EXECUTIVE SUMMARY

The purpose of this final ieport ir to summarize the activities and products deiived fiom a contract of collaboiation between the Population Council and The National Institute of Public Health of Mexico to institutionalize operations research (OR) The Population Council provided US$5,730 undei the contract (No C193 52A) which covered the peiiod from August 1993 to February 1995 The following five main objectives were programmed (1) cumcular development in the School of Public Health of Mexico, (2) technical assistance and teaching in the Continuing Education Programs, (3) compilation of technical and bibliographical material, (4) technical assistance in field woik, and (5) dissemination of information A calendai of activities for each objectwe was planned From the implementation of such activities, some of the main products obtained included (a) analysis of the curriculum involving OR in the academic activities of the School of Public Health, (b) provision of OR lectures within the masters level courses, (c) involvement of giaduate students field woik and the development of their masters theses, (d) development of an operations research project to piovide piactical experience for graduate students, (e) dissemination of results thiough academic participation in national and international conferences and publications, (f) development of a specialized collection of OR materials at the National Insbtutlonal Public Health libiary Finally, the report concludes with recommendations regarding the implementation of the above mentioned activities placing special emphasis on the importance of (a) continuing the collaboration between the two organizations to ensure the institutionalization of OR, and (b) supporting the development of a Operations Reseaich Department within the Institute The ieport includes 9 attachments supporting the achievements of this collabaration

P BACKGROUND

On August 24, 1993, the Population Council (PC) established a contract of collaboration with the National Institute of Publ~c Health (NIPH), with the mutual objective to strengthen the operations ieseaich (OR) capacities within the NIPH The Population Council proportioned a total of US$5,730 to the NIPH in support of related activities The signed contract would last for 12 months dating fiom August 15, 1993 The NIPH solicited an extension, that the PC granted, for an additional 6 months under the amendment No 1 of the original contiact The contract was thus extended until February 14, 1995, without modification to the original budget

During the period of August 15 1993 to February 14, 1995, the NIPH experienced changes in its executive direction and internal structure On Februaiy 14, 1994, the contract was delegated to the newly formed Diiection of Administrative ~ciknces within the School of Public Health of Mexico (SPHM) Subsequently, Dr C6sar Macias Chapula was named Pioject Coordinator, thus replacing Dr Juan Luis Duriin Arenas

Since the beginning of the contract, Lejeune Y Lockett, M S P H has served as assistant to the Project Coordinator through her activities as an International Population Fellow (IPF) with the NIPH

This report describes the objectives, piincipal activities, and the results obtained by the collaboiation Finally, conclusions and recommendations are pioposed by the Project Coordinator

2 OBJECTIVES

The geneial objective of the project is to institutionalize operations research as an academic entity within the NIPH In ordei to achieve such a goal, the following 5 strategies weie specified (1) curricular development in the SPHM, (2) technical assistance and teaching in the Continuing Education programs, (3) compilation of technical and bibliographical mateiial , (4) technical assistance in field woik, ( 5 ) dissemination of information

The semi-annual progress report dated March 10, 1994 (Attachment 1 ) describes in detail the principal activities that weie implemented according to each specific project objective

3 ACTIVITIES

3 1 CURRICULAR DEVELOPMENT IN THE SPHM

In order to conduct this activity, a team was formed consisting of 3 masters students from the SPHM (Leonard0 E Cruz Robles, Marta B Rodriguez Uteia, and Rafael Heinhdez Santiago), an International Population Fellow at the SPHM (Lejeune Y Lockett) and the Project Coordinator (Cksai A Macias Chapula) The team conducted a thorough revision of the courses offered through the SPHM during the 1993 academic year The team selected those couises that could be related to OR and then conducted three work sessions to discuss and draw conclusions about the content of those courses and the potential for including OR infoimation The findings were analyzed by the team and further discussed in a later meeting with Ricardo Vernon (PCIMexico) A document was diafted (Attachment 2) that was distiibuted to vaiious academics in the SPHM Fernando Chac6n Sosa, Director of Administiative Sciences, Dr Francisco Bafiuelos Tillez, Head of the Administiation Sciences Department, and Guadalupe E Rodiiguez, Head of the Didactic Depaitment

It was concluded that the SPHM has the potential to increase the presence of OR in its curiiculum and that this could be done through the organization of a specialization in OR and additional Continuing Education couises in the area It was agreed that the curricular review must continue and be conducted in yet a more extensive manner Furthermole, in older to implement a specialization, continuous technical and financial support are required

3 2 TECHNICAL ASSISTANCE IN TEACHING AND THE CONTINUOUS EDUCATION PROGRAMS

In this activity, the following pal ticipated a Population Fellow (Lejeune Y Lockett), two PCMexico officers (Antonieta Martin and Anne Staunton), and the Pioject Coordinator (Cksar A Macias- Chapula) They piovided technical assistance in 2 courses of "Information Systems 11" within the Masters of Sciences in Health Systems piogram of the SPHM for a total of 8 hours The support for those courses will be ietained systematically in the subsequent academic calendal s

During the fiist semestei of 1994 and with the suppoit of PC/Mexico (Ricardo Vernon), a 4-hour workshop was given foi the Masters of Sclence in Health Systems students The workshop was focused on aspects of methodology and the planning of projects ielated to OR The final objective was to help guide the students in the elaboiation of their thesis pi oposals

A Continuing Education course "Health Management Informat~on Systems" was held fiom June 17 to July 2, 1994 in Oaxaca City, Oaxaca, Mexico The course was administered to approximately 40 state level manageis and administrators of the Ministiy of Health The course consisted of a 2-houi session designed to discuss concepts and techniques of OR ielated to information systems and emphasized the importance of tiansfelling what was learned in the course to other levels within the Ministiy

The Continuing Education course "Operations Research in Health Systems" was planned and programmed for August 21-25, 1995 at the NIPH (attachment 3) The course is designed for health care managers, ieseaicheis, and graduate students The intention is to launch it as a "pilot course" in older to explore the viability of curricular changes and the piomotion of future courses of this type at a regional level in Latin Ameiica and the Caribbean

3 3 COMPILATION OF TECHNICAL AND BIBLIOGRAPHICAL MATERIAL RELATED

This activity was supported by the participation of three representatives of the SPHM (Andree Lera, Lejeune Lockett and Cksar A Macias-Chapula) and two fiom PCMexico (Antonieta Martin and Ricardo Vernon) Institutions were identified that could donate OR related information and publications to the NIPH These organizations were then contacted and the materials have been arriving at the NIPH library on a regular basis The library processes the mate~ials and files them in a spec~ally designated collection that IS bemg developed for OR The journal "Stud~es in Family Plannmg" has been of particular interest to the users of the NIPH libraiy The attachment 4 iefers to the collection received

3 4 TECHNICAL ASSISTANCE IN FIELD WORK

Three masteis students in the SPHM were supported (Leonaido E Cruz Robles, Malta B Rodriguez Utreia, and Rafael Hernindez Santiago), in the elaboration of three research proposals The thiee research proposals weie related to family planning services in the

region of Los Tuxtlas In the state of Veracruz, Mexico A proposal ("Quality of medlcal attention at a non-government institution -MEXFAM- in the reglon of Los Tuxtlas, Veracruz, Mexlco", by Rafael Hernandez Santiago), culminated as them wolk towards the fulfillment of a maqtels degree The other two proposals helped the students in the lnltlal stages of definmg the conceptual framework of the11 thesls The students mentioned above received guldance from Lejeune Y Lockett, Cksar A Macias-Chapula and Luis Durin-Arenas, on behalf of the SPHM, and from Rlcardo Vernon, on behalf of the PCIMexico

In Parallel, the resealch project "An analysls of non-government family plannlng selvlce dellvery and utilization in rural Mexico In terms of quallty and sustalnabllity" was developed Thls project is headed by Lejeune Lockett, with Cksar A Macias-Chapula, Luis Duran-Arenas and Rafael Heinandez Santiago particlpatlng as co- investigators Thls project employs the conceptual framework of OR and Situation Analysls with the goal to feed the results obtained from the field work back into the teaching activities of the SPHM

With the final objectwe to implement the actions derived from the projects mentioned above, varlous academic sessions were conducted with the graduate students, and field work vlslts to the region of Los Tuxtlas, Veracluz wele plogrammed In preparation for these vlsits, admlnistratlve as well as loglstlc actlvlties had to be coordinated such as for the travel, room and board, establishing local contacts, etc The clinic located In Catemaco, Veracruz played a critical role as the mam unit of analysis Its dllectoi, Roberto Barrdn, offered full access to the facllity so that the research projects could be conducted

By February 14, 1995, the advances of the projects reached the level of testing the instruments that were designed to obtain various indicators The attachment 5 describes the research proposals that were developed, and attachment 6, the instruments in progress up to the pilot stage

3 5 DISSEMINATION OF INFORMATION RELATED TO OR

Existing methods withln the SPHM were identified to disseminate information mtra and mter lnstitutlonally The results of t h ~ s

analysis weie integrated in a document (Attachment 7) that exploied the viable mechanisms that could be used to divulge information about OR by the NIPH Antonieta Maitin (PCIMexico) and Ad~iana Robledo (NIPH) pal ticipated in these activities

