Other incidents reported were: six (6) landslides (Region IV-A and NCR), eight (8) drowning (Regions IV-A and IV-B); one (1) vehicular accident (Region IV-A), two (2) collapsed structures (Regions IV-A and IV-B), one (1) lightning (Region IV-B) and three (3) maritime incident (Region IV-B). (TAB A)

B. EFFECTS

1. Affected Population (TAB B)

A total of 992,562 families / 4,451,711 persons were affected in 2,764 barangays / 181 municipalities, 36 cities in 17 provinces of Regions I, III, IV-A, IV-B, VI and NCR

Region III suffered the most with 584,584 families/2,549,849 persons affected in 1,408 brgys of 83 municipalities and 8 cities in 6 provinces namely: Pampanga, Tarlac, Bataan, Zambalez, Nueva Ecija and Bulacan. Region IV-A ranked 2nd with 241,066 families/1,087,683 persons affected in 618 barangays of 56 municipalities and 6 cities in Batangas, Laguna, Rizal and Cavite. NCR ranked 3rd with 90,121 families/419,555 persons affected in 264 barangays, 16 cities and 1 municipality

2. Casualties (Tab C)

There were 130 casualties: 112 dead, 14 injured 4 missing. In terms of the number of deaths, Region III has the most number 48 followed by NCR 41 and Region IV-A 15. The cause of death was mostly due to drowning (70), landslides 12 and the rest were due to other causes

3. Damages to Properties

3.1 Damaged Roads and Bridges (Tab D) At the height of the SW Monsoon, a total of 22 roads were rendered impassable due

to floodwaters while 3 bridges incurred, road cuts, collapsed slope protection, and damaged approaches

In Region III, (3) bridges in Bataan were damaged: Biaan Bridge (Collapsed protection and damaged 2nd approach); Banayam Bridge (Collapsed slope protection and damaged 2nd approach); and Paysawan Bridge (heavily damaged)

3.2 Damaged Houses (Tab E)

A total of 14,280 houses were damaged in Regions I, III, IV-A, IV-B and VI: 3,871 totally and 10,409 partially.

Of the total number affected,, Region IV A has the most number 3,125 totally and 7,315 partially, followed by Region III 427 totally and 1,030 partially damaged

Power and Water Interruptions The following areas experienced water and power interruptions at the height of massive

floodings: NCR (six (6) barangays in San Juan, two (2) barangays in Navotas and some barangays in Malabon) and in Region III (1 city in Zambales, six (6) municipalities in Pampanga, all cities and municipalities in Tarlac, four (4) municipalities in Bulacan, and Orani in Bataan)

6. Dams, Rivers and Waterways Monitoring Six (6) dams namely Angat, Ipo, Ambuklao, Binga, San Roque, Magat and La Mesa

reached their critical levels which prompted PAGASA to issue recommendations to the management of National lrrigation Administration and National Power Corporation on the opening of gates and subsequent release of water as part of precautionary measures.

The Marikina River and Tullahan River overflowed which resulted in the massive

floodings in some barangays in Marikina City and other parts of Metro Manila

Shanty dwellers attempted to rescue personal belongings after the Tullahan River overflowed

II. EMERGENCY MANAGEMENT Emergency management in terms of preparedness, warning information

dissemination, coordination meetings, search, rescue and evacuation, damage assessment and needs analysis (DANA) as well as the relief and recovery operations

had been effected through the concerted efforts of the NDRRMC implementing agencies namely AFP, PCG, DILG (BFP, PNP), DSWD, DOH, Dep Ed, DPWH and DA

The United Nations and the Philippines Humanitarian Country Team, through UN Resident Coordinator Ugochi Florence Daniels, expressed sympathy to the victims of Southwest Monsoon and assured to complement Philippine Government's relief and recovery efforts, if requested.

A.PREPAREDNESS /ACTIVATION

NDRRMC OPCEN was activated to Blue Alert Status on 7 August and Teams A, B

and C from the OpCen Staff rendered duty 24 hours on rotation basis. As the situation worsened, the Center was elevated to Red Alert Status and the representatives of NDRRMC rendered duty at OPCEN 24 hours on rotation basis to facilitate easy coordination and monitoring of actions on the requirements in the field; immediate conduct of damage and needs assessments; and immediate provision of assistance/delivery of basic services to the victims of disasters.

NDRRMC member agencies and RDRRMCs-OCDRCs concerned likewise activated

their respective emergency operations centers (EOCs) to establish continuous coordinations; alerted their emergency responders; pre-positioned all available resources in terms of manpower (response teams/units), equipment and rescue assets and relief supplies (food and non-food items).

B. WARNING INFORMATION DISSEMINATION

NDRRMC OpCen continuously disseminated gale warnings, flood outlook and weather

advisories, status of rivers and waterways issued by PAGASA through, SMS facilities, facsimile and website to RDRRMCs likely to be affected for widest dissemination to local DRRMCs (provincial, municipal and city levels): continuously uploaded NDRRMC update on the periodic situation reports of the evolving situations at the website, twitter and facebook for information of the public as a whole; referred numerous urgent calls received from the residents at risks in heavily flooded areas in Marikina, Quezon City and CAMANAVA (Caloocan Malabon, Navotas and Valenzuela) area to the responding units stationed at the NDRRMOC namely AFP, BFP, PNP and PCG for immediate actions; and maintained continuous link with various cooperating and implementing agencies through coordinations,

RDRRMCs-OCDRCs I, III, IV-A, IV-B, NCR and CAR OpCENs maintained Red Alert status manned by staff / personnel 24 /7 basis; continuously disseminated warning advisories to all Local DRRMCs, RDRRMC member agencies and media practitioners utilizing the SMART info board email, social media network, facsimile and SMS; and continuously advised Local DRRMCs to take appropriate action, watched the alert level, and discussed local preparedness for possible emergency response operations

C. COORDINATION MEETINGS CONDUCTED

NDRRMC conducted an Emergency Meeting with His Excellency President Benigno Aquino III at the NDRRMC Conference Room on August 7, 2012, attended by DSWD, DILG, DOST, MMDA, DPWH, DOH, Dep Ed, DOE,PAGASA, AFP, PNP, BFP, PCG, other agencies and print / broadcast media.

