Notes: 1. The hurricane deductible shall apply on an annual basis to all covered hurricane losses that occur during a
calendar year, regardless of the policy term. 2. In accordance with Florida Statutes 627.701(3)(b)3, the offer of a $500 hurricane deductible is not required
provided the insurer offers a policy that it guarantees it will not non-renew for reasons of reducing hurricane loss for one renewal period and that contains up to a 2 percent hurricane deductible.

3. General Underwriting Requirements
A. Dwellings must be insured to 100% of replacement cost. B. Dwellings must be protected by smoke detectors in good working order located close to the kitchen and
all sleeping areas.

4. Ineligible Risks (these eligibility guidelines apply to both New and Renewal business, unless shown otherwise)
A. Property Type and Characteristics 1. Roof covering is not: 15 years old or newer if 3-tab composition shingle; 20 years old or newer if
architectural composition shingle; 30 years old or newer if metal; 40 years old or newer if slate or tile.
2. Electrical system is not a minimum of 100 amp service. Knob and tube wiring, aluminum wiring,fuses, Federal Pacific Electric (FPE) or Sylvania or Zinsco electrical panels, and no Ground FaultCircuit Interrupters (CFCI) outlets where water might come in contact with the electrical wiring, are
unacceptable.3. Plumbing is not copper, cast iron or PVC. Lead, galvanized steel and polybutylene are unacceptable.
Plumbing not updated within the last 20 years.
4. Dwellings that contain a Central heating system more than 20 years old, or a furnace that has notbeen completely rebuilt within the last 20 years.
5. Townhouses or rowhouses in buildings of more than 4 units, unless separated from units on either
side by masonry firewalls, with parapets extending through the roof at least 15 inches.6. Historical dwellings (i.e. listed on a historical register).7. Dwellings that are in the course of construction for more than 9 months.
8. Dwellings in poor condition, with unrepaired damage, and/or that have poorly maintained paint, stain,caulking, screens, windows or doors.
9. Dwellings with EFIS (e.g. Dryvit) or asbestos.
10. Dwellings with market value less than 80% of replacement cost.11. Dwellings with a roof covering other than: Composition shingle, metal, slate or tile.12. 1- and 2-family properties with flat or slightly pitched roofs (less than 3:12 pitch) that are covered with
any type of rolled composition material and/or painted-on roof tar, roof patch, or roofing cement olderthan 5 years.
13. Mobile/manufactured homes, motor homes, pre-fabricated/modular homes, houseboats, house
trailers or trailer homes.14. Dwellings with open foundations (unless completely enclosed by masonry skirting), homes built on
pilings/piers for flood control purposes are acceptable.
15. Dwelling with wood stoves, pellet stoves, coal stoves, and/or fireplace inserts (unless inspected toverify proper installation that meets current code).
16. Risks that have been previously rejected, canceled or non-renewed for something other than
reduction of hurricane exposure should be referred to underwriting.17. Dwellings also insured under another insurance policy, except Citizens Property Insurance
Corporation (CPIC) and the National Flood Insurance Program (NFIP) policies.
B. Location 1. Dwellings located entirely or in part over any body of water.2. Dwellings located in Protection Class 10, unless qualifying under Protected Subdivision Rule 160.C.
3. Dwellings where prior sinkhole activity has occurred and repairs have not been made to thepremises. Documentation must certify that the property has been stabilized and all necessary repairshave been made.
4. Isolated properties, which are defined as those that are not visible from at least two other dwellingsor in sight of a public road.
5. Properties subject to brush or wild fire.
6. Dwellings located in an area that has been condemned for any reason, including urban renewal orhighway construction.
7. If Windstorm or Hail Coverage is excluded, the property must be protected by a CPIC policy with
matching coverage limit.8. Dwellings located in Special Flood Hazard Areas (SFHA) (Zones A, AO, AH, A1-30, AE, A99, AR,
AR/A1-30, AR/AE, AR/AO, AR/AH, AR/A, VO, V1-30, VE and V), unless a National Flood Insurance
Plan (NFIP) policy has been purchased with matching building and content limits (or the maximumavailable).

