(Urology) Department of Urology CSMMU Lucknow, U.P., 226003 India E-mail: [email protected]要約 膀胱子宮瘻(VUF)患者の腹腔鏡手術中の瘻管の特定は今なお難しい。従来の VUF 診断法は子宮造影 および色素の膀胱内注入であり、現在は超音波検査、CT および/または MRI がそれにとって代わりつつ ある。残念ながら、上記の検査はいずれも腹腔鏡手術中の瘻管特定の助けとならず、腹腔鏡下切除を容 易にもしない。本報では VUF の診断を助けるとともに腹腔鏡下切除を簡易化する単純な手順 (cystohysteroscopy[膀胱子宮鏡法]と命名した)について記述する。膀胱鏡を用いて瘻管を通じ子宮 腔内にガイドワイヤー/尿管カテーテルのコイルを形成させる。cystohysteroscopy により、このガイド ワイヤー/尿管カテーテルを半硬性尿管鏡で膣から引き出す。cystohysteroscopy の利点は、ガイドワイ ヤー/尿管カテーテルが瘻管を通っているため、線維化が存在しても腹腔鏡下切除の助けとなることと、 膀胱子宮瘻部の切除時の牽引/操作に使えることである。本報は、cystohysteroscopy としての半硬性尿 管鏡の使用を記述した初の文献報告である。 尿路観察における共焦点レーザー拡大内視鏡の 2.6 mm プローブと 1.4 mm プロ ーブの比較 1

緒言:ミクロンレベルの分解能でのダイナミックな in vivo 尿路観察が可能な技術としてプローブ式の共
焦点レーザー拡大内視鏡(pCLE)が台頭しつつある。pCLE における 2.6 mm プローブと、軟性内視鏡
に対応する 1.4 mm プローブとの比較分析を行った。
対象および方法:膀胱腫瘍の切除が予定された 67 例を登録した。2.6 mm プローブおよび 1.4 mm プロ
ーブを用いて pCLE 観察を実施した。各プローブの画質を調べ、標準的な組織病理検査と比較した。
結果:2.6 mm プローブの画像が、細胞形態の分解能に優れていた。1.4 mm プローブは視野が広く、微
細構造の描出に優れていた。2.6 mm プローブの画質が優れていたが、1.4 mm プローブは軟性膀胱鏡に
対応し、操作性が保たれ、大きなプローブで従来アクセス困難であった膀胱の領域の観察が可能であっ
た。
結論:尿路の組織病変の鑑別には 2.6 mm プローブの光学的仕様の方が適している。上部および下部尿

結果:患者背景および結石の大きさに関して 2 群間に差はなかった。平均推定失血量(166.4±98.3 mL vs 178±102.4 mL)、平均入院日数(4.5±1.9 日 vs 4.4±1.4 日)、平均術後鎮痛時間(2.2±0.9 日 vs 2.4±0.9 日)、術後輸血率(5.5%vs 6%)、および結石消失率(100%vs 96%)に関して LRP と
PCNL の間に統計的有意差はなかった。平均手術時間は LRP 群で有意に長かった(130.6±38.7 分 vs 108.5±18.7 分)。腹腔鏡群の 1 例(1.8%)のみ制御できない出血のため開放手術に移行した。
結論:RLP は、適切な症例選択を行えば大型の腎盂結石患者に適した術式の一つである。しかしながら、
ほとんどの症例では PCNL がやはり標準治療である。

目的:血管クランプを行わずに腹腔鏡下腎部分切除術(LPN)中の止血を達成するための新しいデバイ
スの可能性を検討すること。
材料および方法:腎門部クランプを行うことなく凝固と切開のタスクを同時に実行するハンドヘルド機
器から成る新しい高周波(RF)使用デバイスと、腎門部クランプを行う標準的手技とを比較する実験研
究を行った。ブタモデルを使用し(各群 10 頭)、17 日後に屠殺した。
結果:新しいデバイスの推定失血量は標準的手技よりも有意に少なかった(15.5±23.7 vs 79.4±76.3 mL)。切除時間は新デバイスの方が長かったが(10.7±13.7 vs 2.1±1.2 分)、総手術時間は有意に短
かった(35.3±13.7 vs 60.2±10.5 分)。局所尿逸流の所見(尿瘤)は両群で等しかった(各 5 例)。し
