Clot-Removing Stroke Treatments Late one night, William Johnson returned to the home he shares with his sister. The 54-year-old knew something was wrong when he awoke and couldn’t turn and his left arm and leg were paralyzed. He then fell, waking his sister. “Deborah asked me what had happened, but I couldn’t talk,” Johnson says. Deborah Johnson called 911. The EMTs recognized that Johnson had probably suffered a stroke and rushed him to the Texas Stroke Institute at The Medical Center of Plano, a key regional hub in a net- work of stroke care centers. A team headed by interventional neurologist Vallabh Janardhan, M.D., applied a cutting-edge treatment to restore Johnson’s health. OPENING THE TREATMENT WINDOW “Normally a treatment called tissue plas- minogen activator (TPA) can dissolve the clots that cause 86% of strokes,” explains Dr. Janardhan. But a patient must get TPA within three hours of a clotting stroke. Because Johnson had arrived outside that time, TPA would have increased his risk for dangerous internal bleeding. Happily for Johnson, The Medical Center of Plano also offers clot removal, an option for eligible stroke patients who have missed the deadline for TPA. In fact, we are the only hospital in Collin County that can offer this comprehensive stroke care. The medical team quickly assessed Johnson’s brain with a CT scan and a specialized “brain attack MRI” to localize his clot as well as understand how much brain had already been damaged by the stroke. The clot, which blocked a vessel to the right side of Johnson’s brain, left a large amount of his brain at risk but still salvageable seven-and-a-half hours after Johnson had gone to bed able to move. Dr. Janardhan continues, “The MRI showed that we could safely retrieve and remove the clot even at this late time window.” AN EXCELLENT RECOVERY Johnson was then taken to The Medical Center of Plano’s neurointerventional suite, which has one of the most advanced cameras in the country. Dr. Janardhan says, “It’s like having a GPS for navigating the brain’s blood vessels. In addition, we can do a CT scan during the clot removal procedure in real time.” Since only about 4% of patients tend to arrive within the three-hour time win- dow to receive TPA, several mechanical devices have been developed to remove clots detected later. Dr. Janardhan says, “Using these mechanical devices to reopen blood vessels is a huge tech- nological advance.” The clot-removal procedure was successfully performed and Johnson’s blood vessels were com- pletely reopened. “Less than 10 hours after his stroke, Mr. Johnson was mov- ing his arms and legs until we finally had to tell him to stop moving.” One month after his stroke, Johnson says, “I am able to talk and walk again. Clot-Removing Stroke Treatments

Late one night, William Johnson returned to the home he shares with his sister. The 54-year-old knew something was wrong when he awoke

and couldn’t turn and his left arm and leg were paralyzed. He then fell, waking his sister. “Deborah asked me what had happened, but I couldn’t talk,” Johnson says. Deborah Johnson called 911.

The EMTs recognized that Johnson had probably suffered a stroke and rushed him to the Texas Stroke Institute at The Medical Center of Plano, a key regional hub in a net-work of stroke care centers. A team headed by interventional neurologist Vallabh Janardhan, M.D., applied a cutting-edge treatment to restore Johnson’s health.

women’s health

Sometimes if a little bit is good, more is even better—especially when it comes to healthy hab-its and heart disease. In fact, the American Heart

Association (AHA) has pinpointed seven habits that have been found to contribute to a healthy heart.

heart diSeaSe: riSk FactOrS “There are some risk factors for heart disease—such as age and heredity—that we can’t control,” says Poonam Agarwal, M.D., a cardiologist on staff at The Medical Center of Plano. “But we can learn to make better lifestyle choices—which does have a powerful effect on our health. Start slowly and build from there. Even small changes will do your heart good.”

cOunting the WayS tO heart healthThe AHA’s seven heart-healthy habits are:• Manage blood pressure. High blood

pressure is the most significant risk factor for heart disease. Optimal blood pressure is less than 120/80 mm Hg.

Seven Simple Habits That Could Save Your Life

• Get active. Getting moderate exer-cise 30 minutes a day helps lower blood pressure; increase HDL, or “good,” cholesterol; control blood sugar; and control body weight.

