● Still no Therapy Cap extension process - What does that mean?

○ No therapy services provided above the hard cap of $2010 for PT & SLP, and $2010 for OT will be reimbursed

○ There is no option to use the KX modifier at this time

○ The therapy cap is not currently being applied to hospital outpatient facilities

○ Services provided at Critical Access Hospitals are included in the Therapy Cap

● Still no Therapy Cap extension process - What is next?

○ According to the APTA the first opportunity to address the hard cap will come when Congress takes up a spending bill that must pass in order to keep the government open after January 19.

○ However current news outlets are reporting that there may not be a spending bill passed this week due to disagreements over immigration and DACA, so that means no extension to the Therapy Cap.

○ We have to wait an see.○ In the interim, run Medicare cap reports regularly.○ Make sure patients above the cap receive and sign an ABN,

if treatment continues, in case a bill is passed but is not retroactive.

● In 2017 a lot of time was spent enhancing the therapist caseload

management tab, to improve the ability of therapists to be

proactive in managing visits and discharges.

Patient Safety Alerts

Creating Alerts from Allergies and Contraindications


● TVIST has a section to document allergies, and also patient specific

contraindications and precautions.

● Precautions and contraindications have always been case specific, and

allergies are only visible if the PMH is accessed.

● This raises a patient safety concern, as significant issues may be overlooked,

or not even visible to other providers.

Creating Alerts from Contraindications


● A new column has been added to the Contraindications/precautions section on

TVIST to allow a provider to create an alert for that item

● When this is selected an alert with the subtype of “Contraindications” will be

created when the note is signed off.



● By default this alert type is set to

show only when a note is

opened, but this can be adjusted

in the alert setup



● The alert will be created, and

now will show for all providers

when they open a visit record.



● If the alert needs to

be more specific,

you can choose to

edit, and add

additional filters

such as the case

that it needs to

display for



● Creating alerts from contraindications is currently available in the Early

Adopter service pack

● Next step: Create an option to create an alert for allergies documented

in the PMH.