C. Occupancies 1. Dwellings which are part of a “working farm.” “Working farms” are defined as properties with any
livestock (such as cows, horses, hogs, goats, etc.), properties on which row crops are grown (other than a domestic garden), or properties with income producing farming operations.
2. Vacant properties, properties unoccupied more than 9 months in a 12-month period, or properties in foreclosure.
3. Properties with continuous unoccupancy of over three consecutive months during any one year period unless it is protected by at least one of the following: a. Central station fire and burglar alarm system b. Located in a gated community c. Located in a multi-unit building protected by a security guard and/or with controlled access d. Actively managed by a property management firm who checks on the property on a regular (e.g.
at least weekly) basis 4. Multi-family dwellings for three or more families. 5. Dwellings primarily used for business or with incidental business occupancies, other than an office. 6. Dwellings with home day care exposures. 7. Dwelling with assisted living activity exposures.
D. Applicant Characteristics and Loss History 1. Applicants who have ever been convicted of insurance fraud, including arson. 2. Applicants who have experienced two or more losses of any kind (except for a documented
catastrophe loss) in the previous three (3) years, unless it can be demonstrated that the causes of loss have been corrected (evidence must be submitted to the Company for approval prior to binding). No account will be non-renewed due to a loss that is the result of an Act of God, a partial sinkhole loss, or one water damage claim, unless the insured has not taken action to prevent a future similar occurrence of damage to the insured property. Applicants with previous fire or liability losses should be referred to the company prior to binding.
E. Liability Exposures 1. Animal exposures
a. Any livestock or saddle animal exposure exists on the premises (see Rule 730 for exceptions). b. Any vicious, dangerous, or exotic animals that are owned or kept by the insured or a tenant,
including, but not limited to dangerous breeds of dogs, lions, tigers, snakes, and other exotic animals. 1) The following breeds or types of dogs are deemed dangerous per se (due to temperament,
strength/musculature, and/or size), thereby rendering the risk ineligible to receive this coverage under any circumstances: a) Akita b) Alaskan Malamute c) American Bull Dog d) American Eskimo Dog (member of the Spitz family) e) American Staffordshire Terrier f) Beauceron g) Bullmastiff (all Mastiffs) h) Bull Terrier i) Cane Corso j) Caucasian Ovcharka (Mountain Dog) k) Chow Chow l) Doberman Pinscher (miniature Dobermans are acceptable) m) German Shepherd n) Giant Schnauzer o) Korean Jindo p) Mastiff (all Variations) q) Perro de Presa Canario r) “Pit Bull” s) Rottweiler t) Siberian Husky u) Staffordshire Bull Terrier v) Thai Ridgeback

a) Any mixed breed dog containing any of the above breeds b) Any dog that has ever been trained as and/or used as a guard dog or attack dog c) Any dog that has ever been trained or used by the military or police for: d) Enforcing public order by chasing and holding suspects by the threat of being released,
either by direct apprehension or a method known as “Bark and Hold” e) Dogs trained for search and rescue, explosive detection, arson investigation, etc. are
not ineligible for coverage solely due to their training and experience f) Any dog belonging to a breed that was historically bred for fighting g) Any dog that has ever bitten anyone or has exhibited aggressive behavior towards
people 3) Contact UPC’s Underwriting Department for clarifications or further discussion in the event
of any uncertainty or questions. 2. Other exposures.
a. Applicants who own off-road all-terrain/recreational vehicles (see Rule 730 for exceptions). b. Swimming pools and hot tubs:
1) Unless the pool is protected by a locking fence at least four-feet high, or by an alternately approved, secure enclosure, or (for above ground pools) the outer ladder steps flip up and are locked into place to prevent unwanted entry.
2) Diving boards, diving platforms, and slides are not permitted. 3) Unless the hot tub has a locking cover or is protected by a locking fence at least four-feet
high. c. There is a trampoline on the premises. d. There is a tree house on the premises. e. Limit the extension of liability to a maximum of four rental properties that are owned by our named
insured(s).

