The IHI/NPSF Lucian Leape Institute takes its name from the internationally recognized leader of the patient safety movement, Lucian Leape. Dr. Leape is a health policy analyst whose groundbreaking research on patient safety and the quality of care began with the publication of his seminal article “Error in Medicine” in the Journal of the American Medical Association in 1994. He was also a member of the Institute of Medicine's Quality of Care in America Committee, which issued To Err Is Human: Building a Safer Health System in 1999 and Crossing the Quality Chasm in 2001.
Dr. Leape is one of the founders of the National Patient Safety Foundation, the Massachusetts Coalition for the Prevention of Medical Error, and the Harvard Kennedy School Executive Session on Medical Error.
Keynote Dinner Speaker:Kirsten Bibbins-Domingo, PhD, MD, MASDr. Bibbins-Domingo is the Lee Goldman MD Endowed Chair in Medicine and Professor of Medicine and of Epidemiol-ogy and Biostatistics at the University of California, San Francisco (UCSF). She is director of the UCSF Center for Vulnerable Populations at Zuckerberg San Francisco General Hospital, a research center focused on discovery, implementation, policy and advocacy, and community engagement for populations at risk for poor health and inadequate health care.
Dr. Bibbins-Domingo is a recognized expert in evidence-based prevention in both clinical and public health settings. She is the Chair of the US Preventive Services Task Force , an inducted member of the American Society for Clinical Investigation and the National Academy of Medicine, and a practicing general internist at Zuckerberg San Francisco General Hospital.

The IHI/NPSF Lucian Leape Institute
In 2017, as the Lucian Leape Institute celebrates ten years of providing vision for the field of patient safety, its parent, the National Patient Safety Foundation celebrated twenty years of pursuing freedom from harm for patients and the health care workforce. In this same milestone year, NPSF has joined forces with the Institute for Health-care Improvement (IHI), combining our knowledge and resources to focus and energize the patient safety agenda in order to build systems of safety across the continuum of care. Effective May 1, 2017, IHI and NPSF merged into one organization, under the name Institute for Healthcare Improvement.
The IHI/NPSF Lucian Leape Institute, named for interna-tionally known patient safety leader Dr. Lucian Leape, who served as the initial chairman and still serves now as a member, continues its work to identify new approaches and develop a strategic roadmap to improving safety in health care at the systems level.
In their initial work, the Institute members outlined five concepts fundamental to making sustainable improve-ments to the safety of our health system: Transparency, Care Integration, Patient/Consumer Engagement, Restora-tion of Joy and Meaning in Work, and Medical Education Reform.
Subsequently, the Institute members began their work exploring the concepts in depth and reporting on their findings and recommendations. The series of completed reports has revealed how much the issues overlap and intersect. It is difficult to imagine robust patient and family engagement without greater transparency, for example. Likewise, greater patient and family engagement is essen-tial if we are to work together to improve care integration.
It has become particularly clear that strong leadership and a culture of safety are essential for lasting improvement in patient safety. But changing culture takes time, and not all leaders know where or how to begin. The move
from fee-for-service to value-based payment models is forcing CEOs, and board members, to become much more engaged in quality and safety issues than they have been traditionally. Leaders, direc-tors, and others who hold the power to improve patient and workforce safety need to ask the right questions and demand results.
It is important to note that we have made progress in patient safety. System flaws are now much more widely recognized as causes of medical error than they were 20 years ago. Research about human factors, adverse events, and sources of errors has proliferated, with a number of journals now focused specifically on quality and safety. Indeed, the field is now recognized as a unique medical discipline, with 1,600 people worldwide holding profes-sional certification in patient safety.
But changing the status quo requires more than research, science, and the development of protocols. It requires—from providers, industry, and all those determined to ensure the delivery of safe care—leadership, commit-ment, vision, and the will to make the right choices.
Recommendations from LLI command attention and action. Engaging at both the influencer and grassroots levels, this work, as intended, has resulted not only in awareness of critical issues and needed corrective mea-sures, but implementation efforts as well.
We are encouraged by the progress that has been achieved and the attention generated by the Institute's activities. It is a time of great change in health care, and we look forward to the Institute’s continued impact, and your support for this essential work, in the years ahead.
