Iam delighted to report the participation of TheTransplantation Society to successfully integrate thecrime of organ trafficking into the agenda of the

United Nations Sustainable Development Goals.I represented TTS at an “Ethics in Action” conference atthe Pontifical Academy of Sciences along withrepresentatives from the WHO (Nunez), PontificalAcademy (Delmonico), and experts in the areas ofhuman trafficking, child labor, slavery and prostitution.The meeting led by senior UN advisor Jeff Sachs(University Professor and Director of the Earth Instituteat Columbia) and Monsignor Marcelo Sanchez Sorondo(Chancellor of the Pontifical Academy of Science)brought together a global group to ultimately inform theUnited Nations General Assembly. The “Ethics in Action”final statement included organ trafficking as a dimensionof human slavery equally important to the UnitedNations as forced labor and prostitution.

I also have the honor of representing TheTransplantation Society to meetings planned with thegovernments and Ministries of Health in Costa Rica,Mexico, and the Philippines because these are locationsof a current concern for organ trafficking, expressed byour professional colleagues in these countries. Alongwith colleagues from the PAS and the nationaltransplant community, we have been invited by localcolleagues to meet with respective health authoritiesto articulate issues of national self-sufficiency withreference to WHO Guiding Principles and theDeclaration of Istanbul.

I am nearing the closure of my term as TTS President.Although I am sorry to see my term end, I am confidentin the future of TTS. Our organization has vibrantparticipation from members around the globe. Yourincoming President, Prof. Mehmet Haberal has been along-term TTS member, a strong leader, and atransplant pioneer in Turkey and the Middle East. Hehas been central to the organization of transplantefforts in the Turkic countries as well. Prof. Haberal willassume his duties as President at our business meetingin Madrid.

Our elections are completed and I am delighted toannounce that you have selected Marcelo Cantarovichfor President-Elect for the term 2018-2020 (becomingPresident for 2020-2022). Marcelo has ably led ourTTS Education Committee for the last 5 years. Whilehailing from Montreal, he has strong ties to our LatinAmerican colleagues.

The Transplantation Society is happy to announce theselection of Buenos Aires as the site for the 2022 TTSCongress. The recommendation from the SelectionCommittee and the decision by Council was based onseveral factors including the results of the membershippoll. In the end, it was a difficult decision as all threesites – Buenos Aires, Istanbul and Montreal – advancedwonderful proposals. The 2022 meeting in BuenosAires represents an important milestone as the first TTSmeeting to be held in Latin America.

Finally, my heartfelt thanks to Officers, Councilors andstaff of TTS for all the support you have given me forthe past two years as President. But I would also like tothank all members who have given so freely of theirtime to further the mission of our Society, and the carefor our patients.

Iam both honored and excited to have the opportunityto serve as President of The Transplantation Societyfor the 2018-2020 term.

TTS is the only global transplantation society thatencompasses all of the disciplines in the field oftransplantation and has grown exponentially since itsestablishment just over 50 years ago. Today, the societyhas 6500 members from more than 100 countries.Currently, there are eight separate official sections tomeet the varying needs and areas of expertise of itsmembers, and 34 affiliated national and regional societiesare gathered under the umbrella of TTS.

Since its inception, TTS has driven global action in thedevelopment of the science and clinical practice, scientificcommunication, continuing education, guidance onethical issues and legal practice. While the focus andactivities of our members’ work may vary within theirrespective fields, extending the reach of our efforts hasnever been more important. The changing needs ofpatients and professionals alike necessarily impact social,medical, ethical, and legislative structures.

We cannot ignore that there continue to be strikingdisparities in transplantation throughout various areas ofthe world. However, TTS continues to take the lead inestablishing best practice in institutions throughout theworld. The ISN-TTS Sister Transplant Centers Programand the newly created TTS-ILTS Paired Transplant CentersProgram are prime examples of the difference that suchinitiatives can make in the creation and development ofkidney and livertransplant programs in low- and middle-income countries. In the coming years we shall persist inthis vein, with the ultimate purpose of establishingtransplantation in every country with comparablestandards of care for the patients.