Said meeting focused on the action plan on damage assessment, assets mobilization,

Search and Rescue (SAR), relief allocation and distribution to augment the LGUs and affected communities. His Excellency raised some specific issues and provided general guidance to the response and relief agencies to require the LGUs concerned to go around their respective areas through the Brgy Captains and provide necessary assistance whenever necessary.

President‘s directive to the agencies were the following: DSWD to find a place where they can put tents as additional evacuation centers; DOH to initiate the provision of potable water for those who depend on deep wells and to address flooding in hospitals; DILG to identify all inundated /flooded barangays in Marikina City, require all LGUs to go around their areas through the barangay captains and to check floodings in Tarlac and Northern Luzon (La Union, Pangasinan and Ilocos Norte; DOST to fast track the preparation of hazard map of Luzon; and DPWH to study the long term solution for Laguna Lake.

On August 8, 2012, His Excellency President Benigno Aquino III, DND Secretary and

NDRRMC Chairperson Voltaire Gazmin, MMDA Chairperson Francis Tolentino and Muntinlupa City Mayor Aldrin San Pedro personally inspected the Tunasan Elementary School in Brgy Tunasan, Muntinlupa City to assess the situation of the evacuees

Rapid Damage and Needs Assessment

The NDRRMC deployed 2 Rapid Damage and Needs Assessment (RDANA) teams on 15 August 2012 to conduct damage and needs assessment to heavily inundated areas: Calumpit, Bulacan, Sto Tomas, Pampanga (Region III), and Calamba City and Bay Laguna in Region IV-A. Each team was composed of OCD, DSWD, DOH, Dep Ed, DA, PRC, PDPWH, DILG and PNP.

Findings of the team: food sustenance for the next three (3) months; needs identified were potable water and portalets at the evacuation centers. Routine vaccination of children was stopped since most of the Brgy Health Stations and other health facilities were submerged with floodwaters. Supplemental vaccinations are not yet implemented

Undersecretary Parisya Hashim-Taradji visited the evacuation centers in San Fernando and Sto.Tomas, Pampanga

RDRRMC III Chairperson joined the President and party in their visit to the flood affected

areas of Central Luzon and personally distributed the relief goods to the flood affected victims and conducted aerial inspection at Pampanga area on 7 August 2012 together with DILG, DSWD, DOH, NEDA, PIA and AFP.

President’s Visit in Central Luzon

PNP II provided security during the visit of the President. PDRRMC Chairperson, Gov.

Hermogenes E. Ebdane Jr. conducted ocular inspection at Bucao Dike and assessed the damage caused by the heavy rains. Olongapo City Rapid Damage Assessment and Needs Analysis (RDANA) Team conducted assessment in various areas within the city.

MGEN Ricardo Banayat, AFP Commander of the 1st Air Division conducted Aerial Survey over Region III together with Chairperson, RDRRMC III, Dir Dijan of DILG, Dir Baldago of PIA, Dir Apostol of DSWD, and ARD Mariano of NEDA onboard PAF SOKOL Helicopter.

D. SEARCH AND RESCUE (SAR) AND EVACUATION

SAR operations were effected through the concerted efforts of the SAR units of AFP,

PCG, PNP, BFP and DPWH with 19,579 Officers and Enlisted personnel, 699 vehicles and 283 sea crafts mobilized to various areas in Regions I, III, IV-A, IV-B and NCR (Tab G))

NDRRMC OpCen coordinated with the Office of the Press Secretary and Presidential Communications Development and Strategic Planning Office regarding list or database of individuals to be rescued.

DSWD SWAD teams coordinated with the local counterparts regarding the inquiries, request for relief assistance and rescue of the families in areas still submerged in deep water.

Details of Search and Rescue Efforts of NDRRMC Member Agencies and

RDRRMCs/ Local DRRMCs (Tab h ) E. RELIEF AND RECOVERY OPERATIONS (Per Cluster)

1. Camp Coordination/Camp Management (CCCM)

The CCCM Cluster through the DSWD has been working with the local authorities to

determine the needs in the evacuation centers. The DSWD is supporting the LGUs in the delivery of food packs and essential relief items to the IDPs and other flood victims. The DSWD is deploying social workers to augment Camp capacities in evacuation centers and in the sites that are expected to accommodate IDPs for an extended period. The CCM Cluster gathered, consolidated, and reporting information on the location and situation inside evacuation sites

A total of 167,246 families/771,202 persons were displaced in Regions I, III, IV-A, IV-B, NCR and VI who were evacuated and provided temporary shelter such as: schools, gymnasiums, public buildings and homes of relatives and friends

From August 7 to Sept 6, 2012, a total of 437,887 families /2,033,852 persons persons

were served both outside and inside evacuation centers boken down as follows: Inside 1,809 Evacuation Centers (ECs) 167,246 families /771,202 persons

Outside ECs (relatives/friends) 270,641 families /1,262,650 persons

2. Food and Non-food Items (NFIs) Estimated Cost of Assistance (Tab I) NDRRMC OPCEN recommended the release of 5,000 sacks of rice to Regions I, III, IV-

A, CAR and NCR at 1,000 sacks each region for relief operations in said regions

NDRRMC Member Agencies Fifty percent (50%) of affected population were served with food and non food items

worth PhP193,961,749.11 from the combined resources of the following agencies: DOH

(PhP15,034,035.95; DSWD, (PhP71,706633.62); LGUs (PhP94,069,426.29); and NGOs (PhP13,151,653.25)

DSWD DSWD –DROMIC released 12,900 family packs to the legislators for their constituents in

the cities of Manila, Muntinlupa, Taguig, Quezon, Caloocan, Pasig and Valenzuela. Assistant Secretary Florita Villar and DSWD FO III’s Operations and Programs Division provided relief assistance to 1,100 families in Apalit, Pampanga.

