PEPERIKSAAN UNIVERSITI SAINS MALAYSIA
ARAHAN-ARAHAN PEPERIKSAAN UNTUK CALON-CALON
1. Calon-calon mesti mengambil perhatian yang teliti tentang
jadual waktu peperiksaan dari segi tarikh, masa, tempat dan susunan
tempat duduk untuk kertas-kertas peperiksaan masing-masing.
Calon-calon dinasihatkan supaya sentiasa melihat papan-papan
kenyataan jika terdapat sebarang pindaan terhadap jadual waktu
peperiksaan. Pindaan seperti itu, jika ada, akan dicetak di atas
kertas biru dan dipaparkan di semua papan kenyataan di Universiti.
Kesilapan membaca jadual waktu tidak diterima sebagai alasan untuk
gagal menghadiri peperiksaan. (Calon akan diberi Gred F*).
2. Calon-calon mestilah tiba tepat pada waktu peperiksaan dan
membawa Kad Pengenalan, Kad
Pendaftaran dan Slip Peperiksaan masing-masing. Calon-calon yang
tidak mempunyai Slip Peperiksaan tidak dibenarkan masuk sehingga
kelayakannya untuk mengambil peperiksaan itu dibuktikan.
Calon-calon yang tidak mempunyai Kad Pengenalan akan dikehendaki
menyakinkan Ketua Pengawas/ Pengawas yang bertugas di dewan
peperiksaan berkenaan dengan pengenalan diri mereka. Calon tidak
akan dibenar mengambil peperiksaan bagi kursus yang tidak tercatat
di dalam slip peperiksaan. Slip Peperiksaan adalah dokumen rasmi
Universiti dan calon tidak dibenarkan menulis pada mana-mana
bahagian di atas slip peperiksaan.
3. Calon-calon tidak dibenarkan masuk ke dewan peperiksaan
sehingga Pengawas yang bertugas memberi
isyarat untuk berbuat demikian. Calon dibenarkan masuk sepuluh
minit (10 minit) sebelum peperiksaan bermula. Setelah dibenarkan
calon hendaklah mengambil tempat duduk dengan senyap dan teratur.
Calon yang hadir lewat lebih dari setengah jam (30 minit) tidak
dibenarkan masuk ke dewan peperiksaan.
4. Selepas berada di tempat masing-masing calon-calon
dikehendaki:
[a] mengisi Borang Kehadiran dan meletakkannya di atas meja di
sudut sebelah kanan;
[b] meletak Kad Pengenalan dan slip peperiksaan di atas Borang
Kehadiran yang telah dipenuhi. Borang Kehadiran akan dipungut oleh
Pengawas/Pembantu Pemeriksa. Calon-calon diingatkan supaya
menyimpan Kad Pengenalan dan meletakkan slip peperiksaan di sudut
kanan bawah meja selepas diperiksa oleh Pengawas/Pembantu
Pengawas.
5. Di atas kulit skrip jawapan, Angka Giliran hendaklah ditulis
dengan menggunakan ejaan dan juga angka
seperti `Tujuh Kosong Kosong Kosong Satu' (70001). Calon-calon
mesti menulis Angka Giliran dengan angka sahaja di atas semua
kertas jawapan yang digunakan. Calon-calon dilarang menulis NAMA
mereka di atas buku/kertas jawapan.
6. Calon-calon yang tidak menulis Angka Giliran atau tersilap
menulis Angka Giliran di skrip
jawapan/borang OMR, skrip jawapan berkenaan tidak akan diperiksa
oleh Pemeriksa dan calon-calon tidak akan menerima sebarang
markah.
7. Calon-calon diingatkan supaya meneliti dan mematuhi segala
arahan yang tercatat di atas kulit skrip
jawapan. Catatan kasar mestilah dibuat di atas kertas jawapan
sahaja. Skrip jawapan Tambahan akan dibekalkan kepada calon-calon
jika diminta. Semua kertas jawapan sama ada digunakan atau belum
mestilah ditinggalkan di dewan peperiksaan. Calon-calon tidak
dibenarkan membawa keluar skrip jawapan dan/atau kertas jawapan
dari dewan peperiksaan.
8. Buku, kertas dokumen, nota atau gambar, kecuali bahan-bahan
yang dibenarkan oleh pemeriksa tidak
boleh dibawa masuk atau dibawa keluar oleh calon dari tempat
peperiksaan. Semasa berada di tempat peperiksaan, calon boleh
menerima daripada Pengawas buku-buku, kertas-kertas,
dokumen-dokumen, gambar-gambar atau sebarang bahan yang telah
dibenarkan oleh Dekan/Pengarah di atas nasihat Pemeriksa atau
Majlis Pemeriksa.
9. Calon-calon tidak dibenarkan merokok di dalam dewan
peperiksaan semasa peperiksaan.

