Prof. Cindy L K Lam, our Chief Censor and Honorary Fellow (2011), was awarded the Medal of Honour (MH) (Chinese: 榮 譽勳章, MH). Our College is also deeply grateful for Prof Young and Prof. Lam’s dedication over the past years, which have substantially advanced the discipline of Medicine. We would like to send our heartiest appreciations to their outstanding contributions to our community. I have just attended the Advisory Committee on Primary Care Directory. The important discussion point is the Implementation of maintenance for listing in Primary Care Directory. I must stress that the College assists Members to gather their CME points and through the Hong Kong Academy of Medicine, a CME Administrator to submit to the Medical Council. The College provides the most diverse CME/CPD activities for primary care doctors in Hong Kong. Primary Care Office (PCO) is to proceed with the checking mechanism proposed and to exercise discretion for the first round of CME/CPD checking in 2017 for all existing enrolled doctors and dentists in the Directory. For those not fulfilling the requirement they would be inactivated from the Directory if such requirement is not fulfilled upon the second round of CME checking in 2018. Enrolled doctors are to attain at least 30 CME points for the period 1 January to 31 December 2017, or 1 July 2017 to 30 June 2018 in order to obtain the Medical Council of Hong Kong (MCHK) CME Certificate for that period for maintaining in the Directory. Subsequent application for re-activation will require submission of the CME Certificate issued by the MCHK for the past calendar year, i.e. 2018. Under the CME Programme for Practising Doctors by the Medical Council who are not taking CME Programme for Specialists, a doctor who has acquired 90 or more CME points in a “three-year CME cycle” will be given approval to use the title "CME-Certified”. On the topic of analogy of DHC to a Mega Health Maintenance Organization (HMO), the Bureau is to include conditions in the tender application to prevent preferential treatment or resource allocation to a particular HMO’s affiliated doctors. All Primary Care Directory listed doctors working in the district will automatically be included in the network of doctors unless he/she opts out. There will be no doctors in the DHC Hub or Satellite Centres. The Service Model of DHC is to provide primary prevention activities and some secondary prevention activities. Tertiary prevention activities will not be provided by DHC and must be implemented by doctors only. There will be no “reverse referral”. All referrals to dietitians, physiotherapists or occupational therapists will only be made by doctors. The DHC will emphasise patient empowerment. There will be a fixed subsidy or reimbursement scheme to doctors in the network for care of patients with chronic diseases. For new patients who have been triaged by DHC and referred to network doctors, government subsidy will be provided for 2 consultations, one for diagnosis and one for initial assessment. For network doctors’ “old” patients with these known chronic diseases, government subsidy will be provided for one consultation each year for annual assessment. Doctors’ involvement in advising, monitoring and assessing DHC services is an integral part of the plan and the significance is recognised. I have just attended the Advisory Committee on Primary Care Directory. The important discussion point is the Implementation of maintenance for listing in Primary Care Directory. DHC will fill the present gap of difficult access to allied health services in the private sector in a particular district. We have every reason to support the establishment of the DHC, to utilise them and to consolidate our role as patient’s family doctor.
Our College has lost a great comrade in the path of establishing Family Medicine as a discipline in Hong Kong. There are no words to describe the tremendous sadness we felt upon hearing the shocking news of the passing away of Dr. Siu Che Hung, Paul on 31 July 2018. Dr. Siu had selflessly served the College for many years. He had held a number of important posts in the Council in the early days
of our formation. He was a long time Council member and Examiner of our College with enthusiastic participation in different Boards, Committees and College activities. He was a towering figure and impressed us with his pony tail hair style. Last year he received the long service award as Examiner of the Conjoint Examination at our Conferment Ceremony in 2017. Dr. Siu was one of our leaders and his contribution to the development of Family Medicine in Hong Kong will never be forgotten. The friendship and memory of Dr. Siu with the College will always be long lived and be in our hearts. May he rest in peace!

It came as the saddest news to know that our longtime friend Dr. Siu Che Hung, Paul passed away on 31 July 2018 after a brave battle against a cancerous disease.
Paul graduated at The University of Hong Kong and had special interest in Obstetrics and Gynaecology in the initial years after working at Alice Ho Miu Ling Nethersole Hospital, United Christian Hospital and Our Lady of Maryknoll Hospital. He soon developed keen interest in Family Medicine and attained the Fellowship of our College. He had been working as Council member, examiner and member/convener of various committees of the College for years.
Paul had actively committed to community works, too, medically or non-medically related. He had served in prominent positions in many community committees, including St. John’s Ambulance Brigade, Hong Kong Road Safety Association, Wanchai District Junior Police Call etc, just to mention a few. He was a District Councilor of Wanchai District 2004 – 2011.
Paul had diversified hobbies, ranging from yachting, golf, and fast cars in the early years, to reading and gourmet food hunting in the retiring years.
The deadly disease came hideously one year ago, and he faced the treatment calmly and bravely.
Paul is survived by his wife Judith, and daughter Yvonne.