On Novembei 2, 1994 the NIPH was inscribed in the Mexican Institute of Systems and Ope~ations Research, INC w ~ t h the intent to communicate and divulge the activities of the NIPH related to OR, and likewise participate and become familial with what is being development in othei institutions

The presentation "An analysis of non-government family planning seivice delivery and utilization in iural Mexico in terms of quality and sustainability" was made at the 122 Meeting of the American Public Health Association (APHA), in Washington, D C , October 30- Novembei 3, 1994

The piesentation "Aniilisis de 10s servicios iurales y no gubeinamentales de planificacicin familiar en tkrminos de accesso, calidad y a~itosuficiencia" was made at the VI National Congress of Public Health Research, in Cuernavaca, Moielos, Mexico, Januaiy 23- 25, 1995

An abstract entitled "Sustaining non-government family planning services in iural Mexico A community oriented model" was submitted to the 123 Meeting of the American Public Health Association (APHA) This event will occur in San Diego, California, October 29- November 2, 1995

The first draft of a document will be submitted to be considered for publication in an inteinational scientific jouinal The aiticle likewise reports the expeiience obtained from in the application of OR methods with the support provided by the PC

The four presentations mentioned above report the preliminary results of the application of OR at the NIPH accomplished wtth the support of the PC in the form of techn~cal assistance in the field work

In the magazine "Mexico Insight", a documentary was wiitten that refers to the activities that the g ~ o u p conducted in Catemaco, Vei acruz

Finally, the Contlnulng Education coulse on "Operations Research in Health Systems" will be offeled In August, 1995 and integrated wlth the rest of the cou~ses offeled by the NIPH This integration was accomplished vla the piintmg of 5000 copes of a coulse catalogue that 1s belng dlstilbuted thioughout the entire Latm Amel lcan/Cm ibbean legion

(e) Technical assistance was provsded to three masters students sn the SPHM resultmg sn the elaboration of three research proposals and two graduate theses

(f) A reseaich pioject wa(; developed (and is In progress) that includes field woik in the region of Los Tuxtlas, Veraclliz, Mexico

(g) Two presentations wele made In two sclentlfic events, one natlonal and the othei lntelnational

(h) Two documents wele submitted for conslderatlon in an international congress and a scientific joulnal

5, CONCLUSHONS AND RECOMMENDATIONS

In continuation, conclusions are discussed that refer to the achievement of the original' objectives and the development of the project in general For each conclusion, recommendat~ons are given that are considered relevant to the institutionalization of OR in the NIPH

1 Interest exists on the part of the SPHM to institutionalize OR The actual programs of the SPHM offer the infrastructure and reflect an mplicit interest in OR in its courses Such is the case of the "Information Systems I and 11" courses, for example Nevertheless, in order to define the mechanism of curricular modification (be it via a specialization, a mastels degree or even basic required courses o n OR), it is necessary to not only conduct an exploratory and documental analysis, but to also establish a consensus with the other actors involved in the academlc system It is recommended to continue with a second phase of curricular analysis using as a base the initial document that was elaborated for this purpose (Attachment 2) It is suggested, for example, to conduct market studies to obtain an indicat~on of the pool of potential students, and of the most appropriate contents for the course

Furthermore, it is recommended to evaluate and follow-up the Continuing Education course that will be conducted on OR in August, 1995 The comments from the students and instructors of t h ~ s course will be utilized in both the long and short term to integrate a curriculum proposal on OR This will also perpetuate the promotion of OR courses on a national as well as regional level in Latin America and he Canbbean

2 The incorporation of OR in the information systems courses at the SPHM has resulted from the interest of the students, mainly those who are obtaining a masters of science degree in health systems It is recommended not only to maintain t h ~ s type of courses, but to communicate the results and recent experiences derived from the application of OR to the personnel of the NIPH The results denved from the present contract for example, can be incorporated with the courses of 1995 in order to share experiences with the students, foster the use of OR and promote involvement in future research projects or t h e m proposals

3 Under this contract, the initiation of a specialized collection of OR materials could be coordiqated in the NIPH library Nevertheless, it is necessary to elaborate a Program of development and follow-up that is rn accordance wlth academic and curricular development

leading to the in~titutionalization of OR in the NIPH It is iecommended, foi example, to select and acquire didactic mateiial, making an exhaustive bibliohemeiigraphic review of varlous data bases, ident~fying previously the specific information needs on the pait of academic and students Once the material is acquired, it will be processed and selectively distiibuted among the community of potential useis Equally, it is recommended to conduct follow-up studies legaiding the use of the collection to assess the effects of the promotion A bibliogiaphic data base on OR that includes the productivity of the iesearchers at the NIPH is highly recommended foi the effects of diffusion and utilization of knowledge about OR in the region

4 The technical assistance in field work on OR resulted to be of great relevance to the achievement of the pioject It represented the ideal mechanism in oidei to tiansfei and apply knowledge of OR The field woik benefited the students and researchers, as well as the health institutions in the legion of Los Tuxtlas, Veracruz On the other hand, it also demanded time and resources in order to support the logistics ielated to the vaiious procedures for tiavel and expenses It is iecommended to look for a more practrcal mechanism in the management of the assigned funds by the PC to the NIPH, for example, through a mole flexible administration and without limiting the budget to exclusively cover tiavel expenses This would help to make use of the resources in a more efficient manner

5 In geneial, the pioducts obtained fiom the contract exceed the goals originally established For example, the participation in national and inteinational congiesses was achieved and also sufficient mateiial was accumulated that can be used for future publications

6 The impoitance of OR within the NIPH is evident and it is highly recommended to (a) solicit fuither support from the PC in ordei to enhance the stiengthening OR in the NIPH, and (b) support the development of an OR Department within the NIPH

The attachment 9 summai~zes a short and mid-term proposal to suppoit the continuation of a pioglam of collaboration with the Population Council in oidei to institutionalize operations reseaich at the National Institute of Public Health of Mexico

RESUMEN

El propósito de este reporte final es el de resumir las actividades y productos que derivaron del contrato de colaboración entre el Population Council (PC) y el Instituto Nacional de Salud Pública de México (INSP) para institucionalizar la investigación de operaciones (10) El PC proporcionó US$5,730 dls bajo el Contrato No C193 52A, el cual abarcó el período de agosto de 1993 a febrero de 1995 El contrato cubrió los siguientes cinco objetivos (1) Desarrollo curricular en la Escuela de Salud Pública de México (ESPM), (2) Asistencia técnica en enseñanza y programas de educación contínua, (3) compilación de material técnico y bibliográfico sobre 10 , (4) asistencia técnica en trabajo de campo, y (5) diseminación de información Para cada uno de los objetivos se desarrolló un calendario de actividades De la implementación de dichas actividades se obtuvieron los siguientes productos principales (a) análisis curricular sobre la inclusión de 1 0 en las actividades académicas de la ESPM, (b) participación académica en los cursos de maestría de la ESPM, (c) apoyo técnico en trabajo de campo a los alumnos de maestría para trabajo de tesis, (d) desarrollo de un proyecto de investigación con técnicas de 10, involucrando alumnos, (e) diseminación de resultados a través de la participación académica en conferencias nacionales e internacionales y publicaciones científicas, y (f) desarrollo de una colección especializada de materiales bibliohemerográficos sobie 1 0 en la biblioteca del INSP Finalmente, el reporte describe las conclusiones y recomendaciones sobre el desarrollo de las actividades y del contrato, haciendo especial énfasis en la importancia de (a) continuar la colaboración entre los dos organismos para asegurar la institucionalización de 10, y (b) apoyar el desarrollo de un Departamento de 10, dentro del INSP El reporte incluye nueve anexos que respaldan los logros obtenidos a través de la colaboración

1 ANTECEDENTES

El 24 de agosto de 1993, el Population Council (PC) establece un contrato de colaboración (No C193 52A) con el Instituto Nacional de Salud Pública (INSP), con el objetivo común de fortalecer las capacidades de investigación de operaciones en el INSP El total asignado por el PC para estos propósitos fué de US$5,730 El contrato firmado establecía una duración de doce meses, a partir del 15 de agosto de 1993 A solicitúd del INSP, se propuso una extensión de seis meses y bajo la enmienda No 1 al mismo contrato, el PC autorizó dicha extensión hasta el 14 de febero de 1995, sin cambios al presupuesto originalemente establecido

Durante el período del 15 de agosto de 1993 al 14 de febrero de 1995, el INSP realizó cambios en sus directivos y en su estructura interna El 14 de febrero de 1994, el presente contrato quedó bajo la Dirección de Ciencias Administrativas de la Escuela de Salud Pública de México, y se nombró al Dr César A Macías-Chapula, como coordinador del proyecto en sustitución del Dr Juan Luis Durán Arenas

