to facilitate that trade-off.
Governments have argued that increased revenues from foreign tourists actually
strengthen public healthcare, or that hospitals receiving public subsidies must provide a
certain percentage of care to the poor and indigent citizens of the host country.
5. Other Ethical Criticism:

These surgery packages have been criticized as:
(1) potentially subjecting patients to procedures in private clinics that are not accredited
and by surgeons who may or may not be skilled and qualified to do the procedure, thus
subjecting the patients to undisclosed potential post-operative complications,
unsatisfactory results, and risks to their general health;

complications arise and revision surgery is required²the costs for which may not be
covered by health insurance in the patient¶s home country