A church service will be held at 2:30pm on 25 August 2018 (Saturday), in St. Mary’s Church at 2A Tai Hang Road (just opposite St Paul’s Hospital), to be followed by cremation in Cape Collinson.

Hong Kong Primary Care Conference 2018“Family Physician: Nexus of the New Era of Primary Care”
Our 8th Hong Kong Primary Care Conference was successfully held from 23rd to 24th of June 2018, with overwhelming response of over 500 registered delegates. Once again, this annual hallmark conference has unfailingly shown its importance as a fertile platform for bringing family physicians, academicians and various primary care professionals together to enhance primary care.
This year’s theme “Family Physician – Nexus of the New Era of Primary Care” rekindled the vital role of family physicians in responding to the ever challenging healthcare ecosystem and to delivering cost-effective, sustainable and patient-centred health care. The conference was graced with eminent icons in Primary Care including Prof. Sophia Chan (Secretary for Food and Health, Food and Health Bureau, HKSAR), Prof. Donald Li (The WONCA World President-elect), Dr. Constance Chan (Director, Department of Health, HKSAR), among many others. We were honored to have three distinguished plenary speakers, namely Dr. Constance Chan, Prof. Richard Hobbs and Prof. Samuel Wong, who have inspired us on the increasing role of Family Medicine in building and sustaining a robust primary health care system to meet the various challenges.
Besides the stimulating and thought-provoking plenary lectures, we also had a wide array of relevant and well-structured programmes delivered by a range of excellent speakers with diverse backgrounds and experiences.
I would like to take this opportunity to express my sincere appreciation to the officiating guests, speakers and moderators for their valuable support, sponsors for their generosity, conference advisors for their guidance, all members of the Conference Organising Committee and secretariat for their hard work, last but not the least, all the delegates for their participation which have made the conference a huge success.

We are extremely grateful towards the government officials, university colleagues, our sister colleges’ representatives from overseas, representatives of the Hong Kong Academy of Medicine and local specialty colleges, overseas dignitaries and doctors’ organisations for their participation in celebrating the joyful successes of our Conjoint Fellows, Exit Examination and Diploma in Family Medicine (DFM) colleagues. We are also most thankful towards the aforementioned friends, colleagues and peers who have made our College’s academic events and the conferment ceremony highly successful.
After the introduction of the Official Platform Party by our Public Orator Dr. Maria Leung, our College President Dr. Angus Chan officially declared the ceremony opened. Dr. Kaye Atkinson, Censor-in-Chief Elect of the Royal Australian College of General Practitioners (RACGP), represented our Australian sister College to give a welcome speech also. There were 20, 20, 15 and 16 doctors awarded HKCFP Fellowship, RACGP Fellowship, Exit Examination Certificate, and Diploma in Family Medicine, respectively. Awards of the Dr. Peter C. Y. Lee Best Candidate in Fellowship Examination 2017, Outstanding Candidate in Exit Examination 2018, the HKCFP Best Research Award 2017, and HKCFP Research Fellowship 2018 were delivered on stage also. The ceremony was concluded by Prof. Sophia Chan, Secretary for Food and Health, Food and Health Bureau of the Hong Kong Special Administrative Region and our Dr. Sun Yat Sen Orator this year, delivering the 29th Dr. Sun Yat Sen Oration titled, “Time to Change: Recommitting Primary Health Care”.
We organised group photo session with successful candidates after the ceremony proper. We are very much obliged for all the College officials’ support and the cooperation of our successful candidates by staying behind at the venue after the ceremony.

