Case Provider: Personnel that oversees a “Provider List” which consists of ~10 students with IEPs

Transition Plan: Post-Secondary Plan for students with disabilities

IEP Input Form: Form used to gather input from multiple teachers/professionals prior to a student’s IEP meeting to help guide what services will be provided

Section 7 is at the beginning of the IEP and is a narrative that describes the student's’ strengths, needs, and current performance in the following areas:● Academic● Behavior● Social● Functional● Independent Living● Transition (within the school and post-secondary)● Communication● Developmental● Language

Section 7 is a comprehensive narrative addressing all aspects of the student. It is what justifies the rest of the IEP.

IEP Input Form Importance

● The input of both the general and special education teacher is essential to write an effective and accurate IEP

● Multiple perspectives/sources of data are required to accurately determine a student’s strengths and needs

● This is a legal requirement

IEP Input Form Protocol

● IEP Input Form will be sent via e-mail to General and Special Education teachers at least 20 school days prior to the scheduled IEP Meeting

● General and Special Education Teachers have 5 school days to complete the IEP Input form to the best of their ability and e-mail the Case Provider when the form is completed. *Please provide as much information as possible*

● It is very important that the IEP Input Form is completed on time in order for the Case Provider to complete their IEP Draft on time - 10 School Days Prior to the Scheduled IEP Meeting

SSM: How to make a Caseload

1. Go halfway down the home page and find the title “My Students”2. Click on “Edit” next to “My Students”3. Click “Add Students to Standard Caseload” in the top left hand corner4. Use student name and/or ID to search for individual students5. Check the box next to the student’s name and it should be highlighted

in green6. Click the Green Button: “Add Student Marked Above to Standard

Caseload”*The student will now be on your Home Page under “My Students”

General Education IEP Paperwork Requirements

● Read and Maintain your IEP/504 Binder○ Trade out NEW paperwork when you receive it (New paperwork will be

placed in your mailboxes)○ Shred any old paperwork (DO NOT just throw it in the garbage)○ Keep your binder in a locked drawer/cabinet

● Using your Rosters, create your Caseload of students in SSM (IEPs and 504s)

● Keep all files and information confidential