In so doing, we will continue to support the activities of theDeclaration of Istanbul, which is celebrating its 10th Anniversaryin 2018. The important changes in legislation and practice thathave taken effect in various countries – and most notably inChina – are a testament to how fundamentally it has addressedthe needs and concerns of the international transplantcommunity. I would like to take this opportunity to thank allour colleagues who have worked tirelessly to this end. Our aimas the transplant community should now be to work towardsa system of meeting the organ demand entirely with deceaseddonation. This will not only result in the reduction of unethicaltransplantation activities, it will also make an enormousdifference to those patients awaiting transplants in which livingorgan donors are not an option.

TTS will continue to play a major role in tackling ongoingeducational, legal, regulatory, ethical and public healthchallenges, in collaboration with the World HealthOrganization, the United Nations, the Council of Europe,national governing bodies, as well as national, regional andinternational societies. Among our primary concerns will bethe ethical means of meeting the growing demand for organsthrough ethical organ donation and procurement, promotingscientific understanding and equality in standards of clinicalpractice and patient care, transparency in transplantationactivities, and of course educational programs to meet theneeds of our members. Communication has been particularlyeffective this past term, with a noticeable increase in electroniccommunications with the membership, especially with theimplementation of the weekly electronic bulletin, TribunePulse. In addition, the Transplantation and TranplantationDirect journals have also been flourishing, with an improvedimpact factor and rising quality of published papers.

We have an exciting term ahead of us. We will begin planningthe 28th TTS Congress in Seoul in 2020. Building on the strongwork done by my predecessors, our leadership team will alsobe reviewing the strategic plan of the society and will worktowards advancing the priorities it sets out. By firmlyestablishing our vision and mission, we will better be able todetermine and realize the impact we wish to makescientifically, socially, legally, and ethically. We will be takingimportant strides to ensure that equal attention is given to the6 regions that are represented within TTS, thereby ensuringthat members from all regions are given equal chance toaccess TTS activities and educational opportunities.

Our membership will play an important role in this process andwe look forward to receiving your thoughts and suggestions.

Mehmet Haberal MD, FACS (Hon), FICS (Hon),FASA (Hon) is a pioneer in the fields oftransplantation and burn treatment since 1975.After training with Thomas Starzl in Denver, hereturned to Turkey and performed the firstsuccessful kidney and liver transplants in thecountry. In addition to several burn societies, heis Founder and Immediate Past President of theMiddle East Society for Organ Transplantation,and Founder and President of the TurkishTransplantation Society, the Turkic WorldTransplantation Society, and the InternationalHaberal Transplantation and EducationFoundation. He served as President of theInternational Society for Burn Injuries (2006-2008), and as TTS Councilor for the Middle Eastand Africa (2004-2008). He received the TTSMillennium Medal in 2000. He is currentlyGovernor and Turkey Representative of the ACS.He is Founder and Founder President as well asPresident of the Executive Supreme Board ofBaskent University, which has established 10hospitals/outpatient clinics and more than 20dialysis centers throughout the country.

DECLARATION OF ISTANBUL CUSTODIAN GROUP

Ensuring DOI remains a valuablesource of ethical guidance

for the next decade

The Declaration of Istanbul Custodian Group (DICG) iscelebrating the 10th Anniversary of The Declaration ofIstanbul this year. To mark this special event a working grouphas prepared an update of the Declaration. The revisionsmade in this 2018 Edition are intended to ensure that theDeclaration of Istanbul remains a valuable source of ethicalguidance for health professionals and policy makers for thenext decade in the face of persisting and emergingchallenges in organ trafficking and transplant tourismaround the world. During March and April we conducted aglobal public consultation and asked for feedback on thedraft 2018 Edition. We’ve received comprehensive feedbackfrom our DICG members, members of organisations thathave endorsed the Declaration of Istanbul, and members ofthe broader transplant community.

As part of the celebrations, the DICG is holding a one dayworkshop during of the Pre-Congress program at theInternational Congress of The Transplantation Society. Theworkshop takes place on July 1, comprised of a half-daymorning program addressing common ethical and clinicaldilemmas in the management of organ donation andtransplantation cases that may involve organ trafficking;and, a half-day afternoon program exploring the Declarationof Istanbul (2018 Edition) and the results of the publicconsultation. The event features several DICG members asspeakers, and we hope to see many more transplantprofessionals among the workshop participants. n

In 2008, some 152 professional from 78countries and from 20 national andinternational organizations andprofessional societies met in a Summitin Istanbul from April 30 to May 2 tocombat organ trafficking, transplanttourism and transplant commercialism.

