폐쇄성 수면무호흡증후군(obstructive sleep apnea synd-rome, OSAS)은 수면 중 비정상적인 상기도 폐쇄가 발생하여
수면의 질을 악화시키고, 주간졸림, 업무 수행 능력 저하, 인지
기능 장애 등을 유발시켜 환자의 삶의 질을 떨어뜨린다. 또한,
고혈압과 심근경색 등의 심혈관계 질환 발생률을 증가시키고
당뇨, 우울증, 녹내장, 파킨슨병, 불임증 등의 원인으로도 작용
할 수 있다.1-8) 이러한 폐쇄성 수면무호흡증후군의 유병률은 매
우 높아 남성의 4%, 여성의 2%에서 관찰되며, 우리나라에서도
남성의 4.5%, 여성의 3.2%에서 수면무호흡증후군이 관찰된다.9,10)
OSAS의 치료에는 수면자세 조정이나 체중 감소와 같은 보
존적인 방법을 비롯하여 구강 내 장치, 비강 및 인두 부위에 대
한 이비인후과적인 수술, 지속적상기도양압술(continuous pos-itive airway pressure, CPAP) 등 다양한 방법이 사용되고 있
다. 이렇게 다양한 치료방법이 있다는 사실은 어느 방법도 쉽
고 확실하게 환자를 치료하지 못한다는 사실을 반영하기도 하
지만, 현재까지는 안전성과 치료 효율적인 측면에서 CPAP가
가장 널리 적용되고 있다.11) 이러한 CPAP는 성인에서는 이미
중증 수면무호흡증후군 환자의 일차적인 내과적 치료로 자리
를 잡았고, 중등도 이상이나 주간졸림증이 동반된 수면무호흡
증후군 환자에서 적극적으로 사용이 권장되고 있다.12) Hoff-stein13)은 수면무호흡증후군 환자의 치료법들에 관한 연구에
서 CPAP가 75%로 구강보조기의 42%에 비해 상대적으로 높
은 효과를 나타내었다고 보고하였다.
CPAP는 안면 마스크와 펌프 장치로 구성되며, 상기도에 양
압을 형성시켜 수면무호흡 환자에서 발생하는 상기도의 주기
적 허탈을 방지한다. CPAP의 지속적인 사용으로 인한 효과들
로 주간졸림증 감소, 성기능 개선, 혈압 강하, 환자와 가족의
삶의 질 향상, 급성 심인성 폐부종 치료효과, OSAS로 인한 사

Though continuous positive airway pressure (CPAP) is the primary treatment modality and im-proves mortality and quality of life for moderate to severe obstructive sleep apnea syndrome (OSAS), its compliance is surprisingly low and limits its overall effectiveness in OSAS treat-ments. This paper is focused on examining a multiplicity of possible factors associated with compliance to CPAP. Recent studies revealed that factors that influence compliance to CPAP include disease severity, patient characteristics, equipment-related, psychological and social fac-tors, and side effects. Relative scarcity of evidence about the effect of upper airway problem on compliance is also noted. In consideration of multi-factorial nature, multidisciplinary approach is needed for enhancing CPAP compliance. Korean J Otorhinolaryngol-Head Neck Surg 2014;57:7-14
C-flex 등) 등의 가변압력 메커니즘도 사용되고 있다. 이 중 AP- AP는 환자에게 필요한 적정압력을 자동적으로 조절하는 방
법이며, BiPAP는 호기시와 흡기시 서로 다른 압력을 제공하
는 방법이다. 마지막으로 flexible pressure relivery 방법은 환