5. Binding Authority Agents have the authority to bind coverage on any risk that is not identified as “ineligible” in the Ineligible Risks section of this underwriting guide. Agent’s authority is for the limits stated and the forms of coverage outlined in the various sections of this guide. Any exceptions must be referred to the Company for approval prior to binding. Agent’s binding authority for new business, or for increases in coverage on existing business is suspended immediately when the National Hurricane Center (NHC) of the National Weather Service has issued a Tropical Storm Watch, Tropical Storm Warning, Hurricane Watch, or Hurricane Warning for any coastal area of Florida. Renewals will only be issued on an “as expiring” basis for coverage and perils; limits will only be increased to keep pace with established inflation factors. The Company will restore binding authority as soon as the NHC lifts the storm designation for all affected Florida coastal areas. If any exceptional circumstances exist which warrant the Company giving consideration to acceptance of additional liability, the line must be submitted to the Company’s Underwriting Department for approval. An example is when coverage is required in order to close on the purchase of a home – in this situation, the agent must contact the Company for approval, and proof of the real estate closing must accompany the application along with the appropriate payment.

6. Application and Submission Requirements
A. Applications All submissions must be made on forms approved by the Company and must be signed. Applications must be complete and accompanied by a check for the full amount of the calculated premium, or installment premium on Premium Payment Plans, with checks payable to United Property & Casualty Insurance Company. Checks must be forwarded to the Company within 5 business days of binding. Original signed applications and all other required documentation must be maintained in the agent’s files. The Company will perform periodic audits of the agent’s files to ensure compliance with all rules and guidelines.
B. Additional Information: Submissions may require information other than that shown on the application. Agents should obtain any information that helps to support acceptance of the risk being submitted, especially when the information on the application calls for more explanation or when this manual indicates that additional information is required. Such cases include, but are not limited to:
Documentation of updates to roofing, heating, wiring, plumbing, etc.
Determination of replacement cost
Appraisals on jewelry or fine arts schedules
Evidence of CPIC or NFIP coverage, if required

8. Payment Plans (see Rule 170. for details)
1 Pay Plan: Full payment at inception.
2 Pay Plan: 50%/50%, plus a $3 service charge with each installment.
3 Pay Plan: 40%/30%/30%, plus a $3 service charge with each installment.
4 Pay Plan: 25%/25%/25%/25%, plus a $3 service charge with each installment.
11 Pay Play: 11 equal monthly installments, automatically deducted from insured’s bank account, plus a $1 service charge with each installment.
Quarterly Pay Plan: 40% of the total premium, plus fees, at the inception of the policy with the remaining 60% of the premium due in three equal installments of 20% 90, 180, and 270 days after inception of the policy. A $3 service charge is applicable to all installments.
Semiannual Pay Plan: 60% of the total premium, plus fees, at the inception of the policy with the remaining 40% of the premium due 180 days after the inception of the policy. A $3 service charge is applicable to both installments.

9. Individual Insurance Financial Scoring
A. The Insurance Financial Scoring of all named insureds will be reviewed in compliance with section 626-
9741, F.S. and Florida Rule 69B-125.004. Credit is evaluated along with prior claim history, and other risk factors.
B. Credit history is ordered and an Insurance Financial Score is determined on the Name Insured only for all new business policies. Agent must obtain permission from the applicant prior to requesting the credit report.
C. Any applicant with a score lower than 600 and a non-catastrophe loss in the last 3 years OR score lower than 600 and a year of construction before 2002 is ineligible for coverage.
D. In no instance, whether when reviewing new business or renewals, will the insurance financial score be the sole basis of determining an individual’s acceptability.

