Mercy Health Hospital and Mercy Health at Home Hospice, Rockford, IL
Serve as the pharmacist member of the inpatient and outpatient palliative care services and Mercy Health hospice team.
Attend weekly interdisciplinary Mercy hospice rounds; review medication profiles for hospice patients and provide pharmacotherapy recommendations as well as education to team members.
Respond to questions/pharmacotherapy consults from hospice nurses and hospice medical director on ongoing basis outside of rounds.
Review medical chart and medication profiles for inpatient palliative care patients
and hospitalized patients electing hospice upon discharge, including discharge
medication review and reconciliation.
Participate in weekly outpatient Supportive Oncology Clinic, and meet with
patients/caregivers to perform medication reconciliation, assess for medication-
related problems and symptoms, provide patient/caregiver education, and assist
palliative physician with drug selection and optimization.
Identify and discuss medication-related issues with non-palliative care providers for
palliative care patients seen in supportive oncology clinic and in the hospital
(including oncologists, primary care providers, hospitalists, cardiologists, and other
specialists) to ensure safe and effective medication use and help coordinate care for
patients.
Investigate and resolve medication access issues for outpatient palliative care
patients, including completion of prior authorization requests for approval or
appropriate quantities for supportive care medications.
Attended inpatient palliative care rounds and outpatient Advanced Illness
Management (AIM) rounds from 2016-2018. Continue to answer questions from
AIM provider related to pharmacotherapy for AIM patients.

Since April 2018, function as the pharmacist member of the biweekly Geriatric
Assessment Clinic (GAC) in Janesville, WI in which patients and caregivers rotate
through assessments with myself, the physician, social worker, and
neuropsychologist during a 2 hour visit.
In GAC clinic, provide pharmacotherapy recommendations to GAC team and
provide patient and family with medication education.
Following GAC clinic, provide written documentation of patient medication taking
behaviors and provide pharmacotherapy recommendations to the primary care
provider.
With pharmacy students as “pharmacist extenders,” developed a geriatric inpatient
pharmacy service for patients ≥ 75 years old (service provided roughly nine months
of the year), which includes comprehensive medication review and resolution of
mediation-related problems

March 2013- Palliative Care Volunteer Service
April 2015 Developed volunteer program for pharmacy students at SwedishAmerican Hospital
Rockford, IL
The intent of the program was for students to provide companionship to hospitalized
palliative care patients who did not have regular visitors. From the volunteer service,
students gained experience interacting with patients and developed communication
and listening skills
Students received formal orientation training from myself and hospital chaplain once
yearly in evening program at the college of pharmacy
Each year, the service had 4-8 regular volunteers
First visit with inpatient palliative care patient was supervised by myself; on subsequent visits, I provided students with a debriefing before and after visit.
Information learned by the student from the visit was also shared with palliative care
team to appropriately address unmet needs, including spiritual, existential, and
