Internal Mishap Reporting: The unit or activity Commander will immediately notify the Wing Commander, Wing Safety Officer, and State CAP-USAF Director of any accident, incident, or mishap involving bodily injury or property damage to aircraft or vehicles in his or her unit.
If the Wing Commander is unavailable, the unit Commander shall notify the following in order of precedence: Vice Commander, Chief of Staff, Director of Operations, Wing Secretary, or Legal Officer. The Wing Safety Officer and the CAP-USAF State Director shall be notified in all cases.
A list of reportable accidents, incidents, and mishaps is contained in CAPR 62-2 Attachments 1, 2 and 3.
4b. Unit or activity Commanders are responsible for submission of an online CAP Form 78 within 48 hours of a mishap.
