2011 Vol. 31, No 6 Diagnostic Ultrasound: Past, Present, and Future K. Kirk Shung Ultrasound has been used as a diagnostic tool for more than 40 years. Many medical applications have adopted ultrasound, mostly notably in obstetrics and cardiology. Started by a few scientists and clinicians in different parts of the world in the early 1950s, it did not become an established diagnostic tool until the early 1970s when grayscale ultrasonography was introduced. Modern ultrasound scanners are capable of producing images of anatomical structures in great detail in grayscale and of blood flow in color in real-time. State-of-the-art four-dimensional scanners that yield three-dimensional volumetric images in real-time are pushing the present technical capability to its limit. Ultrasound is currently the second-most used clinical imaging modality after conventional X-ray radiography. Although ultrasound is considered to be a mature technology, technical advances are constantly being made. The most significant achievements in ultrasound recently have been the developments of approaches capable of the quantitative measurement of tissue elastic properties, namely ultrasound elastography and radiation-force imaging, high-frequency imaging yielding improved spatial resolution, and therapeutic applications in drug delivery and high-intensity focused ultrasound surgery. The miniaturization of scanners has become a trend. In this paper, the history and current state of medical ultrasound are reviewed and future developments are discussed.

Efficient Segmentation Algorithm for 3D Bone Models Construction on Medical Images Chung-Yi Huang, Lai-Jun Luo, Pei-Yuan Lee, Jiing-Yih Lai, Wen-Teng Wang, Shang-Chih Lin Three-dimensional (3D) surface construction from computed tomography (CT) slices has received extensive attention as many biomechanical analyses and designs rely on the development of a reliable 3D model. This study proposes a method for constructing the triangular models of anatomic structures from two-dimensional slices of CT images. A region growing algorithm is developed for the segmentation of the volume data. Each voxel on the volume data represents a pixel of 3D images. A modified marching cubes algorithm, a 3D reconstruction algorithm, is then employed to establish the triangular model. Problems caused by imprecise image data are mitigated. In addition, a data reduction algorithm that combines a pair of voxels along each coordinate direction is developed, in which piecewise linear interpolation is implemented to maintain the accuracy of the reduced model. Several examples are presented to demonstrate the feasibility of the proposed method.

Quantitative Thermal Imaging for Plasmonic Photothermal Therapy Shiou-Han Wang, Chen-Wei Wei, Shiou-Hwa Jee, Pai-Chi Li Continuous-wave (CW) laser-induced thermotherapy (LIT) can heat tumors to above 42ºC and lead to tumor destruction. During LIT, antibody-conjugated gold nanorods (AuNRs) can bind to tumor tissue and improve therapeutic efficacy when implementing plasmonic photothermal therapy (PPTT). This study uses a noninvasive photoacoustic (PA ) technique to monitor temperature during PPTT. A pulsed laser is used to generate the PA signal. AuNRs are used to further increase the PA-signal contrast during PA imaging. Ultrasound (US) is combined with PA imaging for anatomical imaging. The bimodal system of US and PA imaging is superior to conventional systems at locating tumors and recognizing nanoparticles, and facilitates the selection of an optimal treatment plan. This technique allows temperature to be monitored in real-time during therapy, thereby ensuring both safety and efficacy. The technique is based on the fact that the Grüneisen parameter (which determines the acoustic pressure amplitude) is linearly related to the temperature of soft tissue. The results of a phantom study show that the temperature and the thermal mapping of the laser-irradiated area can be exactly measured and drawn quantitatively. Under the ANSI Z136.1 safety guidelines, inducing thermotherapy without AuNRs is difficult with low laser power density. However, by using specific antibody-conjugated AuNRs, targeted thermotherapy can be implemented selectively. An in vivo study is performed to confirm the feasibility of the proposed technique. A quantitative thermal image of the laser-irradiated tumor shows that the temperature is above 45ºC, indicating the induction of thermal therapy. The tumor after thermotherapy is examined histologically. The tumor architecture is destroyed whereas the peripheral and intratumor noncarcinoma cells are preserved despite also being irradiated by the CW laser. This indicates that antibody-conjugated AuNRs can facilitate targeted thermotherapy to selectively kill tumor cells. Both phantom and in vivo imaging results demonstrate that real-time quantitative thermal imaging is feasible for AuNR-assisted PPTT.

