2 IDENTITY SERVICE

The OneHealthPort Identity Service has been operational for twelve years as a community registration authority and private certificate authority. The service vets the identity of healthcare providers or healthcare trading partner organizations and their users for organization level identifiers and individual user identities. The OneHealthPort Identity Service does not currently enumerate OIDs for trading partners because historically the systems of relying partners were not capable of consuming the OID structure. A private number convention, the OneHealthPort Organization Identifier (OrgID), of nine alphanumeric characters is currently used but can be mapped to an OID structure if the trading community is ready to support that convention. The OneHealthPort OrgID is currently cross-walked to an OID structure in the OneHealthPort Health Information Exchange (HIE) for in-state support of current identifiers and external use of the OID standard for external HIE to HIE messages.

3.2 TRADING PARTNER ORGANIZATIONS

Trading partner organizations require a hierarchy in order to attribute data or reports to multiple levels of an organization. Attribution is essential for quality reports and analysis of root causes or locations of data trends. Since meta data provided in the SOAP headers of many healthcare transactions with repositories is used to augment analysis of data from different sources, multiple layers of enumeration allows attributes to define parent and child organizations but also source systems at either level. Source system analysis allows for vendor and trading partner feedback to correct data omissions or data quality issues at the user and system level.

In the National Health Information Network (NHIN) Authorization Framework v3.0 the Attribute Statement describes the various identifiers. OneHealthPort has enumerated to support this structure:

3.2.2 Attribute Statement (from NHIN Authorization Framework Specification v 3.0) The <AttributeStatement> element describes a statement by the SAML authority asserting that the requesting user is associated with the specified attributes. The <AttributeStatement> is required to contain <Attribute> elements as defined by the OASIS XSPA profile of SAML and described in the sections that follow. The NHIN defines the use of the following 8 attributes in the saml:AttributeStatement (use of additional attributes is allowed, but not defined, by this specification): 1. Subject ID - OneHealthPort Security Officer = “Sue Merk” 2. Subject Organization – HIE name = “NRAA Health Information Exchange” 3. Subject Role 4. Purpose Of Use 5. 4 CLINICAL DATA REPOSITORY

OneHealthPort offers a Clinical Data Repository (CDR) as a service for any trading partner interested in sponsoring lives in the CDR. CDR Sponsors might also be trading partners in the HIE so the enumeration of separate OID namespace for the CDR sponsors simplifies the routing and identification of messages intended for the CDR.

The CDR has a requirement that inbound messages have a namespace extension with the terminal OID arc representing a “.1” for documents identifier and a “.2” for patient identifier fields. 5 HIE TO HIE, CDR, OR REGISTRY IDENTIFIER CONVENTIONS

Each HIE, CDR or registry may enumerate their trading partners and exchange in the fashion that best suits their business needs. When exchanging messages on behalf of trading partners with an external HIE, the two exchanges



must exchange meta data for repository and message delivery purposes. The recommended national standard is a SAML wrapper and a SOAP envelope. If all parties to the exchange are not using SAML wrappers on individual documents/messages then a secondary convention that provides the same identifiers in a more traditional HL7 header is an acceptable convention. The required attributes regardless of the header or wrapper approach should include to and from for:

a. Home Community ID = specific Exchange identifier (OHP HIE or NRAA HIE) b. Organization ID = tradingPartner or tradingPartner plus tpChild c. Resource ID (Optional) = vendorSystem or trading partner hosted system

A single object identifier from the sender identifying level a, b or c to a receiver (push message) or query (pull message) should also identify the receiver organization at the appropriate level:

a. Home Community if a repository being queried. b. Organization ID when a request or push is sent to a specific entity organization in an HIE. c. Receiver resource ID if the HIE has identified specific optional systems (EPIC for CCD, Dell Rad for

PACS images, NXTGT for master person index queries). The trading partner must request enumeration of their vendor or system description if the HIE has not currently listed that offering with an OID.

Headers and Wrappers

a. Generic xml header: Use of object identifiers assigned by the respective HIEs for the entity organization is required in both the “to” and the “from” fields alternatively called “SenderID” and “RecipientID”. b. Traditional HL7 Message Header (MSH): Use of OneHealthPort organization IDs or OIDs can be used in traditional HL7 MSH segments. Typically, OneHealthPort trading partner organization IDs are used in MSH 4 and MSH 6 to identify the submitter and the intended recipient. For public health reporting identifiers are used with additional granularity in the MSH and FHS or BHS (if used when batching data submissions). In MSH-4.1 the OneHealthPort OrgID for the submitter is used and in MSH-4.2 the OneHealthPort OID is used. Detailed instructions for use of these identifiers for traditional HL7 messages (sent individually or batched) are outlined in the respective message implementation guides or on Department of Health websites specific to the public health transaction or registry.
