Improving AccessI believe our Mission calls us to be always thinking about improving access—making it easier for people to receive care that will alleviate their suffering—even if there are many different visions about how to get there. This is something on which I would hope we can all agree.

Pine Rest, as an organization, needs to continually be a part of access solutions.

Sometimes this means expanding our services in underserved communities—like the new Portage Clinic. Or, partnering with providers who have similar missions in order to lower overall cost to serve, such as we’ve done with the Christian Counseling Center. Or, providing services in a unique combination that best serves an individual’s diagnosis such as the integrated mental health and substance use treatment at our new Redwood inpatient unit.

We’ll need creative models to make the most of our existing resources that improve access—like the urgent care outpatient psychiatric access project that provides access within 48 hours being piloted right now at the North Shore Clinic. Or, consolidating Pine Rest Community Services that serve mostly low income individuals to one location in downtown Grand Rapids in order to help alleviate transportation issues and make outpatient care easier for this population to access. Or, the psychiatry residency program, which will operate an outpatient psychiatry clinic that should greatly reduce the wait time for psychiatric care.

Our breadth of talent and resources position us perfectly to contribute to research that can help establish new treatments and provide better outcomes to patients such as studying the effectiveness of ECT treatments to help alleviate agitation and aggression in Alzheimer’s and dementia patients. Or, looking at how the spiritual practice of forgiveness impacts the healing process.

The Affordable Care Act and Mental Health Parity provide greater access to mental health services, but the programs still leave gaps in coverage to all as well as the affordability of residential services and copayments for those with middle to low incomes. We’ll need to call on our friends, partners and the greater community to help us provide more access to services through our Patient Assistance Fund.

Our Mission calls us to provide healing and to alleviate suffering. Today that means providing better access to care through programs that are available in a timelier manner, in a more convenient location, and positioned for the best outcomes. We strive for not just access to services…but access for everyone to wellness and hope.

Many people have heard that there is a growing shortage of primary care physicians. But we are also facing a shortage of psychiatrists in West Michigan. Both the public and private sectors struggle to meet the demand for assessments and treatment for individuals who are in need of psychiatric care. The American Psychiatric Association recommends one psychiatrist per 10,000 people. West Michigan presently has only one per 14,000 people, and this shortage is expected to increase as Baby Boomer-aged psychiatrists retire.

“The Psychiatry Residency Program is a direct response to community need for more psychiatrists,” says Pine Rest President and CEO Mark Eastburg, PhD. “Pine Rest receives hundreds of calls each month from people who need to see a psychiatrist and yet have to experience long waits.”

In fall 2013, over 1,000 applications were received for the new psychiatry residency program. From those applications, 80 students were chosen to visit our campus from October through January for interviews, a tour of campus and lunch with faculty. From this process the Pine Rest Psychiatry Residency Program staff members ranked the candidates and the candidates ranked the programs where they interviewed. During the nationwide “Match Day” in March 2014, residents and residency programs learned which residency programs and individual medical students matched.

“I cannot begin to express in words how excited I am that we as an organization have a truly incredible and talented first year class of residents. We attracted very competitive and accomplished

medical students from across the country,” says Psychiatry Residency Program Director Dr. William Sanders. “They have a sincere passion and drive to become excellent psychiatrists. Having worked with them, I can say they each possess the essential traits of kindness, compassion and intelligence.”

Starting in 2015, the residency program will operate an outpatient psychiatry clinic that should greatly reduce the wait time for psychiatric care. We are hopeful that many of these residents will choose to stay and practice in our community after they have completed their training.

Residency Program Addresses Psychiatry ShortagePine Rest is building its Psychiatry Residency Program and will have 32 residents when all four years are filled in 2017. The first residency class started on July 1, 2014.

“The residency program means better and quicker treatment for our patients,” says Dr. Sanders.

Pine Rest offers residents the opportunity to work in an inpatient, outpatient, and residential environment. Working within the complete continuum of behavioral health services in the same organization is unique. In addition, rotations will include general medicine experience, consultation liaison in acute care hospitals and exposure to neurology.

The need for counseling, therapy and other outpatient services has never been greater. Mental health parity laws have given people improved ability to access behavioral health services. And younger generations are more willing to talk about and seek help with their mental health.

In 2014, Pine Rest added two clinics to its outpatient clinic network—Christian Counseling Center and the new Pine Rest Portage Clinic. In the past fiscal year, 267 Pine Rest clinicians provided over 260,000 client visits. Now with 19 locations throughout Michigan and Iowa, our outpatient clinic network can provide over 300,000 visits per year.