Transplantation Direct is an early success story thatpromises to fill a niche in the transplantation publicationarena. Its close affiliation with The Transplantation Societyand the most cited transplantation journal,Transplantation, contributes to its reliability andcredibility. Indeed, our internationally known EditorialBoard has worked very hard to establish the fundamentalsnecessary for building a respectable new journal in acompetitive field. Importantly, Transplantation Directoffers an efficient review process, averaging a firstdecision in just over 2 weeks after submission (16.4 days).Manuscripts undergo a thorough selection process beforebeing sent for review in Transplantation Direct, ensuringan uncompromised level of quality. Recognition of thisquality is steadily bringing more submissions each year,with global submissions from 19 countries in 2017,resulting in an upward trend in the number of publishedarticles (averaging 8 per month in 2017). Staying with thetimes to maximize article exposure, the journal’s Twitter@TXPDirect has a growing presence.

Besides standard article types, Transplantation Direct alsooffers other article formats for new methodologies andregistry/society meeting reports, as well as encouragingbrief, preliminary reports of especially interesting earlyexperimental or clinical results; the latter article type,together with the fast review and publication time, allowsrapid reporting of important early results with the additionof completely open access availability. Steady developmentover the 3 years since its launch brought an early positiveassessment from indexing authorities in August 2016,resulting in all articles available and fully searchable onPubMed Central (indexed on PubMed). TransplantationDirect is also indexed in the Directory of Open AccessJournals, which is your assurance that the journal has beendeemed a legitimate open access journal. n

EDUCATION COMMITTEE

donation awareness,healthy lifestyles, and

leadership development

The Education Committee (EC) is made up ofmultiple working groups which are continuouslydeveloping content and activities.

In April, the EC brought together transplant recipients,transplant medical professionals, kidney donors, donorfamilies and students from a Montreal area high schoolfor a video recorded session. The Students, supervisedby a teacher (who is also a kidney transplant recipient)prepared questions to address to the panel ofparticipants. The students conducted interviews whichwill be used in the educational online teaching moduleabout awareness on organ donation and ransom stationfor school children. This is a joint project with thecollaboration of the Canadian Society of Transplantation,the Dutch Transplant Foundation and TTS. The modulehas been translated from the one produced by the DutchTransplant Foundation. Snapshots of the interviews arebeing shown during the congress.

The EC in collaboration with World Transplant GamesFederation (WTGF) is working on the preparation of ahealthy lifestyle module. This module is designed fortransplant recipients. The goal is to provide transplantrecipients with tools to increase the physical activity andhelp with issues related to mental health and diet.

The TTS Leadership Series is wrapping up. The EC ishosting a Lunch & Learn session at TTS 2018 on July 2nd

which will discuss the leadership series and highlight acouple projects from the leadership series participants.

In the Fall, the Education Committee, spearheaded byDr. Valeria Mas, will launch a Webinar Series onTranslational Medicine. Stay tuned to hear more aboutthis new and exciting series. n

The TTS Congress is here and we are excited to seea truly action-packed transplantation scienceprogramme. The Congress is preceded by a

number of excellent meetings during the weekend, andwe would like to highlight the Post-Graduate Course aswell as the Global Transplant Laboratory meetings asproviding excellent broad overviews of current science.The Post-Graduate Course comprises both clinical andbasic science, with participants free to jump in betweensessions. There is a world-class line-up of speakers, withthe opportunity to discuss and network with clinicians andscientists who are experts in their fields. We wouldrecommend this for attendees at any level – for some itwill provide an introduction and for others, an update! TheGlobal Transplant Laboratory meeting is a valuable updatefor those running studies in transplantation that rely heavilyon immune monitoring and laboratory techniques.

