During the night of 09/10 August 2012 engineering work was taking place on the line that runs between Nuneaton and Birmingham.

The work activities were coming to a close when, at about 04:20 hrs, an on-track machine (Stoneblower 80212) collided with a stationary on-track machine (Ballast Regulator DR77002) within the work site, on the down Arley line.Ballast Regulator DR77002

There were four membersof staff on the ballast regulator, and three onthe stoneblower. One person was taken tohospital and released laterin the day. Others with minor injuries were treated on site by paramedics.

The collision caused the leading bogie of the stoneblower to derail and damage was caused to both machines.

Learning points

When controlling an engineering train, OTM or OTP movement instruct the driver to make the movement at a speed where they can stop within the distance they can see the line ahead to be clear, regardless of the conditions.

All movements within a possession or a worksite must be made at no more than 5mph unless the ES/PICOP and driver/operator have held a safety conversation where appropriate alternative speeds in the journey have been agreed and authorised by the ES/PICOP.

Only the ES/PICOP can authorise appropriate alternative speeds, which should be appropriate for the conditions such as darkness, fog, gradients, low adhesion, curvature of the line or anything else that may affect the driver’s ability to stop short of any obstruction. The maximum permitted speed that can be authorised is 40mph.

*Version 2 is an updated version of the original Arley briefing that takes into account changes that came into force in the December 2013 Rule Book update; specifically the removal of the need to instruct drivers to proceed ‘at caution’. As the message and intent of both versions of the briefing are the same, there is no need to re-brief individuals who received the original briefing on Version 2. Rule Book updates should already have been briefed out to relevant staff locally.