Professor Cindy Lo-Kuen LAM, JP, our Chief Censor and Honorary Fellow (2011), was awarded the Medal of Honour (MH) (Chinese: 榮譽勳章 , MH) on 1 July, 2018, in recognition of her contribution to health services and corruption prevention. The Medal of Honour is awarded to distinguished members of the society who have made important contributions to community service in a district or in a particular area for a long period of time.
Professor Lam is the Danny D. B. Ho Professor in Family Medicine and Head of the Department of Family Medicine and Primary Care of the University of Hong Kong.
She graduated with the M.B.B.S. (HK) and obtained the M.D. (HK) from the University of Hong Kong. She is a specialist in Family Medicine, Fellow of the Hong Kong Academy of Medicine (Family Medicine), Fellow of the Royal College of General Practitioners and Honorary Fellow of the Hong Kong College of Family Physicians. She is Chief Censor of the HKCFP and a Co-Chair of the Grant Review Board of the Health and Medical Research Fund of the Food and Health Bureau, Government of Hong Kong SAR. She is a member of various local professional committees including the Steering Committee for Primary Healthcare Development and the Task Force on Colorectal Cancer Screening, Government of Hong Kong SAR. She served as a member of the Corruption Prevention Advisory Committee, ICAC from 2011 to 2017. She is a member of the Working Party on Research and Working Party for Mental Health of the World Organization of Family Doctors (WONCA). She has a special interest in continuous quality improvement in primary care. Her main research areas are health-related quality of life measurement, primary care health services outcome evaluation and Mental health in primary care. She has published more than 300 peer-reviewed journal articles and book chapters.
Our College is delighted that Professor Lam’s dedication to the advancement of Family Medicine and primary care was recognized with honour. We would like to add our cheers to her outstanding contributions to our community.

A reflection of “How Patient Suicide Affects Physicians“Dr. Wun Yuk Tsan (an old doctor)
I feel sad on reading the article in the Young Doctors Column in the July issue of the Family Physicians Links. I first experienced the same tragedy when I was young and naive in general practice. But, alas, it was not the only tragedy of the same kind in my professional life.
The first patient was a teenager whom I had attended throughout his years in secondary school. His mother brought him to me because he was very disappointed with his results in the public examination (then called School Leaving Certificate examination, equivalent to the DSE nowadays). I tried to comfort him and encouraged him to face the challenge for more than 30 minutes. I let him go when I saw a glimpse of relief on his face, ensuring him that he could come to talk with me again if he wished. I did not charge for that consultation as I knew the whole family well. He never returned. He jumped to his death from his home 7-10 days later. It took me some hard time to pull myself out from the bad feelings after the tragedy. And, only years later that I realized my mistake in that consultation. I should have let him do the talking, instead of the patronizing “comforting and encouraging”.
I described one of my later tragedies in a case report published in the Hong Kong Practitioner (January 1996;18(1):10): “Depression In General Practice: Two Case Reports” with a psychiatrist’s comments. We had very limited clinical training on real patients with suicidal ideation. We usually learn the management of depression from textbooks and role-plays. But the real patients with severe depression behave differently. Moreover,
self-concealment (actively hiding distressful information from others) is common in suicidal patients. One study reported that up to 78% of hospitalized patients denied having suicidal thoughts in the week before taking their own lives (Busch KA, Fawcett J, Jacobs DG. Clinical correlates of inpatient suicide. Journal of Clinical Psychiatry. 2003;64:14–19).
There is another article that we should read: “The Family Physician: What Sort of Person” by Brody (Fam Med 1998;30(8):589-93). He said that the virtues of a healing person included humility, compassion, and forgiveness. Humility means “a realistic knowledge and acceptance of one’s limits and weakness”. Compassion is derived from “to suffer with”. Physicians “must feel comfortable admitting mistakes to peers, confident that they will be understood and forgiven. The abilities to forgive our peers, forgive our patients, and forgive ourselves are all linked.”
I am not defending myself, nor my colleagues, for making mistakes with suicidal patients. Though I wish not, I would predict that similar mistakes would be made by doctors in future. We have been told not to make mistakes, been taught to learn from mistakes (made by us or others), but seldom been taught how to recover from the psychological trauma consequent on our mistakes. We need time and help to pass through grief.