The Transplantation Science Committee is proud to besponsoring and co-organising the ESOT Basic ScienceMeeting in Rotterdam, from 11th-13th October 2018. The Young Members Committee (YMC) continuesto grow with international representation at thetrainee and junior faculty levels. Our mission

focuses on promoting career development opportunitiesfor our youngest members. This past year has beenextremely productive as we have developed a consistentpresence in the Tribune Pulse with a quarterly articlefocused on young member related topics, as well as,spearheading TTS sponsored webinars. The YMC hasalso connected individuals and institutions. The latestexample is the sharing of information and developmentof critical transplant programs both at the University ofAlabama at Birmingham and the University of CapeTown (see recent Pulse article featuring YMC membersDr. Locke and Professor Muller).

We have also been working hard to help make theupcoming 27th Congress of The Transplantation Societyin Madrid, Spain, the best yet for young members. TheYMC has developed several program proposals focusedon career development both at the trainee and juniorfaculty levels. We will also be hosting anothernetworking event, as the launch of our first evernetworking event met with great success in Hong Kongat the 2016 congress. The event will feature knowledgeand opinion leaders in the field and will be the perfectopportunity to identify potential mentors and interactwith other outstanding young members of TTS. We hopeto see you there!

We are excited to be a part of TTS and to continue tomove the mission of TTS forward. The upcoming yearwill be filled with additional opportunities for youngmembers to engage within TTS as we aim to develop awebinar series on career development for 2018-2019.Stay tuned for more great things coming from the YMCof the TTS. n

ETHICS COMMITTEE

The Ethics Committee published its first statementof ethical considerations related to live kidneydonation, based on a forum held in Amsterdam in

2004. In 2006, a subsequent forum was held in Vancouverto review the ethical considerations involved in theexpanded areas of lung, liver, pancreas and intestine livingdonor transplantation. The rapid evolution of all areas oftransplantation has resulted in even more complex ethicalissues in the last decade concerning altruistic donors,multiple organ donors, domino liver transplants, pairedexchange, advanced donation programs, and the use ofsocial media. Liz Pomfret (USA) will be leading the EthicsCommittee in updating the documents of the Amsterdamand Vancouver forums, as well as reviewing the complexnewer issues related to expansion of the living donor pool.

The Ethics Committee will also be reviewing the GlobalKidney Exchange (GKE) initiative, which has recentlyemerged. There has been significant concern that theGKE program should be halted due to ethicalconsiderations. The intent to globally exchange livingkidney donors between low/middle income countries(LMIC) and high income countries challenges establishedethical principles in organ donation. Multipleorganizations, including TTS, the Declaration of IstanbulCustodian Group, the Council of Europe Committee onOrgan Transplantation and the Ibero-LatinamericanCouncil for Organ Donation, have come out in strongopposition to GKE between economically disparatecountries. Rudolf Garcia-Gallont (Guatemala) will beleading the Ethics Committee in reviewing the currentethical challenges associated with GKE at the meetingof the committee in Madrid, with the intent of identifyingthe ethical issues it encompasses, and identifyingsounder strategies for facilitating transplant services inthe LMIC.

Finally, the ethics committee has planned for twoexciting sessions at the TTS 2018 Meeting in Madrid.The State of the Art (SOTA) is scheduled on Monday,July 2, and is entitled Upholding the Ethics ofTransplantation: The Global View. This symposiumincludes talks from transplant leaders in Pakistan, Iran,and China – and will review ethical strategies forincreasing transplant rates without compromising ethicalstandards. The second session is scheduled forWednesday, July 4, and will address the challengingissues involved with informed consent with living anddeceased donors – another area the Ethics Committeewill be tackling during the next two years. n

Perioperative care remains the “missing link” in thecontinuum of care of transplant patients and livingdonors alike. The newly established TTS Anesthesia

and Critical Care Committee is dedicated to fill this gapand provide state of the art updates on perioperativemanagement of transplant recipients and living donors.Preoperative evaluation and intensive care managementof transplant recipient is the focus of the first State-of-the-Art Session at the upcoming annual meeting inMadrid. The committee also aims to increase awarenessof the importance of deceased donor management andresearch, facilitate continuous education of health careproviders unfamiliar with deceased donor management,and provide best practices for donation after brain deathand after cardiac death. The new addition of anesthesia/critical focused abstract sessions will further help toexchange best practices and experience in the care ofour complex patients. n