DSWD SWAD teams coordinated with the local counterparts regarding the inquiries, request for relief assistance and rescue of the families in areas still submerged in deep water

DSWD-QUART continuously prepositioned family packs and Non food Items ready for distribution

DSWD IV-A deployed 10 teams to Taytay, Angono, Rodriguez, Cainta and San Mateo in Rizal; Biñan, Sta Rosa, Bay, Sta Cruz, San Pedro and Calamba City in Laguna to assess the needs of the families inside the evacuation centers and to provide Psycho Social Process (PSP) thereat. The teams implemented the Disaster Assistance Family Access Card (DAFAC) system monitored the camp management and distributed family food packs through administration of DAFAC. Also, facilitated the distribution of family food packs in Rodriguez and Cainta, Rizal during Pres. Noynoy Aquino and Sec Dinky Soliman's visit and distributed family food packs, plastic mats, blankets, hygiene kit, bottled water, mosquito nets, and used clothing to the affected families.

PNP PNP with the Command Group, PCRG personnel, Rotary Club, and PNP- accredited

NGOs conducted relief operations in Tumana, Marikina and provided 1,722 assorted relief packs in said barangay. Also provided 344 packs in Sta Maria, Bulacan. PNP SAF deployed 48 Police women, 77 PNCO and 3 PCO to assist in the repacking of relief goods at DSWD Warehouse Pasay City. PNP-PRO1 assisted various MDRRMCs relief operations as well as the pre-emptive evacuations in flooded areas. PNP Sta Maria, Bulacan conducted relief operations and distributed relief goods to Brgy Caycio and Brgy Lalakhan, Sta Maria, Bulacan. PNP Bulacan continuously conducted relief operations to the affected municipalities of Obando, Malolos, Calumpit, Hagonoy and Meycauayan, Bulacan

PRO IV-A launched "Mula sa Puso ng Pulis" project spearheaded by Police Community Relations Division where in the PRO IV-A personnel donated money to purchase food items for the affected families along the Laguna de Bay region. The initial fund raising activity raised money to purchase 200 food packs distributed to the families of Brgy Banadero, Calamba City.

DPCR-NHQ and PRO IV-A personnel conducted relief distribution at Brgy Linga, Calamba City and provided 6x6 trucks to assist in the continuous relief operations in the provinces of Rizal and Laguna upon the request of DSWD IV-A to RDRRMC IV-A.

3. Logistics Cluster

DSWD accepted the offer of WFP for logistical support in terms of transporting relief

supplies from DSWD National Resource Operations Center Warehouse in Manila to Regions III, IV-A within NCR. So far, WFP had dispatched 21 trucks carrying government

supplies such as family packs, blankets, rice, and 52.5 metric tons of WFP HEB biscuits. The Logistics cluster continuously worked with DSWD and LGUs and other clusters to ensure that relief goods can be dispatched and delivered promptly

AFP Relief Operations

AFP-PAF airlifted 1,400 food packs to the two (2) isolated barangays in Lubao,

Pampanga. SOLCOM deployed one M35 of C COY and delivered relief goods at Brgy. Hall of Malanday. PDRRMC and PSWWDO Tarlac delivered/airlifted relief goods to isolated families in Brgy. Bueno, Capas thru the air assets of the TOG3

NGOs PRC San Juan City conducted relief operations and provided food assistance in Brgys

Rivera, Maytuna and Balong Bato; PRC Rizal provided food packs to the evacuees in San Mateo, Tanay and Rodriguez, Rizal; and hot meals to families temporarily sheltered at the Dagupan City Astrodome, PRC Mandaluyong City provided a first aid station, hot meals and relief goods. PRC Rizal (Marikina Branch) distributed 1,245 packs of relief goods at the evacuation centers in Marikina. PRC Pangasinan provided hot meals to families sheltered at Dagupan City Astrodome. Quiapo Church Disaster Ministry provided food assistance to four (4) evacuation centers in Quiapo, Manila. PRCs Pasig- Potrero, and Makati City conducted relief operations and distributed food packs at the evacuation centers in respective cities

Volunteers of CDRC, STPRC and CREST repacking relief goods at the CDRC warehouse in Quezon City and the STPRC warehouse in Los Banos.

RDRRMC I MDRRMC Bugallon, Pangasinan put up temporary shelters to serve as emergency

evacuation area as flood waters coming from Agno River continued to rise. Evacuees were provided with relief items from the following sources: 920 family packs from Pangasinan Provincial Government; 592 family packs from Tina Dungca of the GMA Foundation; 300 family packs from Vice President Jejomar Binay; and

200 family packs from LGU Bugallon RDRRMC III PDRRMC Zambales distributed relief assistance to the affected families in Iba,

Zambales and activated Local Price Coordinating Councils. PDRRMC Pampanga distributed food and drinks to the affected municipalities of Sto. Tomas, Pampanga; established an evacuation center at the Provincial Engineering Office compound with 30 families/120 individuals catered; and provided relief assistance to 1,100 families in Apalit. PDRRMC Nueva Ecija prepositioned 500 packs of relief goods, 2 dump trucks and 210 cavans of rice. PDRRMC Tarlac delivered truckloads of sand and sacks in Camiling, Malayep, Salapungan and Bantog, Victoria in Tarlac for sand bagging operations. MDRRMCs San Antonio, San Felipe, Sta Cruz, Botolan and Iba in Zambales provided assistance to the affected families in respective areas.