Christian Counseling CenterIn May 2014, Pine Rest welcomed Christian Counseling Center (CCC)—an organization with a similar mission and values plus a long history of similar work. Our coming together enhances both of our missions and is a natural fit.

It’s important to note that the CCC has been a strong organization. That said, its board and leadership recognized that in the coming years it would be increasingly difficult for a standalone entity to afford the infrastructure necessary to thrive in a highly networked, digital era. By coming together, we can exercise stewardship by not requiring two like-minded organizations to duplicate that infrastructure.

More Access to Hope, Help and HealingA couple needs marriage counseling to help save their marriage after the death of a child. A college student is feeling anxiety and depression from the weight of his class load, part-time job and financial pressures. A child is having trouble concentrating in school, acting out and being bullied. A recently retired teacher puts her life on hold to care for her sister who has just been diagnosed with Alzheimer’s.

New Outpatient Clinic in Portage“Since the very opening of the Kalamazoo Clinic, we have had requests to provide services at a location more convenient to persons living in Portage and the other communities south of Portage,” says Jon Weeldreyer, MA, manager of both the Kalamazoo and Portage clinics.

In March 2014, Pine Rest opened a new outpatient clinic in Portage, Michigan, with seven therapists. Individual, family, couple and group services are available for age groups from young children to older adults. The clinic provides services for mental health disorders, substance use disorders, as well as integrated services for persons dealing with both.

Integrated Treatment Provides Better Quality of Life

Historically, when mental illness and a substance use diagnosis co-occur, managing treatment of both separately has been difficult. However, current research has shown that treating both conditions together consistently leads to healthier outcomes for patients, including better medication management and reduction of psychiatric relapses, substance use, violence, suicide, legal problems, and utilization of the emergency room or inpatient services.

Pine Rest has seen an increase in patients with co-occurring mental health and substance use diagnoses over the past decade. One of only a few in the country, the new Redwood Unit at Pine Rest is a comprehensive, integrated treatment center dedicated to the treatment and care of patients with these types of co-occurring disorders.

“We estimate that more than one-third of our total annual inpatient visits are people diagnosed with co-occurring disorders,” says Lisa Schirmer, Ph.D., Director of Clinical Practice and Utilization Management. “This unit will allow us to treat these patients in one place to integrate their treatment experience.”

Co-occurring Treatment Provides Better Quality of LifeAccording to the Journal of the American Medical Association, approximately 50 percent of individuals diagnosed with a severe mental illness are also affected by a substance use disorder. In addition, those living with mental illness are especially sensitive to the effects of alcohol and other drugs. In some cases, individuals have used these substances to self-medicate or manage certain psychiatric symptoms.

Center. The new 20,000 square foot wing includes four single barrier-free rooms, six single rooms and six semi-private rooms.

The Redwood Unit was part of the 3-phase LEAP construction project which resulted in a facilities investment of $12.24 million and hiring of an additional 40 full time staff.

Several Pine Rest programs relocated into the new space, including StreetReach, Community Case Management, an Integrated Medicine Clinic and the DeMey Clinic. The four programs work with about 800 clients annually.

“These programs had outgrown their locations and many of their leases were expiring, so we started looking for other options that made sense for our clients,” says Harmony Gould, Director of Fiscal Operations, Community & Residential Services. “Dwelling Place is a great partner. We have many clients in common, most of whom have transportation issues that can be a barrier to getting to appointments and the help they need. The partnership was a way for Pine Rest to combine services and provide a more convenient and welcoming presence in downtown Grand Rapids.”

The new facility includes 16 rooms that are used for interviews, including telepsychiatry, therapy and exam rooms as well as group and larger meeting rooms. Services offered include psychiatry, medication management, individual and group therapy, addiction services, peer support specialists and case management.

“The office area is friendly, inviting and therapeutic, but also takes into consideration both confidentiality and safety issues,” says Gould. “Our clients have given very positive feedback about the new space and location.”

In addition, the centralized office is able to schedule clients for same-day assessments. Already this practice has helped to decrease the no-show rate by nearly 50%, since many times clients have difficulty finding transportation and getting time off from their employer for a future date.

Easier Access to Services in the Heart of Grand RapidsOn March 17, 2014, Pine Rest Community Services opened at its new location in downtown Grand Rapids on Commerce Avenue between Bartlett and Goodrich Streets. The 15,100 sq. ft. space runs along Commerce Avenue. It is part of the Herkimer renovation project owned by Dwelling Place of Grand Rapids.

Dementia patients who were severely agitated and aggressive benefitted from electroconvulsive therapy.