不 經 不 覺,已 經 行 醫 七 年 了。入 醫 學 院 已 經 是
十 多 年 前 的 事。朋 友 問 我 當 初 為 甚 麼 選 擇 醫
科,不 禁 勾 起 很 多回 憶。
還 記 得 高 中 時 的 我,最 喜 歡 是 生 物 科。大 學 聯
招 時,我 首 七 個 志 願 都 是 七 間 大 學 的 生 物 系。
後 來 是 因 為 一 些 師 友 的 勸 告,才 在 最 後 一 刻 改
報 醫 科。
那 時 我 很 喜 愛 在 圖 書 館 看 生物 科 技 的 書 籍。這
世 界充 滿 奇 妙 的 生 命,是 遠 超 越 我 們 所 能 理 解
的。我 記 得 我 在 一本 圖 文 集 中 看 見一 隻 名 叫 蠑
螈 的 生 物。牠 有 四 肢 再 生 的 能 力。就 算一 整 隻
手 被 咬 斷,他 也 能 把 整 隻 手 再 生 出 來。這 能 力
是 其他 生物所 沒有 的。我 想:如 此 神 奇 的 生物,
是 偶 然 出 現 的 麼?還 是 有一 位 奇 妙 的 造 物 主 創
造 牠?我 偏 向 於 相 信 後 者。但 為 什 麼 這 位 奇 妙
的 造 物 主 只 給 蠑 螈 有 這 種 能 力,而 其 他 生物 卻
沒有?這一點 我 就 想不通了。
入了 醫 學 院 以 後,在 無 盡 的 課 業 中,我 漸 漸 地
忘 記 了 蠑 螈。每 天 都 是 上 課、上 病 房。在 醫 學
院,考 試 合 格 不 是 必 然 的 事。我 們 很 多 同 學 都
活 在 壓 力 下。為 求 合 格,我 們 對 人 體 讀 得 很 深
入,甚 至 於 一 條 血 管、一 條 神 經 的 走 向 也 能 滾
瓜 爛 熟,但 對 人 體 以 及 生 命 的 奇 妙 卻 麻 木了,
當 然 更 失 去了欣 賞 大自然 的 閒 情。
直 至 有 一 天,在 整 形 外 科 的 課 堂 上,我 又 看 到
了蠑 螈。教 授 說:我 們 可以複 製 生物,又可以 製
造 多 種 藥 物 改 變 人 體,卻 始 終 想 不 透 蠑 螈 的 再
生 能 力。小 小 的 蠑 螈 所 擁 有 的 奇 妙 力 量,遠 超
人 類 所 能 測 度,也 許只有 造 物主 才 能 明白。
看 着 小 蠑 螈 的 照 片,聽 著 教 授 的 話,我 又 想 起
造 物 主。衪 創 造了 奇 妙 的 世 界,但 祂 的 心 意 也
蠑螈與人生
許 不 是 人 所 能 測 度 的。如 何 才 可 以 認 識 祂 呢?
如 果 一 條 小 蠑 螈 也 被 給 予 能 力,那 麼 我 們 人 類
又 被 賦 予 了 甚 麼?人 類 比 蠑 螈 高 等 多了。我 們
的 存 在,會不會 也 是 被 賦 予 的 呢?
我 在 醫 學 院 第 五 年,在 整 形 外 科 病 房 裏,認 識
了小 B。她 是 個 一 歲 的 小 女 孩,因 被 人 關 在 家
裏,後 遇 上 火 災,她 逃 不了,被 烈 火 燒 傷了四 成
皮 膚。她 本 來 活 不 了,卻 被 消 防員 救了。教 授 替
她 做 了多次 手 術,把 人 工 皮 膚 植 上。她 身上 到
處 是植 皮,看了也 心 酸。
我 每 天 探 望小 B。看 着 她 流 淚 的 臉,和 滿 身 的人
工 植 皮,我 又 想 起 了蠑 螈。為 什 麼 造 物 主 不 把
再生能 力給 予小 B?對 蠑 螈 來 說,斷手 斷 腳 也 是
小 事。如 果小 B 有 再生能 力,這 點 燒 傷 就 算 不了
甚 麼。但 她 是 人,沒 有 再生 能 力。我 閉 上 眼 睛,
就 想 起 她 被 困 家中 烈 火 燒 身 的 情 景。全 知 全 能
的 造 物 主 容 許 這 場 火 災 發 生 在 她 身上,卻 又 藉
我 們 的 手 救了她,把 人 工 皮 膚 植 上,使 她 重 獲
新 生。這 是 為 甚 麼 呢?
小 B 適 應 了新 的 皮 膚,愈 發 健 康,常 常要 爬 出 牀
外玩 耍。我 在 畢 業 前夕,再次 回 到 病 房,想 看小
B。一 看,她 的 牀 已 經 空了。護 士 說 她 已 經 出 院
了。我 發 呆 良 久,但 也 感 到 欣 慰,因 為 我 知 道 小
B 已 經 像 一 條 康 復 了的 小 蠑 螈,回 到 她 本 來 的
世界了。
以 後 我 每 次 行 山 涉 水,看 見 蠑 螈,我 都 會 想 起
造 物 主 的 奇 妙。衪 也 是 我 們 所 不 能 測 透 的。衪
容 許 世 間 無 盡 的 苦 難 發 生,卻 又 給 予人 心 有
愛,又 賦 予 我 們 各 有 能 力,使 我 們 可 以 改 變 別
人 的 生命,讓 社 會再生。也 許人 類 社 會,就 是 一
隻 大 蠑 螈,在 無 盡 傷 痛 中,不 斷 再生。
“ 神 造 萬 物,各 按 其 時 成 為 美 好,又 將 永 生 安
置 在 世 人心 裏。然 而,神由始 至 終 的 作 為,人不
能 參 透。"望 各 位 醫 生 在 忙 碌 中 也 抽 片 刻 停 下
來,觀 察自然,思 想 造 物之奇 妙。同 時 也發 揮 所
學,照 顧 病 人,幫 助 世 人在 苦 難 中 重 建 人 生,再
現 生命 中 的 美 好。

