    MAIN PARTS OF AN EMPLOYMENT APPLICATION

    A. PERSONAL INFORMATION

    This includes your name, current and previous addresses, age, social security number, available

    hours, emergency contact information, and other questions about you as a person.

    B. EDUCATIONAL INFORMATION

    This should include the name of your college and/or trade schools. Include names and addressesof schools, the diplomas or GEDs, and degrees you earned. Be sure to include the dates you

    attended each school.

    C. WORK HISTORY

    This requires you to state past jobs and responsibilities. Include names, addresses and the dates

    of each employer. Also include a description of the duties you were responsible for at each job,

    the salary you earned, the name of your supervisor and the reason for leaving. You may also be

    asked to include a list of special skills (i.e. computer skills, machines you have operated) and or

    information that you think qualifies you for the job. Include military experience (if any) and

    volunteer work.

    D. REFERENCES

    You may be asked to provide the names and addresses of two to three people who could provide a

    character reference for you. Be sure to include their names, job titles, company names, addresses

    and telephone numbers. Make sure that you notify your references before they are contacted by

    potential employers.

    TIPS FOR COMPLETING EMPLOYMENT APPLICATIONS

    Have all of your information prepared ahead of time and carry it with you so that you know the

    answers to all questions that apply. Be accurate and neat.Always apply for a specific job title! In the space Position applying for, never write anything.

    There is no job called anything, and you will probably get nothing. Be definite.

    Use blue or black ink only. Carry a pencil, paper clips and a pen with an ink eraser or some

    correction fluid. Neatness counts.

    Use Standard English. Do not use slang.

    Sometimes you are asked to complete your work history or education history in chronological

    order. Reverse chronological order means that you start with your current job and go backwards

    to your very first job. Be sure to mention special certificates, licenses, professional organizations, and other

    business-related honors, and achievements that could give you an advantage over other

    Manual assembly of Samplers and Displays and all associated hand work involving stickering, packaging,

    caging, or any re-configuration of product.

    1. Manual assembly of products and displays.

    2. Operate, troubleshoot and maintain numerous pieces of relatively low technology equipment.

    3. Inspect finished product and ensure compliance with all quality standards.

    4. Maintain the highest GMP and safety standards.

    5. Track and keep records of all daily production.

    6. Able and willing to back up Machine Operator positions as needed.

    QUALIFICATIONS

    1. High school diploma or equivalent required.

    2. One year previous assembly line or manufacturing experience required

    3. Forklift experience preferred.

    4. Good eye/hand coordination and strong manual dexterity is required.

    5. Solid communication skills, both oral and written.

    6. Good self-starter and ability to work with minimum supervision.

    7. Ability to communicate (written and verbal) in English.

    JOB SPECIFIC FUNCTIONS:

    Essential Job Functions, Duties, Responsibilities and Tasks1. Assists with daily operations of recreation programs, equipment and facilities2. Assists with facility schedules and rental requests, supervision of temporary staff, participants

    and volunteers

    3. Responsible for recreation facility operations while on duty, and responsible for maintainingdiscipline in recreation areas

    4. Assists in maintaining facilities in a clean, neat and safe manner5. Performs public relations and may be assigned to work the front counter or other control areas6. Responsible for inventory, maintenance, and cleaning of program equipment/supplies/facility at

    various locations

    Other Duties and Responsibilities1. Assists with special events in other program areas

    2. May cover the duties of the Facility Assistants position as neededIncumbent is accountable for all duties of this job and other projects and responsibilities may beadded at the Citys discretion.

    POSITION REQUIREMENTS/WORKING CONDITIONS/PRE-EMPLOYMENTREQUIREMENTS:

    Education, Experience, Skills, Formal Training, Licenses and CertificationsRequired:

    A minimum of 18 years of age

    High school diploma or G.E.D.

    Some experience in teaching and supervising in a recreation and/or educational setting

    At least one year experience, additional training and/or education in leisure services

    Must possess or be able to obtain a CPR and Standard First Aid Certificate within ninety (90)

    days of employmentPreferred: Participation in adult sports, especially softball, soccer, volleyball or basketball

    Any equivalent combination of education, training and experience, which would provide the

    required knowledge and skills, may be considered.Physical RequirementsWork is physical in nature and involves:

    Constant near acuity and hearing is needed to perform daily functions and communicate withpatrons and staff

    Constant standing, walking on uneven terrain, and climbing stairs in a multi-level facility to