PSWDO/PDRRMC TARLAC and CADRE Company of 70IB conducted repacking and distribution of relief goods to the affected families; delivered/airlifted relief goods to isolated families in Brgy. Bueno, Capas thru the air assets of the TOG3

Region IV-A

RDRRMC IV-A Chairperson alerted PNP PRO IV-A to assist DSWD IV-A in the relief operations in the provinces of Rizal, Laguna, and Cavite. Tanauan CDRRMC provided food packs to evacuees under their AOR.

RDRRMC V Team Albay – OCD 5 team assisted in the repacking efforts of ABS-CBN Sagip

Kapamilya Foundation for possible relief distributions to affected communities

NCR LGU Makati City distributed food packs and relief goods to individuals affected by

3. Health Cluster DOH-HEMS deployed 8 Medical Teams and 24 Public Health Team to provide

medical services (consultation, vaccination/immunization, dental service and health education) and technical services; activated the Surveillance in Post Extreme Emergencies (SPEED) system with 35 health facilities participating and advised respective CHDs to activate SPEED in their respective areas; prepositioned stand-by Doxycycline capsules, prophylaxis, and assorted drugs and medicines to the LGUs concerned; provided medicines and medical supplies to augment the Regional Health

Offices in affected areas; set up its Command Center; in East Avenue Medical Center, strengthened advisory and health promotion campaign on the preventive measures of Leptospirosis cases and other flood and water borne related diseases; distributed Vitamin A, doxycycline, and Hyposol solution; conducted vaccination of attenuated measles vaccine (AMV) and measles, mumps, rubella (MMR) to the children; and deployed medical teams in Quezon City, Bataan, Bulacan, and Laguna to conduct medical consultations with 18,572 affected families catered; The most common causes of consultations were acute respiratory infections fever, wounds, skin diseases, and high blood pressures

RDRRMC III and DRRMOs concerned, in coordination with PHAPS CARES Foundation Inc provided medicines to the flood affected LGUs in Bataan, Bulacan and Pampanga.

CHD III provided PhP50,000.00 worth of drugs and medicines to the Provincial Health Team Office (PHTO). CHDs III, IV-A, CAR and NCR

RDRRMC III and DRRMOs concerned, in coordination with PHAPS CARES Foundation Inc provided medicines to the flood affected LGUs in Bataan, Bulacan and Pampanga.

CHD IV-A provided PhP350,000 worth of assorted medicines and PhP100,000 worth of doxycycline capsules and prophylaxis to the evacuation centers to anticipate the influx of diseases especially Leptospirosis.

Team Albay-OCD5 Medical Group was activated to provide support to the WATSAN team and conducted Post Disaster Health Assessment (PDHA) in the cities of Malabon and Valenzuela

CHD IV-B and MHO San Jose, Oriental Mindoro representatives conducted consultations and provided the evacuees with assorted drugs and medicines

Usec Benito Ramos and Dir Susana Cruz together with the medical mission of Philippine Medical Association (PMA) and DOH conducted medical mission in Malabon City

INGOS UNFPA has initially prepositioned 10,000 dignity kits for distribution to women of

reproductive age, particularly pregnant and lactating women. PRC also provided dignity 2,000 kits.

World Health Organization provided human resources and technical support to DOH Health Emergency Staff.

Save the Children International conducted rapid assessment in Taguig, Pateros, CAMANAVA, Pampanga and Bulacan

PRC Pampanga Chapter and Volunteers conducted health and sanitation seminar and set-up First Aid Station and Welfare Desk to help evacuees in the Municipality of Sto. Tomas and San Fernando, City, Pampanga; distributed relief goods (family packs) in San Fernando City and municipalities of Sto. Tomas, Guagua, and Sta. Rita, Pampanga.; and set up 3,500 liters and 5,000 liters of water bladder at Brgy Kalayaan Hall, Angono, Rizal serving 219 families in 3 evacuation centers; and at Joaquin Guido Elementary School, Angono, Rizal serving 261 families, respectively.

4. Water Sanitation and Hygiene (WASH)

• WATSAN Contingent Albay team led by Dr Rempilio of CHD V brought 1 tank lorry and 1 Water Filtration Machine and delivered response services based on areas prioritized by NDRRMC OpCen. The team was dispatched to Malabon City upon the request of the City Mayor. The team was able to distribute 68,000 liters of potable water to the residents of Brgys Tonsuya, Catmon and Kaunyagan utilizing the Water Filtration Machine and the distribution was made using the Mobile Lorry with 12,000 liters capacity

5. Nutrition Cluster

• CHDs III and IV provided vitamin A supplementation in evacuation centers and distributed Multiple Micronutrient powder to children with age from 6-59 months. CHD-NCR provided Vitamin A supplementation during the medical consultation and reported one (1) case of acute malnutrition in Quezon City for validation. Dr Jose Fabella Memorial Hospital coordinated with CHD NCR for human milk supply which was disseminated to all Nutrition Cluster partners for referrals

• The Nutrition cluster shared the Infant and Young Child Feeding in Emergencies (IYCF-E) Guidelines to the CHDs at evacuation centers in Regions III, IV-A and NCR for better management of nutrition response

• World Food Program (WFP) delivered 3 metric tons of Plumpy Doz (a highly nutritionally-densed food supplement ideal for complementary feeding among children aged from 6 to 59 months to CHD-NCR for distribution to the different evacuation centers prioritizing Pasig City as the primary LGU recipient

• UNICEF provided HEMS with Micronutrient Powders for the children 6- 59 months who are non- recipients of Plumpy Doz

• Dir. Gonzaga with UNICEF representatives, Dir. Banatin of DOH-CHD IV-A HEMS (WASH and Nutrition Cluster) had a joint monitoring of evacuation centers in the municipalities of Sta Cruz, Pila, Bay and Biñan City in the province of Laguna

• Regional Director deployed a mobile water treatment unit in Malabon which provided purified drinking water to people in the evacuation centers in Metro Manila.

