ENHANCED FDB Medications

• New referrals entered prior to 3/11/20 with a SOC date completed on or after 3.11.20 will NOT require a conversion of any medications entered on the referral or imported from one of the medication history interfaces nor will the enhanced FDB medications be available for entry within a SOC visit

• Any episode on which the recert order due IDG meeting is created on or after 3/11/20 will require a conversion of existing medications on the patient’s medication list to enhanced FDB medications as part of that IDG meeting details.

Hospice recertifications & Enhanced FDB medications• Conversion of existing medications will re required as hospice

episodes are recertified and the recert order due IDG meeting is created

• The conversion can be completed in PointCare or the back office

• The Registered nurse IDG Role will be required to convert each medication and the medical Director will be given the opportunity to review the patient’s medication list with the new enhanced FDB medications prior to signing the meeting details

To convert meds in back office via the IDG console • Select the recert order due meeting and

select the view details button

• Select the IDG meeting details for the Registered Nurse role and select edit/sign details:

• If patient is due to have their meds converted to Enhanced FDB meds a message will display in the upper right corner and a convert medication button will be enabled

Convert meds via back office cont. • Once conversion has been initiated all meds must

be converted

• Selecting a med will enable the convert button

• Clicking the convert button will open the med picker and will display the new medications search screen

Medication conversion from back office • Clicking in the med search field

will display a list of potential enhanced FDB meds that match the med info displayed

• The following fields will be auto-populated:• Frequency• Start date• Financial responsibility

• Complete the rest of medication fields

• As each med is converted to the appropriate enhanced FDB med the user is returned to the list of the patient’s current active meds and the meds will be displayed in green text reflecting the conversion

Converting existing unlisted medication to Enhanced FDB medications • Expanded list of available meds

within the Enhanced FDB meds many of the previously unlisted meds will now be available.

• Selecting unlisted meds will enable the convert button

• The med picker will then display the new med search screen

• Clicking med search field will display a list of potential enhanced FDB meds that match the med info displayed

• Once the correct med name/dosage is selected the new med picker will open the add/edit med form so the conversion can be completed

Converting existing unlisted meds to enhanced unlisted medications

• Some meds will still not be available in the expanded list

• These types of existing unlisted medications will have to be converted to enhance unlisted medications

• Selecting medication will enable the convert button

• Follow steps listed above for med conversion

been converted, the Save and Close button will be enabled

Med conversion continued for back office• The conversion of

existing non-enhanced medications to enhanced FDB medication will create a hospice physician order as documentation of the conversion

• The order will be inserted as approved and will be marked as DO Not SEND so that no actions are required

Back office med conversion FOR MD• The IDG meeting detail must be

signed by the IDG Registered Nurse for the medications conversion to be considered complete so it is very important that the meeting details be signed in a timely manner

• The conversion of existing medications will prompt review of medications by the IDG role-medical director

• The conversion will trigger an already existing message on the IDG details for the MD indicating medications have changed.

• The MD can review medications from the medical record info button

Back office Med conversion for MD • Selecting the

review meds will check the medications for interactions and mark the meds as reviewed

• Once the Medical Director has closed the patient’s med list, they are returned to the IDG meeting details for the IDG role

Conversion to FDB meds via the IDG meeting

• From the visit list screen in POINTCARE, select the menu and the IDG meetings option and from the drop down, select the patient to open a list of the available IDG meetings for the selected patient

• This will be available only on the Recert Order Due meeting

Med Conversion on PointCare Device

• Tap the recert order due meeting for the patient

• If the med conversion is required a message will be displayed at the top of the screen advising the medication must be converted to save and sign the meeting details and a new section will be displayed titled “converted medication” with a medications button.

• The save button is not available until all medications have been converted.

Med Conversion on PointCare Device • Each med on the

patient’s med list will have to be converted individually

• To initiate the conversion, tap and hold the medication to be converted.

• Select Convert

Med Conversion on PointCare Device• PointCare will search the FDB

Medications data for all possible matches for that specific drug name

• The medication information section will include the existing Routed Medication, Dose, and Amount. Using this information, select the appropriate Enhanced FDB Medication from the medication search results.

• Once the correct Enhanced FDB Medication has been located in the search results, select that medication, and the Edit Medication screen will be displayed.

• The following required fields on this screen will be auto-populate based upon the information related to the medication being converted

• Frequency• Start Date• Financial Responsibility

Med Conversion on PointCare Device

• Once these two fields have the correct values, any instructions needed can be added, and the Save button selected to save the conversion. The user is then returned to the Patient’s Medication List with the converted medication shown with a green check to indicate it has been converted This same process will need to be repeated for each medication listed for the patient.

Med Conversion on PointCare Device

• Once all existing medications have been converted, the Save option can be selected from the menu. This will return the user to the IDG Meeting Details screen on which the Medications button will be disabled. The IDG Meeting Details can now be saved and the user has the option in the menu to Sign Details:

• Then SYNC. Medical Director will be prompted to review this steps listed above

Hospice In paitent

• Will follow back office procedure above

• When the conversion is complete for hospice patients in the inpatient unit the non-enhanced med will move into discontinued and the converted meds will display converted along with the date/time of the conversion

• Once medications are converted to Enhanced FDB Medications, new schedules will need to be established for the newly converted scheduled meds.

• Once the newly converted med has a schedule established, the med will be displayed on the report with the respective order date and shading of blocks.