“The experience is frightening for patients, who often are unable to describe what they are thinking or feeling,” says Louis Nykamp, MD, Pine Rest ECT Medical Director. “And it causes distress for family members and caregivers.”

Dementia patients who were severely agitated and aggressive benefitted from electroconvulsive therapy (ECT) in a study conducted by researchers at Pine Rest, McLean Hospital at Harvard Medical School and Duke University School of Medicine. The preliminary study involved 23 patients, and most showed significant improvement after treatment.

“Unfortunately, a lot of people’s information and understanding of ECT comes from older movies, where it is dramatized as barbaric,” he said. “The actual procedure is radically different than what is portrayed in the movies.”

In the treatment, patients are given medication to fall asleep, followed by medicine to relax the muscles. Two electrodes are placed on a patient’s scalp, and deliver a five- to seven-second electrical current that induces a brief seizure in the brain. After 20 minutes in a recovery room, the patient is ready to return home or go back to his or her hospital room.

The Pine Rest clinicians received positive feedback from some family members of the patients in the study. “They were pleased to see loved ones in substantially less distress,” he said. “to be able to interact with them, to know they could be fed and clothed and cared for without a boxing match at every turn provided a good deal of relief.”

“We don’t have the medication tools in our toolbox often to sufficiently help with the degree of agitation and distress that we sometimes see in severe dementia,” he said. “(ECT) represents an effective approach, and with a good margin of safety. Results were encouraging and suggest that ECT may be a rapidly acting, safe, and effective treatment for certain patients with dementia and behavioral disturbances that do not respond to or tolerate standard behavioral interventions and medications. Future research is warranted.”

The Pine Rest research group hopes to collaborate and be a part of this larger study to confirm the results of this preliminary trial.

The study was published in the May 2014 issue of the International Journal of Geriatric Psychiatry for their contributions to the research project. The Pine Rest clinicians involved in the research are psychiatrists Eric D. Achtyes, MD; Jack A. Mahdasian, MD; Louis J. Nykamp, MD; nurse Lori Van der Schuur White, RN; and LeAnn Smart, Research Coordinator.

Collaborating on Dementia Treatment ResearchAgitation and aggression are among the most frequent and disruptive behavioral complications of dementia that contribute to increased cost of care, hospitalization, caregiver burden, and risk of premature institutionalization. Dementia patients may refuse clothing changes or bathing. And when caregivers try to help, those with the complication of dementia may yell, hit, kick, shove and bite.

Integrated Treatment Provides Better Quality of Life

In October 2013, Pine Rest Pastoral Servicesopened enrollment for a study in collaboration with Fuller School of Psychology to investigatethe impact of forgiveness and spiritual distress on the treatment outcomes for individuals in apsychiatric inpatient setting.

Rev. Karl VanHarn, DMin, Director of Pastoral Services, is the principal investigator, andRev. Janet Carter, MDiv, BCC, Chaplain, is the co-investigator. The two are working closelywith adult inpatient staff to identify patients for the study.

The overarching purpose is to examine the natureand possible impact of forgiveness (i.e., interpersonal, self, and divine) on the spiritual functioning ofpatients in an inpatient psychiatry program. The study draws on qualitative questions andstandardized quantitative assessments.

“The Pastoral Services staff discussed a list of topics for research. We wanted it to be a topicthat would benefit our patients and the care we provide at Pine Rest,” said Rev. Van Harn.“The research results could influence the way we lead groups and the conversations we havewith patients.”

Patient participation in the study is voluntary and is open to men and women of all races, religionsand national origins. Two questionnaires are completed while at Pine Rest. A third is completedtwo months after discharge.

Researching the Impact of Forgiveness Can working on forgiveness during an inpatient stay lead to better mental and spiritual health? Does practicing forgiveness reduce spiritual distress? What are the unique spiritual and mental health consequences of the three different types of forgiveness?

Pine Rest Pastoral Services is studying the impact of forgiveness and spiritual distress on treatment outcomes.

“One patient thought the study would be difficult and uncomfortable, but he felt it would help him talk about his issues,” says Rev. Carter. “Other patients have said they enjoy being able to make a difference. It makes them feel good.”

However, what if you are suffering from a mental health issue that requires urgent psychiatric attention? All too often, the answer is a long wait for access to care that could take several weeks, while your condition gets progressively worse.

In July 2013, Pine Rest’s North Shore Clinic implemented a pilot program that reduces long waits for urgent access to psychiatric care and provides a much needed lifeline to those suffering from an urgent mental health issue.

“This new model of care is much needed in the community,” says Megan Zambiasi, MA, manager of the North Shore Clinic during the pilot.