6. Education Cluster

On 07 August 2012, the Office of the President issued Memorandum Circular No. 33 on the suspension of work in government offices and classes in college level in National Capital Region and the provinces of Zambales, Bataan, Pampanga, Pangasinan, Tarlac, Bulacan, Laguna, Cavite and Rizal, with the exception of those agencies whose functions involve the delivery of basic and health services, preparedness and response to disasters and calamities, and/or the performance of other vital public services

On August 8, 2012, classes both private and public at all levels were suspended in some areas in NCR, Regions III and IV-A. Some establishments and works in offices both government and private offices were also suspended

Dep Ed reported that 224 schools so far are being utilized as evacuation centers and mobilized teachers to conduct classes in QC; and prepared a contingency plan (1-2 hr additional for regular classes) to complete 180 days mandatory or make up classes; conducted site visits and assessment with Save the Children and World Vision in the most heavily affected areas in Regions III, IV-A and NCR to determine priority sites for intervention or provision of educational emergency supplies.

8. Infrastructure Cluster

DPWH Manila reported that three (3) bridges in Region III were rendered impassable due to scoured approach; rechannelled /diverted water ways of the two bridges and filled up embankment over cut –off approach utilizing one (1) unit bulldozer and one (1) unit backhoe, three (3) dump trucks of the local government units and one (1) unit pay loader from private firm.;

DPWH IV-A deployed its personnel to the flood prone areas in the region to assist the motorist traversing along the national roads

Provincial Engineering Office IV-A mobilized heavy equipment (loader, backhoe and dumptrucks) to augment in the clearing operations in Lemery, Batangas

CDRRMCC Muntinlupa built temporary footbridges at the affected areas near Laguna Lake

PNP- HQ deployed eight (8) PNP mobile cars to monitor the roads and bridges within Metro Manila.

G. CLEARING AND SAND BAGGING OPERATIONS

DPWH in NCR and III continuously monitored flooded thoroughfares and cleared the debris; conducted repair/restoration of affected roads, deployed heavy equipment and maintenance crew to the areas affected by landslide, uprooted trees and road sections with scoured pavements; and provided barricades and warning signs on road sections with road cut, eroded slope and affected bridges.

Marikina City Clean Up Operations

DPWH-IV-A deployed its personnel to the flood prone areas in the region to assist the motorist traversing along the national roads in the region. DPWH CAR continuously cleared impassable roads in cooperation with private

contractors. DPWH III Installed warning signs on road sections with road cuts, and eroded

slope in Hermosa, Bataan BFP–SRU III continuously conducted water rationing and flashing in

Dinalupihan, and Pilar Bataan and government offices, schools, and major roads in Zambales.

RDRRMC CAR

PDRRMC Benguet assisted the DPWH in road clearing operations with heavy equipment and manual work done.

NCR

OCD -NCR joined with MMDA in the conduct of clean-up operations in Marikina City

RDRRMC III

RDRRMC III issued 50,000 pcs of empty sacks to Candaba, Pampanga on 7 August 2012 to augment the sand bagging operations being conducted by the LGU.

PDRRMC Pampanga issued 500 empty sacks to Parian, Mexico and 2,000

empty sacks to San Isidro, Anac, San Isidro Matua, Malauli, Sta Lucia, Paguiba and Masantol for sand bagging operations; and released empty sacks to city/municipalities. CDRRMC Angeles released sacks to the following Brgys: 500 pcs to Sapang Bato, 500 pcs Pampanga, 200 pcs Sto Rosario, 700 pcs to Cutcut, 200 pcs to Sta Trinidad, 400 pcs to Cuayan, and 1,200 pcs to San Nicolas all in Angeles City.

PDRRMC Tarlac, in coordination with CADRE Company of 70I and PSWDO, delivered truckloads of sand and sacks in Brgys Malayep, and Salapungan in Camiling, Brgy Bantog in Victoria, Brgy. Poblacion. Sur in San Clemente, Brgys Villa Paz & Malayep in Gerona, Aquino Bridge in Mabini, Tarlac City, La Paz and Moncada for sand bagging operations.

H. DECLARATION OF STATE OF CALAMITY (Tab K)

The Sangguniang Panlalawigan (Province), Sanggguniang Panlungsod (City) and

Sangguniang Bayan (Municipality) declared their respective areas under a State of Calamity through Resolutions to utilize portions of the local Calamity Fund for relief and recovery operations. The body of Deo Socajil was recovered along the coastal area of Brgy. Gui-ob. On or about

12:00 PM 5 August 2012, the victim was reported missing after their banca capsized

due to the big waves while fishing.

SAR Operations was conducted by the Brgy. Officials and CPR was applied. The victim was

immediately rushed to RHU but was pronounced dead on arrival.

AFP AFP RESCOM NHQ assisted in the evacuation in Gen. Trias and Bacoor, Cavite. RESCOM deployed fifty (50) raincoats and twenty six (26) boots. 703rd Brigade with one (1) platoon, two (2) KM450 and NHR truck was stationed at OCDR III

for possible deployment and easy coordination of possible augmentation to flood affected areas.

