Factors

O Very important

O Normal body temperature 37◦C

O Heat production less than heat loss

O Cold, windy, wet, unfit, injured

O Heat produced in muscles during exercise

Page 3: Hypothermia - Coniston Mountain Rescue Team · Hypothermia Algorithm for Fell Use continued Patient Is Shivering Shelter from wind Change wet clothing for dry clothing Hot packs/

Problem

O Shivering produces heat

O Below 33◦C shivering stops so body

temperature falls much faster

O Mild hypothermia 33-37◦C

O Severe hypothermia below 33◦C

O Usual scenario exposure (? due to injury)

– collapse with hypothermia

Page 4: Hypothermia - Coniston Mountain Rescue Team · Hypothermia Algorithm for Fell Use continued Patient Is Shivering Shelter from wind Change wet clothing for dry clothing Hot packs/

Symptoms

O Mild

O 37 - 35◦C

O “chilly”

O Stiff fingers

O Numb

O Muscles uncoordinated

O Shivering starts

Page 5: Hypothermia - Coniston Mountain Rescue Team · Hypothermia Algorithm for Fell Use continued Patient Is Shivering Shelter from wind Change wet clothing for dry clothing Hot packs/

Symptoms

O Moderate

O 35 - 33◦C

O Stumbling

O Confused

O Weakness

O Speech slow

O Shivering stops!

Page 6: Hypothermia - Coniston Mountain Rescue Team · Hypothermia Algorithm for Fell Use continued Patient Is Shivering Shelter from wind Change wet clothing for dry clothing Hot packs/

Symptoms

O Severe

O 32 - 30◦C

O Unable to walk or stand

O Slow pulse

O Irritable

O Reduced respiratory rate

O Severe muscular incoordination

Page 7: Hypothermia - Coniston Mountain Rescue Team · Hypothermia Algorithm for Fell Use continued Patient Is Shivering Shelter from wind Change wet clothing for dry clothing Hot packs/

Symptoms

O Even lower!

O 30 - 28◦C

O Muscles rigid

O Semi-conscious

O Pupils dilated

O Pulse & breathing may be difficult to

detect

O Below 28◦C Unconscious/ cardiac arrest

Page 8: Hypothermia - Coniston Mountain Rescue Team · Hypothermia Algorithm for Fell Use continued Patient Is Shivering Shelter from wind Change wet clothing for dry clothing Hot packs/

Treatment

O Follow algorithm

Hypothermia Algorithm for Fell Use (MRC England & Wales)

Initial Casualty Assessment

Personal & casualty security

ABC & conscious level

Head to toe examination

Look for signs of life Assess skin temperature

If possible check pulse, BP, temp, GCS

Is casualty shivering?

Hypothermic???

Hypothermia Algorithm for Fell Use continued

Patient Is Shivering

Shelter from wind

Change wet clothing for dry

clothing

Hot packs/ hot air

Food & warm drinks

Insulate

Evacuate (carry off)

Mild Hypothermia

Hypothermia Algorithm for Fell Use continued

Patient Not Shivering

Signs of Life (YES)

Insulate

No external heat

Do not remove clothing

Evacuate Smoothly

Helicopter if possible

Severe Hypothermia

Hypothermia Algorithm for Fell Use

Patient not shivering

Severe Hypothermia

No signs of life

NO

Do not perform CPR

Insulate

No external heat

Do not remove clothing

Evacuate smoothly

Helicopter if possible

YES

Diagnose death

Injuries compatible with death?

Frostbite

O Thankfully rare

O Freezing of tissues

O Toes, fingers and nose
