Target Population and Setting Our program is for senior
citizens who are above the age of 60 Since one of the primary
locations that seniors visit to socialize with other seniors is a
senior center, Jump for Your Silver Life will be introduced to such
senior centers in select neighborhoods of Brooklyn and Queens
Neighborhoods include: Flushing, Jackson Heights and Bayside in
Queens along with Sheepshead Bay, Dyker Heights, and Bay Ridge in
Brooklyn

Purpose Americas aging population is steadily increasing. There
are 40 million Americans who are over the age of 65. By 2050, the
population will increase to 89 million In New York City, there are
more seniors in Brooklyn and Queens than in other boroughs
Nutritional status of seniors 38% of those aged 65 and above are
overweight. Approximately, 25.9% are obese. There are more than
820,000 seniors who are physically inactive! In Brooklyn, there are
34.7% of elders aged 65 and above that are obese. In Queens, there
25.7% elders that are obese

Implications Centers for Disease Control and Prevention asserts
that obesity can increase ones risk of: 1. Coronary heart disease,
stroke, and high blood pressure, type 2 diabetes. Two out of three
older Americans have multiple chronic conditions 94.3% of those
older than 65 are prescribed for blood pressure medications 2.
Cancers, such as endometrial, breast, and colon cancer. 3. High
total cholesterol or high levels of triglycerides. 4. Liver and
gallbladder disease. 5. Sleep apnea and respiratory problems. 6.
Degeneration of cartilage and underlying bone within a joint
(osteoarthritis). 7. Reproductive health complications such as
infertility. 8. Mental health conditions 12% of those older than 65
do not receive either rarely or never receive emotional OR social
support

How does the future look? Increased demand of health care
services and medical costs This is where Jump for Your Silver Life
comes into action! We recognize that Americas seniors need
immediate attention and care because they are lacking
social/emotional support, face increasing medical costs, and are
already at risk of chronic diseases. It lends a helping hand and
the needed resources so that seniors can transform their later
years into their first act of life ( I wanted to say they are
becoming young, and energetic herewhat is a better way to end
it?)

Goals Seniors in New York are not physically active. Engaging
in regular exercise can prevent obesity, age- related diseases
(i.e. sarcopenia), experiencing falls, type 2 diabetes, and heart
diseases With that in mind: Our goal is to encourage seniors of age
60 and over to increase their physical activity levels and take
control of their nutritional status.

Objectives Increase physical activity level of seniors 65 and
over by 45% by the end of 2 years Decrease the percentage of adults
65 and over with BMI over 30.0 to 25% by the end of 2 years

Program Design In order to promote daily physical activity, our
team will hold regular group fitness classes to enforce that
behavior Fitness classes will be led by fitness instructors three
times a week. Exercises will help seniors meet their physical
activity guidelines A variety of exercise routines will used in
order to target the common health problems seniors face today Tai
Chi- improves balance and reduces their risk of falls

At times, outdoor physical activity will replace exercises
routines at senior centers to motivate seniors to use their
communitys resources to achieve their goals In our wish to increase
physical activity among seniors, we would also like to create a
supportive environment for them. That is why our fitness classes
will be for groups of individuals. Exercising in a group setting
will help provide social support that some seniors lack today. Ooh
yeah, lets party now! 1, 2, Cha Cha Cha !

Program Design (contd.) To improve seniors nutritional status:
Seniors will also have a chance to receive nutritional counseling
by a Registered Dietitian Counseling sessions will be scheduled
every two months to monitor their progress

Evaluation Mechanisms In- Process Evaluation Focus group
interviews, surveys, will be conducted to assess the effectiveness
of the program. Is the program helping to alleviate their fear of
making movement? What are their thoughts on the typical format of
our classes? Are our services being delivered to the right
audience? Is our program being marketed well? Outcome Evaluation
Can be conducted by our Registered Dietitians to evaluate our
participants improvement on nutritional status, adoption of healthy
food choices

Take- Home Message Jump for Your Silver Life seeks to provide a
holistic edge to physical activity programs directed towards older
Americans. It takes a step further by extending nutritional
counseling, coupled with opportunities for social networking, to
support not only the older populations physical health, but also
their mental health.
