The contract terms and conditions in this agreement will be firm through April 25, 2017. Enclosed, for your records, is an executed copy of your agreement with the Lake County School Board. Per contract specifications, it is your responsibility to provide the Purchasing Department with current copies of your certificate of insurance naming the School Board of Lake County, Florida, as an additional insured and to maintain the required coverages for the duration of the contract term. If you have not already done so, please forward this information as soon as possible. Failure to submit a fully completed original certificate of insurance signed by an authorized representative of the insurer providing such coverage may cause your company to be in default and subject to cancellation of the contract. BACKGROUND and RATIONALE:Lake County Schools has experienced increased growth over the past ten years, the implementation of a new ERP system and decreased funding from the State. These are a few factors that increase risk in our IT Center. In addition, there had never been a formal independent risk assessment of our IT organization and IT systems that support the School Board's business and educational needs in the School District by a professional audit group. Due to the complexity of the assessment and the anticipated dollar value a Request for Proposal (RFP) was issued. On April 27, 2015, RFP # 4080DB IT Risk Assessment was awarded and approved by the Board which included in the terms and conditions of this contract the option to renew for two (2) additional 12- months terms. This contract period is currently scheduled to expire April 26, 2016. The contract has been reviewed by the Board attorney.

The vendor below has indicated that they would renew their agreement for the first renewal term. If the renewal is approved by the Board, the expiration of the agreement would be April 26, 2017. All other terms and conditions of the original agreement would remain the same.

RECOMMENDATION:I recommend that the Board approve the execution of the First Extension to Agreement for RFP #4080DB - IT Risk Assessment as shown below.

FISCAL IMPACT:The cost of the IT Risk Assessment will be $50,000.00. The IT audits will be performed on an as needed basis at a blended rate of $155.00 per hour. Both IT Risk Assessment and any IT Audits will be paid for utilizing general operating funds.
