- Cumulative GPA (if pertinent) and internships

Provide RN license #, the state(s) you are licensed as an
RN, and the expiration date of your license.
Provide LVN license #, state(s) licensed in as an LVN, and
the expiration date of your license.
If not licensed as a nurse:
- Include the month and year you plan to take NCLEX-RN