자가 숨을 내쉬기 시작하는 시점의 압력을 낮추어 호흡의 편안
함을 제공하고, 상기도 허탈이 발생하기 쉬운 호기 후기 단계
에서는 세팅된 압력을 제공한다. 이와 관련한 연구에서, flexi-ble pressure를 적용한 환자군에서 상대적으로 CPAP를 더 규
칙적으로 더 많이 사용하였다는 보고도 있으나,36) 많은 연구

들에서는 flexible pressure 여부가 순응도의 차이를 유발하지
는 않았다.55,56) APAP 기능에 관한 meta-analysis에 의하면 고
정압 CPAP에 비해 순응도의 차이는 없었으나, 젊은 환자군에
서 APAP를 선호하는 경향이 있는 것으로 나타났다.57) 또 다른
무작위 연구들에서도 전체적인 순응도는 차이가 없었지만 CP- AP 사용과 관련된 부작용을 호소한 비율은 고정압 CPAP 군
에서 높았으며, 하나의 부작용이라도 경험한 환자군에서는 APAP
군에서 고정압 CPAP 군에 비해 순응도가 더 높았다.58,59) 사용
량과 관련해서는 사용일수는 차이가 없었으나 auto-titration
군에서 사용시간이 근소하지만 유의하게 더 길었다는 보고도
있으며(median CPAP 사용시간 각각 4.65시간, 4.51시간, p< 0.001),60) CPAP 적정압력이 10 cmH2O 이상인 환자에서는
APAP 군에서 유의하게 사용시간이 길다는(306±114분, 271
±115분) 보고도 있어, CPAP 관련 부작용의 경험이 있거나, 평
균 압력이 높은 환자들에서는 APAP 사용이 순응도 향상에
도움이 될 수 있을 것이라 생각된다.
상기도 이상 및 수술의 영향(Upper airway problem)
CPAP 치료에 영향을 미칠 수 있는 상기도의 구조적 이상
이나 질환으로는 비강에서는 비중격 만곡증, 부비동염, 알레
르기 또는 비알레르기 비염, 하비갑개 비후 등을 생각해 볼
수 있고, 구강 및 인두에서는 긴 목젖, 대설증(macroglossia),
설근부 비후, 구개편도 비대, 하악후퇴 등을 고려할 수 있다.
이 부분에 대한 연구는 다른 인자들에 비해 다소 부족한 것
으로 생각되나, 최근 CPAP 치료에 이비인후과 의사의 참여
가 늘어남에 따라 그 관심도가 증가하고 있다.
이 중 음향비강통기도검사(acoustic rhinometry)를 통해 산
출한 최소비강단면적(minimal cross-sectional area)이 CPAP
적정압력과 반비례하며, 비폐색으로 인한 비저항의 증가가 ad-herence에 영향을 미친다는 보고들이 있다.58,59,61-63) 또한 Na-kata 등62)은 하비갑개 부위의 비강 단면적이 0.6 cm2보다 작
을 경우, 75%의 민감도와 77%의 특이도로 CPAP 사용 실패
를 예측할 수 있다고 보고하였다. 또한 코막힘이 있는 CPAP
사용 환자에서 비중격만곡증에 대한 수술을 시행하였을 때
CPAP 적정압력이 2~3 cmH2O 정도 감소하는 것으로 알려져
있다.64,65)
CPAP 사용에 대한 구인두 수술의 영향에 대해서는 아직까
지 논란의 여지가 있다. Mortimore 등66)은 1996년에 시행한
임상실험에서 구개수구개인두성형술(uvulopalatopharyngo-plasty)을 시행한 환자가 깨어있는 상태에서 CPAP 치료를 시
행하였을 때 대조군에 비해 CPAP 치료를 잘 견디지 못하며
mouth leak 많다는 것을 발견하여 CPAP 치료에 잘 견디지 못
할 것으로 예상하였다. Han 등67)도 비슷한 결과를 보고하였으
나, 구개수를 보존하는 변형된 구개수구개인두 성형술을 시행
한 환자에서는 이와 같은 mouth leak이 발생하지 않는 것을
발견하였으며, Khan 등68)은 오히려 구개수구개인두 성형술 시
행 후 CPAP의 사용 압력이 유의하게 감소하였다고 보고하여,
최근에 많이 시행되는 구개수를 보존하는 구개수구개인두 성
형술은 과거의 방법과는 달리 CPAP 사용에 악영향을 주지 않
을 것으로 생각된다. 하지만 아직까지 구인두 수술이 CPAP 사
용시간에 실질적으로 도움을 준다는 보고는 없으며, 이에 대
한 연구가 향후 필요할 것으로 생각한다.
