HEALTH & SAFETY POLICY

The management of Jacksons Cranes & Heavy Haulage is committed to a safe and healthy working environment for everyone using the premises as a place of work or visiting on business.

Management will: Set health and safety objectives and performance criteria for all employees and

work areas. Annually review health and safety objectives and managers performance. Encourage accurate and timely reporting and recording of all incidents and

injuries. Investigate all reported incidents and injuries to identify all contributing factors

and, where appropriate, formulate plans for corrective action. Actively encourage the early reporting of any pain or discomfort. Provide treatment and rehabilitation plans that ensure a safe, early and durable

return to work. Identify all existing and new hazards and take all practicable steps to eliminate,

isolate or minimize the exposure to any significant hazards. Ensure that all employees are made aware of the hazards in their work areas and

are adequately trained so they can carry out their duties in a safe manner. Encourage employee consultation and participation in all health & safety matters. Ensure that all contractors and subcontractors are actively managing health and

safety for themselves and their employees. Promote a system of continuous improvement, including annual reviews of

policies and procedures. Meet our obligations under the Health & Safety in Employment Act 1992 (as

amended by the Amendment Act 2002) (the HSE Act), the Health and Safety in Employment Regulations 1995, codes of practice and any relevant standards or guidelines.

Every employee is expected to share in the commitment to health & safety. Every employee has a responsibility for the health & safety of employees working

under their direction.Each employee is expected to help maintain a safe and healthy workplace through: Following all safe work procedures, rules and instructions. Properly using all safety equipment and clothing provided. Reporting early any pain or discomfort. Taking an active role in the company’s treatment and rehabilitation plan, of their

early and durable return work. Reporting all incidents, injuries and hazards to the appropriate person.

The Health & Safety CommitteeThe Health & Safety committee includes management, union and employees. The committee is responsible for the implementation, monitoring, review and planning of health & safety policies, systems and practices.

We are committed to continuously strive for improvement and it is Jacksons Cranes & Heavy Haulage goal to achieve excellence in Health & Safety.

Manager

Management Commitment and Responsibilities1. Ensure that Jacksons Cranes & Heavy Haulage meets the requirements of the HSE Act

(1992), its amendments, and relevant Regulations and Codes of Practice.2. Establish health & safety responsibilities and performance criteria within our H&S system

for all personnel.3. Set annual health & safety objectives and regularly review our performance.4. Identify and review all existing and new hazards, and take all practicable steps to

eliminate, isolate or minimise the exposure to any hazards.5. Conduct an annual review of the Health and Safety System including completion of the

WSMP audit tool as an annual self-assessment.6. Ensure that all personnel are trained and supported to perform their duties in a safe

manner.7. Ensure that all personnel understand and are trained in health & safety requirements

relative to their position in the company.8. Encourage employee participation in all matters relating to health & safety.9. Establish effective procedures to deal with emergencies.10. Accurately record, investigate and report any work related incidents and accidents.

Review Health and Safety Management system following critical event or change in work procedures

11. Promote a system of continuous improvement as an integral part of all Jacksons Cranes & Heavy Haulage policies, procedures and work practices.

12. Support the process of injury management and rehabilitation within the workplace, using alternative duties and light work options.

13. Induct all new employees and contractors.

Employee Commitment & Responsibilities1. Actively contribute to hazard identification and management.2. Report immediately any unsafe work condition, accident or incident.3. Wear all personal protective equipment provided.4. That all employees adhere to Best Practice Guidelines/ Approved code of practice

applicable to the area in which they are currently operating.5. Follow safe work practices and ensure that their actions do not cause harm to

themselves or others.6. Participate in health & safety activities, meetings and training.7. Assist with the review of the health and safety system and/or instigate change in work

procedure following a critical event.8. All employees will comply with the principal company’s health & safety and

environmental requirements.

Health & Safety Team1. Accident investigation.2. Coordinate and maintain health & safety system.3. Help induct, new employees on health and safety issues.4. Assisting injured workers to return to work safely, in conjunction with the injured worker,

company doctor and employer, including medical and vocational needs.5. Hazard Management in conjunction employees.6. Assist with implementing light duties for injured workers.7. Ensure all health and safety documents are controlled to Jacksons Cranes & Heavy

Haulage procedures.8. Report on health & safety performance of the contractor and sub contractors.9. Ensure compliance with all regulations, codes of practise and principal’s safety

requirements.