7ID consist of 71st DRC, 7DRU, 7FMC with 36 personnel prepositioned 2 rubber boats, 1 M35 truck, 1 KM 250 truck and 1 ambulance in Sindalan, Pampanga

conduct of DANA in the provinces of Pampanga and Bulacan. 202nd Bde deployed its DRTFs in 15 towns around Laguna Lake and conducted evacuation

and SAR operations of affected families and transported them to the evacuation areas; 740CG personnel were deployed at the 1st district of Laguna; 1lB personnel at 2nd to 4th District of Laguna while 161B and 541EB DRU were deployed at Rodriguez, San Mateo and Cainta, Rizal

PNP PNP NCR deployed a total of 250 SAF Personnel (125 in CAMANAVA Area and 125 in

Marikina) for SAR Operations; four (4) 6x6 Trucks for “Libreng Sakay” Operations and other assistance to stranded motorists/passengers; and one (1) 6x6 truck and two (2) PNP Personnel to help transfer two (2) patients from Delos Santos Hospital to Philippine Heart Center. one (1) tow truck with 11 PNP HPG Personnel to help stranded motorists. Deployed one (1) tow truck with 11 PNP HPG Personnel to help stranded motorists.

PNP IV_A Dispatched one (1) team in San Pedro, Laguna to assist in the evacuation of affected residents of the municipality

PNP MG dispatched eight (8) inflatable rubber boats and 1 flat boat to MMDA for rescue operations.

PNP Academy (PNPA) placed its cadets in stand-by alert status for deployment in the affected areas to assist ongoing efforts on the SAR of the LDRRMCs and RDRRMCs.

PNP PDG Nicanor Bartolome with the Command Group, PCRG personnel, Rotary Club of Camp Crame and PNP accredited NGO’s conducted relief operations at Tuman, Marikina. and

Deployed one (1) tow truck with 11 PNP HPG Personnel to help stranded motorists. PPO Pangasinan deployed SAR personnel composed of 8 PCO’s and 154 PNCO to

different affected areas within their AOR and deployed Disaster Response Teams with assets (vehicles, rubber boats, etc) to conduct / assist in evacuation, rescue and relief operations

PPO Bataan deployed 53 personnel, 2 M35 trucks, 1 HILUX, 1 rubber boat with OBM, 1 rubber boat with paddle, 2 fiber banca, 4 OBM, 20 Life vest and two (2) life rings in Dinalupihan, Bataan.

PNP Bulacan conducted SAR and relief operations to affected municipalities such as Obando, Malolos, Calumpit, Hagonoy and Meycauayan.

Police Regional Office IV-A proceeded to Brgy Landayan, San Pedro, Laguna for the

Conduct of Rescue Operation as requested by the San Pedro, Laguna Mayor. PRO4A dispatched a 6x6 truck to be used on the operation.

PNP CAR assisted in the monitoring of road conditions and status of communities at high risk areas. Response teams with tools, equipment were on standby for deployment

MMDA MMDA deployed rubber boats in Bgry Sto Domingo, Talayan, Roxas District, and Parkway

Village, Muñoz, Quezon City PRC PRC NCR deployed WASAR team at Brgy San Jose , Rodriguez, Rizal and rescued 10

persons. PRC San Juan City deployed two teams with 4x4 trucks to conduct rescue operations in Brgy

Salapan, San Juan City. BFP BFP III conducted SAR operations in Bataan, Bulacan, Pampanga, Tarlac, and Zambales;

conducted water rationing and flashing in Dinalupihan, Pilar, and Hermosa in Bataan, government offices, schools, and major roads in Zambales; and conducted OPLAN LIKAS in Calumpit, Bulacan and OPLAN TAWID BAHA in Malolos City, Bulacan

SRU of BFPR 3 was deployed in the flood affected areas of City of San Fernando, Pampanga and conducted rescue and evacuation

BFP IV_A deployed Search and Rescue Unit to flooded areas assisting LDRRMCs in their evacuation and SAR operations.

DPWH DPWH III dispatched dump trucks to PDRRMC Pampanga to augment the on-going

evacuation operation DPWH IV-A Deployed its personnel to the flood prone areas in the region to assist the

motorist traversing along the national roads in the region; provided two (2) dump trucks to support DSWD relief operations.

RDRRMC/ Local DRRMCs Region I OCDRC I coordinated with CGDNL for possible deployment of personnel with rubber boats to

conduct evacuation of affected families in Brgy Guelew, Pangasinan PDRRMC Pangasinan alerted its member agencies for possible response operations and

deployed two (2) dump trucks for San Fabian and Aguilar; and Gov. Amado Espino announced the suspension of classes for all levels. Dispatched 2 WASAR Teams with PHERT personnel, 2 dump trucks and rubber boats to Brgy Lasip and Balococ, Lingayen, Pangasinan.

MDRRMC Bautista deployed thirty six (36) personnel and five (5) PNP vehicles to assist in evacuation and distribution of relief goods and conducted pre-emptive evacuation of affected families in Brgy Poponto

Region III RDRRMC III conducted briefing to the Cuayan, Isabela Rescue Team to discuss the situation

in Pampanga and coordinated with LTO re availability of 2 dump trucks to augment the assets mobilized in Pampanga.

PDRRMC Tarlac dispatched Tarlac WASAR team for SAR operations at Brgy Tibag, Tarlac City; dispatched trucks and 1 DART with rescue boat for relief and rescue operations; deployed personnel to Sto Tomas, Pampanga and conducted rescue operations in Minalin and Sto Tomas, Pampanga; and convened a meeting attended by all council members and other line/support agencies at Capitol Building.