One of the Heart & Lung Committee’s major focusis on delivering a wider membership andincreased submission not only to the

Transplantation journal but also for the TTS 2020Congress in Seoul. The number of accepted heart & lungrelated manuscripts by the journal remains static at justover 30 per annum, a figure that clearly needs increasing.There are currently over 430 TTS members who haveindicated one of their specialties as being Heart andLung. To gain a clearer definition of this interest, we willbe creating a questionnaire that will allow theCommittee to communicate more efficiently and start anearnest dialogue to promote greater academic, scientificand practical collaborations. This was discussed at arecent meeting of a few of the Committee membersduring the ISHLT conference in Nice, France. PeterHopkins (Brisbane, Australia), Tobias Deuse (SanFrancisco, USA), Sandeep Attawar (Chennai, India), PeterMacdonald and Kumud Dhital (Sydney, Australia) wentthrough the rationale for the formation of the H & LCommittee and how it could work in partnership andwithout conflict with the wider heart and lung transplantcommunity represented by the ISHLT. It was agreed thatthis should be explored further including the writing ofconsensus documents with a multi-organ focus; however,we need to start by ratifying clear Terms of Referencewhich are being drawn up.

The other area of practical collaboration is beinghighlighted by a planned case-based Heart TransplantWorkshop that will be delivered in Ho Chi Minh City,Vietnam by the Heart Transplant team from St Vincent’sHospital in Sydney. Led by Kumud Dhital, PeterMacdonald and Hon Chee Chew, eight clinicians andnursing staff traveled to Vietnam in June this year andprovided a comprehensive educational workshop thatstretched from recipient selection all the way through topost-transplant follow-up. It had endorsement andsupport from TTS, the Transplantation Society ofAustralia and New Zealand (TSANZ) and the AustralianOrgan and Tissue Authority; a full report will besubmitted to the next Tribune edition. It is hoped thatthis model can subsequently be taken to other regionsand tailored for other organs as appropriate. n

WIT LEADERSHIP

The WIT Steering Committee will announce its new 2018-2020 co-chair at TTS 2018. Elaine Reed will be steppingdown as Chair of WIT after two years of dedicatedleadership. Lori West, who has been the co-chair of WITfor the last two years, will become the Chair of WIT for2018-2020.

The WIT Working Group: Pillar 1 - Advancing and InspiringWomen Transplant Professionals, will also be announcingnew leadership at TTS 2018 as Lorna Marson will bestepping down as the chair. Lorna Marson is currently thePresident of the British Transplantation Society. n

On September 26-29, 2017, a Joint Congress was heldwith the Canadian Society of Transplantation (CST) inHalifax, Nova Scotia. It received the highest accolades forinternational quality of science from CST, together withletters of appreciation from the Prime Minister of Canada,Justin P. J. Trudeau, and the Premier of Nova Scotia,Stephen McNeil. An exciting gala evening took place onthe water in Halifax and included future, present and pastCTRMS Presidents in attendance and celebrated 5CTRMS Young Investigators for award presentations.

A state-of-the-art symposium on July 3, 2018 will lookat: current outcomes in clinical beta-cell replacementand how they should drive decision-making in specificsituations faced by patients with complicated type 1diabetes; a consensus paper on definitions of successfuloutcomes for pancreas and islet transplantation recentlypublished by experts from IPITA and ESOT; andapplication of these criteria to stem cell-derivedtransplants will also be explored in this symposium.

A morning symposium on July 4th, will take you to thefuture of beta-cell replacement, beyond pancreas andislet transplantation as they are performed today. Willtransplantation of xenogeneic or stem cell-derivedinsulin-producing tissues be the ultimate solution? Willgenome editing be instrumental in the development ofsuch strategies? The 14th International biennial meeting of theISVCA will be held September 29 to October 1,2019 in New Delhi, India. The ISVCA meeting will

be held in parallel to the ISOT/CAST meeting in thesame venue and will have one common plenary session.This will enable members from the Asian and Indiansocieties and the VCA community of the world to haveclose interaction

India, which has a rich experience in solid organtransplantation, came into the VCA scene with the firstbilateral hand transplants done at the Amrita Institute,Kochi on January 13, 2015. Subsequently, the same centrehas carried out four more double upper limb transplants.Two more centers followed with double hand transplantsin one patient each this year. Two uterine transplants werealso carried out successfully in Pune, India this year.

