In the effort to continuously improvement of treatment, our hospital has joined with other
hospitals to document, store, and evaluate as many and as detailed as possible medical results
from cases of intracranial germ cell tumors. The aim is that patients will benefit from new
knowledge about treatment and causes of this disease. To avoid confusion or double
registration, your personal data/the personal data of your child (surname, forename, date of
birth, address) will also be registered.

In dem Bemühen, die Behandlungsmethoden ständig zu verbessern, hat sich unsereKlinik mit anderen zusammengeschlossen, um möglichst viele und genaue medizini-sche Befunde aus den einzelnen Krankheitsverläufen zu dokumentieren, zu spei-chern und auszuwerten und eine rasche Zusammenarbeit der Kliniken untereinander
zu gewährleisten. Um Verwechslungen, Doppelerfassungen und ähnliches auszu-schließen, erfolgt diese Erfassung zusammen mit den Personalien (Name, Vorname,Geburtsdatum, ggf. Geburtsname, Adresse). 1.1 Accompanying studies of Germ Cell Tumor biology and prognostic markers
It is evident from the brief overview of the cytogenetic profiles in GCTs provided in this protocol (see chapter
1.2.3) that a cyto- and molecular-genetic characterization is necessary in order to prospectively assign eachindividual tumor to the appropriate biologic subgroup. This is particularly important, because the histologicevaluation does not allow for distinguishing e.g. YST of the adult testis from YST of the infantile testis. The cut-offbetween the infantile and the adult type can be estimated between an age from 5 to 15 years. However, the riskof recurrence and therefore the clinical management differs between the two.In addition, it is mandatory in certain situations to critically evaluate the constitutional karyotype, because GCTs atcertain sites may be associated with constitutional chromosomal aberrations. Therefore, a conventionalcytogenetic analysis of blood lymphocytes must be performed in each male patient with a mediastinal GCT and ineach female patient with a malignant ovarian GCT (see above). It can be assumed that the detection ofconstitutional chromosomal aberrations will have a significant impact on the estimation of the risk ofmetachronous GCTs and of prognosis (with respect to both survival and reproductive function after cure fromGCT) and therefore, on clinical management.
Cytogenetic analysis of blood lymphocytes is recommended for the detection ofconstitutional sex chromosomal abnormalities in:
• male patients with mediastinal GCT• female patients with malignant ovarian GCT
The studies aiming for the screening for potentially constitutional sex-chromosomal abnormalities will becommunicated to the treating physician. However, prior to communication, positive PCR based assays will be
confirmed with FISH technique. If both are positive for aberrant presence of Y-chromosomal material, thephysician will be informed. In this instance, the letter will include the explicit recommendation that evaluation ofblood lymphocytes is advisable and that a consultation of a board-certified human geneticist must be initiated.
1.1.1 Aims of the accompanying biologic studies
The associated biologic studies aim for the evaluation of cytogenetic (e.g. CGH profiles, loss of heterocygosity(LOH)) and molecular markers (e.g. microsatellite instability) that may be associated with clinical outcome and/orresistance to therapy. If suitable (fresh-frozen) tissue is available, GCTs with mixed histology will be analyzedafter microdissection of the different histologic components in order to evaluate potential steps of tumorprogression. The associated molecular biologic studies will also address the question to what extent sex
chromosomal abnormalities at the sub-microscopic level contribute to the development of ovarian and mediastinalGCTs.
1.1.2 Coordination of the biologic stud ies
The biologic studies will be coordinated by the MAKEI coordination center. Part of the analyses will be performedby the research group at the Clinic of Pediatric Oncology, Hematology and Immunology in Düsseldorf (PD Dr.D.T. Schneider). The tissue bank will be managed in Düsseldorf. Currently, cooperation with the U.S. ChildrensOncology Group (COG) is planned. This cooperation will allow for performing the analyses focusing on a potentialclinical correlation of biologic markers in cooperation with the ongoing COG protocols on childhood GCTs.Research groups from other institutions are encouraged to apply for the distribution of tissue samples for
complementary studies. The decision regarding these applications will be made by the MAKEI study coordinatorsafter review by the Ethic’s committee of the Heinrich-Heine-University. First priority will be given to applicants who