120. Eligibility
A. Form HO 00 03 - A Homeowners Policy may be issued:
1. To the owner-occupant(s) of a dwelling which is used exclusively for private residential purposes (except as provided in Rule 120.F.) and contains not more than 2 families and with not more than 2 boarders or roomers per family; or
2. To the purchaser-occupant(s) who has entered into a long term installment contract for the purchase of the dwelling and who occupies the dwelling but to whom title does not pass from the seller until all the terms of the installment contract have been satisfied. The seller retains title until completion of the payments and in no way acts as a mortgagee. The seller’s interest in the building and premises liability may be covered using Endorsement HO 04 41 – Additional Insured; or
3. To the occupant of a dwelling under a life estate arrangement when the Coverage A amount is at least 80% of the dwelling’s replacement cost. The owner’s interest in the building and premises liability may be covered using Endorsement HO 04 41 – Additional Insured; or
4. To cover dwellings in the course of construction provided the policy is issued only in the name of the intended owner-occupant(s) of the dwelling. It is permissible to extend the Homeowners Policy, without additional premium charge, to cover the interest of a non-occupant joint owner in the building and for premises liability. Use Endorsement HO 04 41 – Additional Insured.
B. Form HO 00 04 - A Homeowners Policy may be issued to: 1. The tenant(s) (non-owner) of a dwelling or an apartment situated in any building; or 2. The owner-occupant(s) of a dwelling, cooperative unit or of a building containing an apartment not
otherwise eligible for a Homeowners Policy under Rule 120.A. above; provided the residence premises occupied by the insured is used exclusively for residential purposes (except as provided in Rule 120.F.), and is not occupied by more than one additional family or more than two boarders or roomers.
C. Form HO 00 06 – A Homeowners Policy may be issued to: A Homeowners Policy may be issued to the owner(s) of a condominium or cooperative unit which is used exclusively for residential purposes (except as provided in Rule 120.F.), and is not occupied by more than one additional family or more than two boarders or roomers.
D. Subject to all other sections of this rule, a Homeowners Policy may be issued to cover a seasonal dwelling. (See Rule 140).
E. A Homeowners Policy shall not be issued to cover any mobile home, trailer home, or housetrailer. F. Certain business occupancies are permitted, provided:
1. The premises is occupied principally for private residential purposes; and 2. There is no other business occupancy on the premises. When the business is conducted on the residence premises, refer to Rule 550 for Section I Coverage and Rule 820 for Section II Coverage. When the business is not conducted from the residence premises, this coverage is not available.
G. All policies are issued with the mandatory HO 04 96 04 91 – No Coverage – Home Day Care exclusion endorsement. No policy shall be issued for an occupancy where child care is provided to unrelated individuals for compensation, unless all of the following criteria are met: 1. A current copy of a Certificate of Insurance from the insurer providing Commercial Liability coverage
at limits equal to or greater than the Company Personal Liability limits is provided to the Company. 2. A current copy of the Florida Department of Children and Families “Family Child Care Home
Certificate of License”, or a copy of the current Child Care License for the covered premises issued by the county of jurisdiction is provided to the Company prior to issuance or renewal of each policy period.

170. Method of Payment
The Company accepts the following methods of payment: A. 1 Pay Plan: Full payment of the total annual premium plus all policy fees at the inception of the policy. B. 2 Pay Plan: 50% of the total premium, plus fees, at the inception of the policy with the remaining 50% of
the premium due 60 days after the inception of the policy. A $3 service charge is applicable to both installments.

C. Hurricane Deductibles 1. The hurricane deductible subjects the policy to a percentage deductible amount that applies to windstorm
loss that occurs during a hurricane. This deductible applies beginning at the time a hurricane watch or warning is issued for part of Florida by the National Hurricane Center of the National Weather Service and ends 72 hours following the termination of the last hurricane watch or hurricane warning issued for any part of Florida by the National Weather Service.
2. Hurricane means a storm system that has been declared to be a hurricane by the National Hurricane Center of the National Weather Service. The National Weather Service currently uses the Saffir/Simpson Hurricane scales to categorize hurricanes. This scale specifies that a hurricane is a storm that originates

