We currently have an exciting opportunity for a Worksite Nurse Health Coach to work in the Grinnell Regional Medical Center in Grinnell, Iowa. The Worksite Health Coach is responsible for providing lifestyle behavior change coaching, education, self-management support, and care integration for Grinnell employees and spouses (participants), the primary focus is on individuals with chronic diseases minimizing the risk for preventable complications and acute care events. The coaching approach employed is based on the principles of behavioral change and motivational interviewing. This coach teams with GRMC providers to integrate and coordinate expertise and support of the other health professionals, family members, HealthPartners health advocates and other resources available through the employer and community.
LOCATION:Grinnell Regional Medical Center in Grinnell, IA
ACCOUNTABILITIES: 1. Communicates and collaborates on patient care plan with primary care and specialty providers in support of optimal patient outcomes.2. Build and maintain effective relationships with Grinnell employees and spouses (participants) with focus on lifestyle behavior change coaching and on health behavior issues based on participant inquiries or physician referral and/or with chronic diseases as identified through the disease registry. Core areas of focus include physical activity, nutrition, tobacco cessation, stress management, weight management, and other health-related areas of preventive coaching along with collaborating with other clinicians to support treatment of chronic diseases and health improvement of people who have chronic diseases.3. Provides coaching in individual and group settings as appropriate, utilizing multiple communication channels, including face-to-face, telephone and secure e-mail.4. Collaboratively develop care goals with the participant and his/her clinician team and schedule coaching sessions to support the participant in attaining personal goals and self-care efforts, focusing on the circumstances, needs and preferences of the individual.5. Integrates patient’s individual clinical, functional and health behavior information to develop action-oriented and times-specific planning and implementation of appropriate interventions.6. Utilize personalized health coaching techniques that combine motivational interviewing, intrinsic coaching and shared decision making to support participants in achieving their health and lifestyle goals.7. Assist participant in selecting health improvement programs including all programs offered by Grinnell and health-related community programs and events.8. Utilizes accepted guidelines and standards of care for management of chronic diseases.9. Effectively and routinely communicates with employees to provide them with a better understanding of their health and to facilitate successful health outcomes. 10. Facilitates successful collaboration resulting in high employee and care provider satisfaction levels.11. Maintains current and accurate documentation of all disease management and coaching activities and assists in tracking database outcomes.12. Maintains confidentiality of information in accordance with department and corporate policies.13. Act as a health education resource and facilitate referrals as appropriate to onsite medical resources, onsite chaplains, HealthPartners health advocates, EAP, and community resources.14. Assist in the development, promotion, and delivery of programs, products, and services as requested.15. Coordinate, implement, and evaluate worksite wellness activities, including onsite events, that are focused on changing the culture. 16. Conduct employee seminars/presentations on a variety of health topics. 17. Perform other duties as assigned.
REQUIRED QUALIFICATIONS: Registered Nurse with current licensure in the State of Iowa, or equivalent; BSN preferred.3 years of clinical proactive experience with current clinical knowledge as it relates to chronic disease management.Health behavior change coaching experience.Experience in effectively collaborating and problem solving with patients and health care team members.Strong written and verbal communication skills to communicate effectively with individuals at all levels of the organization.Demonstrate working knowledge of quality improvement and understanding of health care.Experience in use and management of automated medical management systems.Ability to engage patients with various dispositions onto the programs.Knowledge of data, data analysis, measurement systems, evaluation methods, preferably study designs.
PREFERRED QUALIFICATIONS:Master’s degree in health education, exercise physiology, public health, or other health-related field.Previous experience in one-on-one lifestyle/behavior change coaching and facilitating group classes in a worksite setting.Proven ability to motivate individuals to make lifestyle/behavior changes.Knowledge and experience in health risk assessments, lifestyle risk reduction, disease management, case management, and health promotion is recommended.Individuals who have had work experience(s) requiring autonomy and independent decision making, budget responsibility, management skills, and business acumen are preferred.Ability to quickly understand and adapt to the culture and leadership of GRMC.Ability to build relationships based on trust, an understanding of the local health care system, and strong collaboration skills are pre-requisites to success.

Key Facts About Selective Service Registration
(NAPSA)-Young men in the United States often find turning 18 can bring a number of opportunities. It also brings an important obligation-registering with Selective Service.
Registration is man-dated by the federal gov-ernment and is required in order to be eligible for college loans, federal jobs and more.
To help make the pro-cess more understand-able, here are some key questions and answers.
Q. Who must register with the Selective Service and when must they reg-ister?
A. All young men, in-cluding immigrants, re-gardless of their status, are required to register within 30 days of their 18th birthday. Immigrants who arrive after their 18th birthday must register within 30 days of their ar-rival if younger than 26.
Q. In addition to being in compliance with the law, are there benefits to registering?
A. Proof of registration is required when applying for federal college loans and grants, including Federal Pell Grants, Fed-eral Supplemental Educa-tional Opportunity Grants (FSEOG), Direct Staf-ford Loans/PLUS Loans, National Direct Student Loans, and College Work-Study.
Registration is also re-quired for federal jobs, and the U.S. Citizenship and Immigration Services (USCIS) makes registra-tion with Selective Ser-vice a condition for U.S. citizenship if the man first
arrived in the U.S. before his 26th birthday.
It is also necessary for jobs with many state and municipal governments as well as government contractors. Some states even require it when ap-plying for a driver’s li-cense.

A. Yes. A man who fails to register may, if prose-cuted and convicted, face a fine of up to $250,000 and/or a prison term of up to five years.

It is also possible to register when applying for Federal Student Fi-nancial Aid (FAFSA form). Simply check “Register Me” on Box #22 of the form. In addition, some high schools have a staff member or teacher ap-pointed as a Selective Service Registrar who can assist with the registration process.
