Ce groupe d’experts issus de ces sociétés, a établi des recommandations concernant les ratio de changement d’opioïde à partir de la morphine. L’argumentaire et les recommandations de ce travail ont été établis selon la méthodologie proposée par la HAS d’élaboration des recommandations de bonnes pratiques. Grade des recommandations Niveau de preuve scientifique fourni par la littérature A Preuve scientifique établie Niveau 1 - essais comparatifs randomisés de forte puissance ; - méta-analyse d’essais comparatifs randomisés ; - analyse de décision fondée sur des études bien menées. B Présomption scientifique Niveau 2 - essais comparatifs randomisés de faible puissance ; - études comparatives non randomisées bien menées ; - études de cohortes. C Niveau 3 - études cas-témoins.

La recherchebibliographiqueaété réaliséepar interrogationdeMedline, et basée sur lesmots clés suivants: morphine, douleur, cancer, soins palliatifs, rotation opioïde, ratiod’équiantalgie.Elle a été complétée par une recherche manuelle, à partir de la revue de la littératureeffectuéeparl’EAPC(réf23).Les études retenues sont résumées dans le tableau 1, cité en annexe. Peu d’études font

• Dans lesensmorphineorale->oxycodoneorale,unratiomorphine:oxycodonede2:1(diviserladosedemorphinepar2pourobtenirladosed’oxycodone)(AMM).
§ Danslesensoxycodoneorale->morphineorale,unratiooxycodone:morphine de 1: 1,5 (multiplier la dose d’oxycodone par 1,5 pourobtenirladosedemorphine)(gradeC).
§ Quelle que soit les doses calculées, la prudence est de mise enl’absence de données dans la littérature pour les posologiessupérieures à 400 mg/24h d’oxycodone PO et 600mg/24h demorphinePO.
• Dans le sens morphine orale -> hydromorphone orale, un ratiomorphine:hydromorphonede7.5:1(diviserladosedemorphinepar7.5pourobtenirladosed’hydromorphone)(AMM).

Unpatient traitépar120mgdemorphineLPx2, soit240mg/j:240:7,5=32mgd’hydromorphone/jsoit16mgLPx2avecdesinterdosesde morphine à libération normale de 25 à 40 mg/prise (ou desinterdosesd’oxycodoneàlibérationnormalede10à20mg/prise).Unpatienttraitépar32mgd’hydromorphone/j:32x5=160mgdemorphine LP par jour, soit 80 mg x 2 de morphine LP, avec desinterdosesdemorphineàlibérationnormalede15à25mg/prise.
- convertir la posologie de l’opioïde à stopper en EquivalentMorphine Oral (EMO).Vérifier les autresmédicaments pris par le patient susceptible depouvoir interagiravec la méthadone (inducteurs et inhibiteurs du cytochrome P450 et allongeantl’espace QT), vérifier l’espace QT à l’électrocardiogramme (QTc). Une attentionparticulièreseraportéelorsdel’introductiond’unnouveaumédicamentconnupourallongerl’espaceQToususceptibledemodifierl’équilibreionique.
