before heart screen #1

• Come on time for heart screen appt in fasting state (no breakfast! we feed you)
• Bring completed forms with you• Arrange any remaining amount due• Complete each heart screen station• Eat a heart-healthy breakfast• Receive tote bag with CHIP materials• Attend first CHIP session on Monday