Desde el inicio del contrato, la Mtra Lejeune Y Lockett se ha desempeñado como asistente del coordinador del proyecto, a través de sus actividades como "international Population fellow" (IPF) con el INSP

Este reporte describe los objetivos, las actividades principales y los resultados obtenidos de la colaboración entre ambas partes del contrato Al final, se presentan las conclusiones y recomendaciones propuestas por el coordinador del proyecto

2 OBJETIVOS

El objetivo general del proyecto fué el de institucionalizar la investigación de operaciones en el INSP Para lograrlo, se plantearon los siguientes cinco objetivos específicos a seguir (1) desarrollo curricular en investigación

operativa (10), en la Escuela de Salud Pública de México (ESPM), (2) asistencia técnica en enseñanza y progiamas de educacion contínua, ( 3 ) compilación de material técnico y bibiiográfico sobre 10 , (4) asistencia técnica en trabajos de campo sobre 10, y (5) diseminación de infoimación sobi e 10

Para lograr los objetivos, se planearon la estrategia y las actividades a seguir Estas se describen en detalle en el Reporte Semi-anual del 10 de marzo de 1994 (Anexo 1) A continuación se refieren las principales actividades realizadas para cada uno de los objetivos específ~cos

3 1 DESARROLLO CURRICULAR EN 10, EN LA ESPM

Para conducir esta actividad, se integró un grupo de trabajo, conformado por tres alumnos de la ESPM (Leonardo E Cruz Robles, Marta B Rodriguez Utrera, y Rafael Hernández Santiago), un investigador visitante en la ESPM (Lejeune Y Lockett) y el coordinador del proyecto (César A Macías-Chapula) El grupo realizó una revisión exhaustiva de las unidades didácticas de los cursos ofrecidos por la ESPM, durante 1993 Se seleccionaron las unidades didácticas relacionadas con 1 0 y se condujeron tres sesiones de trabajo donde se discutió y concluyó sobre el tema Los resultados encontrados por el grupo fueron analizados y discutidos en una reunión posterior con Ricardo Vernon (PC/Mexico) Se obtuvo un documento (Anexo 2) que fué puesto a la consideración de diversos académicos de la ESPM, entre otros el Mtro Fernando Chacón Sosa, Director de Ciencias de la Admnistración, el Dr Francisco Bañuelos Téllez, Jefe del departamento de Ciencias de la Administración, y la Lic Guadalupe E Rodríguez, Jefe del Departamento de Didáctica

Se concluye que la ESPM cuenta con el interés de institucionalizar 1 0 en su curricula y que ello podría ser a través de la organización de una especialidad en 1 0 y de la propuesta de cursos de educación contínua en el área Se acuerda que la revisión curicular debe continuar y conducirse de una manera mas extensa, además de que para implementar una especialidad se requerirían fondos y apoyo técnico contínuos

3 2 ASISTENCIA TECNICA EN ENSEÑANZA Y PROGRAMAS DE EDUCACION CONTINUA

En ésta actividad participaron un investigador visitante en la ESPM (Lejeune Y Lockett) dos oficiales del PC/Mexico (Antonieta Martin y Anne Staunton) y el coordinador del proyecto (César A Macias- Chapula) Se otorgó asistencia técnica a dos cursos de "Sistemas de Información 11", dentro de la Maestría en Sistemas de Salud/Maestría en Ciencias de la ESPM, con un total de ocho horas El apoyo a estos cursos se mantendrá de manera sistemática en los calendarios escolares subsiguientes

Durante el primer semestre de 1994 y con el apoyo del PCIMexico (Ricardo Vernon), se otorgó un taller de cuatro horas de duración a los alumnos de la Maestiía en Ciencias, en Sistemas de Salud de la ESPM El taller se enfocó a los aspectos metodológicos y de planeación de proyectos relacionados con 1 0 El objetivo final fué orientar a los alumnos en la elaboración de sus protocolos de tesis

Se apoyó el Curso de Educación Contínua "Sistemas de Información Gerencia1 en Salud", que se llevó a cabo del 17 de junio al 2 de julio de 1994, en la Ciudad de Oaxaca, Oax , Mexico El curso fué dirigido a aproximadamente cuarenta gerentes y administradores d e la Secretaría de Salud del Estado Se incluyó una sesión de dos horas para discut'ir conceptos y técnicas de 10, relacionadas con sistemas de información y se hizo énfasis en la importancia de transferir lo aprendido en el curso, a los diversos niveles de la Secretaría

Se planeó y progiamó el curso de educación contínua "Investigación Operativa en Sistemas de salud", el cual se otorgará del 21 al 25 de Agosto de 1995 dentio del Programa de Educación Contínua del INSP (Anexo 3) El curso está dirigido a gerentes del área de la salud, a

investigadores y docentes de post-grado, interesados en el area El propósito es lanzarlo como "curso piloto" para explorar la viabilidad de cambios curriculaies y la promoción de cursos posteriores a nivel regional en América Latina y el Caribe

3 3 COMPILACION DE MATERIAL TECNICO Y BIBLIOGRAFICO SOBRE 10

En ésta actividad participaron tres miembros de la ESPM (Andree Lera, Lejeune Y Lockett y César A Macías-Chapula) y dos del PCfMexico (Antonieta Mai tin y Ricardo Veinon) Se identificaron las instituciones que podrían enviar en calidad de donación las publicaciones relacionadas con 1 0 Se procedió a solicitar el material seleccionado, y éste llega con regularidad, directamente a la biblioteca del INSP La biblioteca procesa el material y se integra a la colección especializada que se está desarrollando sobre 1 0 La revista "Studies in Family Planning" ha resultado de particular interés a los usuarios de la biblioteca del INSP El Anexo 4 refiere la colección recibida

3 4 ASISTENCIA TECNICA EN TRABAJO DE CAMPO, SOBRE 10

Se apoyó a tres alumnos de maestría de la ESPM (Leonardo E Cruz Robles, Marta B Rodiíguez Utrera, y Rafael Hernández Santiago), en la elaboración de tres protocolos de investigación Los tres protocolos se relacionaron con los servicios de planificación familiar en la región de Los Tuxtlas, Veracrúz, México Un protocolo ("Calidad de la atención médica en una institución no gubernamental -MEXFAM- de la región de Los Tuxtlas, Veracrúz, México", por Rafael Hernández Santiago), culminó en trabajo de tesis para obtener el grado de Maestría Los otros dos protocolos apoyaron a los alumnos para definir el marco conceptual de su trabajo de tesis Los alumnos arriba mencionados recibieron asesoría por parte de Lejeune Y Lockett, Cesar A Macías-Chapula y Luis Durán- Arenas, de la ESPM, y de Ricardo Vernon, por parte del PCIMexico

En paralelo, se desariolló el proyecto de investigación "An analysis of non-government family plannrng service delivery and utilization in

rural Mexico in terms of quality and sustainability" En éste proyecto, encabezado por Lejeune Y Lockett, participan César A Macías-Chapula, Luis Durán-Arenas y Rafael Hernández Santiago Este proyecto utiliza enfoques de 10 y análisis situacional con el propósito de retroalimentar las actividades de docencia de la ESPM, con los resultados obtenidos en trabajo de campo

Con el fin de implementar las acciones derivadas de los proyectos arriba indicados, se realizaron diversas sesiones académicas con los alumnos y se programaron visitas de trabajo de campo a la región de Los Tuxtlas, Veracrúz Para ello, hubo que coordinar actividades tanto administrativas como logísticas para los pasajes, viáticos, contactos locales, etc Especial rol jugó la clínica de MEXFAM, ubicada en Catemaco, cuyo Director, el Lic Roberto Barrón, brindó todas las facilidades para conducir los proyectos de investigación

Al 14 de febrero de 1995, el avance de los proyectos llegó al nivel de prueba de los instiumentos que se diseñaron para obtenei diversos indicadores El Anexo 5 describe los protocolos de investigación que se desarrollaron, y el Anexo 6, los instrumentos utilizados haqta su fase de piueba

Tanto César A Macías-Chapula (ESPM) como Ricardo Vernon (PC/Mexico) asesoraron a Leonardo E Cruz Robles en la elaboración de su protocolo de tesis y trabajo de campo sobre el "desarrollo y prueba de un sistema de evaluación que otorgue datos sobre la calidad de la atención en planificación familiar, en la clínica de MEXFAM, Oaxaca, México"

3 5 DISEMINACION DE INFORMACION SOBRE 1 0

Se identificaron loq medios existentes en el INSP para difundir y diseminar información intra e interinstitucional Los resultados de dicho análisis se integraron en un documento (Anexo 7) que explora los mecanismos viables a utilizar para difundir información sobre 10 , por parte del INSP En estas actividades participaron Antonieta Martin (PC/Mexico) y Adriana Robledo (INSP)

Con fecha del 2 de noviembre de 1994, se inscribió al INSP, al Instituto Mexicano de Sistemas e Investigación de Operaciones, A C, con el fin de comunicar y divulgar las actividades del INSP en materia de 10, y a la vez, de participar y conocer lo que se está realizando sobre el área en otras instituciones