• Control cholesterol. Too much LDL, or “bad,” cholesterol in the blood can clog arteries and increase the risk for heart attack and stroke. Your total choles-terol should be below 200 mg/dl.

• Eat better. Include more vegetables, fruits, whole-grain products, and nonfat or low-fat dairy products in your diet. Also reduce your intake of sodium and highly processed and high-fat foods.

• Lose weight. Obesity is a major risk factor for heart disease. Visit www.nhlbisupport.com/bmi to find your body mass index (BMI). If it’s 25 or higher, start losing excess pounds. Your waist circumference is another important number to know. Dr. Agarwal says, “Excess belly fat raises your risk for heart disease. A waist-line greater than 40 inches in males and 35 inches in females is a marker for increased cardiac risk even if the BMI is within normal limits.”

• Reduce blood sugar. Adults with dia-betes are more likely to have heart disease than those without it. If your blood sugar level is above 100 mg/dl, follow your doctor’s advice.

• Stop smoking. Smoking increases the risk for heart disease, especially if you have other risk factors.

HoW Do You Measure uP? The AHA has established an online tool called “My Life Check.” Visit www.mylifecheck.heart.org to see how you stack up against “Life’s Simple 7” and get advice for improving your own heart health.

You’ve scheduled your annual flu shot. You wash your hands diligently and thoroughly. You steer clear of sick friends. Is there more you can do to protect yourself from the flu—or to relieve your symptoms if you do get sick?

While no food has definitively been shown to vanquish the flu virus, nutrients in the following four may give you the winning edge this year. 1. salmon. A recent study of Japanese schoolchildren suggests that getting a daily dose of vitamin D during winter may reduce the incidence of influenza A, a common strain of seasonal flu.

The children in the study took vitamin D supplements, but oily fish such as salmon is an edible option. Fortified milk and cereals are good sources, too. This nutrient is also made naturally in the body when the skin is exposed to sunlight, but it’s not always enough when the days grow shorter. Ask your doctor how much vitamin D you need.2. Green tea. Several studies have found that drinking green tea may improve your immune system—but can it battle the flu bug? In one study, participants took cap-sules containing two tea components. The results showed the capsules decreased the number of people reporting cold and flu symptoms by about a third. There was also a decrease in how long the symptoms lasted.

3. Yogurt. Probiotics are live microorganisms similar to the “good” microorganisms that live in the gut. They are found in some foods, such as yogurt, and come as dietary supplements.

In a recent study, researchers gave 3- to 5-year-olds pro-biotic supplements to see if that would affect flu symptoms. Some children received one probiotic strain, while others received a combination of two. After six months, both groups showed a reduction in the incidence and duration of flu symptoms when compared with a placebo group. The two-strain group had the best results. 4. Chicken soup. Fluids help your body fight infection, and hot liquids like chicken soup may relieve symptoms, too. In a small study, 30 volunteers with cold- or flu-like symptoms drank either a heated or room-temperature fruit drink. Researchers then tested their nasal airflow and asked about six symptoms: runny nose, cough, sneezing, sore throat, chilliness, and tiredness. While the airflow test showed no significant improvement with either drink, participants who drank the hot beverage reported relief from all six symptoms. Those who drank the cooler drink experienced relief from only three: the runny nose, cough, and sneezing.

Falls are the leading cause of injury among older adults, and 60% of these falls occur at home. Here are some helpful precautions:• Keep floors and walkways free of clutter.• Ensure that all stairwells are adequately lit. Install handrails on both sides. Place

fluorescent tape on the edges of the top and bottom steps for visibility, and non-slip treads on wood steps.

• Install grab bars beside tubs, showers, and toilets. • Use a rubber bath mat or place nonskid adhesive strips in the shower or tub. • Install a nightlight along the route between your bedroom and bathroom. • Eliminate throw rugs. Secure loose area rugs with double-faced tape or slip-

resistant backing.

mature matters

Watch Out for SpotA recent report from the U.S. Centers for Disease Control and Prevention (CDC) reports that an estimated 86,629 Americans receive emergency room treatment each year for fall-related injuries associated with a pet dog or cat. It appears canine friends are slightly more responsible for the injuries: Nearly 88% of falls associated with pets involved dogs.