External H&S Consultants Responsibilities1. Liaise with Jacksons Cranes & Heavy Haulage management and employees on Health

and Safety Issues.2. Assist with accident investigation when required.3. Provide Health and Safety training.4. Assist with health and safety systems following a critical event.

Annual Health & Safety ReviewA health and safety review of the current system, policies and documentation will be held annually.Health and safety objectives will be set and reviewed annually and each health and safety meeting will discuss how we are working towards these objectives, incident trends and analysis will be discussed and injury prevention initiative implemented where relevant.

This system audit discussion is to also be used as a means of getting feedback on the effectiveness of the H&S system and making changes to improve its effectiveness in managing work place safety. A self-assessment will be completed using the WSMP audit tool. All audit processes must be fully documented and clearly indicate:

1. All remedial action required.2. Who is responsible for implementing remedial action.3. The timeframe in which remedial action must be completed.

All such action points must show evidence of follow up and sign-off.

Feedback and action points from these reviews will be used to update the Health and Safety System as appropriate.

Annual MeetingAn annual meeting is to be held each year prior to the annual review. This meeting will review the past years performance, and provide feedback and input for development of the next year’s annual plan. Document control is the responsibility of the Health and Safety Team. Any changes made must be introduced at the next available meeting. All changes will be signed off by the staff.

Meetings

Jacksons Cranes & Heavy Haulage will conduct a meeting on a regular basis.A meeting may also be held on other occasions required by the principal or any

when an employee requests one.These meetings will cover:

1. All employees know what their task will be.2. Those under supervision must know who will be responsible for them.3. Cover any accident/ incident reports and their follow-ups.4. Report any defective machinery, equipment and/or processes.5. Update hazards and controls – are they effective?6. Progress toward company objectives.7. Any new safety bulletins and other such information should be reviewed.

Hazard ID, Assessment and ManagementThe management of all workplace hazards and particular those hazards that are deemed to be significant, is a key component of the safety management system. All levels of management and employees need to show continuous commitment to the following areas of the hazard management process:

1. Identification of existing and new hazards. 2. Risk assessment of all identified hazards.3. Development of hazard controls.4. Implementation and documentation of hazard controls.5. Auditing of the hazard management process.

Key documents in the Hazard Identification and Control process are:1. Hazard Register.2. Site Inspections.3. Industry Codes of Practice.4. Meetings.

Hazard IdentificationEmployees of Jacksons Cranes & Heavy Haulage will identify hazards and develop controls as new hazards arise. All hazard information and their control mechanisms must be documented in the Hazard Register. Other sources of hazard information are:

1. Codes of Practice.2. Best Practice guidelines.3. Manufacturer’s instructions.4. Industry safety bulletins.5. ACC website.6. DOL website.

Employees also need to be aware of the particular hazards outside of the immediate workplace such as:Workplace culture – including the commitment and priority of health & safety within your workplace.Work organization – including issues of workload, long hours.People management – including information and communication, education, training and supervision.Task/focus of the task/objects – including cranes, trucks, vehicles, equipment, cranes lifting objects, housekeeping, hand tools, power tools, maintenance. Environment – including weather, electrical and fire.

Hazard AssessorsThe person(s) carrying out the hazard ID process on new or modified equipment must be signed off as suitably qualified.Appropriate qualification would include:

1. Personnel with appropriate experience and training. 2. Ongoing training will be required at least every two years for those leading hazard

management.3. External specialist if required.

New HazardsFor new or redefined hazards (not in register), including new equipment and/or work processes, a hazard identification process must be carried out to ensure compliance with the HSE act. Such hazard information can come via:

1. Physical inspections.2. Accident/ incident investigation findings and outcomes.

Jacksons Cranes & Heavy Haulage maintains feedback into the hazard register by:1. Accident/ incident investigative feedback and outcomes.2. Meetings.

Compliance with the control process will be checked via visual monitoring and audits.

New Machinery/Equipment Induction and Hazard Check

Each new piece of machinery/equipment must have clear operating instructions and/or operators manual. When required training must take place by the designated operators. These must be stored in the site office.The machinery/equipment induction check will also have to cover:

1. Existing and new hazards and controls for the new machine.2. The process in which it will be used. 3. Further work that will be required to develop satisfactory hazard controls for the new

equipment/plant.4. That the new operator has read any operations manuals/instructions.