Se participó en el 122 Encuentro de la American Public Health Association (APHA), que se celebró en Washington, D C , del 30 de octubre al 3 de noviembre de 1994 El trabajo presentado fué "An analysis of non government family planning service delivery and utilization in rural Mexico in terms of quality and sustainability"

Se participó en el VI Congreso Nacional de Investigación en Salud Pública, que se llevó a cabo en el INSP, Cuernavaca, Morelos, México, del 23 al 25 de enero de 1995 El trabajo presentado fue "Análisis de los servicios rurales y no gubernamentales de planificación familiar en términos de acceso, calidad y autosuficiencia"

Se envió a consideración para ser aceptado en el 123 Encuentro de la American Public Health Association (APHA), el trabajo "Sustaining non-government family planning services in rural Mexico A community oriented model" Este evento se realizará en la Cd de San Diego, CA, del 29 de octubre al 2 de noviembre de 1995

Se cuenta con el primei borrador de un documento que se enviará a la consideiación editorial de una revista científica de carcter internacional

Los cuatro trabajos arraba indicados reportan los resultados preliminares de la aplicación de 1 0 en el INSP, a través del apoyo ofrecido por PC, en materia de asiqtencia técnica en trabajo de campo

En la revista "Mexico Insight", apareció un documental de divulgación, haciendo referencia a las actividades que el grupo realiza en Catemaco, Veracrúz

Finalmente, cabe mencionar que el curso de educación contínua sobre "Investigacion Operativa en Sistemas de Salud", que se realizará en el mes de agosto de 1995, se ha difundido de manera integrada con el resto de los cursos que ofrece el INSP Esto se realiza a través de un catálogo de cuisos cuyo tiraje fué de 5000 ejemplares y cuya distribución se realiza en todo el pais

El Anexo 8 integia las fotocopias de las aportaciones científicas y de divulgación mencronadas

4. PRODUCTOS

Los principales productos obtenidos a lo largo de los dieciocho meses que duró el contiato fueron los siguientes

(a) Se obtuvieion cuatio reportes técnicos, uno sobre la diseminación de resultados de investigación por parte del INSP, otro sobre el avance parcial del contrato, otio más sobre el análisis curricular de la ESPM, y el presente reporte final del contrato

(b) Se participó en tres cursos diferentes de la ESPM, con un total de doce horas dedicadas a 10

(c) Se planeó, piogiamó y piomovió un curso de educación contínua sobre 10, a realizaise en el mes de agosto de 1995, en el INSP

(d) Se inició el desariollo de una colección bibliohemerográfica sobre 10, en la biblioteca del INSP

(e) Se otorgó asistencia técnica a tres alumnos de maestría de la ESPM, para la elaboración de tres protocolos de investigación y dos tesis de postgrado

(f) Se desarrolló un proyecto de investigación (aún en proceso) que implicó trabajo de campo en la región de Los Tuxtlas, Veracrúz, México

(g) Se participó con dos trabajos en dos eventos científicos, uno nacional y otro internacional

(h) Se cuenta con dos trabajos para ser enviados a la consideración de un congreso internacional y de una revista científica

5 CONCLUSIONES Y RECOMENDACIONES

A continuación se describen las conclusiones referentes al logro de los objetivos originalmente planteados y al desarrolio del proyecto en general Según competa, para cada conclusión se emiten las recomendaciones que se consideran relevantes a la institucionalización de 1 0 en el INSP

1 Existe interés por parte del INSP, en particular por la ESPM, para institucionalizar 1 0 Los programas actuales de la ESPM ofrecen la infraestructura y el interés implícito por 10 en sus cursos Tal es el caso de los cursos de "sistemas de información 1 y 11", por ejemplo Sin embargo, pala definir el mecanismo de modificación curricular (sea éste a través de una especialidad, una maestría, o bién, un "tronco común" sobre 10) , se requiere hacer un analisis, además de documental, exploratorio y de consenso con otros actores involucrados en el sistema académrco del INSP Se recomienda continua1 con una segunda fase de análisis curicular, tomando como base el documento inicial que se elaboró para este propósito (Anexo 2) Se recomienda por ejemplo, conducir estudios de mercado para obtener indicadores de parte de alumnos potenciales, sobre el tipo de contenidos curriculares

Por otro lado, se recomienda evaluar y dar seguimiento al curso de educación contínua que se realizará sobre 10, en agosto de 1995 Los comentarios de los alumnos y docentes de éste curso serán de utilidad para integrar una piopuesta curricular sobre 10, en el coito y mediano plazos Ello redundará en la promoción de cursos sobre 10, tanto a nivel nacional, como iegional en América Latina y el Caribe

2 La incorporación de 10 en los cursos de sistemas de información de la ESPM ha iesultado de interés para los alumnos de la ESPM, principalmente aquellos que buscan el grado de maestros en ciencias o en sistemas de salud Se recomienda no solo mantener este tipo de cursos, sino vincular los resultados y experiencias recientes derivadas de la aplicación de 10, por parte del personal del INSP Los resultados derivados del presente contrato por ejemplo, se podrán incorporar a los cursos de 1995, para compartir experiencias con los alumnos, fomentar el uso de 1 0 y promover la participación en proyectos de investigacion o pi otocolos de tesis

3 Bajo este contrato se pudo coordinar el inicio de una colección especializada sobre 10, en la biblioteca del INSP Sin embargo, se requiere elaborar un programa de desarrollo y seguimiento al mediano plazo, que vaya de acuerdo al desarrollo académico y curricular de la institucionalización de 10 Se recomienda por ejemplo, seleccionar y adquirir material didáctico especializado, haciendo una revisión bibliohemerográfica exhaustiva en diversas bases de datos, identificando pieviamente, necesidades específicas de información por parte de académicos y alumnos Una vez adquirido el material, éste se procesará y difundirá selectivamente entre la comunidad usuaria potencial Igualmente, se recomienda conducir estudios de seguimiento al uso de la colección para efectos de promoción y depuración de la misma Una base de datos bibliográfica sobre 1 0 , que incluya la productividad de los académicos del INSP, es altamente recomendable para efectos de difusión y uso de conocimientos sobre 10, en la región

4 La asistencia técnica en trabajo de campo sobre 1 0 resultó ser de gran relevancia a lo largo del proyecto Representó el mecanismo ideal para transferir y aplicar conocimientos en materia de 1 0 El trabajo de campo benefició tanto a alumnos como a académicos e instituciones del área de la salud en la region de Los Tuxtlas, Veracrúz Por otro lado, también demandó tiempo y recursos para apoyo logístico referente a los diversos trámites de pasajes y viáticos Se recomienda buscar un mecanismo más piictico del manejo de los fondos asignados por el PC al INSP, por ejemplo, a través de una adminitración más ágil y flexible y no limitarlos al ejeicicio exclusivo de pasajes y viáticos Ello ayudará a aprovechar los iecuisos de una manera más eficiente

5 En general, los productos obtenidos del contrato rebasaron las metas originalmente planteadas Se logró participar por ejemplo en congresos nacionales e internacionales y se cuenta con suficiente material para su publicación posterior

6 Es evidente la importancia de 10 dentro del INSP y se recomienda continuar en el camlno del fortalecimiento de 1 0 en el INSP Para ello, se recomienda (a) solicitar mayor apoyo al PC, y (b) apoyar el desarrollo de un Departamento de 1 0 en el INSP

El Anexo 9 resume a manera de propuesta al corto y mediano plazos, el programa que se somete a la consideración del PC para este fin

INTRODUCCION:

Los ob-jetivos planteados por el sector salud dentro del

programa de planificación familiar están fundamentalmente

dirigidos a la reducción de la tasa de natalidad y de fertilidad

llevando consigo ayudar a alcanzar el tamaño de familia adecuado

con el espaciamiento de hilos deseados y mejorar los indicadores

de salud materna relacionados con la salud reproductiva, quedando

sin consideración la percepción que tienen los usuarios acerca

de la calidad y eficiencia de los serncios de P.F. recibidos y

sobre el nivel de satisfacci6n entre los mismos

Además del sector salud existen otras instancias no

gubernamentales que dedican sus recursos en la prestaci6n de 1

servicios de salud y en este caso de planificacibn familiar sin

fines de lucro, donde además de buscar y contribuir al logro de

los ob~etivos planteados por el sector salud, tratan de hacer

rentables sus proyectos y servicios a través del patrocinio de

otras instituciones afines o con interés en estudios de población

y por el cobro no oneroso de sus servicios. donde para efectos

de este proyecto estamos considerando a la institución denominada

MEXFAM en una de sus sedes ubicada en Catemaco Ver. La cuál a

través de sus estrategias administrativas y gerenciales busca

elevar la calidad de sus servicios de P.F. contribuyendo con esto

a aumentar su demanda y mantener ba2o su sistema una población

constante que solicite sus servicios

PLAbrmEAMIg#TO DEL PROBLEMA:

Considerando que esto establece una especie de competencia

desigual entre el sector salud, en donde los servicios de

son gratuitos, y los esta

no, dlcha competencia esta dada por la demanda de servicios que

tiene esta instituci6n con relaci6n a las de el sector salud y

que, de acuerdo a los datos de aportados por la institución y

comparados con las cifras oficiales del sector salud el ~mpacto

no evaluado de esta institucibn sobre la tasa de natalidad en la

región de los tuxtlas podria tener cierta significancia en la

misma, por los aAos proteccibn parela que gana con cada método

otorgado. considerando que de acuerdo con los datos de poblacibn

la región de los tuxtlas es donde existe una mayor relación de

hi]os por parela. Por otra parte existen evidencias o

percepciones acerca de la calidad de servicios que otorgan las

instituciones del sector salud, motivo por el cu61 el ptíblico

usuario opta por demandar servicios privados en cuanto su i

capacidad de pago o situación econbmica se lo permite y para el

caso de la planificaci6n familiar, no solo existe el factor costo

sino que se agregan toda una serie de factores de tlpo cultural

y social que influyen de manera directa sobre la decisión de

elegir algún método anticonceptivo, motivo por el cuál las

instituciones no gubernamentales que ofertan este tipo de

servicios mantienen sus estrategias administrativas considerando

estos factores, de tipo cultural social y económico, sin afectar

la calidad de servicios que dicen tener y mantener un flu-~o de

demanda constante de servicios y sin de)ar que el usuario se

salga de su cobertura.