PDRRMC Pampanga conducted force evacuation in the municipalities of Sto. Tomas, Minalin, Macabebe, Masantol, Apalit, Sta. Rita, Lubao, Guagua, Sasmuan with the support of MDRRMOs; established an evacuation center at PEO compound with 30 families/120 individuals; and conducted various rescue and evacuation operation within Pampanga area.

PDRRMC Zambales dispatched two (2) WASAR Teams with PHERT personnel, 2 dump trucks and rubber boats to Brgys Lasip and Baloloc, Lingayen Pangasinan.

Rescue 206 Santiago City conducted rescue operations at Heroes Hall in City of San Fernando, Pampanga.

Rescue 922 Cauayan City, Isabela conducted rescue operation in Dinalupihan, Bataan on August 7 and reported to OCD III on August 9.

CDRRMC Palayan City Rescue Team conducted rescue operations in Sto. Tomas, San Matias and Minalin Pampanga.

CDRRMC Olongapo City conducted rescue and evacuation operations in the 16 Brgys of Olongapo City

MDRRMCs Subic, Castillejos, San Narciso and Cabangan assisted in evacuation of affected families in low lying Barangays and distributed relief goods and sandbagging.

Region IV-A RDRRMC IV-A Chairperson alerted PNP PRO IV-A to assist DSWD IV-A in the relief

operations and assessed the on-going evacuation and SAR operation in the provinces of Rizal, Laguna and Cavite; and requested PDRRMC Quezon and CDRRMC Batangas for possible assistance in SAR Operation in Rizal province; and assessed the evacuation and SAR operation of the MDRRMC.

PDRRMC Laguna was instructed to conduct pre-emptive evacuation in the municipalities along Laguna de Bay.

PDRRMC Quezon dispatched an eight-man (8) response team with Water Search and Rescue (WASAR) to Rodriguez and San Mateo, Rizal.

CDRRMC Batangas City volunteered to be deployed in the flooded areas of Rizal to augment SAR contingents; sent ₱ 500,000.00 worth of relief goods to Cainta, Rizal and launched “Handog ng Batangueño” project to gather support for the affected population in the province of Rizal.

Calapan City, Mindoro Public Safety Department conducted ocular inspection; monitored the affected areas; and deployed personnel and prepositioned rescue equipment for possible SAR operations.

Region IV-B MDRRMO Abra de Ilog Occidental Mindoro placed their Operation Center on alert status and

advised pre-emptive evacuation especially to those living near the river. Region V RDRRMC Bicol sent a contingent of Team OCD5-Albay composed of WASAR teams from

the naval Forces southern Luzon (NAVFORSOL), Tactical Operations Group 5 (TOG)-PAF and the BFP5 together with the Water Sanitation (WATSAN) team equipped with Water Filtration System. The team which is headed by the Deputy Commander of NAVFORSOL as the Task Force Head proceeded to the Flood Control Information Center (FCIC), MMDRRMC for the coordinating instructions.

NCR OCD-NCR facilitated request of City of Manila for fire trucks; request of Marikina for heavy

equipment, additional trucks and manpower; request of Muntinlupa for 3,000 family packs; and the requirements of the cities of Malabon and Valenzuela for WATSAN or Water Filtration System. and conducted a briefing at the MMDA OpCen to 3 WASAR Teams from BFP-SRU 5, NAVFORSOL and TOG5 PAG and were deployed that resulted to the rescue of 278 persons and transporting to the different evacuation centers. The team distributed 68,000 liters of potable water to residents of Brgys Tunsuya, Catmon, and Kunyugan of Malabon City utilizing the Water Filtration Machine and the distribution was made using the Mobile Lorie (12,000 liters capacity).

Parañaque Rescue dispatched two (2) dump trucks for the stranded passengers in Evacom and SM in San Dionisio, Parañaque and deployed one (1) rubber boat, one (1) dump truck and rescue personnel in Brgy San Isidro to evacuate families from the creek side to the Brgy Hall.

LGU Pasay City deployed two (2) rubber boats with rescue personnel to assist rescue operations.

LGU Malabon City formed an augmentation team consisting of MDRRMO, PNP Malabon, Special Action Force, Fire Malabon, City Hall Employees, Navy, Coast Guard, MMDA, Health, CSWDD and volunteer personnel.

LGU San Juan City dispatched the rescue vehicles for flood monitoring and assisting evacuation of constituents in AOR.

QC DPOS deployed Rescue Team with rubber boat to the following areas: Brgy Damayan Lagi, Brgy Sto Domingo, Brgy Damayan District I, Roxas District and Brgy Apolonio Samson.

SOUTHWEST MONSOON RELIEF AND RECOVERY OPERATIONS 7-11 August 2012

DSWD

DSWD NCR released a total of 12,900 family packs for the request of some legislators for their affected constituents in the cities of Manila, Muntinlupa, Taguig, Quezon, Caloocan, Pasig, and Valenzuela and prepositioned 136,730 food packs in the National Resource Operations Center amounting to ₱ 42,450,000.00. and a total of 124,405 food packs were released to the affected areas amounting to ₱ 31,101,250.00.

DSWD IV-A deployed eight-man team in Cainta, Rizal to provide Psycho Social Process (PSP), implemented the Disaster Assistance Family Access Card (DAFAC) system, and monitored camp management and distributed family food packs. and deployed 10 teams in Taytay, Angono, Rodriguez, Cainta and San Mateo in Rizal; Biñan, Sta Rosa, Bay, Sta Cruz, San Pedro and Calamba City in Laguna to assess the needs of the families inside the evacuation centers through administration of DAFAC; facilitated distribution of family food packs in Rodriguez and Cainta, Rizal during Pres. Noynoy Aquino and Sec Dinky Soliman's visit; and distributed family food packs, plastic mats, blankets, hygiene kit, bottled water, mosquito nets, and used clothing to the affected families.

