You know..John might have the right idea…..
• For years we have been fed a lie..that flying is safe..Flying is not safe. Pilots and maintenance technicians need to take every action within reason to minimize threatening situations.
Each flight is a series of links in a chain
• As we prepare for a flight, we need to:
flight environment, personal skills, equipment, navigation requirements and
weather.
Identify -Access - Manage - Monitor
There are 7 steps
• Define Mission• Identify Safety Concerns• Access Risks• Identify Options• Balance Risk vs. Gain• Execute Your Decision• Monitor
Step 1: My Mission
To Fly my 1953 Cessna 170 B toGrand Cayman Island for the International Aviation Week
2nd Step: Identify Concerns
• 1. Fuel
• 1a. Airworthiness
• 1b. Personal Skills
• 1c. Weather
• 1d. Survival Equipment
• 1.e NAV/COM Equipment
• 1f. Flight Environment
3rd Step: Access Risks
Probability
Severity
Rare
Survivable
Acceptable
Acceptable
Exposure Manageable Acceptable
Planes don’t just fall out of the sky.
People survive ditching in the ocean with proper equipment and training.
45 minutes from land at the most distant point.
4th Step: Identify Options
• Avoid Risks
• Accept Risks
• Reduce Risks
• Take the Airlines
• Understand your personal risk tolerance
• Proper preflight planning and maintenance will minimize problems.
5th Step: Risk vs Gain
• Analyze risk level for the mission with your controls in place. This step serves as a “Reality Check” to verify the desire to undertake the mission.
6th Step: Execute The Decision
7th Step: Monitor The Situation
Risk Management Model
Hazards Associated with Task
Mission
Severity Probability Exposure
Risk Level - Is it Acceptable?
No - The Risk Is Not Acceptable
• Eliminate Risk
• Reduce Risk• Modify Risk
• Cancel Mission
Risk Management is a Continuos Process
Yes - The Risk Is Acceptable
• Control Severity, Probablity & Exposure
• Execute Mission
• Monitor The Mission
Risk Management is a Continuos Process
Enjoy It!(I was still thinking about the trip home)
