In this article, we present an overview of fracture treatment options, and offer a simple algorithm for management based on the type and severity of the fracture. Patients and Methods: Twenty-three patients with frontal sinus fractures were analysed. All patients were male, with the mean age of 31. The average follow-up length was 13 months. Of 23 patients, 8 patients had anterior wall fractures and 15 patients had anterior and posterior wall fractures. Their types of fracture, combined injuries, surgical methods, surgical results, and complications were analysed. Results:Six patients had conservative management and 17 patients had surgery. Of 17 patients who had surgery, 6 patients had only repair of the fracture, 3 patients had repair of the fracture with sinus obliteration, and 8 patients had repair of the fracture with cranialization of the frontal sinus. The most common associated injury was fractures of other facial bones ( 83%) , especially the orbit. Traumatic optic neuropathy was the most common complication associated with the frontal sinus fracture or its accompanying injuries. Conclusion:Frontal sinus fractures were safely managed with repair of the fracture, sinus obliteration, or with sinus cranialization which was used irrespective of type and severity of fractures. ( Korean J Otolaryngol 2000 ; 43 : 396-401) KEY WORDS:Frontal sinus fracture·Sinus obliteration·Sinus cranialization·Traumatic optic neuropathy. 서 론 전두동 골절은 전체 안면골 골절의 약 5∼12%이고, 1) 강한 외상에 의해 발생하기 때문에 신경외과적 손상이나 다른 안면골 골절 등과 동반되는 경우가 흔하다. 전두동 골 절의 치료는 골절의 유형과 심한 정도, 동반된 손상의 종류 와 정도 등에 따라 달라진다. 1)2) 많은 경우에서 동반된 손 상의 치료를 위해 다른 과와의 협력수술이 필요하다. 저자들은 외상성 전두동 골절 환자 23명을 분석하여 골 절의 유형과 정도에 따른 치료 법과 결과, 동반된 손상, 합병증 등을 분석하여 향후 치료 법의 결정에 도움을 주 고자 하였다. 환자 및 법 1992년 3월부터 1999년 3월까지 외상성 전두동 골절 로 치료받은 환자 23명을 대상으로 후향적으로 분석하였 다. 환자의 대부분은 이비인후과와 신경외과가 협력하여 수술하였고 일부는 이비인후과, 혹은 성형외과 단독으로 수술하였다. 23명 모두 남자였으며, 평균 나이는 31세(17 에서 65세)였다. 23명의 환자 중 전두동 전벽만 골절된 환자가 8명, 전후 벽이 같이 골절된 환자가 15명이었으며 후벽만 골절된 환 자는 없었다. 비전두관 골절은 전벽 혹은 전후벽 골절과 동 반되어 3명에서 있었다. 전후벽이 같이 골절된 15명 중 전 논문접수일:1999년 10월 22일 / 심사완료일:2000년 2월 12일 교신저자:진홍률, 361-711 충북 청주시 개신동 62번지 충남대학교 의과대학 이비인후과학교실 전화:(0431) 269-6157·전송:(0431) 265-6157 E-mail:[email protected]

through and through fracture)이 6명이었다(Table 1,
Fig. 1).
수술은 골절로 인해 전벽 혹은 후벽이 심하게 전위되었
거나 전위된 전후벽과 함께 두개내 손상이 동반된 경우에
시행하였다. 환자의 상태가 안정되면 가능한 조기에 수술
하였고, 두개내 손상 등으로 인해 신경외과적 응급수술이
필요한 경우는 환자의 상태를 보아서 신경외과와 협력수술
을 시행하였다.
수술시 관상절개(bicoronal incision) 혹은 전두부에 열
상이 있는 경우에는 열상을 이마의 주름에 따라 연장한 절
개를 통해 골절부위에 접근하였다. 후벽이나 비전두관의
골절이 없고 전벽만이 골절된 경우에는 조각난 골절편을
작은 bone hook 등을 이용하여 제위치로 정복하거나 골절
편 가장자리의 점막을 제거한 후 미니플레이트를 이용하여
고정하였다. 비전두관 골절이 동반된 경우에는 비전두관을
작은 골편과 측두근을 이용하여 막고, 전두동의 상피를 드
릴로 전부 제거한 후 측두근을 이용하여 전두동을 폐쇄하
였다. 전후벽이 골절되고 전두부의 피부에서 전두개와로
연결된 개방성 전후벽 골절의 경우 뇌실질 손상이나 출혈,
경막 파열 등의 심한 손상이 흔히 동반되었기 때문에 먼저
신경외과에서 전두개두술(frontal craniotomy)을 하여 손
상된 뇌실질을 절제하거나 지혈하고 파열된 경막을 수선하
였다. 이후 조각난 후벽은 제거하고 비전두관은 폐쇄하였
고, 점막은 드릴로 전부 갈아낸 뒤 전위된 골절편을 소독하
여 전벽을 재건하는 전두동 두개화(cranialization of fron-
tal sinus)를 실시하였다(Fig. 2).
수상시기부터 수술까지의 기간은 1년 전의 전두동 골절
로 인한 전두부 함몰을 교정하기 위해 수술을 시행한 한
명을 제외하면 평균 7일이었다. 수술 후 평균 추적관찰 기
간은 13개월이었다. 골절의 원인은 대부분 교통사고였고
(83%) 이외에도 폭력사고, 추락사고, 직업성재해 등이 있
었다.
이들을 대상으로 골절의 유형에 따른 치료 방법, 수술의
결과, 동반된 손상의 종류, 합병증 등에 대하여 조사하였다.
결 과
전두동 전벽 골절 환자 8명 중 1명은 골절편의 전위가
없어 보존적 치료를 시행했으며, 5명은 골절편의 전위가 있
어 개방정복과 내고정을 시행했고, 나머지 2명은 골절편의
전위와 함께 비전두관 골절이 있어 개방정복, 내고정과 함
께 전두동을 폐쇄하였다(Fig. 3). 전후벽 골절 환자 15명
중에서 전후벽 모두 골절편의 전위가 없던 5명은 보존적인
치료를 하였고, 후벽만 전위된 1명은 개방정복과 내고정,
후벽만 전위되고 비전두관 골절이 같이 있던 1명은 개방정