Statistical Techniques for Detection of Optic Disc and Macula and Parameters Measurement in Retinal Fundus Images Cemal Köse, Cevat İkibaş Morphological structures such as the optic disc and macula have been extensively utilized in automatic diagnosis and screening systems for diseases such as diabetic retinopathy (DR), age-related macular degeneration (ARMD), and glaucoma. This paper proposes simple statistical techniques for the detection of the optic disc and macula for calculating the diameter of the optic disc and the distance between the optic disc and macula. The results can be used in automatic diagnosis or monitoring systems developed for retinal diseases. A tool is provided for verifying the location of the optic disc and macula to increase reliability. This study also uses the weighted-distance method to extend the healthy parts of a retinal image. About four hundred retinal fundus images of different qualities are used for testing the system. A detection rate of 97% of the optic disc and macula, an accuracy of 95% of the diameter of the optic disc, and an accuracy of 97% of the positions of the optic disc and macula are obtained.

Detection of the Optic Disc on Retinal Fluorescein Angiograms Shangping Liu, Ji Chen A fast and efficient approach for localizing and segmenting the optic disc in fluorescein retinal images by means of mathematical morphology and the gradient vector flow (GVF) snake model is proposed. The localization algorithm utilizes the similarity in gray intensity between blood vessels and the optic disc. In the approach, the retinal images are first enhanced using two top-hat operators, where the optic disc is localized using Otsu’s threshold and image subtraction. After localization, pixel-level preprocessing is performed to remove blood vessels, where the optic disc boundary is detected using the GVF snake model. The proposed method is evaluated using a database of 60 fluorescein images. The accuracy rate of disc localization is 96.7%. The boundary detection method has average sensitivity values of 95.1% for images with a defined optic disc and 90.4% for images without a defined optic disc.

A Modular Framework for Shape- and Image-prior-based Anatomical Structure Contour Extraction Ayse Betul Oktay, Yusuf Sinan Akgul The automatic extraction of anatomical structure contours from medical images is a challenging task in the presence of missing or unrelated parts, occlusions caused by other structures, and image noise. Employing prior information about the anatomical structures has been one of the most popular ways of addressing these challenges. This paper presents a novel framework that incorporates both shape and image priors into the contour extraction process with deformable contours. The framework handles the deformable contour evolution and the prior information integration separately by stopping the evolution of the deformable model and regularly re-initializing the expert-delineated contour with the most similar image and shape properties. The method can be used with any deformable model without complicating the deformable model functional. An explicit training phase is not required for the construction of the prior model. Moreover, it can be applied to any medical shape contour extraction task with simple modifications. The system is tested on echocardiographic images and cardiac magnetic resonance imaging (MRI) slices for left ventricle border extraction under the level set framework. The visual and numerical results show the effectiveness of the proposed method.

Automated Fuzzy Segmentation Approach for Vessels in Computed Tomography Images Gang Yu, Pan Lin, Junfeng Gao, Can Liu, Xuanqin Mou This paper presents an automated approach for extracting vessels from computed tomography (CT) images that is based on the multiscale analysis of the vessel structure in the image. According to the scale responses to local line-like structures, the feature vessel image is built to reduce noise and enhance vessel structures. The multiscale linkage model is used to build a parent-child relationship between feature images with adjacent scales. Fuzzy distance measures are developed to describe the fuzzy similarity of vessel structures in multiscale images. The fuzzy similarity is embedded into conventional fuzzy clustering framework to build new multiscale vessel segmentation algorithm. The proposed multiscale similarity linking fuzzy C-means (MSLFCM) algorithm optimizes segmentation on all scales of the feature images. The inter- and intra-scale constraints are combined to automatically extract vessels. The segmentation experiments are performed on synthetic and CT pulmonary vessel images. The experiment results demonstrate the satisfactory performance of the proposed approach.