450. Additional Amounts of Insurance – Form HO 00 03 Only A. The policy provides loss settlement for buildings insured under Coverage A or B on a replacement cost basis
without deduction for depreciation, if, at the time of loss, the amount of insurance on the damaged building is 80% or more of the replacement cost of the building immediately before the loss. However, in no case shall the payment under the replacement cost provision exceed the limits of liability stated for coverage A in the declarations.
B. The policy may be endorsed to provide additional insurance for Coverage A only when loss to the dwelling building exceeds the limit of liability shown in the policy declarations.
C. When the following option is selected, the Coverage A limit of liability shall be at least 100% of the full replacement cost of the dwelling building at policy inception or at the time the endorsement is added to the policy.
D. Specified Additional Amount of Insurance for Coverage A Only
1. An additional amount of insurance equal to 25% of the Coverage A limit of liability may be selected. This additional amount is available when loss to the dwelling building exceeds the Coverage A limit of liability shown in the Declarations.
2. The additional amount cannot be applied to any other coverage nor does it increase the Coverage A limit.
3. The additional premium for this option is calculated by multiplying the previous step in the rating algorithm by the following factor: 0.08
E. Do not use this endorsement when any endorsement which modifies the required percentage of replacement value is attached to the policy.

The blanket limit of liability for Coverage C is 50% of the Coverage A limit of liability. This limit may be increased, in 5% increments, to a maximum of 75% of the Coverage A limit of liability. The additional premium rate per $1,000 of increased Coverage C limit is as follows: $2.50
B. Reduction in Limit – Form HO 00 03 The blanket limit of liability for Coverage C is 50% of the Coverage A limit of liability. This limit may be reduced, in 5% increments, to a minimum of 25% of the Coverage A limit of liability. The premium credit per $1,000 of reduced Coverage C limit is as follows: $1.25
C. Coverage C Exclusion – Form HO 00 03 Coverage C may also be excluded completely, upon written request by the insured (form UPC 179) and acceptance by the company, per 627.712 Florida Statutes. The Exclusion will apply to all subsequent renewals until removed. This endorsement may only be removed on the renewal anniversary date of the policy and upon payment of the appropriate premium. Multiply the Premium Subtotal B in the rating algorithm by .09 to determine the credit.
D. When Coverage C is excluded the following rules do not apply and endorsements are not available to

Personal property items used in a trade or business are not eligible for scheduled personal property coverage. A. Provides coverage for loss caused by hurricane to aluminum framing for screened enclosures and carports permanently attached to the dwelling on a replacement cost basis. This endorsement does not provide coverage for screen material or costs associated with removing or replacing screens. This endorsement does not increase the limit of liability for Coverage A. This coverage is available in increments of $5,000 up to a maximum of $75,000.

630. Water Back Up and Sump Overflow
A. The policy forms exclude coverage for loss resulting from water or waterborne material which backs up
through sewers or drains or which overflows or is discharged from a sump, sump pump or related equipment. When the optional Limited Water Back-up And Sump Discharge Or Overflow Coverage endorsement is attached to the policy, coverage is provided with respect to direct physical loss, not caused by the negligence of an “insured”, to property covered under Section I, caused by water or waterborne material which originates from within the dwelling where the named insured resides and backs up through sewers or drains or which overflows or is discharged from a sump, sump pump or related equipment.
B. The policy may be endorsed to provide such coverage for a limit of liability of $5,000 subject to a $250 deductible. No other deductible option is available.

640. Water Loss Prevention Credit
A. A credit is available to insureds who can certify that the plumbing system in the insured dwelling has been updated or checked for soundness and is free of any visible defects. All appliances connected thereto, and the attachments between the plumbing system and the appliances, must also be checked for soundness and lack of defects.
B. This certification consists of a licensed plumber completing the Water Loss Prevention Credit Certification (UPC-136 Form).
C. This credit applies automatically to all new homes completed in the same calendar year as the initial effective date of the policy. In this case the requirement for a Water Loss Prevention Credit Certification is waived.
D. This credit expires after three (3) successive policy terms unless a new Water Loss Prevention Credit Certification pertaining to the continued soundness of the plumbing system is provided to the Company.
E. To develop the premium credit, multiply the previous AOP rating step by a factor of .05.