10. Calon-calon tidak dibenarkan membawa masuk ke dalam dewan
peperiksaan kamus elektronik, sebarang jenis beg, dompet, kotak
pensel, kotak geometri atau bekas lain. Mesin kira boleh dibawa
masuk tanpa sarungnya jika dibenarkan oleh pemeriksa. Sila ambil
perhatian, penggunaan Kamus Elektronik dan telefon bimbit semasa
peperiksaan adalah dilarang. Calon-calon hanya boleh membawa masuk,
pen, pensel dan alat tulis lain yang dibenarkan sahaja. Telefon
bimbit juga boleh dibawa masuk tetapi perlu dimatikan dan
diletakkan di sudut kanan bawah meja calon.
11. Calon-calon dikehendaki meletak slip peperiksaan di sudut
kanan bawah meja calon selepas disemak oleh Pengawas/ Pembantu
Peperiksaan. 12. Calon-calon tidak dibenarkan berhubung di antara
satu dengan lain semasa peperiksaan, sama ada
melalui percakapan atau cara yang lain. Jika calon ingin
berhubung dengan Pengawas, beliau mestilah mengangkat tangan.
13. Ketua Pengawas/Pengawas yang bertugas akan memberi isyarat
yang nyata dan jelas untuk
menandakan bahawa peperiksaan telah tamat. Sebaik sahaja isyarat
ini diberikan, semua calon hendaklah berhenti menulis.
14. Calon-calon adalah bertanggungjawab untuk mengikat skrip
jawapan (jika perlu) dan terus berada di
tempat masing-masing sehingga skrip jawapan dikutip oleh Ketua
Pengawas/Pengawas dan calon-calon hanya dibenarkan keluar dari
dewan peperiksaan setelah dimaklumkan oleh Ketua Pengawas/
Pengawas.
15. Calon-calon hanya dibenarkan meninggalkan tempat peperiksaan
selepas setengah jam (30 minit)
peperiksaan bermula. Walau bagaimanapun, tidak seorang calon pun
dibenarkan meninggalkan dewan peperiksaan lima belas minit (15
minit) sebelum tamat peperiksaan.
16. Calon-calon yang jatuh sakit semasa peperiksaan sedang
berlangsung mestilah melaporkannya kepada
Pengawas yang bertugas di dewan peperiksaan dengan secepat yang
mungkin. 17. Calon-calon yang tidak dapat menduduki peperiksaan
bagi mana-mana kertas yang didaftarkannya
hendaklah memberi sebab-sebab mengapa mereka gagal menghadiri
peperiksaan [secara bertulis] kepada Pendaftar [melalui Ketua
Penolong Pendaftar (Unit Peperiksaan dan Pengijazahan)], disokong
oleh dokumen-dokumen yang sah [Sijil Cuti Sakit hendaklah
dikeluarkan oleh Pusat Sejahtera
(Kesihatan & Pergigian) USM, doktor panel Universiti atau
hospital/klinik kerajaan] dalam tempoh 48 jam selepas peperiksaan
kertas berkenaan dijalankan. Kegagalan untuk berbuat demikian akan
dianggap bahawa calon tidak mempunyai sebarang alasan yang sah dan
berkemungkinan diberi gred " F* " bagi kertas tersebut.
18. Jika seseorang calon didapati meniru/menipu ataupun
mempunyai bahan-bahan yang tidak dibenarkan
untuk dibawa ke dalam dewan peperiksaan semasa peperiksaan
diadakan, tindakan disiplin yang sewajarnya akan diambil terhadap
calon yang berkenaan. Jika seseorang calon disyaki
meniru/menipu dia boleh disingkir dari Universiti.
19. Calon-calon yang tidak menghadiri kesemua kertas peperiksaan
tanpa sebab yang dapat diterima oleh
Majlis Peperiksaan Universiti ataupun tanpa kelulusan pihak
Universiti terlebih dahulu, akan dianggap gagal dalam keseluruhan
peperiksaan dan boleh dikeluarkan daripada Senarai Pelajar
Berdaftar Universiti.
20. Bagi calon-calon yang mengalami masalah kesihatan, sila
dapatkan nasihat Pengarah, Pusat Sejahtera
(Kesihatan & Pergigian) Universiti Sains Malaysia atau
doktor dari Hospital/Klinik Kerajaan.