Hypertension is a common disease and draws attention from both medical professions and the general public. Automated blood pressure measuring devices are now commonly used by the public. However, how is their accuracy evaluated?
There are several protocols to evaluate the accuracy of the device, namely AAMI (Association for the Advancement of Medical Instrumentation) protocol which is required by the FDA before products can be sold in the US market; EN 1060-4 protocol developed by European Committee for Standardization (CEN) which is required for CE mark before products can be sold in the EU market. In addition, the BHS (British Hypertension Society) protocol and ESH-IP (European Society of Hypertension International protocol) are also commonly used, which are not formally required by any market.
In 2009 the International Organization for Standardization (ISO) developed another standard which was largely based on the AAMI and CEN standard. A revised version of the American National Standards Institute (ANSI)/AAMI/ISO was developed in 2013. This year, AAMI/ESH/ISO are jointly developing a universal protocol for the growing need for validation of blood pressure machine.

The validation protocol involves recruitment of participants with their blood pressure measured by test device compared against mercury sphygmomanometer or accurate non-mercury devices by same-arm sequential BP measurement. The ESH-IP requires 33 participants compared to 85 for the ANSI/AAMI/ISO. ESH-IP recruit general population of adults aged >25 years and require separate studies for special populations (e.g. pregnancy), while ANSI/AAMI/ISO recruit participants aged >12 years and additional 35-45 participants for subgroup study. ESH-IP recommends use of mercury sphygmomanometer, whereas ANSI/AAMI/ISO accepts any manometer with maximum error +/-1 mm Hg.
Both ESH-IP has pass/fail criteria for individual BP reading and individual participants based on the number of readings with absolute test-reference BP difference <5, 10, 15 mm Hg, while ANSI/AAMI/ISO based on mean BP difference and standard deviations. Interestingly, the two approaches are statistically interchangeable with the two protocols allowing similar maximum levels of probability for tolerable error of <10 mmHg in BP measurement.

HKCFP Trainees Research Fund 2018 /
HKCFP Research Seed Fund 2018
The Research Committee of HKCFP is proud to continue to offer the two research funds, The
Trainees Research Fund and the Research Seed Fund.
The Trainees Research Fund will be opened to all registered HKCFP trainees and is made of
four awards (each up to HK$5,000). It is envisaged it will help trainees especially (but not limited
to) those doing research projects as their exit examination. Those who have funding support
elsewhere will not be considered.
The Research Seed Fund is open to all HKCFP members when a maximum of $10,000 award will
be made to the successful applicant to assist the conduction of a research project.
Winners of the award will receive 50% of the approved grant up front and the remainder 50%
upon completion of the project.
**Please note that each applicant can only apply either one of the above Funds**
Assessment Criteria for both funds:
1. Academic rigor of the paper (e.g. originality, methodology, organisation and
presentation);
2. Relevance and impact to family medicine & primary care (e.g. importance of the topic
and the impact of the findings on the practice or development of the discipline); and
3. Overall budget

