Acknowledgement

Having this Monthly Medical Assistance to the Provincial Jail Inmates will

not be feasible without the following:

Firstly, Almighty Allah, for giving us strength and wisdom in our everyday

task and providing us light of guidance in order for us to perform our duties well;

To Hon. Governor Esmael “Toto” G. Mangudadatu for vast support on the

health of deprived sectors of the community and as well as constituents inside the

jail.

To Provincial Administrative Officer Engr. Abdulwahab V. Tunga and OIC-

Provincial Budget Officer Lynette A. Estandarte, Asst. PPDO Nulfarid S. Ampatuan

for allowing us to conduct this monthly health program and providing assistance

in performing this activity and in making this report.

To the DOH-ARMM Secretary Dr. Kadil Sinulinding, Jr., Provincial Health

Officer Dr. Tahir B. Sulaik, and OIC-PHO Dr. Elizabeth A. Samama in envisioning

quality health services to the constituents of Maguindanao and strong support on

the health programs of the province.

To Provincial Jail Warden Kasan L. Odin, for deepest concerns on the health

of every inmate and providing them assistance in any possible means he can offer

to address their health problems.

And to everyone, who graciously rendered their time and effort for the

success of every medical assistance, a lot of thanks to you.

OVERVIEW

The Provincial Jail of Maguindanao is located at P.C. Hill, Cotabato City and

account for more than 100 inmates. The jail has limited rooms for male and

female and facilities for the welfare of the inmates.

When there is a medical or dental problem, they are brought to Cotabato

Regional and Medical Center as soon as the problem reaches severity. Only in

medical outreaches were majority of the inmates could have access to medical and

dental consultations. This activity is done occasionally. When this dilemma was

raised during one of the local health board meetings, the Provincial Governor

Esmael G. Mangudadatu sent advance team to assess the situation.

On February 17, 2011 the Provincial Governor conveyed the Mobile Hospital

equipped with staff for medical, dental and minor surgeries. All inmates were

catered on that one (1) day activity.

During this medical mission, the team found out different diseases from the

inmates including cyst, hypertension, diarrhoea, pneumonia, diabetes and other

minor medical problems. Those with cysts were successfully operated on the

mobile hospital with the help from the doctors of the IPHO-Maguindanao.

With these prevailing health problems, routine health assessment to the

inmates including sanitary inspections to the jail building is of recommended

solutions to improve their health status. Thus, provision of medical team

composed of One (1) doctor, Three (3) nurses and One (1) sanitary inspector will

greatly affect the status of each inmate. The team will conduct routine physical

assessment and individualized treatment. This will also include assessment of

their sanitary condition and shall recommend proper sanitation program.

Moreover, to continuously assess their concerns, the team will conduct this

activity every last Thursday of the month or as the date requested by the province

and the jail personnel.

OBJECTIVE

Provincial Government of Maguindanao and IPHO-Maguindanao are in

collusion to treat the underlying medical problems of the Provincial Jail

Inmates.

To properly assess the inmates’ medical condition thus preventing advanced

and complicated diseases.

To provide inmates with free medicines and other supplies needed to

improve their health condition.

To assess the sanitary condition of the jail thereby preventing diseases

caused by improper sanitation.

To provide health promotions thru audio-visual presentations and

recommend guidelines on proper hand washing and tooth brushing.

SCHEDULING

The medical team will conduct the activity once a month from 8:00am to

4:00pm and this includes medical consultations, minor surgical procedures when

there is a case of wound, provision of medicines, sanitary inspections and Health

Promotions.

Basing on the data and approval from the Provincial Office, when there is a

need to conduct tooth extraction, it will be scheduled on the next visit.

