DB Orthodontics’ aim is to provide the Orthodontic profession with the most user friendly mini implant system.

We have strived for absolute perfection from the design stage to the manufacturing of the Infinitas mini implant system.

We are continually developing the system, working with leading Orthodontic experts to ensure that we meet the highest standards expected by todays Orthodontic profession.

In essence, the Infinitas mini implant system has been designed by an Orthodontist, for Orthodontists, so that precise, reliable bone anchorage can be achieved using as simple a clinical process as possible.

Infinitas mini implants system has been designed with Dr Richard Cousley who is a Consultant Orthodontist working in the UK hospital service and private practice. He has published papers and lectured widely on the technical aspects of bone anchorage, and developed the Infinitas mini implant system to overcome some of the limitations of existing mini implants, especially in terms of their lack of insertion precision and overly complex inventories.

A combination of clinical and computerised trials have proven that the Infinitas mini implant system is reliable and provides comfort to both patient and clinician.

When deciding which mini implant system to purchase there are a number of considerations a clinician will take: How easily will auxiliaries such as closing coil springs or elastomeric chain attach to the head? How comfortable will it be for the patient? How easily will it integrate with my current treatment mechanics? Will I need to drill a pilot hole or will the mini implants self-drill?

With this in mind we have designed all components of the Infinitas mini implant system to meet all the indications for use as we hope to exceed the expectations of the clinician.

The contents of this literature will explain all the features of this sophisticated and intelligently designed system which has already made it the system of choice for many Orthodontists.

The head of the Infinitas mini implant is designed for easy and direct attachment of all forms of traction. In particular there is no need for custom made closing coil springs.

Infinitas mini implants are self drilling and when placed in keratinised gingiva can be placed directly through this tissue using the Infinitas screwdriver.

The internal fit of the Screwdriver securely holds the neck of the Mini-Implant during insertion.

The introduction of bone anchorage to Orthodontics has provided the clinician with an increasing number of innovative treatment possibilities.

It is our belief that such innovations should make orthodontic daily practice easier and in order to do this should integrate with minimal effort into both the practice and treatment; Infinitas has been designed to do just this.

Having only one multi-purpose head design simplifies the choice for the clinician and allows you to keep a low inventory. The use of the ergonomically designed standard handle, which houses the Screwdriver, Mucotome and Cortical bone punch inserts, makes the system cost effective for re-equipping and more comfortable for the clinician. Cortical Bone Punch

During placement in areas of dense cortical bone eg. posterior mandible and mid-palate it is beneficial to use the Cortical bone punch.

Only minimal force is required for the 1mm diameter and 2mm length of this insert to perforate the cortical bone reducing insertion torque and preventing slipping of the mini implant tip when being placed at an oblique angle.

Incisor IntrusionThis was an adult patient with absent upper left canine and upper right 1st premolar teeth and previous aggressive periodontal disease. Limited treatment was undertaken for maxillary arch alignment and levelling, whilst avoiding extrusion of the buccal teeth, and this then enabled placement of a bonded retainer. Infinitas mini implants were used to maximize intrusion of the incisors and overbite reduction. These were 1.5mm diameter and 6mm body length versions. Direct traction was applied using powerchain to the lateral and then central incisor brackets.

Intermaxillary TractionThis 15 year old boy presented with a Class II division 2 malocclusion, absent lower right 1st premolar and significant displacement of the lower centreline to the right side. Extraction of the upper right 1st premolar was then required to achieve a Class I buccal occlusion and incisor relationship. Initial fixed appliance treatment involved re-opening the lower right premolar space as the labial segment was advanced in order to normalise the lower centreline and reduce the deep overbite. Full anchorage support was then required for mesialisation of the lower molar teeth and subsequent use of class III traction (to preserve the upper centreline).

A 1.5mm diameter 9mm body length Infinitas mini implant was inserted mesial to the lower right canine (once this tooth was in the correct position) following simple manual perforation of the mandibular cortex using the Infinitas bone punch. Direct traction was applied to the 1st molar attachment hook using elastomeric modules until the 1st premolar space was closed. An inter-maxillary elastic was then worn full-time from the mini implant to the upper right 1st molar hook. Hence, full control was achieved over space closure in relation to preserving the lower, and then the upper centreline corrections.

Indirect AnchorageHypodontia patients frequently have inadequate alveolar bone volume in the areas where permanent teeth fail to erupt. Therefore, mini implants are better inserted in more apical positions as shown in this case where the upper canines erupted in very distal positions. 9mm body length, 1.5 mm diameter mini implants were inserted to assist with mesialisation of the molar teeth. Indirect anchorage was utilised by securing a 19x25 steel wire through the Infinitas head (with composite) and a cross-slot placed on the main archwire.

Molar IntrusionThis adult patient was originally scheduled for posterior maxillary impaction to correct her anterior open bite. Instead, after initial alignment, Infinitas mini implants were used for intrusion of the upper buccal segments. Extraction of the premolars was also undertaken, but no space closure was undertaken until incisor contact had been achieved. The patient also had extraction of the 3rd molars once the intrusion was proceeding successfully. Prior to this a significant step had developed between the 2nd and 3rd molar crowns, as the former teeth were intruded.