환자의 심리적 인자(Patient character)
환자의 성격이나 심리상태도 CPAP 순응도에 영향을 미칠
수 있다. 폐쇄공포증(claustrophobia)과 관련된 이전의 연구
들에서 수면무호흡증후군으로 진단받은 환자들의 약 50%가
만약 자신이 폐쇄공포증을 경험한다면 CPAP를 사용하지 않
을 것이라고 하였다고 답하였으며, CPAP 사용시간이 긴 환자
군에서 치료 전 평가한 폐쇄공포증 점수가 유의하게 낮았다.14,69)
이러한 폐쇄공포증이 있는 환자의 경우에는 먼저 낮 동안에
마스크를 착용하게 하여 환자로 하여금 마스크 사용에 익숙
하게 한 후, 점차 주간에서 야간으로, 이후 착용시간을 늘려가
는 방식으로 탈감작(desensitization)을 유도하면 CPAP 순응
도를 높일 수 있다.50)
폐쇄공포증 외에도 환자의 우울증 여부나 성향도 순응도에
영향을 미칠 수 있다. 비록 아직 논란의 여지가 있기는 하지만,70,71)
Wells 등42)은 수면무호흡 증상의 자각, CPAP 치료를 통한 증
상 변화, 부작용에 대한 인식에 있어 우울증이 있는 환자와 그
렇지 않은 환자 간에 유의한 차이를 보였다고 하였다. 또한 CP- AP 치료를 통해 OSAS 증상 호전을 체험한 환자들에서 순응
도가 높았으며, 치료 후에도 OSAS 증상의 변화가 없는 환자
들에서는 더 심한 우울증 증세를 보이는 것으로 보고하였다.
환자의 성격과의 관련성도 알려졌는데, 일반적으로 낙천적
이고 활발한 성격은 CPAP 치료에 긍정적인 동기를 유발시켜
순응도를 높게 한다.55) 반면 Wild 등28)과 Edinger와 Radtke50)
는 우울한 성격이나 건강염려증(hypochondriacal persona-lity)을 보이는 환자에서 CPAP adherence가 상대적으로 낮
았다고 보고하였다. 스트레스 상태에서의 대처 경향 중 하나
인 회피대처전략(avoidant coping strategies: 어려움이 닥쳤
을 경우 이를 회피해버리는 성격) 성향의 환자의 경우에도 ad-herence가 낮다는 보고가 있다.14)
최근 CPAP adherence를 높이기 위한 행동학적 측면에서
의 접근이 시도되고 있는데, 환자에게 CPAP 치료에 대해 완
전히 이해시키고, CPAP 사용에 대한 동기를 부여하여, 환자
스스로가 확신을 갖고 치료에 임할 수 있도록 하기 위함이다

(인지행동치료, cognitive behavioral therapy).21,36,72) 또한
CPAP를 사용함으로써 환자가 느끼는 주관적인 만족도에 대
해 환자 스스로 체감하도록 하고 치료 과정에 환자를 참여시
키는 것이 adherence를 높이는 데 효과적이다.73,74)
질환의 중증도에 대한 환자의 인식도 순응도에 영향을 미
친다. 순응도가 높은 환자의 경우 질환의 심각성에 대해 더 잘
인식하고 있는 경우가 많다.28) 반대로 환자가 주간졸림증과 같
은 주관적인 증상이 없는 경우, 치료에 대한 필요성을 인식하
지 못하여 순응도가 낮은 경우가 많다.75) 하지만, 이러한 환자
들에서도 다중수면잠복기검사(multiple sleep latency test)와
같은 검사상에서는 주간졸림이 확인되는 경우가 많으므로 이
를 환자에게 인지시키는 것도 중요하다.75)
