Some Terms...
Case Manager: Personnel that oversees all Students with IEPs and 504s
Case Provider: Personnel that oversees a “Provider List” which consists of ~10 students with IEPs
Transition Plan: Post-Secondary Plan for students with disabilities
IEP Input Form: Form used to gather input from multiple teachers/professionals prior to a student’s IEP meeting to help guide what services will be provided
Assistive Technology: Assistive/Adapted Devices

Section 7: Language and General Considerations
Section 7 is at the beginning of the IEP and is a narrative that describes the student's’ strengths, needs, and current performance in the following areas:● Academic● Behavior● Social● Functional● Independent Living● Transition (within the school and post-secondary)● Communication● Developmental● Language
Additional parts of Section 7
Curriculum-Based Assessment Data: Projects, quizzes, tests, midterms, finals, etc.
Standardized/District-Wide Assessment Data: COMPASS, EPAS, Reach, etc.
Transition Assessment Data: Student interest surveys, career/college match surveys, student interviews, independent functioning assessments, etc.

Parent Concerns and Input
The Importance of Section 7
Section 7 is a comprehensive narrative addressing all aspects of the student. It is what justifies the rest of the IEP.
The combination of teacher narratives, assessment data, and intervention results determines the services the student receives from the Diverse Learner Department. *All three are necessary*
The Transition Plan and Section 7 drive the rest of the IEP

IEP Input Form Importance
● The input of both the general and special education teacher is essential to write an effective and accurate IEP
● Multiple perspectives/sources of data are required to accurately determine a student’s strengths and needs
● This is a legal requirement
IEP Input Form Protocol
● IEP Input Form will be sent via e-mail to General and Special Education teachers at least 20 school days prior to the scheduled IEP Meeting
● General and Special Education Teachers have 5 school days to complete the IEP Input form to the best of their ability and e-mail the Case Provider when the form is completed. *Please provide as much information as possible*
● It is very important that the IEP Input Form is completed on time in order for the Case Provider to complete their IEP Draft on time - 10 School Days Prior to the Scheduled IEP Meeting

● We will be using the same Google Form that was used last year ○ We will be asking for feedback later to make necessary improvements for
this year
● The Google Form will be sent via-email with a link to complete the form○ Please contact the Case Provider and/or Case Manager if the link is not
working
● Please provide as much narrative as possible in each section of the Input Form

SSM Sign-In
You can access YOUR students’ IEPs using the online SSM System*Do not look at students’ IEPs that you do not physically teach due to confidentiality*
1. Go to the IMPACT Home Page2. Click on SSM at the top of the Page3. Use your CPS Username and Password to Sign-In

SSM: How to make a Caseload
1. Go halfway down the home page and find the title “My Students”2. Click on “Edit” next to “My Students”3. Click “Add Students to Standard Caseload” in the top left hand corner4. Use student name and/or ID to search for individual students5. Check the box next to the student’s name and it should be highlighted
in green6. Click the Green Button: “Add Student Marked Above to Standard
Caseload”*The student will now be on your Home Page under “My Students”

It should say “IEP” under Type
5. Check the box next to the student’s name and it should be highlighted in green6. Click the Green Button: “Add Student Marked Above to Standard Caseload”

SSM: Navigating through the IEP
*The Special and General Education Teachers should read the entire IEP in order to fully understand the student’s strengths and needs*

Modifications
How do you find these sections in SSM you ask?
1. On your homepage find “My Classes” on the right side of your screen2. Click on a class
3. You can check on the right side of your list of students to see who has an IEP or 504
4. Scroll over the small arrow on the left side of the page next to the student ID number and the “Student Profile” and “Documents” will show up.5. Click on “Documents”
3. Click on “Individual Education Program (IEP)”
1. Navigate to the different sections of the IEP by placing your curser over the “Cover Sheet” button in the top left corner under “My Home Page”
*The “Cover Sheet” button will change depending on the section you are viewing in the IEP*
General Education IEP Paperwork Requirements
● Read and Maintain your IEP/504 Binder○ Trade out NEW paperwork when you receive it (New paperwork will be
placed in your mailboxes)○ Shred any old paperwork (DO NOT just throw it in the garbage)○ Keep your binder in a locked drawer/cabinet
● Using your Rosters, create your Caseload of students in SSM (IEPs and 504s)
● Keep all files and information confidential
