Creating a Culture of SafetyCreating a Culture of Safety Monthly rounding by Senior Leadership Interacting with front line staff to hear their concerns

regarding safety and how the next patient may be harmed Open lines of communication and lets staff identify

defects in processes or equipment to improve patient care.

A Culture of Safety is ensuring every patient, every nurse, every physician, EVERY TIME receives best practice evidence based medicine.

SENIOR LEADER & UNIT PARTNERSHIP / WALK-ROUNDS GOAL: Establish an environment of trust for frontline caregivers (“We are here to learn and improve, not to blame or audit”) COMMON DISCUSSION TOPI CS: Tell me what is working well today? Is there any staff that we should be recognizing? Is there anything we can do better? Do you have the tools and equipment to do your job? Are you aware of any recent Incident reports? Please describe how the next patient in this clinical area will be harmed. Can you think of a patient harmed because of less-than-safe care? What could this unit do on a regular basis to improve safety? Are there any immediate safety concerns that you need assistance with? Are there any current patient needs that you need assistance with? Review actions taken/not taken to address issues from previous rounds. Remember: The % of caregivers exposed to rounds over time should be maximized (# rounds as well as connecting with different caregivers each round). Other (Recent Huddles, etc.)