OBJETIVOS GENERALES:

l.-Evaluar la calxdad de los servicios de planificaci6n

famlliar que otorga MEXFAM en la región de los Tuxtlas.

2.- Conocer la percepción que sobre calidad tienen los

usuarios de los servicios de planificación familiar de MEXFAM

OBJETIVOS BSPECIFICOS:

1.- Ubicar el grado de satisfaccibn de los usuarios y su

complacencia por el pago de los servicios de planificacibn

familiar

2,- analizar las relaciones entre satisfaccibn del usuario

y la satisfacci6n del proveedor de servicios

3.- Ubicar el grado de información sobre el método de P.F.

que utiliza la usuaria y el grado satisfacción del mismo.

4,- conocer factores de tipo cultural, social y econbmico

que influyen de manera directa en la decisibn de llevar o

utilizar algún método de planificación familiar

5.- ubicar la capacidad técnica de los proveedores así como F

de los recursos con que cuentan para prestar sus servicios de P. F

MARCO CONCEPTUAL:

Para los fines de este trabalo se utilizaran los marcos

teóricos que sobre calidad de la atención médica ha desarrollado

Avedis Donabedian, y calidad de los servicios de planificación

familiar que ha desarrollado Judith Bruce, considerando en ambos

sus dimensiones, como son ESTRUCTURA PROCESO Y RESULTADO y LA

ELECCION LIBRE E INFORMADA, VARIEDAD DE METODOS ANTICONCEPTIVOS

DISPONIBLES, CAPACIDAD TECNICA, RELACIONES INTERPERSONALES,

CONTINUIDAD OMECANIMOS DE SEGUIMIENTO CON EL SERVICIO Y UN

CONJUNTO DE SERVICIOS APROPIADOS QUE INCLUYAN LOS DEL CUIDADO

MATERNO INFANTIL respectivamente

JUBTIFICACION:

La pertinencia de un estudio de estas características

estriba en los siguientes puntos;

1.- el obleto de incidir en la tasa da natalidad tiene sus

razones tanto demográficas como econ6micas y una de las

estrategias por las cuales se puede se puede llegar a impactar

en ella es la planificación familiar programa que ha sido

retomado con la emisión de la norma oficial para la P.F. emitida

el 30 de mayo de 1994 donde se incluye a la calidad para la

prestación de estos servicios

2.- La calidad de atención en servicios de planificación familiar

viene a ser la piedra angular donde descansa el exito o el

fracaso del programa y lógicamente la afectaci6n directa positiva

o negativa sobre la tasa de natalidad.

3.- no basta el otorgar servicios de planificación familiar desde

el punto de vista institucional, sino se tiene conocimiento de

factores culturales o sociales que afectan de manera directa la

decisión de las pare)as o usuarias, pues el rechazo del programa

en algunas ocasiones obedece al poco respeto que sobre sus

costumbres e intenciones reproductivas tiene cada parela

METODOLOGIA:

El estudio comprende la recolección de información sobre

aspectos organizacionales de la institución así como de sus

estrategias administrativas al respecto, aspectos demogrtificos,

socio-económicos y culturales de la región

-Datos sobre el programa de planificación familiar comprend~dos

en esa 3urisdicción sanitaria para compararlos con los datos

sobre su programa de planificaci6n famillar [etapa ya terminada]

- elaboracidn, prueba y estudio piloto de mstrumentos para la recolección de información específica [etapa ya terminada]

-Determinación del tamaño de muestra para determinar el numero

de encuestas a levantar tanto a proveedores de serviclo como a

usuarios de métodos designados como trazadores.(anticonceptivos

orales y diu) [etapa que termina en la primera semana de marzo

- Adiestramiento a encuestadores dos días de la 3ra semana de marzo de 1995 [etapa pendiente programada para el dia 11.12.18,19

de marzo de 19951

- Levantamiento de la informaci6n de a través de las encuestas. durante el mes de abril

-Elaboraci6n de la base de datos y captura de la información

del 8 al 31 de mayo

- del 1 de )unlo al 15 de julio análisis estadlstico de la base

PROJECT DESCRIPTION

The present diagnostic study addresses the concern of family planning organizations' capabilities to successfully implement fees or donations as 2 cost recovery intervention towards sustainability. Based on the assumption that quality 1s an important factor, this study attempts to analyze the process and organization of service delivery and identify the socio-demographic and economic factors that influence utilization patterns, the users' satisfaction with the cervices they receive, and their willingness to pay.

Using both quantitative and qualitative instruments, this study will apply the principals of Operations Research and more speciflcally, Situation Analysis, to evaluate the structure an operatlons of MEXFAM in terms of its goals towards quality service delivery and financia1 sustainability

It 1s estimated that by the year 2000 the cost of providing family planning in developing countries will reach approxmately $2 6

billion (Janowitz et al., 1990). To meet the anticipated shortfall in public funding, international donors are pressing local family planning organizations to become more self-sufficient and encouraging the non-government organizations (NGOs) and commercial sector to assume an increased role in the provision of family plannlng services.

U.S.A.I.D. is a malor funder of family planning programs and has initiated a 5 year phase-out effort aimed to discontinue the provision of fully subsidized contraceptives supplied to its pro~ects worldwide. In the case of Mexlco, the ma]ority of A.1.D support for family planning has been channeled largely through non- government family planning associations and missions and, to a lesser degree, through government agencies. Thus, the trend in family planning programs is towards sustainability to offset the gradual reduction of externa1 funding Mexico 1s one of the countries that has the capability to operate independently relatively sooner than some other countries due to its infrastructure and financial potential.

WIDER OBJECTIVE- To evaluate the services of a non-government (NGO) provider in a defined geographical location in terms of the quality of service provision and to explore the variety of socio- demographic and economic factors that influence utilization patterns, the usersf satisfaction with the service they receive, and their willingness to pay for those services.

IMMEDIATE OBJECTIVES:

1) To analyze the process of providing family planning within a rural primary health care setting.

2) To analyze what perceptions providers have of effective 56Xservice and how satisfied they feel with the services they deliver.

3) To analyze what perceptions users have of effective service and how satisfied they feel with the servlces they receive.

4) To analyze the relationship between usersf satisfaction and their willingness to pay for services.

5) To assess the financia1 capacity of the user population and their ability to pay for MEXFAMfs services

6) To assess MEXFAMfs policies and the process of reaching its goal of becoming economically sustamable

7 ) To assess the social and cultural factors that may modify the relationship between satisfaction and willingness to pay

THE TARGET GROUP:

The study will be conducted in Catemaco, Veracruz and surrounding areas in the primary health care facility of MEXFAM and its targeted communities (inhabitants >lo0 and ~1000). Samples will be drawn from MEXFAMts 42 target communities located within the 4 counties of Santiago Tuxtla, San Andrés Tuxtla, Catemaco, and Hueyapan de Ocampo. These counties are al1 rated high in terms of socio-economic deprivation on a state and national scale. The state of Veracruz has been identified as high priority gmen the large number of rural communities without access to family planning and primary health care services where they reside. The study will focus on users, providers and service delivery at the MEXFAM site and in the communities that they serve. It will also include the feedback of key managers and pol;cy makers within the organization

PROYECTO DE ASISTENCIA TECNICA PARA DIWLGACION E INSTITUCIONALIZACION DE LA INVESTIGACION OPERATIVA

INSTITUTO NACIONAL DE SALUD PUBLICA CUERNAVACA, MEXICO

REPORTE DE ACTIVIDADES OCTUBRE-NOVIEMBRE, 1993

El Instituto Nacional de Salud Pública (INSP) y el Population Council han celebrado un convenio con el obletivo primordial de difundir e institucionalizar el uso de la investigación operativa en programas de salud.