복과 내고정 후 전두동을 폐쇄하였으며, 전후벽 모두 전위
가 있던 8명은 개방정복, 내고정과 함께 전두동 두개화를
시행하였다. 전두동 두개화를 시행한 8명 중 6명은 개방성
전후벽 골절이 있어 피부에서 전두개와까지 통한 환자였다
(Table 1). 이상의 전두동 골절에 대한 저자들의 치료 방
침을 간단하게 요약하여 도식화하였다(Fig. 4).
동반된 손상으로는 다른 안면 부위의 골절, 얼굴의 연조직
손상, 정형외과적 손상, 두개내 손상 및 두개골 손상 등이 있
었다(Table 2). 다른 안면부위의 골절은 19명(83%)에서 동
반되었는데 안와의 골절이 65%로 가장 많았다(Table 2 and
3). 두개내 손상 및 두개골 손상은 모두 9명(39%)에서 동
반되었는데 뇌척수액 누출이 6명, 두개저 골절이 3명, 경
막외 혈종이 3명, 뇌실질 출혈이 2명, 지주막하 출혈이 1
명 있었다(Table 4). 이러한 두개내 손상 및 두개골 손상
은 대부분이 전후벽 골절, 특히 개방성 전후벽 골절에서
(83%) 많이 발생하였다.
전두동 골절 혹은 전두동 골절과 동반된 손상에 연관되

상흔이 6명에서 발생하였고, 측두근을 이용하여 전두동 폐
쇄를 한 3명의 환자에서 경미한 측두 부위의 함몰이 발생
하였다. 이 외에도 누관계(lacrimal apparatus) 손상이 3
명, 외상성 비중격 만곡증이 3명, 지속적인 복시가 3명, 안
검하수가 1명에서 발생하였다(Table 5). 외상성 시신경병
증 7명 중 2명은 초고용량 스테로이드의 투여와 함께 두개
내 손상의 처치를 위해 신경외과에서 전두개두술을 시행하
면서 전두개와를 통해 시신경 감압술을 동시에 시행하여 시
력의 회복이 있었고, 또 다른 2명은 초고용량 스테로이드를
투여하였으나 1명은 실명하였고 1명은 시력 회복이 없었다.
나머지 3명은 발견이 늦어 치료를 못하였다(Table 6).
고 찰
전두동 골절 중 가장 흔한 유형은 전위된 전벽 골절이며,
다음으로 전후벽 골절이 같이 있는 경우이며, 전벽 골절 없
이 후벽만 골절된 경우는 매우 드물다.3-5) 저자들의 경우
에는 전벽 골절보다 전후벽이 동시에 골절된 경우가 65%
로 더 많았고 후벽만 골절이 된 경우는 없었다. 전벽의 골
절은 이마의 함몰로 인한 미용적인 변형을 초래하지만 후
벽의 골절은 두개내 손상과 동반되는 경우가 흔하다. 비전
두관의 골절은 전후벽의 분쇄골절, 전두동저(floor of fr-
ontal sinus)를 침범하는 수직 골절, 비사골 골절과 동반된
전두동 골절에서 흔히 동반된다. 비전두관의 골절은 방사
선학적 검사에서 발견하지 못하는 경우도 많고, 제대로 치
료하지 않을 경우 점액농류(mucopyocele), 뇌막염, 두개
내 농양 등의 합병증을 유발하기 때문에 전두동을 폐쇄하
는 방법으로 치료하여야 한다.6) 저자들의 경우 3명에서 전
벽 혹은 전후벽 골절과 동반되어 비전두관 골절이 발생하
였고 모두 전두동 폐쇄를 시행하였다.
전두동 골절의 진단에는 신체 검사와 방사선학적 검사가
중요하다. 신체 검사에서 전두부 열상이나 뇌척수액 비루
가 있거나, 촉진으로 이마의 함몰 등이 있으면 전두동 골절
을 의심할 수 있다. 방사선학적 검사로는 일반 두개골 촬영
을 선별검사로 사용하는데, 특히 Caldwell’s view와 la-
teral view가 유용하다. 골절선이 보이지 않더라도 기수면
