Record any significant work related injury or illness especially those that result in:

– Death– Loss of Consciousness– Days away from work– Restricted work activity or job transfer– Medical treatment beyond First Aid– Medically removing the employee under an OSHA

standard– Needle stick or “sharps” injury contaminated with

potentially infectious material (another person’s blood).– Hearing Loss

Lift Inspections

• Annual Inspection by “qualified” 3rd party

• Employees must have documented training on usage