BUDGETARY REQUIREMENTS

Transportation Allowance : = P2, 000

Communication Allowance : = P200

Food Allowance : Breakfast and Lunch x P75 X 5 pax = P750

Honoraria of the Health Personnel

1. Doctor : 1 pax X 1000 = P1,000

2. Nurses : 3 pax X 500 = P1,500

3. Sanitary Inspector : 1 pax X 500 = P500

TOTAL= P5,950

All medicines, equipment and instruments will be taken from the supplies of

the Mobile Hospital. Television and Disk Player that will be utilized for health

promotion shall be provided by the Provincial Jail.

FLOW OF ACTIVITY

Three days prior to the first conduct of medical assistance, the health

personnel shall coordinate with the jail warden and have the complete list of

names of the inmates.

At Registration Area

i. The Medical Staff and Jail Warden shall confirm the

identity of the inmate from the listed names provided by

the jail personnel.

ii. The Medical Staff shall take the vital signs of the inmate.

iii. The medical staff and security personnel shall guide the

patient to the waiting area.

At Waiting Area

i. The assigned personnel shall start the audio-visual

presentations for health promotions.

ii. While inmates/patients are waiting for their

medical/surgical consultation, they will be watching

health promotions thru audio-visual presentation.

iii. Once called, the assigned staff shall guide the patient to

the medical consultation.

At Medical Consultation Area

i. The medical practitioner shall assess the patient and

prescribe the medicine needed. When the case is minor

surgery, the patient will be operated after the consultation

period.

ii. The assigned personnel shall guide the patient to the

Pharmacy/Medicines or at Minor Surgery Area.

At Pharmacy/Medicines Area

i. The assigned staff shall provide the medicines prescribed

by the Doctor.

ii. The Security Personnel shall guide the patient back to

their respective rooms.

At Minor Surgery Area

i. The Assigned Staff shall validate the name of the patient

and the operative site.

ii. The Assigned Nurse shall prepare the instruments and

other supplies for the specific procedure to be done.

iii. The Medical Practitioner together with the assigned scrub

nurse shall perform the surgical procedure.

iv. After the procedure, the staff shall bring the patient to the

recovery area.

Post-Assessment

i. The health personnel, provincial staff and the provincial

jail personnel shall conduct the post-assessment activity

to identify weakness and transform it to strength for next

month’s visit.

Note:

All patients diagnosed as having complicated problems or

diseases will be referred to the nearest hospital upon the

request of the doctor and approval from the head of the facility.

Exit and Preparation for next visit

The records will be kept individualized with strict

confidentiality and put it into the data bank where it will be

used again as reference for the next scheduled activity.

1st Monthly Visit: June 06, 2011

The Maguindanao Medical Team has catered 98 patients during its first visit

in the jail and within this number of patients are 24 visitors and Provincial Jail

Staff acquired for consultation with free medicines. Patient may have one or more

health complaints. The table below shows the distribution of health complaints

with corresponding number of patients.

As shown above, the highest number of complaint is Cough (38 patients)

which may be caused by easy transmission of droplet and airborne diseases from

one person to another. This transmission may be due to crowdedness inside the

area.

Further, as noted, there are also two cases of inmates who were circumcised

by their fellow inmates as per request, using only non-sterile sharp objects. Thus,

this promotes wound infection and long-healing process.

As observed by the sanitary inspector, the sanitation should be improved

since there are poor drainage system and poor waste disposal management.

Health teaching was also provided to increase their knowledge regarding

personal hygiene and other health-related practices.

ASSESSMENT OF SANITARY INSPECTORY AND RECOMMENDATION FOR NEXT VISIT:

Proper utilization of area space to lessen crowdedness.

Installation of exhaust fan is needed.

Waiting area for visitors.

Additional toilets and cubicles.

Improve Personal Hygiene Practices

Provision of free shampoo and bath/laundry soaps

Cleaning of canals as for proper drainage system

Removal of tires and breeding sites.

Improve waste disposal management

Provision of Garbage Cans

Zero Case of Non-Health Personnel Circumcision.

Provision of Circumcision by health personnel on next visit is

recommended