New equipment/plant may also introduce new hazards to the entire operation due to the operation of the equipment/plant or the change in work process that the equipment/plant brings to the business.Ensure the operator is aware and familiar with the new machine, any hazards it brings and the controls in place around it.

Safety Review –New MachineryPrior to the purchase of any new plant and/or equipment Jacksons Cranes & Heavy Haulage will consider the relative Health and Safety aspects of the various models as part of the evaluation process before deciding on which plant/equipment to purchase. Operating instructions must be available for all new machinery/equipment.A new safety installation safety checklist will be carried out.

Hazard RatingsDefinition of a hazardA hazard is any activity, situation or substance that can cause harm.

SignificantSignificant hazard means a hazard that is an actual or potential cause or source of:

1. Serious harm, or

2. Harm (being that is more than trivial) the severity of whose effects on any person depend (entirely or amongst other things) on the extent or frequency of the persons exposure to the hazard or

3. Harm that does not usually occur, or usually is not easily detectable, until a significant time after exposure to the hazard.

Hazard Control and ManagementHazard control can be achieved in three main ways:Elimination Achieved by removing the hazardous plant, equipment, substance or by discontinuing work methods or processes. Substitution is one such form of elimination, by replacing the problem area with a safer alternative.Isolation Achieved by placing some kind of barrier or distance between the hazard and the people in the hazard zone. Using controls such using barriers to keep personnel away from the operation are both methods of isolating hazards.Minimisation If the hazard cannot be managed by above methods, then we must minimise exposure via such actions as:

1. Following safe and accepted work practices as per rules and guidelines.2. Provide suitable PPE and ensure it is maintained and used correctly.3. Maintain all equipment properly.4. Provide adequate supervision and where necessary training.5. Monitor exposure and health (such as health checks) if required.

Hazard Risk Score AssessmentTo determine the combined risk score for the hazard the three categories score are multiplied together to get an overall risk score.Exposure x Likelihood x Consequence = RiskEg 3 x 3 x 3 = 27The higher the final risk score the greater the risk of injury.E = Exposure to hazard L= Likehood of harm resulting C= Consequence (probable

result of potential accident)

Hazard ReviewHazard review and employee feedback back can occur through several forums :

1. Accident/incident investigation2. Meetings3. Audits4. Visitors

All visitors to the site must be inducted into the site:1. Be given an induction into the major hazards of the yard, on site job and out of bounds

areas.2. Have appropriate ppe.3. Sign the visitors book as understanding of hazards.

Personal Protective EquipmentJackson Cranes & Heavy Haulage will ensure all new employees are supplied with all relevant PPE to the task they are to perform. This includes steel capped footwear, eye protection, hi visibility vest, helmet, hearing protection.

Safety Inspections/Equipment and AuditsRegular checks will cover all areas of protective equipment and machinery/equipment checks. Audits and inspections will vary according to the scope of work. Vehicle/Workplace Inspection Checklists will be documented.

Induction, Training and SupervisionInduction of Individual Employees It is the responsibility of Management to ensure all their staff and contractors undergo a formal induction in accordance with Jacksons Cranes & Heavy Haulage induction procedures before commencing any work. The purpose of the Induction process is to introduce the employee to the work environment, and to explain and clarify Jacksons Cranes & Heavy Haulage Health & Safety.

The induction process must cover the following issues:1. Health & Safety Policy.2. Relevant codes of Practice, Critical Rules and Best practice Guidelines.3. Emergency Procedures.4. Accident / Incident reporting.5. Hazard identification & appropriate hazard information.6. Management responsibilities.7. Employee responsibilities.

Essential DocumentationPrior to any employee starting they must complete the following forms:

1. Pre-Employment Medical Questionnaire2. Health & Safety Policy – acknowledged 3. Employee Information Form4. Employee Induction (Power Point if available)

Flow Pre-Employment Screening and Health MonitoringThe Jacksons Cranes & Heavy Haulage Medical Questionnaire filled out during induction is part of a pre-screening process to assess/ confirm suitability for task. All information recorded and/or discussed with Employee must relate to issues that could have a bearing on their ability to perform the work related tasks. Any such information shall remain confidential to management.

Jacksons Cranes & Heavy Haulage may require some new employees to carry out a pre-employment health check. Such checks should clarify any existing illness/ injuries or conditions the employee may have, and set a base line monitoring point for future annual health checks.