A Study on Evaluation of kV-CBCT-image-based Treatment Planning using Anthropomorphic Phantom Padmanaban Sriram, Nagarajan Vivekanandan, Sukumar Prabakar Kilovoltage cone-beam computed tomography (kV-CBCT) based on flat-panel technology is primarily used for patient positioning. It can also be used for re-planning and dosimetric verification in adaptive radiotherapy by validating the accuracy of CBCT-image-based treatment planning. This study evaluates the accuracy of dose calculation based on CBCT images using an anthropomorphic phantom. The Hounsfield unit (HU) – electron density calibration curves for full-fan and half-fan modes of CBCT are obtained using a Catphan® 600 phantom and compared with the conventional CT curve. The stability of the CBCT calibration curve with time is also studied over a period of 8 weeks. CT and half-fan CBCT imaging of the anthropomorphic phantom was obtained. The isodose distributions of a single direct field and wedge fields are compared for CT- and CBCT-image-based dose calculations. Simulated identical targets and organs at risk are delineated in CT and CBCT images for which conformal radiation therapy (CRT) and intensity-modulated radiation therapy (IMRT) plans were generated. The percentage dose differences and dose-volume histograms (DVHs) of CT- and CBCT-based plans are compared. It is found that the maximum difference in HU between half-fan CBCT and CT is 40 HU. No significant change is observed in the calibration curves over a period of 8 weeks. The isodose distributions computed based on CBCT and CT for a single direct field and wedge fields agrees to within 1%. For head and neck and pelvis IMRT plans, the dose calculated using CT agrees with that of CBCT-based calculation to within ± 1.0%, respectively. However, for the thorax, there is a pronounced discrepancy at the 100% isodose line and the dose difference is within ± 3%. It can be thus concluded that CBCT images can be used for dose calculation, but it is necessary to validate the dosimetric data for inhomogeneous tissue regions.

Fast CT Image Processing using Parallelized Non-local Means Hao Wu, Wenhua Zhang, Dazhi Gao, Xindao Yin, Yang Chen, Weidong Wang Reducing the radiation dose delivered to patients has been an important concern since the introduction of X-ray computed tomography (CT). However, low-dose CT images tend to be severely degraded by noise. This paper proposes using parallelized non-local means (PNM) under a computation framework for improving low-dose X-ray CT images. For the proposed PNM method, the pixel intensities are processed based on the self-similarity properties of tissues with various levels of attenuation across large-scale neighborhoods. In the experiment, CT images from a Siemens CT scanner with 16 detector rows are collected for various dose levels. Results on both phantom and clinical CT images from various human parts validate the performance of the proposed accelerated parallel approach in terms of noise and artifact suppression and feature preservation.

Non-intrusive and Non-contact Frequency-domain Photothermal Radiometry of Carie Yasser H. El-Sharkawy, Mahmoud El-Sayed Gadallah The effects of pulsed photothermal radiometry (PPTR) using a thermal camera on the optical and thermal properties of biological materials are investigated. Changing the pulse repetition frequency of the laser pulses affects the penetration depth of the laser, which can provide three-dimensional (3D) images in arbitrary planes and allow imaging deep within a solid tissue. A 3D heat conduction formulation that uses 3D optical diffusion is developed to derive a turbid frequency-domain PPTR (FDPPTR) model. The proposed model may be useful for non-contact, non-invasive, in situ evaluation for the depth profilometric imaging capabilities. The model is applied for monitoring carious and artificial subsurface lesions in human teeth.