650. Windstorm or Hail Exclusion Base Premium Credit A. The peril of Windstorm or Hail may be excluded for all coverages, including Coverage D – Loss of Use,
if the property is eligible for such coverage from CPIC. B. The peril of Windstorm or Hail may also be excluded for all coverages, including Coverage D – Loss of Use,
anywhere in the state upon written request of the insured(s) named on the policy and written authorization from any mortgage holder or lienholder with an interest in the insured location, per 627.712 Florida Statutes. If Windstorm or Hail coverage is excluded and the insured property is not eligible for wind-only coverage from CPIC, the policyholder must use form UPC 180 and personally write and provide to the Company in his or her own handwriting, signing his or her name (and signed by any other named insured on the policy) the following statement (the statement must also be dated): “I do not want the insurance on my home/mobile home/condominium unit to pay for damage from windstorms or hurricanes. I will pay those costs. My insurance will not.” A written statement from any mortgage holder or lienholder must also be sent to the Company approving of the policyholder electing to exclude windstorm or hurricane coverage from the policy. All necessary documents must be received and approved by the Company prior to the attachment of the exclusion to the policy. The Exclusion will apply to all subsequent renewals until removed. This endorsement may only be removed on the renewal anniversary date of the policy and upon payment of the appropriate premium.
C. To calculate the adjusted Base Class Premium: 1. Determine the appropriate AOP and Wind Base Class Premiums as described in Rule 300. 2. Subtract the Windstorm or Hail Exclusion Base Credit for the Form and Territory found in Rate Table
300 B from the Base Class Premium from Step 1. 3. Multiply the Adjusted Base Class Premium (excluding Windstorm or Hail coverage) developed in
Step 2 by the Protection/Construction Factor and then by the Key Factor for the desired limit of liability (see Rule 300).

655. Identity Theft Expense and Resolution Services Coverage
Identity Theft Expense and Resolution Services Coverage is available to provide first party coverage for expenses incurred by an insured as a direct result of any identity theft occurrence first discovered or learned of during the policy period. This endorsement provides up to $25,000 coverage for:
A. Costs for notarizing affidavits or similar documents attesting to fraud;
B. Costs for certified mail to law enforcement agencies, credit agencies, financial institutions, etc.;
C. Lost income, up to a maximum of $1,000 per week/$5,000 total, resulting from time taken off work to complete fraud affidavits, meet with law enforcement agencies, credit agencies, and/or legal counsel;
D. Loan application fees for re-applying for loans when the original application is rejected because the lender received incorrect credit information;
E. Attorney fees incurred to defend lawsuits brought against the insured by merchants, financial institutions or their collection agencies; remove any criminal or civil judgments wronly entered against an insured; and challenge the accuracy or completeness of any information in a consumer credit report;

665. Sinkhole Coverage A. Catastrophic Ground Cover Collapse is automatically provided in all Homeowners forms. The premium is
included in the base premium. This coverage cannot be excluded. This coverage is provided in form UPC 176, which is attached to all Homeowner policies.
B. Sinkhole coverage is automatically provided in the HO 00 04 and HO 00 06 forms. On form HO 00 03, new policies may be bound only with Catastrophic Ground Cover Collapse (apply the credit in Table 300.I.). If the policyholder elects the option for Sinkhole Coverage, an approved inspection of the home must be completed prior to adding the coverage. The inspection will document existing damage, evaluate the integrity of the dwelling, and verify that there is no proximate sinkhole activity that has not been disclosed.
C. An “Approved” inspection service is one that has been designated by us as competent to perform the evaluation, and whose report format meets our informational requirements. The insured will contract directly with the Approved inspection service, and pay an arranged fee, and both parties will receive a copy of the inspection. The fee will not be refundable no matter how the underwriting decision is reached.
D. An inspection from an inspection service that has not been designated by us as “Approved” may be submitted by the insured for consideration in meeting this requirement. Such an inspection must have been completed by a professional engineer, professional geologist, or other individual or entity recognized by us as possessing the necessary qualifications to properly complete the inspection, and must meet all requirements outlined above with regard to timing, content and format.
E. Structures that have substantial cracking, shifting of the house on its foundation, doors that won’t close properly, visible depressions or voids in the ground and immediate proximity (less than ½ a mile) to known sinkhole activity may only be eligible for Catastrophic Ground Cover Collapse. Structures with little or no damage may be eligible for Sinkhole Loss Coverage.
F. We will non-renew policyholders maintaining Sinkhole Loss Coverage and provide an offer of coverage to such policyholders which includes Catastrophic Ground Cover Collapse and excludes Sinkhole Loss Coverage. If the policy holder requests the option for Sinkhole Loss Coverage to be added to the new policy, then an inspection will be required as outlined above. The “Sinkhole Loss Coverage Endorsement Request [UPC 221] must be completed and signed. This item F. does not apply to policyholders who currently have an “Approved” inspection and will last for an annual period.