Sub Optimal Test ResultsWhere any results show deterioration of an employee’s health, Jacksons Cranes & Heavy Haulage will firstly review all current work procedures and protective equipment to ensure they have not contributed to these results. All results will be fed back into the hazard management system. All sub optimal results will be managed in conjunction with the employees GP. Medical tests may also be conducted following a critical event and or when an employee ceases employment.

Training Policy and Procedures

Jacksons Cranes & Heavy Haulage is committed to ensuring that all employees are fully trained to an appropriate level of skill necessary for them to work safely, and in an efficient and productive manner. The training matrix will show which employees are competent in their tasks and who is able to train.

Training RequirementsEmployees initial training requirements are set direct from the induction and competency information supplied upon commencing work.

At no time may any employee operate any machinery or plant they are not qualified and/or trained to operate.

Documentation & Management of TrainingA trained or suitably experienced person must undertake any training and/or supervision of employees who are not yet deemed fully competent for task.All informal/in house training is to be noted on the training plan. External trainers must be industry approved and accredited trainer assessors.

For the formal training and assessment of industry standards, will only use persons with the relevant industry qualifications and experience. Annual training reviews and performance appraisals will be completed by management to ensure ongoing monitoring and compliance with safe working practices.The calendar will act as a reminder for renewals of licences and First Aid certificates etc.

Disciplinary ProceduresJacksons Cranes & Heavy Haulage expects all employees and contractors to work in a professional, co-operative and safe manner at all times. Serious misconduct or unsatisfactory behaviour will not be tolerated. Which could lead to disciplinary actions in line with the Employees Individual Employment Contract or Contractors Agreement.

The following list represents some instances of unsatisfactory behaviour, which could lead to disciplinary actions in line with your employment contract and current employment legislation:

Rehabilitation and Injury Management Procedures Jacksons Cranes & Heavy Haulage is dedicated to the well-being and rehabilitation of our contractors and employees, and will ensure that all possible options are utilised to promote a full and speedy recovery of accident victims.

Under no circumstances is anyone to be put back to work in circumstances which may aggravate an injury that has not yet fully healed.

Light DutiesEach individual has a duty to inform the doctor or treatment provider that there is a range of light duty options available for any potential lost time injury.

Jacksons Cranes & Heavy Haulage management will provide an employee with a return to work program.

Definition of Accident, Serious Harm and Incidents

Accident means an event that….

1. Causes any person to be harmed or;2. In different circumstances might have caused any person to be harmed.

Serious Harm (includes death) means…

1. Any of the following conditions that amounts to or results in permanent loss of bodily function, respiratory disease, noise-inducted hearing loss, dermatological disease, communicable disease, musculoskeletal disease, illness caused by exposure to infected material, decompression sickness, poisoning, vision impairment, chemical or hot metal burn of eye, penetrating wound of eye, bone fracture, laceration, crushing.

2. Amputation of body part.3. Burns requiring referral to a specialist registered medical practitioner or specialist

outpatient clinic.4. Loss of consciousness from lack of oxygen.5. Loss of consciousness, or acute illness requiring treatment by a registered medical

practitioner, from absorption, inhalation, or ingestion, of any substance.6. Any harm that causes that person harmed to be hospitalised for a period of forty-eight

hours or more commencing within seven days of the harm’s occurrence.

Incident An undesired event, which, under different circumstances could result in harm to people, damage to property, as loss to work process.

Hazard An activity, arrangement, circumstance, event, occurrence, process, situation, or substance that is an actual or potential cause as source of harm.

Occupational Illness An illness caused by exposure to harmful influences during the course of a person’s occupation.

Notification Procedures1. All employees are responsible to report all accidents, incidents, and hazards to

management as soon as possible. A accident/incident form is to be completed. Failure to report any incident can result in ACC refusal, or instant dismissal under the disciplinary actions for “Serious Misconduct”.

2. For serious harm incidents, Management to notify, local Department of Labour representative immediately. Then in writing within 7 days.

3. Remember to “Freeze the Scene” for all injury or harm incidents.

4. For all other accidents/incidents, the contractor must notify management as soon as possible and complete a accident/incident form, and get it to Jacksons Cranes & Heavy Haulage Management within 24 hours of the incident or property damage situation.