El INSP consta de tres divisiones principales: la Escuela de Salud Pbblica (ESP), el Centros de Investigación en Salud Pública (CISP) y el Centro de Investigación Epidemiológica xxxx (CIEI) . Las actividades que se realizarán dentro del marco de este convenio serán primordialmente con la Escuela de Salud Pública. Se considera que si se consigue la institucionalización de la enseñanza de la investigación operativa como una especialidad de la Maestria en Sistemas de Salud, el modelo podria replicarse en otras Escuelas de Salud Pública en México y el resto de América Latina y el Caribe

Entre los ob]etivos que se proponen en la carta de intención se encuentran los siguientes.

OBJETIVOS:

Apoyar la enseñanza de la Investigación Operativa en la Maestría en Sistemas de Salud y en los programas de Educación Continua de la Escuela de Salud Pública de México.

Recopilar bibliografía técnica, incluyendo articulas, ponencias, libros, instrumentos, manuales y material audiovisual sobre investigación operativa

Reforzar la divulgación e institucionalización de la investigación operativa en el INSP

ACTIVIDADES:

1 Primero se desarrolló un diagnóstico de flu-jos de información en el INSP, que ha permitido conocer los elementos de comunicación interna de la organización, asi como sus productos de divulgación tanto al interior como al exterior

En una de las primeras sesiones de trabajo, el Dr Luis Durán, Director de Ciencias Sociales y Administrativas, reiteró que espera

que con este proyecto (a) se fortalezca la docencia e investigación por medio de la cooperación entre el Population Council y la Escuela de Salud Pública de México; (b) se incrementen los acervos de información sobre investigación operativa por medio del intercambio de materiales; y (c) se difundan los resultados de evaluaciones que hacen ambas instituciones y se discutan estrategias para alcanzar distintos públicos.

El Dr. Durán solicitó que la coordinación del proyecto se haga a través de Leleune Lockett, Becaria de la Universidad de Michigan que se encuentra en el INSP. Hasta la fecha, se han tenido reuniones de trabalo tanto en la Ciudad de México como en Cuernavaca, con las Direcciones de Información y Publicaciones y de Desarrollo y Extensión Académica y con sus diferentes subdirecciones y departamentos, para estudiar las posibilidades de traba30 colaborativo La estructura orgánica del INSP se muestra en la Figura 1.

2. La primera visita a las instalaciaones del INSP tuvo como ob-~etivo colaborar con Leleune Lockett en la impartición de un cursillo de Investigación Operativa a estudiantes de la Maestría en Sistemas de Salud Esta experiencia fue muy dtil para detectar la posibilidad de realizar talleres, seminarios y cursillos en Investigación Operativa para los estudiantes de la Escuela de Salud Pública

El Cursillo de Investigación Operativa dio oportunidad de conocer las necesidades de los estudiantes y el interés que despierta el uso de investigación operativa para megorar los servicios en los programas de planificación familiar Debido a que en general los estudiantes de esta maestría son trabaladores de la salud pdblica que están prestando servicios en alguna institución pública del país, son un público al que nos interesa mucho capacitar en el uso de investlgaclón operativa para que puedan

aplicarla en sus programas de salud. Especificamente, se detectó que los estudiantes*

Tienen mucho interés en saber qué es la investigación operativa.

m Necesitan materiales de consulta en investigación operativa

Requieren asesoria personal para dirección de tesis en investigación operativa.

RECOMENDACIONES

Establecer becas para estudiantes de la especialización en Investigación Operativa.

Realizar seminarios y talleres periódicamente para el personal académico y estudiantes de otras maestrias de la Escuela de Salud P6blica.

3, Dirección de Información y Publicaciones

La reunión de trabalo que se tenla planeada con el Dr. Malaqulas López, Director de Información y Publicaciones, no pudo efectuarse debido a compromisos del Dr. LÓpez. Sin embargo, la sesión de ese dia se llevó a cabo con el Sr. Carlos Oropeza, Sub- Director de Publicaciones y con las Jefes de los Departamentos de Edición y Producción, Dra. Lucila Pacheco y Lic. Patricia Avila, respectivamente, y sus equipos de trabalo. En esta reunión se discutieron tanto los productos impresos como los procesos de producción. A continuación se describen las publicaciones del INSP.

SALUD PUBLICA BE MEXICO

Revista Bimestral, publicada desde 1959, actualmente con un tirale de 3500 elemplares. Salud Pública de Méxlco ha sido aceptada por Current Contents, Index Medicus, Index Medicus Latinus, Institute for Scientific Information, Social Science Citation Index y otros indices internacionales que registran publicaciones cientlficas con un nivel de calidad uniforme y reconocido La revista publica alrededor de 250 articulos al año, en idioma español. Existe gran demanda de los autores para publicar en la revista y generalmente hay una larga lista de espera de artículos que ya han pasado todo el proceso de revisión y que han sido aceptados para su publicación.

La línea editorial de Salud Pública de México es temas de salud ptíblica. El Comité Editorial que establece las politicas generales, está formado por personalidades nacionales e internacionales, incluyendo al Secretario de Salud de México.

Cada año, en el número 6 de la revista se publican los traba~os del Congreso Nacional de Investigación en Salud Pública. La revista Salud Pública de México tiene también Editores "Huésped1@ para número especiales, por elemplo, tuvieron un nlímero especial de Calidad. El financiamiento de la Revista proviene en su totalidad del Presupuesto Federal del Gobierno de México, salvo cuando hay suplementos o publicaciones especiales que son financiadas por agencias no gubernamentales, por e3empl0, la Fundación Mexicana para la Salud. La revista no es autofinanciable, pero recupera algunos de sus costos por medio de venta de suscripciones La Revista tiene convenios de intercambio con la OPS y otras agencias internacionales.

PERSPECTIVAS EN SALUD PUBLICA

Esta serie fue iniciada en 1986 y a la fecha ha publicado 14 números cuatro de los cuales están en inglés. La serie difunde resultados de investigaciones, conclusiones y recomendaciones de conferencias, seminarios y talleres, revisiones bibliográficas sobre un tema especifico y tesis de maestria o doctorado que aporten conocimiento nuevo. Su Comité Editorial está formado por especialistas en diversos temas. Los trabajos propuestos se someten a dictaminación. Su tira-Je es de 2000 e~emplares

SINTESIS EJECUTIVA

Esta serie está dirigida a personas que toman decisiones, por lo que maneja información resumida y con lenguaje accesible, sobre los hallazgos más relevantes de las investigaciones del INSP Se han publicado dos volúmenes de Síntesis Ejecutiva que contienen 25 Sintesis de trabajos.

PORTAFOLIOS EJECUTIVO

Serie dirigida a tomadores de decisiones con un enfoque en un sólo problema, presentando hallazgos y proponiendo soluciones El primer volumen de la serie contiene 13 trabalos producidos por investigadores del INSP.

RECOMENDACIONES

Que el Population Council promueva la Revista Salud Pública de México entre las instituciones con las que colabora en la región, tanto para que compren suscripciones como para que los investigadores interesados puedan publicar sus articulos sobre proyectos de investigación operativa y de salud pública en general.

4. Dirección de Desarrollo y Extensión Académica (DDEA)

La Dra. Lucero Rodrlguez es la encargade de esta Dirección y tlene a su cargo lo que es propiamente la divulgación interna y

externa del INSP. La Dirección de Información y Publicaciones termina su responsabilidad con la producción de los impresos, la Dirección de Desarrollo y Extensión Académina es responsable del flulo de la comunicación interna y externa del Instituto. Como comunicación interna manelan el boletin mensual Ayocuan y un boletfn menos formal dirigido al personal académico, en donde se les informa de las actividades de la semana.

AYOCUAN

Por lo que respecta a comunicación interna, el INSP tiene un Boletfn mensual que se llama Ayocuan. El boletfn circula a 150 lectores del personal académico, de apoyo, y administrativo. Los temas de Ayocuan son sucesos importantes del Instituto, entrevistas a los investigadores, difunde sucesos de interés académico, publica el programa de educación continua.

= INSPTANTANEAS Existe también una publicación, muy informal, casi como un

volante que se llama INSPtantáneas, donde se dan avisos y notlc~as en general al personal del INSP Aparentemente estos volantes los leen más los empleados al nivel administrativo.

RECOMENDACIONES

La Lic. Adriana Robledo recomienda hacer una evaluación de Ayocuan para saber si el boletin tiene el formato, la periodicidad y los contenidos que el personal desea. Parece que nunca se ha hecho algún cuestionario para conocer las opiniones de los lectores. No se sabe si el personal en todos los niveles lo leen, especialmente al nivel de apoyo y administrativo.

Usar Ayocuan e INSPtantáneas para promoción de actividades de Investigación Operativa dentro del INSP.