670. Windstorm Resistive Features of Residential Construction
A. Eligibility
1. When the policy covers the peril of Windstorm or Hail, a risk may be eligible for a premium credit if the dwelling has been built or retrofitted in conformance with the windstorm mitigation elements of the 2001 Florida Building Code (effective 3-1-2002) with respect to:
a. Type of roof covering b. How the roof deck is attached to the structure c. The technique and materials used in connecting the roof to the walls of the structure d. The type and extent of materials used in protecting all openings to the dwelling structure e. The geometry (or shape) of the roof f. The extent of water resistance techniques employed in the roof construction
2. Other factors impacting the level of credit are:
a. The version of the Florida Building Code to which the construction of the dwelling was subject. The program differentiates construction between existing construction and new construction. These terms are defined in paragraph D. of this rule.
b. The location of the dwelling; proximity to the coast and terrain being the most significant factors.
B. Mitigation Credit Tables
Mitigation Credit Tables have been developed for implementation of this rule that apply to either new construction or existing construction by building type.
C. Verification
The company will require proof that substantiates the existence of loss mitigation features. This proof will be in the form of verification by a registered or licensed design professional certified building inspector or licensed General Contractor. A valid Florida Uniform Mitigation Verification Inspection Form (OIR-B1-1802) is available for use in satisfying this requirement. However, other signed and dated documentation from a duly qualified professional may be acceptable subject to underwriting review. The insured is responsible for the expense associated with substantiating the existence of the mitigation features. No verification documentation beyond shutter documentation, appropriate pictures and permit documents are required for dwellings constructed on or after March 1, 2002. Dwellings with a construction permit dated prior to 3-1-2002 must submit a valid Florida Uniform Mitigation Verification Inspection Form (OIR-B1-1802). Photos can be provided to show proof of a Hip Roof.
D. Definitions of Terms
New Construction - Risks constructed subject to the 2001 version of the Florida Building Code.
Existing Construction - Risks constructed prior to March 1, 2002, or not built to the 2001 FBC.
High Velocity Hurricane Zone (HVHZ) – Miami-Dade and Broward Counties.
Terrain
Terrain C - Miami-Dade and Broward counties, all barrier islands and all risks within 1500' of the coastline.
Terrain B - All areas not included in Terrain C.
FBC Wind Speed - Design wind speed (3 second gusts) for the site location as determined by the wind speed map in figure 1606 of the Florida Building Code.
FBC Wind Design Speed - Wind speed (3 second gusts) for which the structure is designed to withstand according to the FBC 2001. Basic (minimum) Wind Design Speeds (SDS) are established for each FBC Wind Speed Zone. When the WDS of the structure (1-4 unit building built on or after January 1, 2002) exceeds the basic speed design, certification is required for premium credit. Use a valid Florida Uniform Mitigation Verification Inspection Form (OIR-B1-1802).