The Indian Society of Organ Transplants (ISOT) leads thescientific activities related to organ transplants in thecountry. This society has subdivisions for different solidorgans as well as supportive services like pathology andtransplant coordinators who all have separate sections inthe national meeting held annually in different parts of thecountry. In 2015, the Indian Society of ReconstructiveTransplants (ISRT) came into being under the umbrella ofthe ISOT. The 2019 national meeting of the ISOT will becoupled with the Congress of Asian Society ofTransplantation (CAST). The CAST will attract participationfrom transplant professionals from all Asian countries.

The meeting will discuss relevant topics like expanding theindications for VCA, VCA in developing countries, VCAprocurement in deceased donors and Immunologyupdates from solid organ transplants. A one-day satellitesymposium is planned on immunology and pathology ofVCA in Kochi, a port city in the southern state of Kerala afew days after the main meeting.

New Delhi, the capital city of India, is well connected to allparts of the globe. It offers a plethora of tourist attractions,besides being a hub for exploring the neighbouringattractions like the Taj Mahal and the pink city of Jaipur orthe hill stations of Uttarakhand. The satellite symposium inKochi will offer a chance to visit the picturesque southernstate of Kerala described in the tourism circles as “godsown country”, offering a mix of hill stations, beaches,backwaters and opportunity to relish the traditionalAyurvedic massage sessions. n

The care of adults and children with intestinal failure hasbecome a well-integrated mission, with intervention ofgastroenterologists, hepatologists, surgeons,anesthesiologists, specialized nurses, and social workers,through procedures and treatments such as long-termhome parenteral nutrition, lengthening surgery, growthfactors administration and transplantation. The ultimateaim of this demanding care is for these patients to lead a“near normal” life. The meeting will unite physicians andallied health professionals on topics such as: the basics ofintestinal regeneration and immunology, the excitingdiscoveries on the numerous positive and deleteriousaspects of the microbiome, the many roads of the gut-liver axis, the challenges of everyday life with parenteralnutrition, the future of rehabilitation such as hormonaltherapy and artificial intestine, the difficulties andsuccesses of intestinal transplantation, the long termoutcomes of these patients, and the many ways toimprove patients’ lives, such as psychological and socialsupport, in the education system and workplace,compliance and transition, telemedicine, and patient carein less affluent countries.

The meeting will address the patients’ care from both thepatient point of view as well as the health worker’s, and atall stages, from childhood to old age, whether hisintestine is absent, transplanted or regenerating. The mission of TID is to promote research andeducation in the prevention, diagnosis, clinicalconsequences, and management of the infectious

disease problems of the transplant recipient. In the lasttwo decades, TID has organized meetings in severalregions of the world, bringing updated information in thefield of transplant infections, with programs thatdiligently include topics of regional interest.

In order to build collaborations with other interestedorganizations, we have recently held an exciting jointsession with the International Society of Heart and LungTransplantation (ISHLT) during the 2018 ISHLT conferencein Nice, France. A similar one-day meeting is beingplanned for November 11, 2019, in Dubai UAE, one-dayprior to the ISODP Congress being held on November12-15, 2019. Members are welcome to suggest similarcollaborations with other meetings.

Our journal, Transplant Infectious Disease, covers state-of-the-art clinical review articles, clinical investigation,reviews of experimental and translational research intransplant infectious disease, and novel case reports.Over the years, TID members have contributedsignificantly to the journal’s success, increasing thenumber of periodic submissions and the growth ofTransplant Infectious Disease impact index.

Above all, we are proud to note that TID members areexpert professionals in their field of activity in all regions,as shown by the recent publication "Recommendationsfor the Treatment of Endemic Diseases and TravelMedicine in Solid Organ Transplant Recipients andDonors: Latin America", which was proposed anddeveloped by a team of experts, many of them activemembers of TID.