Las actividades de comunicación externa comprenden difusión a través de boletines de prensa, convocatorias a nlvel de prensa regional y difusión a través de la Dirección de Comunicación de la SSA. Publican entrevistas y artículos de divulgación en las secciones especiales de los diarios de circulación nacional Tienen también acceso al Canal 11 y al Canal 3 local

La DDEA tiene una base de datos de todas las Escuelas de Salud Pública de América Latina y otra base de datos de funcionarios, rectores de escuelas de salud pública, hospitales privados y asociaciones profesionales de México Tienen una base de datos más por institución, no por persona Las bases de datos de funcionarios es la que consideran más importante y la mantienen actualizada telefónicamente. Sus sistemas de enios es por estafeta o servicio similar y actualizan inmediatamente cuando se les devuelven los sobres Tienen convenios con facultades de medicina

para distribución de materiales y por vía de las embajadas de México en el extranlero envían sus programas de Educación Continua de la Escuela de Salud Pública de México Consideran que deben reforzar y actualizar sus bases de datos.

Recommendation* Establish OR advisory board of individuals in Latin America and the Caribbean.

Recommendation Collect materials to place in a designated "OR sectionw of the library at INSP.

Action A meeting was held (Dec. 12, 1993) with Andreé Lera, the INSP Library Director, Toni Martin and Leleune Lockett. As a result, an agreement was made to receive additional materials from the Population Council to enhance INSPts collection of references. Once the library completes a reorganization process, they will be prepared to file and cross reference OR information in a more easily identified manner. 1. Leleune Lockett, Luis Duran and Cesar Macias are designing an family planning evaluation prolect the applies the techniques of Operations Research The study will evaluate non-government services provided by MEXFAM in rural Veracruz, Mexico and the

project is planned to be completed by January 1995

Recommendation: Use this prolect as an opportunity for INSP masters students interested in Operations Research to gain practica1 experience, help establish OR in the INSP, and apply the data to the writing of their masters thesis. Representatives from both INSP and the Pop Council can serve as thesls advisors The students can continue the procecc of Documentation of OR Materials through their literature search for the prolect. They can also play an integral role in the Development of an OR Currlculum within the INSP.

action: Ricardo, Luis and Leleune met (Sept. 21, 1993) and discussed provlding field training to INSP masters students and creating an institutional link via Pop Councilts technical support. Luis and Ricardo met on (Jan. 4, 1994) and dlscussed the possiblity of providing financia1 support for 2-3 students to participate in the prolect and also discussed the activities mentioned above that the students can complete Lejeune has recruited 3 students and they are now writing thelr thesis proposals based on this study design. Planning meetings with the students and the host organlzations for the study will begin January 26, 1994

ANALYSIS OF OR CURRICULUM DEVELOPMENT

To date there 1s no official OR curriculum established within the INSP. The masters programs and extension courses would be an avenue in which to incorporate OR concepts.

Recommendation: Develop a course for masters students who have an interest in the techniques of OR and/or may want to specialize in OR withln their academic training. We must first analyze the existing curriculum to see what may be lacking and to identify the most appropriate avenues to integrate the concepts of OR

Ton1 and Leleune collaborated in givlng an OR lecture to students of a masters leve1 Health Information Systems course in Sept. 1993 at INSP This course gave the students the opportunity to understand the usefulness and relevancy of OR techniques to improve health cervices and decision making The students atthe INSP are generally public health professionals involved in service delivery and recearch Due to organizational changes wlthin the INSP it was necesary to delay this segment of the collaboration pro-ject and meanwhile focus on the development of the other activities. However, with the participation of the students, the planning of the curriculum will resume.

CB 24 Evaluacion de la atencion medica hospitalaria Con u n indice de garantia de calidad (INGAR) en el caso de la muerte materna Estudio de casos y controles Canales Muñoz JL, Rodrigez Arias E Hospital de Grneco Obstetnoa lnstrtuto Mexrcano del Seguro Social La muerte materna es analizada por los comites medicos enfatizando los aspectos fisiopatologicos y clinicos sin dar un peso especifico al proceso de atencion rnedica La variabilidad esperada de este proceso segun las condiciones de las gestantes no esta sistematizada lo que conduce a decisiones medicas inco- rrectas que pueden asociarse con baja calidad de la atencion La garantia de calidad implica la adecuacion permanente del proceso de atencion rnedica a las condiciones de salud Objetivo determinar la asociacion de la calidad del proceso de atencion rnedica hospitalaria con la sobrevida materna en el Hospnal de Gineco Obstetricia de la Delegacion Jalisco del lMSS Metodologia se hizo un estudio de casos y controles, ambidireccional con seleccion de controles hospitalarios segun el quinquenio de edad y paridad de los casos pero sin pareamiento Se aplico un Indice de Garantia de Calidad (INGAR) construido por ocho indicadores del proceso de atencion con base en normas instducionales recomendaciones del Comite de Garantia de Calidad del Hospital y de la informacion cientifica disponible y que fue validado pre

- viamente Resuiiados se estudiaron 46 casos y 157 controles, no hubo diferencias porcentuales en edad y paridad en los casos hubo 50% mas mujeres con baja escolaridad En ambos grupos se encontro un elevado indice de control prenatal inadecuado (70 0%) De los ocho indicadores del proceso de atencion hubo direrencias en la congruencia diagnostico terapeutica (RMs 14 8 p <O 0000) v medicion obstetrica (RM 1 69 p=ns) El INGAR global (punto decorte=2 0)

- distinguio tres grupos el primero (casos 48% y controles 82%) con un INGAR menor a 2 O segundo con valores hasta 3 5 tercero con valores hasta 4 5 en ningun subgrupo se encon traron diferencias Al ajustar el INGAR la RM se elevo a 2 59 veces (IC 1 11 6 03 p=ns) para atencion de baja calidad en los casos Conclusiones e l INGAR determino los componentes del proceso que no garantizan la calidad de atencion medica Se sugiere evaluar las normas de atencion medica obstetrica y su adecuacion segun las caracteris ticas sociales y biologicas de las gestantes modificando el numero tipo y caracteristicas de los servicios e incluso el tipo de personal e instalaciones requeridas

CB 26 Construccion y validacion de u n instrumento de medicion para satisfaccion laboral del medico familiar Garcia Peña C Reyes Lagunes I Reyes Frausto S, Villa Contreras S, Libreros BangoV Martinez Gonzalez L MuñozO Instrfuto Mexlcano del Seguro Social Objetivo construir y validar un instrumento para la medicion de la satisfaccion laboral de los medicos familiares Material y metodos se definio teoricamente e l constructo de satisfaccion laboral, posteriormente se utilizo la tecnica de red semantica para la construccion de los items Se eligio la escala de tikert con diferencial semantico Se diseño el cuestionario dando un orden aleatorio a los 80 items finales y se aplico a nueve medicos seleccionados aleatoriamente por cada item en la dimension con mayor numero de ellos para su validacion Resuitados elconstructo quedo integrado por las dimensiones satisfaccion laboral instituto de salud donde trabajo mis pacientes y yo como medico El indice de discriminacion fue adecuado (pcO 001 ) Losvalores para alfa de Cronbachvariaron entre O 76 a O 94 En el analisis factorial se obtuvieron cuatro factores en cada dimension a excepcion de mis pacientes que obtuvodosfactores Estos factores explicaron masdel 44% de la varianza del constructo elaborado Conclusiones el instrumento de medicion discrimina adecuadamente tiene alta confiabilidad y los factores obtenidos verifican la validez de constructo La tecnica de red semantica resulto ser util para la construccion del instrumento de medicion

War against disease Barron and Torres, both 44, want campesinos to seek preventative rnedical care Most peasant farmers distrust government health facilities because they are staffed by overworked physicians who are often unfamil- lar with indigenous languages and customs and don t have time to learn

Campesinos even accuse government health officials of dishon- esty, pretending to eradicate dis- cases when in fact they are not For example, Health Secretariat officials say t u b e r c ~ ~ l o s i s has been eradicated in the countryside, bcit Barron and Torres are finding i t is still prevalent

President Carlos Salinas d e Gortari's Solidaridad program was a new approach to reach rural communities by providing - free of charge - materials to build their clinics and doctors to staff them It is now in 16 states By building their own clinics, i t was believed communities would take a more active role in their health care

However, many facilities were never built because of apathy and in some areas the government- appointed leaders in charge of dis pensing the IMSS-Solidaridad funds were accused of stealing the money Another damaging aspect to the program says Gornez is that i t contracts with inexperienced physicians recent medical g rad~i ates and only for one year Gomez believes that tht Catemaco clinii could scrve as a role rnodel for the couiitry

MexFam gave them their grant to start the clinic in Catemaco since the agency had already penetrated the state with family planning programs

But people in the medical field scoffed at their idea, warning them i t would be impossible to make money off the poor Plus gaining the trust of campesino belief systems - which differ among region and ethnicity - would be so diffi cult i t wouldn't be worth the both- er Now the husband and wife team is proving naysayers w rong The key they discovered is respecting local customs

Temich went to the Catemaco clinic For surgery since government health officials failed to detect the tumor They just gave her pain killers and sent her on her way a three-hour trek back to her vlllage Although government facilities are free, they aren t uorth the trouble, she says

A U S health official working with the Catemaco clinic says the experience has taught her that pre- vention and doing more with less could help her nation cut crippling health care costs