Monthly Reporting and Accident RegistersJacksons Cranes & Heavy Haulage has a legal requirement to keep detailed Accident and Incident records, and to review on Frequency Statistics annually.

All Accident, Incident reports are to be discussed at the Health and Safety meeting. Injury prevention initiatives and innovations will be discussed, actioned and signed off.

Jacksons Cranes & Heavy Haulage Accident and Incident trends will be reported and discussed at the safety meetings. Incident data will be posted on the notice board beside the smoko room.

Accident Investigation ReportsJacksons Cranes & Heavy Haulage will complete a formal investigation of Serious Harm accidents.These reports will be filled out on the appropriate Accident Investigation template.

Accident Feedback and ReviewAll accident investigations should provide an assessment of contributing factors and underlying factors that caused the accident. This section should identify the hazards and the breakdown or lack of controls for the hazard that led to the Accident.The Hazard Register will be updated with the new hazard(s) and /or controls resulting from the incident.

Jacksons Cranes & Heavy Haulage Management will ensure that suitable emergency procedures are prepared and implemented and that all employees have been made aware of these procedures.

It is critical that all persons on site have been inducted, and understand the Jacksons Cranes & Heavy Haulage site emergency plan - this includes site visitors.

“First-Aiders” must hold a current first aid training qualification. The First-Aiders must know the correct procedures to follow as per the processes outlined in this emergency procedures

section of the manual.

Emergency Rules:All cranes/trucks/support vehicles and Washdyke Yard are to have ready access to a suitable first aid kit. This kit must be regularly audited to ensure that contents are sufficient in the case of an emergency.

Communication & Testing of Emergency Procedures1. The induction process will clearly explain emergency rules and procedures.2. Jacksons Cranes & Heavy Haulage requires at least 2 emergency procedure drills to be

carried out per year (one every six months). These drills can cover any of the above emergency situations

3. Any safety drills carried out should be recorded.4. A debrief session should be held after each drill to discuss how the drill went and to

highlight any necessary training and/or improvements to process.

The most common emergency situations are likely to be accident emergencies or fire emergencies. Please ensure your employees know what to do, and through the use of drills, have confidence in their abilities and actions.

All critical events will require an emergency response meeting to be held with all personnel on site as soon as possible after the event. Outcomes will be discussed and recommendations be implemented. If required specialist advice may be sought to manage a situation out of Jacksons Cranes & Heavy Haulages control.

Contractor – a person engaged by any other person (other than as an employee) to do work for gain or reward.Jacksons Cranes & Heavy Haulage will select contractors and service agents who are:Well established, competent and certified/ qualified to perform the required work.Can provide if requested, references from previous clients on their performance in regards to Health & Safety and their overall contractual ability.A Contractor Declaration Form may be required to be completed prior to commencement of work.

New Contractor InductionPrior to any contractual works commencing, Jacksons Cranes & Heavy Haulage will provide a Contractor induction for the key Contractor personnel involved .This process should include:

1. Any information associated with any hazards already identified, must be fully shared with the new contractor.

2. An induction into the hazard ID and control system for the reporting of new hazards. 3. Emergency procedures.4. Accident/incident reporting.5. Training and supervision.One off contractors ‘Contractors Induction Checklist form’ to be completed.Long term contractors ‘Contractor Agreement Form’ to be completed.

Contractor Monitoring and ReviewThe on-going monitoring and management of contractors will be the responsibility of Jacksons Cranes & Heavy Haulage.The contractors health and safety performance should be reviewed annually, via Contractors Performance Audit Form.Post contract evaluation to take place at the completion of work carried out, via Contactors Performance Form.

Contractor Non-ComplianceWhere any non-compliance is noted, then Jacksons Cranes & Heavy Haulage will inform the contractor responsible, with documented details. Any such notices must clearly state the remedial action required, allocate who is responsible for remedial action, and give a date by which it must be achieved by:Immediate suspension of that activity or process until the non-compliance is remedied.Contractor given one full working week to remedy issue.Contractor given one month (until next audit) to correct the issue.

Service Agent InductionService agents fall into the category of person(s) who provide a specialist service to the company but on a part time or irregular basis.Groups included may be:

It is recognised that these people carry out their trade in similar businesses and have a reasonable expectations of what to encounter.All service providers should read the visitors induction, sign in and out of the visitor’s book.