가족 및 배우자 관련 인자(Bed partner and family)
가족 및 배우자가 순응도에 미치는 영향에 대해서도 몇몇
연구가 진행되었다. 배우자의 불만은 CPAP 치료의 흔한 실패
요인 중 하나이며,14,76) CPAP 구매에 있어서도 배우자의 역할
이 매우 크다.77) 처음 CPAP를 사용할 경우, 침대에서 CPAP를
착용하고 있는 환자의 모습이 배우자에게는 충격적인 장면일
수 있음을 이해해야 한다. 따라서, 가족구성원, 특히 배우자를
CPAP 시작 전 교육프로그램에 참여시켜 미리 CPAP 치료에
익숙하게 하는 것이 필요하며, 환자의 삶의 질 향상을 배우자
가 함께 느끼고 CPAP의 필요성을 이해하게 할 경우 환자의 ad-herence를 높일 수 있다는 보고도 있다.11,49) 또한 환자의 CPAP
치료는 수면시 배우자의 삶의 질 향상으로 이어지기도 한다.16)
사회경제적 요인(Socioeconomic status)
CPAP는 비교적 고가의 장비로 환자들에게 경제적인 부담
으로 작용하여78) CPAP 치료의 순응도를 저하시킬 수 있으며,
Brin 등77)과 Simon-Tuval 등78)은 사회경제적 수준과 CPAP
에 대한 순응도가 비례함을 보고하였다.
의사관련 요인(Doctor-related factor)
CPAP 사용에 있어 의사의 역할은 매우 중요하다. 환자와의
대화를 통해 환자의 상태를 정확하게 알려주고 치료 받지 않
았을 경우 발생할 수 있는 합병증의 가능성을 알려주며, 치료
의 필요성을 주지시킴으로써 CPAP 치료의 순응도를 높일 수
있다. 주간졸림증은 업무 효율을 떨어뜨리고 교통사고로 이어
질 수 있으며, 심혈관계 질환과 신경인지기능의 장애를 초래함
을 알려야 한다.11) 이 경우 의사는 본인 자신이 확신을 가지고,
환자의 눈높이 맞게 충분하게 설명해야 하며, 수면무호흡으
로 인한 합병증에 대해 경각심을 갖도록 설득해야 한다. 또한
환자로부터 CPAP를 사용하겠다는 구두약속을 받는 것이 순
응도 향상으로 이어진다는 보고도 있다.79)
하지만 이러한 접근은 의사의 권위를 긍정적으로 받아들이
는 초기에는 유효하지만 시간이 지날수록 환자들은 자신의 건
강과 관련된 문제에 대해서는 낙관적인 반응을 보이는 경향이
있으며, 점차 본인의 건강과 관련된 결정을 의사의 판단보다는
자기자신이 판단하려는 경향을 보인다.80) 이런 경우 의사는 환
자의 이러한 욕망을 이해하고 환자와의 대화를 통해 환자가
CPAP 치료의 필요성과 치료를 통한 이득(perceived benefit)
을 인지할 수 있도록 적극적으로 도와주어야 한다. 사회인지이
론(social cognitive theory)에 따르면 인간은 비용을 통해 얻
을 수 있는 이익이 비용보다 큰 것을 인지할 경우 CPAP와 같
은 예방적인 행동 및 생활방식의 변화를 취할 가능성이 더 높
게 된다.21) 또한 지속적인 추적관찰을 통해 환자가 CPAP 치
료에 잘 적응하는지를 확인하는 것이 중요하며, 그렇지 못할
경우 의사는 오히려 순응도를 낮추는 요인이 될 수 있다. 이러
한 비순응도의 가장 큰 원인 중 하나가 환자와의 대화 부족이
다. 따라서 의사는 환자에게 치료의 목적, 효과 및 발생할 수
있는 부작용 등에 대해 자세히 설명해 주는 것이 필요하며, 환
자가 CPAP 치료에 대해 더 잘 이해하게 될수록 환자의 불만
