 How do I place an order? Order Forms will be… Sent home with the elementary & middle school students

Available in the Elementary and High School Offices Available to download on the Lomira School District Website

Return your order form with payment… To your child’s classroom (elementary students only) To the elementary school office Mail to Heidi Zingsheim, Lomira PTC, W455 Super Drive, Lomira, WI 53048

Payment… Make checks payable to Lomira PTC If check returned, you are responsible to cover all bank charges

Deadline… Firm order deadline: Wednesday, March 23rd Orders received after the deadline will be returned

All Sales Are Final…No Returns or Exchanges (No Inventory-Sorry!)

How will I receive my order? On the Order Form, select one of the following… Send Home [Student/Teacher] –

Once the orders are assembled, we will send the items home with your student. Please include the teacher’s name to assist in distribution.

Pick-Up At School [Date TBD] – We will again be assembling the orders at school after all products have arrived. An e-mail notification will be sent informing you of this date. If you cannot pick-up your order at that time, your items must be picked up in the elementary office the following week if alternate arrangements have not been made.

When will I receive my order? Approximately 4-6 weeks after order deadline