Department of Family Medicine and Primary Care of The University of Hong Kong is actively recruiting primary care doctors who are prepared to take medical students into their practices for teaching purposes.
• As part of the Professionalism in Practice Programme (PIP), each student will attach individually or in pairsto the same preceptor for three half-day sessions over the first two years of medical school. Through early attachment to a primary care doctor, students will focus on developing an understanding of what it means to be a doctor and how professionalism is practiced in real life. The dates of the attachment are flexible.
• During the Family Medicine Junior Clerkship – Family Practice Attachment (FPA) of the fourth year of medical school, each student will attach to a family practice in the community for two half-day sessions over nine-week period. These attachments are on a one-to-one basis, and are scheduled at regular intervals throughout the clerkship.
Most doctors find that teaching medical students is a rewarding experience, and that the satisfaction gained often more than compensates for the minor disruption to the practice. Those who volunteer to teach will be offered an honorary academic appointment at The University of Hong Kong, which is a quotable appointment. CME and CPD accreditation has been given to these teaching activities by the Quality Assurance and Accreditation Committee of Hong Kong College of Family Physicians (HKCFP) towards the award of the Certificate of Quality Assurance. Our honorary teachers are entitled to use HKU’s facilities including the library and email access to the HKU portal.
Fellows and members of the HKCFP are most eligible, and enquiries from other doctors would also be welcome. No previous experience is necessary - interest and motivation to pass on your expertise are much more important.

Interest Group in Dermatology – The 66th Meeting held on 7 July 2018

Dr. Lam shared five interesting cases with us.
The first case was an elderly gentleman with extramammary Paget’s Disease (EMPD). EMPD is a neoplastic disease. The majority is primary EMPD (>75%) which arises from intraepidermal apocrine glands or pluripotent keratinocyte stem cells. On the other hand, secondary EMPD arises from epidermotropic spread from underly ing adenocarcinoma in dermal adnexal glands or from internal organ neoplasm. EMPD accounts for 6.5% of all cutaneous Paget’s disease and affects mostly older people (age 50-80), and it is more common in male in Asian population (F:M ratio = 1:6). Vulva is the most common site (up to 65%), followed by perianal area (20%) and male genitalia (14%). EMPD may present as slow-growing, often pruritic, well-demarcated erythematous scaly plaques, sometimes with erosions, giving rise to a “strawberries and cream” appearance. It often mimics benign inflammatory dermatosis, so high degree of suspicion is essential for early diagnosis. Differential diagnoses include infection, intertrigo, psoriasis, eczema, contact dermatitis, SCC and Bowen’s disease. It is important to screen for associated malignancies, e.g. female genitourinary cancer and breast cancer in vulva EMPD, male genitourinary cancer in penoscrotal EMPD, and lower GI cancer in perianal EMPD. The mainstay of treatment is surgical excision. The prognosis is generally good but long term follow-up is required to detect possible recurrence.
The second case was two siblings with tinea capitis. Tinea capitis affects predominately preadolescent children. In HK, the most common causative dermatophyte is Microsporum canis, which is usually contracted from infected cats or dogs. In US and some cities of UK, Trichophyton tonsurans is more common and it is contracted by close contact. Tinea capitis may present as scalp scaling, patchy alopecia with scales/block dots/pustules, and Kerion formation.

Differential diagnoses include seborrheic dermatitis, psoriasis, discoid lupus, impetigo, alopecia areata, traction alopecia, and trichotillomania. Wood’s lamp is helpful as M. canis will show a green fluorescence. Investigations include skin scrapping and hair plucking. Treatment is oral antifungal: griseofulvin for M. canis, while oral terbinafine for T. tonsurans. Affected children should be excluded from school until treatment have been commenced. Familial screening should be done especially in T. tonsurans cases as they are anthropophilic and are highly infectious. Infected pets should also be treated.
Dr. Lam showed the case of Congenital Melanocytic Nevi (CMN) to us. He showed us the concern of CMN including cosmetics, malignant changes and neurocutaneous melanosis. If the CMN size is < 20 cm, the risk of malignant change is less than 1 %. For size of CMN > 20 cm, the risk of malignancy is < 5%. For its management we can observe the lesion if its size is < 20cm. If its size is > 20 cm, we may need to have MRI of the CNS to rule out neurocutaneous lesion. And we can have surgery or laser treatment of CMN. Giant CMN poses a significantly increased risk of melanoma.
Dr. Lam shared a case of CA-MRSA to us. He explained the diagnosis of CA-MRSA based on genetic composition and the criteria of diagnosis. The treatment of CA-MRSA includes oral septrin and doxycycline (not for <12 yrs old). The importance of notification of CA-MRSA to CHP was discussed and treatment by CHP often includes empirical decolonization therapy for all patients and their close contacts.
Finally, Dr. Lam presented a case of rosacea including symptoms and signs with overview of the first line treatment.