' I'm impressed at how much they're able to d o with so little resources," says Lejeune Lockett, 30, who worked at the University of California Medical Center in Los Angeles She is n o w providing technical assistance on family planning and population issues to the National Institute of Public Health in Cuernavaca and was sent to the Catemaco clinic

Uncertain future Government officials, however have not reacted positively to the clinic Instead of embracing it as the wave of the future, they have treated i t as a threat and a pariah

In February, the Health Secretariat in Catemaco chut the clinic down for two days charging that there weren t enough beds in the facility S surgery recovery room After severa1 thousand people, including campesino organizations and town mayors throughout the Sierra Tuxtla region signed peti- tions to keep the clinic open Health Secretariat officials from Mexico City opened i t back up

Barron accepts the victory warily just one more worry of what lies ahead Until then they will contin- u e their work with hopes of expanding to other states

The present aagnoshc study employs the concepts of Situatron Analysis to assess factors mfluenang the abhty of MCSH to successfully implement fees as a cost recovery mkrvention towards sustamb11ity Based on the assumptm that quality s an mportant factor, this study uses the models of Judlth Bruce and of A Donabedim as conceptual framework to develop mdrcators related to the qWty of care, the users sabsfactm wth those semces, and their lfnllingness to pay for the services they receive

Fmadly, among the populatms served by MSCH there are man9 cultural characterrsbcs related to values, customs, and language that are mque In the region Terberos" (herbahsts), "hueseros" (bone setters), 'curanderos' (healers) and 'parteras' ~ d w v e s ) are among the traditionl medlme practiboners that are wdely used rn ths region of Veracruz In fact, Catemaco is famous for its Wqos' (wtches) that perform Irmpias' Qmtual cleansmgs) for l d s and the many vsitors that seek theu services

Although there are many strengths in the objectives, orgamzation and process of the MSCH semce dellverg, there are factors that are weakness and propose a threat to its efficiency and survival in terms of sustamabhtJF Frrst, the Brigade IS a useful tool m the provismn of b s ~ c health services and the promobon 01 the 4th level cbfi.ic!hospital fa&ty However, it does not prove to be cost-effiaent Frequently, the money that is recovered from pabents seen does not cover the cost of sendrng the brigade Although one of the crrterion for the Brigade to go out is a confumed hst of a least 20 pattents previously scheduled by the Cornmulllty Assistant, depending on the health motives and rf there IS no need for further referral t~ the ch ic in Catemam, the costs are not recovered

The answer to this question can only be generated after further investigation and analysis Although not wrt2u.n the realm of tlus prbcular study, a detzuredt financial evaluation of sustamabiltty would also be very beneficla1 to MSCH The 4 instruments that we are currently developmg wrll provide us wth further data regarding an inventory of material and human resowces, satqfachon of the users and the techrucal capacity of the providers as related to the provisi011 of farmly plsnning services We plan to pilot the iflStfillllents in

INTRODUCCION

A ocho afios de fundado, el INSP es todavia una inst~tucion joven pero a la vez suficientemente consolrdada y madura Entre sus objetlvos destacan realizar mvestigacion en temas de salud pliblica que contribuyan al conoc~miento de 10s problemas prioritarios del pals en mater~a de salud, y formar recursos humanos a nivel superior en las diversas areas de la salud publica

En estos ocho afios de trabajo el I N S P ha definido su actuac16n en el area de ~nvestigacion siguiendo el desarrollo de ocho lineas que considera prioritarias y con las cuales se atiende a la mayor parte de las necesidades de investigaci6n en salud pubi~ca que requiere el pals Estas lineas son las slguientes

Del modesto grupo rnrclal de 14 rnvestigadores que tuvo orrgrnalmente el Centro de fnvestrgaclones en Salud Publrca en 1984, el lnstrtuto cuenta hoy en dra con un

equrpo de 252 acadernlcos y profeslonlstas, haclendo de esta una de las mayores

concentracrones en America Latlna de personas con estudros avanzados en las drstintas drscrplrnas biomedrcas, socrales, epidemrologrcas y admlnlstratrvas que trenen relacron con la salud publrca

Para la formacron de 10s recursos humanos que requreren las tareas de salud pibllca, el I N S P ha ofertado 6 programas academrcos en el crclo escolar 1994- 1995, destacando por su relacr6n con las tareas de rnvestrgacron, la Maestrra en Crenc~as de la Salud, con sus areas de concentracron en Salud Reproductrva y Srstemas de Salud entre otras, destacando tambren las Maestrras en Adm~nrstracion de Servicios de Salud y Salud Publrca, esta ultima ahora bajo su modalrdad en Servrcro, con lo que se permlte capacrtar a drstancra un mayor numero de drrectrvos y jefes de servrcro en su propra area de trabajo

Este objetrvo de formaclon de recursos ha permltldo a la Escuela de Salud Publica de Mexico (ESPM), graduar a 574 estudlantes de sus programas de maestrias y espec~al~zac~ones, estando en espera de egresar a la prlmera generation del Doctorado en Clencras No menos rmportante, otras tareas de capacitacron se desarrollan tanto dentro de 10s muros de la rnstrtuc16n como fuera de ellos, lo que ha permltido capacrtar a 3,774 estudlantes en dlversos cursos de educacron contrnua desde 1986

El area de lnvestlgacion de Operacrones (10) 1nrc16 su desarrollo en el INSP en junlo de 1993 En esa fecha, a traves del Programa de lnvestlgacion de Operacrones y Sistemas de lnformacron (IOSI), se def~nicj la vrabrlrdad de un convenlo de colaboracion con el Population Councrl en Mexico y posterror a varras reunrones de trabajo con la frnalidad de desarrollar esta area del conocimrento El 24 de agosto de 1993, se frrrno dlcho convenlo con lo que se rmpulsaron las acclones para la formaclon y asesoria de alumnos con un mayor rnteres en 10, permrtiendo as1 una nueva alternatrva en el analrsrs y desarrollo de protocolos de tesis para ellos A traves de dicho convenlo, se condujo un estudio sobre la rnstrtucronalrzacron de 10 en el INSP, y se rnrcraron actividades de desarrollo en ese camino

Las actiwdades se resumen en las cinco areas estrategicas sigulentes

Con respecto al desarrollo curricular se ha elaborado un revlslon de 10s programas de educacion superror de la ESPM, anallzando las unidades d~dacticas de todos 10s cursos ofrecldos durante 1993 Se hrzo una selecclon de la unidades

dldactlcas relaclonadas con 10 y se tuv~eron d~versas d~scuslones sobre la

rnclusion de I0 en unidades dldactrcas de cursos selecc~onados

Con referencla a la asrstencla tecnlca en ensetianza, se identiflcaron las unrdades didacticas donde se requiere dicha asrstenc~a de manera rnmedrata De esta manera, personal del programa de lOSl y el Population Council partlciparon en el curso Srstemas de lnformaclon Il, dentro las Maestrlas en Clenc~as con areas de concentracron en Sisternas de Salud y Servrclos de Salud Asi mlsmo esta lncorporacion de material de 10 tambien se ha llevado a cab0 en 10s cursos de educacion cont~nua destacando el brindado a D~rectivos y Jefes de Servrclos de la Secretar~a de Salud en el Estado de Oaxaca

Con referencla a la comprlac~on de mater~al tecn~co y-b~bl~ograf~co, se Ilevo a cab0 un d~agnost~co s~tuacronal del acervo ex~stente sobre 10 en la brblroteca del INSP De ~gual rnanera, se detectaron las neces~dades de rnater~al bibl~ohemerograf~co y se ident~frcaron 10s organlsmos e mst~tuc~ones que estuvieran en pos~bilidad de envrar documentos y publicac~ones sobre 10 a la bibl~oteca del lNSP Esta

actrvrdad ha permrtldo la actual~zao~on del acervo exrstente sobre 10 en la biblroteca

Con respecto a la aslstencla tecnlca en trabajo de campo, se asesoran dos tesls de maestria en crenclas y un protocolo de rnvestlgaclon sobre la sustentablhdad de servlcros de planlf~caclon famlhar no-gubermentales en la region de Los Tuxtlas, Veracruz, Mexlco A1 respecto, se ha vlsltado en dlferentes ocaslones la cludad de Catemaco con el propos~to de conocer la vlabllrdad del proyecto y de establecer un marco conceptual para ident~f~car 10s elementos relac~onados con 10 al n~vel local

Finalmente, sobre la disern~nac~on de tnformaclon relaclonada con 10, se han tdentlflcado 10s medlos actuales de drfus~on de informaclon del INSP, y se ha planeado una campafia de promoclon sobre 10 dentro y fuera del INSP Por otro lado, se tlene preparado un manuscrrto lrsto a ser publlcado, sobre las apllcac~ones de 10 en el area de la salud

PROPUESTA Y PERSPECTIVAS

A lo largo de doce meses de actlvrdades sobre la rnst~tuc~onalrzac~on de 10 en el INSP, se puede conclulr lo slgulente