이 줄어들게 되며 순응도 또한 증가하게 된다(인지행동치료,
cognitive behavioral therapy).29)
인지행동치료에 따르면 환자의 이전의 경험에 대한 기억 및
이를 어떻게 인지하는지가 이후의 행동에 영향을 미치므로
환자의 CPAP에 대한 잘못된 지식을 수정해주고 CPAP 치료
로 인한 긍정적인 변화를 인지하게 하면 CPAP 치료의 순응
도를 높일 수 있다.81)
결 론
CPAP 순응도를 높이기 위한 전략
앞에서 우리는 CPAP에 대한 순응도에 영향을 미칠 수 있
는 인자들을 알아보았다. 이들 중에는 질환의 중증도 및 성격
등 환자에 관계된 인자들과 CPAP 기계 및 마스크 등 장비에
관계된 인자들, 의사에 관계된 인자들, 사용시 발생하는 부작
용 등 다양한 원인들이 관여할 수 있다(Fig. 1). 환자들은 이
인자들의 비용-이득(cost-benefit) 관계에 의해 CPAP 사용여
부를 결정하게 되며, 환자의 성격이나 의사와의 관계, 행동치
료 여부 등에 의해 판단의 저울추가 바뀔 수 있다(Fig. 2).
환자가 CPAP 치료를 받아들이기로 결정하는 것은 치료 초
반이 매우 중요하다. 이는 치료시작 후 수일 내에 이루어지며
치료시작 후 3~7일의 순응도 자료로 장기간의 순응도를 예측
할 수 있다.26,45) 또한 CPAP 치료 중단은 치료 시작 후 첫 3개
월 이내에 가장 높게 나타나므로47) CPAP 초기에 발생할 수

있는 여러 부작용들을 적극적으로 해결해 주는 것이 환자로
하여금 CPAP 치료를 유지할 의지를 높이는 데 도움이 될 것
이다. 저자는 이 부분에서 우리 이비인후과 의사의 장점이 있
다고 생각한다. CPAP 초반에 발생하는 부작용의 많은 부분
이 상기도와 관계된 경우가 많으며, 마스크와 접하는 안면부
위의 해부학적인 구조에 익숙한 이비인후과 의사들은 CPAP
착용시 발생하는 많은 부작용들을 쉽게 해결할 수 있다. 따라
서 환자가 CPAP와 관련된 불편함을 호소하였을 때 교과서적
인 지식뿐만 아니라 이비인후과 질환을 다뤄오면서 익히게 된
실질적인 경험을 치료에 사용한다면 더욱 더 좋은 결과를 얻
을 수 있을 것이다.
또한 환자와의 대화를 통해 질환의 중요성을 인식시키고 사
용 이후 환자가 얻게 되는 이득을(주간졸림의 향상, 두통 감
소 등) 인지하게 하는 것이 순응도를 높이는 데 중요한 역할을
한다. 앞에서도 언급하였다시피 치료 초반에는 환자가 의사의
권위를 받아들이고 지시에 잘 따르지만 시간이 지나가면서 본
인의 의지에 따라 치료를 선택하게 된다. 또한 모든 만성질환
의 치료가 그러하듯이 CPAP 치료는 환자에게 있어서는 생활
습관의 변화를 요구한다. 따라서 환자 자신이 치료의 효과를
체득하지 못하면 장기간 치료를 유지하는 것이 쉽지 않다. 그
러므로 환자가 CPAP를 사용함으로써 일어난 많은 긍정적인
변화를 대화를 통해 인지시키고, 치료의 중요성에 대해 정확
한 정보를 알려줌으로써 순응도의 향상을 꾀할 수 있다. 이 경
우 적극적인 교육 프로그램을 시행함으로써 장기 순응도를
향상시키며, 배우자도 함께 교육에 참여시키는 것도 도움이 된
다.76) 행동요법의 하나인 인지행동치료를 제공하는 것도 특히
치료초기 순응도를 향상시키는 데 있어 매우 중요하다.82)