UNIVERSITI SAINS MALAYSIA EXAMINATIONS
EXAMINATION INSTRUCTIONS TO CANDIDATES
1. Candidates must take note of details in the examination
timetable such as date, time, venue and
seating arrangement for their respective examination papers.
Candidates are advised to refer constantly to the notice boards for
any amendments to the examination timetable. Amendments, if any,
will be printed on blue paper and will be posted on notice boards.
Misreading of the examination timetable cannot be accepted as a
reason for failure to attend an examination. (Candidates will be
given an F* grade).
2. Candidates must be punctual for their examination and must
bring their Identity Card, Student Card
and Examination Slip. Candidates without an Examination Slip
will not be admitted into the examination hall until their
eligibility to sit for the examination has been verified.
Candidates without an Identity Card will have to convince the Chief
Invigilator/Invigilator of their identity. Candidates will not be
allowed to sit for the examination paper of a course not listed in
the candidate’s Examination Slip. The examination slip represents
an official University document and candidates are thus not
permitted to write on any portion of the examination slip.
3. Candidates are not allowed to enter the examination hall
until the Invigilator on duty gives the signal
to do so. Normally, candidates are allowed to enter ten minutes
(10 minutes) before the examination commences. Upon entering the
hall, candidates are to proceed to their seats in a quiet and
orderly manner. Candidates who are more than half an hour (30
minutes) late will not be permitted to enter the examination
hall.
4. Upon taking their respective seats, candidates are required
to : [a] complete the Attendance Form and place it at the top right
hand corner of their desk.
[b] place their Identity Card and Examination Slip on top of the
completed Attendance Form. The
Attendance Form will be collected by an Invigilator/Examination
Assistant. Candidates are reminded to keep their Identity Card and
place the examination slip at the right corner underneath
candidates’ desks after checked by Invigilator/ Examination
Assistant.
5. Candidates are to write down their Index Number in words and
figures e.g. Seven Zero Zero Zero
One (70001) on the cover of their answer booklet. Candidates
must write their index number using figures on all answer sheets
used. Candidates are not to write their NAMES on their answer
booklet/sheets.
6. Examiners will not mark the answer scripts of candidates who
fail to write their Index Number or who
write their Index Number wrongly on their answer booklet/OMR
forms and these candidates will not receive any marks.
7. Candidates are reminded to observe all instructions stated on
the cover of their answer booklet.
Rough work may only be done on the question paper. Extra answer
booklets will be supplied upon request. All answer booklet/sheets,
whether used or unused, must be left in the examination hall.
Candidates are not allowed to remove any answer booklet and/or
answer sheets from the examination hall.
8. Books, documents, notes or pictures, except materials
authorized by the examiner, cannot be
brought into the examination hall. In the examination hall,
candidates may be supplied with books, papers, documents, pictures
or any other material authorized by the Dean/Director upon advice
by the examiner or Council of Examiners.
9. Candidates are not allowed to smoke in the examination
hall.

