How Did We Come to CHIP?

• Met Dr. Diehl in Loma Linda, CA• Helped with some CHIP programs• Led a CHIP program conducted by

medical students for medical training

• Conducted local CHIP programs, including one as a study at Carilion

• Helped launch numerous CHIP programs in Roanoke area

folks to travel across country or world• Costs thousands ($5,000 - $20,000)• Requires leaving home environment

and being off work• Return to same environment after

program with no support• Difficult to continue the lifestyle

CHIP Registration Process

1. You decide to take the program (or not)2. Complete the registration forms3. Pay registration fees (prepay or deposit)4. If financial assistance is needed,

complete a scholarship application form5. Select heart screen #1 time slot6. Receive forms to be completed at home

before heart screen #1

CHIP Heart Screen Process

• Come on time for heart screen appt in fasting state (no breakfast! we feed you)

• Bring completed forms with you• Arrange any remaining amount due• Complete each heart screen station• Eat a heart-healthy breakfast• Receive tote bag with CHIP materials• Attend first CHIP session on Monday