PNP –CO sent 70 PNP Personnel to DSWD National Resource Operations Center, Pasay City to assist in the repacking of relief items; deployed 1,114 PNP Personnel from NCRPO to different location in Metro Manila to assist in the relief distribution ; and dispatched eighty (80) personnel at DSWD warehouse Pasay City for repacking of relief goods

PCRG, LSS, ITMS and CL; deployed 10 personnel to Sto Tomas, Pampanga for the conduct of relief operation; deployed personnel and vehicles for the distribution of relief goods at Brgys Santa Monica, San Sebastian and San Jose in Hagonoy, Bulacan and Tumana, Marikina City.

PCRG distributed relief goods and deployed personnel and vehicles for the conduct of relief operations in Brgy San Isidro, Montalban, Rizal and Brgy Tunasan, Muntinlupa City.

CPNP Group proceeded to Brgy. Sumilang, Pasig City to distribute relief goods to the flood victims.

CRG conducted relief goods operations to the flood victims of Brgy. Polo, Valenzuela City. PNP Sta Maria, Bulacan conducted relief operations and distributed relief goods to Brgy

Caycio and Brgy Lalakhan, Sta Maria, Bulacan. PNP Paniqui, Tarlac provided security during the visit of His Excellency President Benigno

Simeon Aquino III. Checked the condition and situation of the evacuees and provided relief goods.

RPSB 3 conducted relief operation at Guagua, Pampanga. PNP NCR distributed food in Brgy Rivera, Maytuna and Balong Bato.

PAF airlifted 1,400 food packs to two (2) isolated barangays in Lubao, Pampanga; 56IB and 7ID successfully delivered 1,000 packs of relief goods at Malolos City, Bulacan HJTF-NCR left station onboard two (2) KM 450 trucks and three (3) M35 truck which

proceeded to CAMANAVA area to distribute relief goods.

PRC Marikina, PRC Pasig-Pateros, PRC Muntinlupa, PRC Mandaluyong and PRC Makati conducted relief operations and distributed food packs in the evacuation centers in Marikina, Pasig, Pateros, Muntinlupa, Mandaluyong and Makati PRC Mandaluyong City provided a first aid station, hot meals and relief goods. PRC Makati distributed food products to 1,201 individuals affected by flooding in Brgy

Rizal, Makati PRC Rizal conducted relief operations, distributed a total of 1,245 relief goods and

provided food packs to the evacuated population in San Mateo, Tanay and Rodriguez, Rizal.

PRC Pangasinan and Rizal provided hot meals to families temporarily sheltered at the Dagupan City Astrodome

PRC IV-A distributed food and non-food items at two (2) evacuation centers in Cainta, Rizal together with the PRC Chairman.

PRC and Rizal Provincial Social Welfare and Development Office and Provincial Health Office distributed 1,000 hygiene kits to the evacuees of Rodriguez, Rizal.

RDRRMC III Chairperson joined the President and party in their visit to the flood affected areas of Central Luzon and personally distributed the relief goods to the flood affected victims; coordinated and facilitated the provision of relief assistance to the flood affected families at Olivas Subd, San Nicolas, San Fernando City.

Provincial Government of Bulacan deployed 4 RFT truck and 56 IB truck to Obando, Bocaue, Marilao, 18SF and Sta Maria

PSWDO/PDRRMCTarlac delivered/airlifted relief goods to isolated families in Brgy. Bueno, Capas thru the air assets of the TOG3; and delivered sand and sacks to Brgy. Pob. Sur in San Clemente, Concepcion, Villa Paz & Malayep in Gerona, Camiling, Salapungan in Tarlac City, Bantog in Victoria, Aquino Bridge Mabini Tarlac City, La Paz & Moncad.

PDRRMC Pampanga distributed food and drinks to affected municipalities of Sto. Tomas, Pampanga.

PDRRMC Zambales distributed relief assistance to the affected families in Iba, Zambales and activated Local Price Coordinating Councils.

PDRRMC Nueva Ecija prepositioned 500 packs of relief goods, 2 dump trucks and 210 cavans of rice.

MDRRC San Antonio, San Felipe, Sta Cruz, Botolan and Iba Zambales provided assistance to the affected families.

Region IV-A PDRRMC Rizal dispatched additional 4 boats to Rodriguez and San Mateo, Rizal and

dispatched Dump truck to Marcos Highway in Antipolo City; coordinated with Manila Water for the provision of potable water in the activated evacuation centers.

CDRRMC Sta Rosa conducted pre-emptive evacuation to 137 families and provided family food packs.

CDRRMC Tanauan provided food packs to evacuees under their AOR.

DRRMC Bacoor City chairman Mayor Strike B. Revilla led the distribution of relief assistance to constituents who have been greatly affected by flood.

DRRMC Bacoor City conducted force evaluation in the flooded areas.

Region V OCD 5 Team Albay assisted in the repacking efforts of ABS-CBN Sagip Kapamilya

Foundation for possible relief distributions to the affected communities.

NCR OCD –NCR coordinated the request of DSWD-NCR of four (4) units of 6x6 vehicles for the

conduct of relief distribution with President Benigno Aquino III in the National Capital Region SJTF deployed two (2) military trucks at DSWD NROC. LGU Makati City distributed food packs and relief goods to individuals affected by flooding in

Brgy Palanan, Brgy Pembo and Brgy Comembo.