But don’t give up your pet just yet. Research has shown that humans who own pets benefit from lower blood pressure, stress, and triglyceride lev-els. They also have more opportunities for exercise and socialization.

The CDC suggests raising aware-ness that certain pet-related situations and activities can lead to falls, as well as reinforcing the American Veterinary Medical Association’s recommendations that emphasize obedience training.

Depression is a serious medical condi-tion affecting 20 million Americans. It can worsen existing health problems, including heart disease and diabetes. The good news? Depression is highly treatable. Here’s how to know when to see your doctor for help.

recOgnizing depreSSiOnMost people have days when they feel down. But you may be grappling with something more serious—depres-sion—if you have several of the follow-ing symptoms for two weeks or more and they interfere with your daily life: • Persistent feelings of sadness, emp-

tiness, worthlessness, or guilt • Decreased interest or pleasure in

favorite activities• Significant changes in sleep, appe-

tite, or energy

Most Depressed Adults Go Without Treatment• Irritability or restlessness• Difficulty concentrating, thinking, or

making decisions• Recurrent thoughts of suicide or death

treatment OptiOnSIf you feel depressed, get help. Effective treatments are available. The two most common treatment meth-ods are antidepressant medications and psychotherapy, or talk therapy. Medications address chemical imbal-ances in the brain. Therapy helps people cope with feelings and symp-toms while teaching them to change thought patterns that may play a role in the illness.

A study in the Journal of the American Medical Association suggested that antidepressants were most effective against severe cases of depression. But

doctors stress that medication con-tinues to help many patients.

If you’re suffering from depres-sion, don’t put off seeking treatment. After all, your health—and maybe even your life—depends on it.

Myths and Facts About Autism

6 | TheMedicalCenterof Plano.com

The numbers are undeniably alarming—the prevalence of autism spectrum disorders (ASDs) increased 57% between 2002 and 2006. Today, nearly 1% of children are diagnosed with an ASD. While researchers say no single factor can explain these statistics, one thing is certain: Parents can best help their children by under-standing the myths—and facts—about this disease.

myth: All people with autism are alike. Fact: ASDs make up a group of developmental dis-abilities that affect social, communication, and behav-ioral abilities. They are called “spectrum disorders” because they affect each person in a different way, and symptoms can range from mild to severe.

myth: Autism is caused by poor parenting, vaccines, and so forth.Fact: Researchers don’t know exactly what causes ASDs. They do know it has nothing to do with how a child is raised.

The idea that vaccines might play a role surfaced after a small 1998 study. Many parents became con-cerned, some refusing to vaccinate their children and putting them at risk for serious contagious diseases. In early 2010, however, the Lancet study was retracted by the journal after flaws in research methods were revealed. There is no scientific evidence that vaccines cause autism.

So what do experts think are risk factors for ASDs? Children with a sibling or parent with an ASD are at higher risk. So are boys, as well as children with cer-tain genetic disorders. But researchers continue to search for answers, and they are looking at everything from brain chemistry, to birth order, to genetic and environmental factors.

myth: Autism can be cured.Fact: Currently there is no cure for ASDs, but early diagnosis and intervention can greatly improve a child’s development. That’s why the American Academy of Pediatrics has recommended that pedia-tricians screen all children for ASDs at both 18 and 24 months.

Research has found that signs of autism may emerge gradually over a child’s first year or more, so in the majority of affected children, it may not be pos-sible to identify an ASD before age 1. Researchers also found that parents do not always recognize ASD signs and would be helped by formal screenings.

To educate parents about developmental milestones so that they can better recognize symptoms, the Centers for Disease Control and Prevention has launched a website, “Learn the Signs. Act Early.” You can find it at www.cdc.gov/ncbddd/actearly.

Help Your Teen Cope with DiabetesThese days, lots of teens—many who are over-weight and inactive—are developing type 2 diabetes. Left uncontrolled, high blood sugar can raise the risk for early heart disease. It also can have emotional effects, making kids feel different or stressed.

