Quando?
Elezione
✓ Nelle exeresi linfonodali radicali
Is It Necessary To Place Prophylactically an Abdominal
Drain To Prevent Surgical Site Infection in Abdominal
Operations? A Systematic Meta-Review.
Wu X, Tian W, Kubilay NZ, Ren J, Li J
Surg Infect (Larchmt) 2016 Dec;17(6):730-738
CONCLUSIONS: Most reviews fail to detect an increased incidence of
surgical site infection in the setting of drainage placement but also fail to
attribute any specific benefits to the presence of a drain in abdominal
procedures.

Drainages in abdominal surgery: (in)dispensable?
Globke B, Schmelzle M, Bahra M, Pratschke J, Neudecker J
Chirurg. 2017 May;88(5):395-400
CONCLUSION: There is evidence that drains should not be routinely used in
elective abdominal surgery, such as cholecystectomy, colorectal resection and
gastrectomy. Even for some cases of pancreatic and liver resection, there is growing
evidence that routine placement of drains is not mandatory. In conclusion, there is a
need for more prospective randomized controlled trials.
Quando?
Urgenza
potenzialmente contaminato
Vantaggi
✓ Funzione di “spia”
✓ Garantire la detersione del sito chirurgico
✓ Permettere di intercettare raccolte post-operatorie per “mobilizzazione”
✓ “Guidare” eventuali fistole all’esterno
✓ Possibilità di eseguire esami colturali post-operatori
✓ Possibilità di eseguire esami biochimici post-operatori
✓ Possibilità di eseguire lavaggio continuo (tecnica aperta vs chiusa)
✓ Possibilità di eseguire fistolografie
Svantaggi
✓ Dolore
✓ Porta di ingresso per infezioni
✓ Lesioni da decubito (emorragie, fistole, …)
✓ “Entrapment” di visceri (omento, tube, …)
✓ Rottura accidentale

Laparoscopic lavage versus resection in perforated diverticulitis with purulent
peritonitis: a meta-analysis of randomized controlled trials
World Journal of Emergency Surgery 2016
Conclusions
The present meta-analysis shows that in acute perforated diverticulitis with
purulent peritonitis laparoscopic lavage is comparable to sigmoid resection in
term of mortality but it is associated with a significantly higher rate of
reoperations and a higher rate of intra-abdominal abscess. No differences in
term of mortality were demonstrated at follow-up. Further studies are needed to
better define the safety and appropriateness of this treatment.