have received funding after peer review. In the past, samples have been distributed to collaborating institutionsfrom Germany, England, and the Netherlands.
1.1.3 Logistics
In every patient, blood (≥ 2 ml, anticoagulated with EDTA, at room temperature) has to be sent to the MAKEIstudy center for comparison of constitutional DNA to tumor DNA from fresh frozen or archival (paraffin embedded)samples. Otherwise, e.g. LOH and microsatellite instability studies and the studies on constitutional sex-chromosomal aberrations are impossible.In addition, the participating hospitals are strongly encouraged to contribute fresh-frozen tissue samples of eachsurgical specimen to the MAKEI tissue bank, if sufficient tissue is left for histologic diagnosis. In Düsseldorf, thepresence of sufficient tumor tissue within the sample will be verified histologically. Tumor samples must be snap-frozen in liquid nitrogen and sent to the address mentioned below. Alternatively, the tissue samples may also besent to the tissue bank in Cologne (BioCase® project of the GPOH Competence Network). After inclusion into thetissue bank, the samples will be stored anonymously, and patient identifiers will be registered in the MAKEIcoordination office, only.

1.1.4 Patient Information and Declaration of Consent
All patients and/or their parents will be informed about the purpose of including parts of surgical tissue samplesand blood for analysis of biologically and potentially also clinically relevant genetic aberrations. The patients haveto sign the declaration of consent (see appendix A) prior to genetic analysis. The forms can be copied form theprotocol or downloaded from the MAKEI homepage. A copy of the declaration has to be sent to the MAKEIcoordination center.

Einwilligungserklärung zur Gewinnung und Lagerungvon Tumorgewebe
(zum Verbleib in der Krankenakte)

Ich bin damit einverstanden, dass Tumorgewebe meines Kindes zur Erforschung von Teratomen und malignenKeimzelltumoren in ihren molekularen, genetischen, immunologischen und anderen, mit der Krankheit direktverbundenen Merkmalen untersucht und gegebenenfalls für die Entwicklung neuer Behandlungsverfahreneingesetzt wird. Die Entnahme des Tumorgewebes ist nicht mit zusätzlichen Schmerzen verbunden; sie erfolgt imRahmen der für mein Kind notwendigen operativen Tumorentfernung bzw. während der zur Diagnosestellungerforderlichen Probenentnahme aus dem Tumor.
Falls bei der Tumorentfernung aus medizinisch chirurgischen Notwendigkeiten gesundes Gewebe mitentfernt
werden muss, darf dieses als Vergleichsgewebe für die Untersuchung von Tumoreigenschaften eingesetztwerden. Eine medizinisch nicht notwendige Erweiterung des operativen Eingriffes erfolgt dazu nicht. Zugestimmtwird der Entnahme einer Blutprobe während der Narkose (je nach Alter 2 – 5 ml) als Vergleichsmaterial für dieEigenschaften des Tumors.
Tumor, Vergleichsgewebe und Vergleichsblut werden zentral in einer Tumorbank der GPOH (Gesellschaft fürPädiatrische Onkologie und Hämatologie) gelagert und kostenfrei und anonymisiert Wissenschaftlern, die inuniversitären Einrichtungen oder in Krankenhäusern tätig sind und in GPOH-Studien kooperativ eingebundensind, für die obengenannten krankheitsbezogenen Untersuchungen zur Verfügung gestellt.
Für die Teratome und malignen Keimzelltumoren erfolgt die Lagerung in der Klinik für Kinder-Onkologie, -Hämatologie und -Immunologie der Heinrich-Heine-Universität in Düsseldorf (Direktor: Professor A. Borkhardt).
Auf diese Weise sollen die Diagnosestellung sicherer gemacht werden, das biologische Verständnis derErkrankung verbessert und neue therapeutische Ansätze gefunden werden.
