letter seven (7) days prior preferred schedule. Employees should

Onsite Data Capture
1. Onsite Data Capture (ODC) is an activity that will be
conducted by accredited personnel / partner of Allcard Plastics
Phils. to capture pertinent data needed for PhilHealth membership
card. The following data will be gathered on this activity:
a. Photo of member
b. Fingerprint of member
c. Signature of member
d. Basic Information
2. Data Capturing can be arranged and scheduled per
group/department of your organization to lessen downtime on your
operations. For a more efficient data capturing process, employees
should register on-line at http://philhealth.allcard.com.ph:81 3.
An NDA between parties can be executed to ensure that these data is
undisclosed and secured4. The membership card costs P90.00 per
member and payable by the employee.
a. Your Company will be remitting the payment to Allcard
Plastics Philippines Inc. in check or cash upon delivery.
b. Payment will be directly remitted by your employee to Allcard
Plastics Philippines after capture
5. Each ODC booth includes two (2) to three (3) personnel from
Allcard Plastics Phils. Inc. These personnel will be bringing in
the following equipments for the Data Capture activity:
a. 17 Monitor
b. Laptop
c. Fingerprint scanner
d. Signature pad
e. Camera
f. Collapsible booth
g. Tables and Chairs
h. Marketing material such as tarpaulin poster w/ stand
6. Size of ODC booth is 5ft x 4ft and must be situated within a
well-ventilated area.
7. Each ODC booth can capture 110 members (minimum) within one
working day (8 hours shift). Multiple ODC booths can be arranged
depending on the population of PhilHealth members in your
organization.
8. Delivery lead time is 15 days in NCR and 30 days to the rest
of the country from data capture date.
9. Allcard would be requesting for an internet connection or
Wi-Fi for the data captured to be transmiited to Philhealth in real
time. Our team will bring its back up pocket wi-fi, but we hope
your company can provide the said request.