“We are now able to provide the kind of urgent mental health care that can help prevent hospitalizations and decrease the number of suicides.” Studies show that before many individuals attempt suicide, they often seek out services from a health care provider.

The point person for the program triages potential cases as they are presented to the clinic. After assessment, the case is assigned to the psychiatry team at the clinic and clients are treated according to the severity of the case. The North Shore program provides urgent access to care within 48 business hours; and for non urgent cases, every attempt is made to offer an appointment within two weeks. It’s a significant accomplishment for any clinic, especially one with a service area that extends from Ludington to Olive Township.

“We started by providing urgent care two hours per week,” says Zambiasi. “We are currently offering eight urgent care hours per week, and these hours can be used flexibly for new

evaluations, appointments for current patients with urgent needs, or a combination of the two. Our model does not substitute for emergency care. Our emergency protocols have not changed and individuals are directed to call 911 in the event of an emergency. However, in an urgent situation, we now have the ability to provide clients the opportunity to be treated before the condition becomes an emergency.”

The model provides a template for similar services at Pine Rest outpatient clinics. “People were not getting in to our offices quickly enough,” says Zambiasi. “Staff assigned to the project really embraced the effort because they recognized the tremendous need that is present in all our communities.”

As you review these numbers, please note the following: • Revenue from our core clinical services grew by 16% due in part from Pine Rest acquiring 22 licensed hospital beds from Saint Mary’s

to support our new residency program. Demand for Pine Rest services remains steady and strong

• Contract services income figure relates to those amounts reimbursed to Pine Rest for expenses incurred in relation to our joint operating agreement with Saint Mary’s Health Care

• Investment related income reflects the gains and losses on all of Pine Rest’s investments, including the Pine Rest Foundation. These gains reflect our participation in the broader financial market’s appreciation in the past few years

• Overall expenses grew by 11%, which reflects the added expense required to support the significant revenue growth

Providing financial support and resources is another way that Pine Rest assures access to compassionate behavioral health care to those underserved in our communities. Sometimes this means that we provide a service for less than the full cost. Other times, we provide services even when we know the patient, client or resident will be unable to pay the bill.

Pine Rest Foundation On the MoveI am pleased to introduce myself in this publication as the new executive director of the Pine Rest Foundation. I began my role in April 2014, and it has been energizing to learn more about Pine Rest Christian Mental Health Services’ long history in our community, including the generations of people who have made such a difference to this institution. I have gained important insights and have begun to initiate ways to enhance the Foundation’s support of Pine Rest over time, and I look forward to sharing progress with you in the years ahead. However, before we move forward, the annual report is about taking a look back. I want to extend my gratitude to the leaders who preceded me. Randy Johnson transitioned out of the executive director role into retirement at the end of 2014. During that transition, Pete Harkema stepped in as interim executive director, ably steering the ship while the search for my position progressed. Under his leadership, new programmatic elements that had been under development were launched.

Mission Builders is a way to stay connected to those who have served Pine Rest in a variety of capacities—from past employment to volunteer service. Many of these individuals remain passionate about our work and offer a unique perspective that we hope to keep at the table.

PR Connect is a group of diverse young leaders who are engaged in reducing the stigma of mental illness among their peers, and the broader community, by opening a dialogue about behavioral health. The group is serving an important role in augmenting our communications, with a goal of making people receptive to seeking care when it is needed.

During this time, the Foundation team continued to raise funds in support of Pine Rest—for the Pine Rest Foundation Endowment, for strategic projects and programs and for a key priority, the Patient Assistance Fund (PAF), which exists to ensure that those with financial need can secure needed treatments.

A portion of the annual endowment distribution goes toward the PAF, but this does not meet the need. Each year, the Foundation also raises current funds to fill the gap; however, the need is growing. That is why programs like Mission Builders and PR Connect are so important. Pine Rest Christian Mental Health Services can meet its mission better by having more advocates who understand the importance of behavioral health care and of providing access to services. We need more people to share the message that a grateful family member recently shared with us:

“My hope is that funding for PAF is made a high priority, especially for those young people who need help. If a family cannot afford to pay, it’s imperative that there is help available for them. Truly, [therapy] saves lives…lives worth saving.”

Who are among our strongest advocates in carrying this message? Our donors. They have taken a step beyond advocacy to action. In the following pages, you will see the names of those who have made a commitment to individuals and families such as this one, a commitment of caring. They are the backbone of our work. Through their philanthropy, they stand arm-in-arm with us as Pine Rest responds to those who are seeking a solution. They open up new possibilities for those who are in need of hope.