A 1.5mm diameter 9mm body length, long neck mini implant was inserted bilaterally in the palatal alveolus between the 1st and 2nd molar palatal roots (after removal of a circle of mucosa using the Infinitas mucotome). Direct elastomeric traction (to bonded cleats) on the molars produced differential elevation of their palatal cusps (as well as bodily intrusion), whilst arch constriction was counteracted by archwire expansion. Some over-correction of the anterior openbite was achieved as evidenced by posterior openbites at debond.

The pre and post-intrusion lateral radiographs demonstrate the alteration in the maxillary occlusal plane and also the resulting favorable mandibular auto-rotation (and pogonion advancement).

Molar MesialisationThis adult male required orthodontic decompensation and then a mandibular setback osteotomy to correct his Class III malocclusion. The lower right 1st molar had a poor prognosis yet space closure would normally be contra-indicated by the need to advance the lower incisors pre-operatively. Conventionally the lower 3rd molar would also be removed during this jaw surgery. Therefore, it was planned to close the 1st molar space by pre-operative mesialisation of the adjacent molars and hence preserve the 3rd molar in this quadrant.

Normally one mini implant (placed in the premolar area) would suffice for mesio-distal anchorage, but a 2nd was added here because space closure was greatly complicated by a substantial loss of alveolar height in the 1st molar area. The Infinitas mini implants were 1.5mm diameter, and had 6mm and 9mm body lengths, inserted buccal and mesial to each premolar root.

Direct traction was applied, via elastic modules, to both a free-sliding powerarm (distal to the 2nd molar) and to the 3rd molar bracket hook. These powerarms enable better bodily movement of teeth with fewer side-effects such as archwire binding.

esIncisor RetractionThis 16 yr old boy presented with a Class II division 1 malocclusion with an absent lower right 2nd molar, moderate anterior crowding, and both centrelines displaced to the left side. Therefore, it was ideal that both upper and lower right canines be retracted for both overjet reduction and centreline correction, but without loss of a further tooth from the lower right quadrant.

The treatment plan included extraction of the upper right 1st premolar and using a mini implant, inserted buccal and mesial to the upper right 1st molar, for direct anchorage. Distalisation of the lower right buccal segment was undertaken using a mini implant placed immediately mesial (and buccal) to the lower right 1st molar. One 1.5mm diameter, 9mm length short neck Infinitas mini implant was inserted in each right quadrant in a self-drilling manner, following initial perforation of the mandibular cortex using the Infinitas manual bone punch.

Elastic modules were used for direct retraction of the upper right canine and then the whole labial segment. In the lower arch, a laceback connected the mini implant to the nearby canine bracket during initial alignment. Simultaneous distalisation of the posterior teeth occurred such that further active traction was not required in this quadrant.

Direct Molar DistalisationThis 13 year old girl had previous surgical exposure of both upper central incisors and removal of severely hypoplastic 2nd molars. The treatment plan could not be finalized until the incisors were brought into the arch, when it became clear that she needed overjet reduction (from 8mm) and correction of ¼ and ½ unit class II buccal segment relationships. Asymmetrical distalisation of the maxillary arch was indicated, but class II elastic traction was contra-indicated because of the short upper incisor root lengths.

A 1.5mm diameter, 9mm body length long neck Infinitas mini implant was inserted, palatal and mesial to the upper 1st molar palatal roots, following removal of a circular piece of mucosa using the Infinitas mucotome. A 3D Infinitas stent was used for accurate positioning of the mini-implants by directly guiding the slow handpiece-driven instruments.

Direct traction was applied to hooks bonded onto the 1st premolar palatal surfaces, and then indirectly to the incisors via labial elastic chain. The buccal teeth were distalised by up to 4mm. Therefore, this case demonstrates the benefit of a customised stent for accurate insertion of mini implants, and the flexibility of this palatal approach for direct distalisation of up to ½ unit.

Tooth Movements and Fixation Without the Use of Fixed AppliancesBoth of these adult patients required orthognathic surgery, but inter-maxillary fixation (IMF) from fixed appliances was contra-indicated because of insufficient root lengths and poor periodontal support. Consequently, multiple mini implants were inserted intra-operatively in both arches for immediate bone-supported IMF (shown here using powerchain fixation). These remained in situ post-operatively until it was clear that no remedial traction was required.

The 2nd patient then required intrusion of an over-erupted upper canine so that the lower arch could be levelled post-operatively. Two 1.5mm diameter, 6mm body length mini implants were inserted mesial and distal to this canine, and powerchain intrusive traction utilised. Subsequently an inter-maxillary elastic was used to extrude the lower buccal teeth, whilst the canine intrusion was allowed to relapse slightly.

Infinitas Mini Implant Courses

The key to successful mini implant placement is knowledge and experience. Our course aims to equip orthodontists with the theoretical and practical knowledge for mini implant anchorage in a wide variety of clinical scenarios, as well as practical experience of placement in bony-like material to simulate actual placement.

Objectives of the course;

>>To consider the history and realise the varied clinical applications of bone anchorage

>>To critically appraise the literature evidence on mini-implant techniques.

>>To explain the design, benefits and clinical usage of the Infinitas mini implant system.

>>To provide practical in vitro teaching on the use of the Infinitas system.