EXAMINATION INSTRUCTIONS TO CANDIDATES 10. Candidates are not
allowed to bring into the examination hall electronic dictionaries,
any bags,
wallets/purses, pencil cases, geometrical instrument cases or
any other containers/cases. Calculators (without the covers) may be
brought into the examination hall if allowed by the examiner. Use
of ELECTRONIC DICTIONARY is prohibited during the examination.
Candidates may only bring in pens, pencils and any authorized
writing materials. Hand phones are allowed to bring in but must
be switched off / in inactive mode and place at the right corner
underneath candidates’ desks.
11. Candidates are required to place examination slip at the
right corner of underneath candidate’s desks after checked by
Invigilator/Examination Assistant . 12. Candidates are not allowed
to communicate with one another during the examination, whether
verbally or through any other means. Should a candidate wish to
communicate with the invigilator, he/she should raise his/her
hand.
13. The Chief Invigilator/Invigilator on duty will give a clear
signal to indicate that the examination has
ended. As soon as this signal is given, all candidates must stop
writing. 14. Candidates are responsible for tying their answer
booklets (if necessary) and are to remain at their
respective seats until their answer scripts have been collected
by the invigilator. Candidates are only allowed to leave the
examination hall upon notification by the Chief
Invigilator/Invigilator.
15. Candidates may only leave the examination hall half an hour
(30 minutes) after the commencement
of the examination. However, candidates are not allowed to leave
the examination hall in the last fifteen minutes (15 minutes) of
the examination.
16. Candidates who fall ill during an examination must report
their illness to the invigilator on duty in the
examination hall as soon as possible. 17. Candidates who are
unable to sit for any examination for which they are registered
have to state (in
writing) to the Registrar [through the Principal Assistant
Registrar (Examination and Graduation Unit)] the reasons for their
absence from the examination, supported by valid documents [Medical
Leave Certificates must be issued by Pusat Sejahtera (Health
Centre) at USM, USM Panel Clinics or Government Hospital/Clinic]
within 48 hours from the end of the said examination paper. Failure
to do so will be taken to mean that the candidate does not have any
valid reason and the candidate will, therefore, be likely to be
awarded an `F*’ grade for the paper.
18. If a candidate is found to have copied/cheated or is found
to be in possession of unauthorized
materials in the examination hall, disciplinary action will be
taken against the candidate. Candidates suspected of
copying/cheating can be expelled from the University.
19. Candidates who fail to attend all their examination papers
without any valid reason acceptable to the
University Examination Council or without the prior approval of
the University will be deemed to have failed the whole examination
and can be deregistered as a student of the University.
20. Candidates who are having health problems are to consult the
Director of Pusat Sejahtera (Health
Centre) at Universiti Sains Malaysia or a doctor from any
Government Hospital/Clinic.

Course (KSCP).

Obey all EXAMINATION RULES. Disciplinary action will be taken
against any

Saya ingin menarik perhatian semua pelajar kepada Arahan-arahan
Peperiksaan Untuk Calon-calon (No. 16) yang dipetik seperti berikut
:
Calon-calon yang tidak dapat menduduki peperiksaan bagi
mana-mana kertas yang didaftarkannya hendaklah memberi sebab-sebab
mengapa mereka gagal menghadiri peperiksaan (secara bertulis)
kepada Pendaftar [melalui Ketua Penolong Pendaftar (Unit
Peperiksaan dan Pengijazahan)], disokong oleh dokumen-dokumen yang
sah [Sijil Cuti Sakit hendaklah dikeluarkan oleh Pusat Sejahtera
(Perubatan & Pergigian) USM, Doktor Panel USM atau
Hospital/Klinik Kerajaan] dalam tempoh 48 jam selepas peperiksaan
kertas berkenaan dijalankan. Kegagalan untuk berbuat demikian akan
dianggap bahawa calon tidak mempunyai sebarang alasan yang sah dan
berkemungkinan diberi Gred " F* " bagi kertas tersebut.
2. Untuk peringatan anda, Sijil-sijil Akuan Sakit yang
dikeluarkan oleh doktor-doktor yang lain dari Pusat Sejahtera
(Perubatan & Pergigian) USM, Doktor Panel USM dan
Hospital/Klinik Kerajaan tidak diterima sebagai alasan yang
sah.
3. Bagi pelajar yang berkenaan, sila ambil tindakan sewajarnya.
Pihak Universiti tidak akan menghantar Surat Tunjuk Sebab kepada
pelajar yang tidak menghadiri peperiksaan.

ABSENCE FROM EXAMINATION KURSUS SEMASA CUTI PANJANG (KSCP)
EXAMINATION, 2019/2020 ACADEMIC SESSION
I would like to draw the attention of all students to the
extract of the Examination Rules to Candidates (No. 16) as stated
below :
Candidates who are unable to sit for any examination for which
they have registered are required to state (in writing) to the