A little parental support can go a long way. Here’s how to help:• Cook the same healthy foods for the whole

family so that your teen isn’t left out.• Plan family walks to help your teen get 60

minutes of exercise a day.• Don’t see your role as the “blood sugar

police.” Watch your teen complete his test-ing until he’s shown that he can keep it under control. Then, give him some space.

• Get your teen a cool journal to help her “vent.”

• Peers can be a great source of support. Find a local camp or group for kids with diabetes at 800.diaBeteS (800.342.2383).

Our eyes can feel scratchy and irritated under many conditions—airplane travel, sitting in an air-con-ditioned room, or spending hours at the computer. But some people actually don’t produce enough tears,

or their tears evaporate too quickly, so their eyes are dry and uncomfortable all the time.

If this sounds familiar, see your ophthalmologist. Your doctor will examine your eyes and perform tests to measure your tears. “The good news is that there are many effective treatments for dry eye,” says Susan Swanson, M.D., an ophthalmolo-gist on staff at The Medical Center of Plano.

let the gOOd tearS FlOWTears lubricate the surface of the eyes, wash away debris, and help protect the eye from infection. Tears are also necessary for healthy eyes and clear vision. Symptoms of dry eye can include:• Stinging or burning • A scratchy feeling, or a feeling that something is

in the eye• Eye fatigue• Redness• Difficulty wearing contact lenses• Excess tearing, caused by irritation • Fluctuating vision

Untreated dry eye can raise the risk for eye infec-tions and lead to scarring on the cornea and vision problems. Dry eye can occur at any age, but it’s more common in women—particularly those past menopause—and older adults. It can be associated with certain medical conditions, like autoimmune diseases, or caused by medications, such as anti-histamines. Radiation therapy and LASIK also can cause dry eyes. So can long sessions on the com-puter or playing video games.

many OptiOnS FOr treatmentOver-the-counter artificial tears, gels, and oint-ments are usually the first line of treatment for dry eye. Additional options include:• Punctal plugs, tiny silicone plugs inserted into

the drainage holes of the eyes so that tears stay in the eyes longer

• Steroid eye drops, to decrease inflammation on the eye

• A prescription medication, RESTASIS®, that controls inflammation on the eye's surface and increases the ability to produce tears

• Special contact lenses that keep air from drying the surface of the eye

“Work with your eye doctor to decide which approach is right for you,” Dr. Swanson says.

Having trouble starting your family or adding to it? You’re not alone. The American College of Obstetricians and Gynecologists estimates

that as many as one in 10 couples fail to get pregnant within six to 12 months of trying.

Your doctor can help. Dawn Bankston, M.D., an obstetrician-gynecologist at The Medical Center of Plano, says that women with fertility problems prefer to stick with the doctor and office staff they’ve come to know. Infertility Problems? We Can Offer You HelppinpOinting the cauSe To solve the infertility puzzle, Dr. Bankston and her colleagues employ a variety of diagnostic proce-dures. These include:• Lab work to test for hormone

irregularities that could interfere with fertility

• Ultrasound imaging to determine if a woman is ovulating

• Hysterosalpingography, an X-ray to show blockages in the fallopian tubes or uterus

• Laparoscopy, an outpatient proce-dure to look for a condition called endometriosis or pelvic scarring, which could prevent pregnancy

A urologist can also help determine if the male partner has enough sperm and if they can swim freely.

Most of the time, your doctor can treat infertility with drugs or minor surgery. Dr. Bankston explains, “We offer oral and injectable fertility medications that stimulate ovulation, as well as intrauterine insemination, called artificial insemination.”

caring While curingUnlike some doctors, physicians at The Medical Center of Plano recognize that infertility can be stressful, and that your emotional well-being matters as much as your physical health. “A lot of women suffer mood problems when they can’t

get pregnant,” says Dr. Bankston.A program at The Medical Center of

Plano called Women’sLink, which offers services for women at every life stage, also addresses the emotional aspects of infertility.

An infertility support group meets at the Women’sLink Support Services Clinic at 4001 West 15th St., Suite 465. Each 90-minute session leaves time for women to discuss their feelings and learn healthy ways to cope.
