CHAPTER SERVICE PROJECT [9TH Grade] “Worth 500 Points”
Portfolio is a purposeful collection of student work that exhibits the student’s efforts, progress, and achievements in one or more areas of the curriculum. The collection must include the following:
Letter of Introduction – This is a letter in narrative format that introduces the Healthcare Science/Pre-Med/HOSA member to the instructor and members. It must be dated and describe career goals and other relevant information – 100 POINTS
Career Cruising -- Students will use the Career Cruising to complete a career entrance survey to determine which careers they are most suited for. Print two[2] copies, one to turn into me and one for your portfolio –

Resume – Using the Career Cruising website (www.careercruising.com), create a one-page resume to include educational and work related experiences, honors, awards, leadership activities, etc. – 100 POINTS
Writing Sample – (Essay) “Introduction of self and Why Medicine? A narrative on who the student is and what made student choose medicine as a career. The sample must be neatly typed and free of spelling and grammatical errors – [2 pages] 200 POINTS
Community Service (25 hours) – Must show dates, times, location, and learning outcomes. May not be part of any other activity related to school or during school hours. – 1250 POINTS
Description: The Healthcare Science/Pre-Med/HOSA member will create a portfolio of evidence of his/her accomplishments in preparation for a health career.
Purpose: To demonstrate the skills, abilities, and attitudes necessary to pursue a career in health care.
Application: In addition to earning recognition through Healthcare Science/Pre-Med/HOSA. The Healthcare Science/Pre-Med/HOSA member is encouraged to keep and use the portfolio as evidence of pre-professional growth and accomplishments when participating in interviews for scholarships, college admission, and jobs.
Portfolio Requirements: The Portfolio must be in a 3 ring binder (orange, hunter green or white in
color) Must have divider for each grade level and section Pages are to be one-sided The letter of introduction and resume must be typed in 12 font double
spaced (2 pages) All material must be completed by the date announced. No late work will be
accepted at all and you will not be allowed to stay in the program.

Letter of Introduction
Hello, my name is Carla Johnson and I am a freshman at John Hopkins Magnet
High School in Mobile, Alabama. I also attended Lee Mae Elementary and Collins
Middle school. I am currently enrolled in the Healthcare Science/Pre-Med Program
with plans to one day become a doctor. I am a well-organized, highly motivated
individual who enjoys challenges in a work environment. I am an active participant
in my area church as well as other local organizations. I have been an active member
in the youth bible study class and the youth group of the Mt. Vision Community
Church for six years. I have participated in two youth mission trips to work on the
homes of low-income residents. The mission trips require a ten-month commitment
of fundraising, as well as the actual ten days of work. I am a Junior Firefighter for
Mobile, a member of the Girl Scouts, and a volunteer for various school and
community functions. After high school, I plan to earn a degree in Cardiac Medicine
and enhance my skills by starting a practice in a small rural town.

RESUME GUIDELINES FOR PORTFOLIO GENERAL TIPS
Order – Prioritize information in order of importance, with the most important text up high and to the left where it is most likely to be read
Header – Include your name and your complete contact information. Be
certain your e-mail address sounds professional (e.g., not “[email protected]”)
Font – Use a standard font (e.g., Times New Roman, Arial, Garamond) Text
size should be 12 points; Your name and section headings can be a bit larger; they should also be in BOLD, and ALL CAPITALS
Format – Maintain uniformity
Paper – Use plain paper (white, light cream, or very pale gray). Do not use
paper with visible flecks in it.
Abbreviations – Minimize use of abbreviations. However, “GPA,” “B.S,” “BA,” and abbreviations of other well-known degrees are okay
Length – Limit your resume to one page; make sure it is visually attractive
and easy to read.

COMMUNITY SERVICE GUIDELINES
Community service is volunteer service within the school, church or community.
Hours may be accumulated at various acceptable places. You MUST send me an
service activity to get my approval AND include that email along with
your community service form in portfolio. FAILURE TO DO SO WILL
RESULT IN DENIED COMMUNITY SERVICE HOURS for that activity.
Any project performed for community service hours must meet the definition and guidelines as stated by the instructor. A student should complete the minimum of 25 hours of service for 9th grader by the date assigned 25 hours of service for 10th grader by the date assigned 50 hours of service for 11th graders by the date assigned 50 hours of service for 12th graders by the date assigned
ALL community service MUST be pre-approved by the site supervisor before starting the service
Community service can be performed after school, evenings, weekends and during spring/summer/holiday breaks
The community service must be completed, and documented before the portfolio is submitted by the student
The student CANNOT profit monetarily or receive credit in another way for the service.
Written verification of the completed hours is necessary for completion of the program. The service MUST benefit others