Roof Cover - The covering applied to the roof deck for weather resistance, fire classification or appearance. The most common roof covering materials in Florida are composition shingles and tiles. A key factor in roof covering performance is the method of attachment of the roof covering to the roof deck. The Florida Building Code 2001 (Section 1504) has material requirements and attachment specifications that are superior to common roof covering building practices in the past. For composition shingles, these requirements include self-seal strips and compliance with ASTM D-3161 (Modified for 110 mph). This requirement is commonly referred to as the "110 mph" rated shingle.
FBC Equivalent - The roof covering specifications of the 1994 SFBC also require improved attachment methods and testing to a similar protocol. Therefore, these roof coverings are considered to be sufficiently similar to FBC roof coverings to be classified in the "FBC Equivalent" category. The rating of roof covering for existing construction can be achieved by requiring the roofing contractor to certify that a prior installation met the 1994 SFBC or the FBC 2001 requirements. Otherwise the current roof covering should be rated as Non-FBC equivalent. Insurers should remind owners of existing houses that when the re-cover their roofs, they need to have their contractor certify that the installation meets the FBC 2001 Chapter 15 requirements in order to receive the new roof covering credit.
Non-FBC Equivalent - Roof coverings that do not meet the 1994 SFBC or the FBC 2001 requirements. (see above)
Roof Deck Attachment - The performance of the roof deck is of critical importance in keeping hurricane loses to a minimum. It usually only takes the loss of a small portion of the roof deck before the losses for the building become substantial. Rain enters into the building and produces water damage to the interior and contents.
The most common roof deck types are plywood and Oriented Strand Board (OSB) decks. Prior to the availability of plywood, the most common roof decking was dimensional lumber or tongue and groove boards. Dimensional lumber or tongue and groove boards are nominally thick and are laid in a fashion that is parallel to the ridge or diagonal to the ridge. These roof decks are fastened by at least two nails per truss/rafter connection. Because of the inherently large number of nails in dimensional lumber or tongue and groove, the uplift capacity is generally far greater than typical plywood/OSB decks.
By far the most important feature of roof decks is the attachment to the framing, which is usually achieved by nail fasteners. Nail size, type, spacing and penetration depth into the truss or rafters determines the uplift resistance of the deck. The difference in uplift capacity of 8d (2 1/2 inch) nails at a typical nail spacing and 6d (2") nails at the same spacing is a factor of about two times stronger, which makes a significant difference in deck performance in hurricanes.
The thickness of the deck material is also important primarily in the determination of the penetration depth on the nail into the truss/rafter. Prescriptive building codes specify longer nails for thicker decks.
Level A - Plywood/OSB nailed with 6 penny common nails at 6" spacing on the edge and 12" in the field on 24" truss spacing. This provides for a mean uplift resistance of 55 lbs per sq ft.
Level B - Plywood/OSB nailed with 8 penny common nails at 6" spacing on the edge and 12" in the field on 24" truss spacing. This provides for a mean uplift resistance of 103 lbs per sq ft.
Level C - Plywood/OSB nailed with 8 penny common nails at 6" spacing on the edge and 6" spacing in the field on 24" truss spacing. Within 4'of a gable end the nail spacing is 4". This provides for a mean uplift resistance of 182 lbs per sq ft. for non-gable end locations and 219 lbs per sq ft for gable end locations.
Level D - Dimensional lumber and tongue and groove decks. Over 90% of the RCMP inspected dimensional lumber decks have 8d or greater nails. The case of two 8d nails per board produces a mean uplift resistance of 338 lbs per sq ft.
There are many technical issues that affect the proper rating of the roof deck including a variety of available nail sizes, nail penetration depths, the consideration of missed nails, etc. Proper inspection guidelines and training are essential to determining the deck attachment of existing residences.
Roof-wall Connection - The roof to wall connection is another critical connection that keeps the roof on the building and acts to transfer the uplift loads into the vertical walls. This connection is key to the performance of the building due to the large negative pressures acting on the roof. Verification of the

type of roof to wall connection requires access to the attic to inspect for accurate house rating. There are several manufacturers of the metal connectors for hurricane uplift connectors and each company has a fairly wide line of products. For practical purposes, a classification is used to distinguish the uplift capacity of these connections based on connector type. The most important feature of any of these connectors - other than toe nails, is that the fasteners used to transfer the loads from rafter/truss to strap to top plate or side wall are always loaded in shear (perpendicular to the nail direction) or the strap is embedded into the bond beam of the masonry wall. Proper installation is critical to connector performance.
Toe Nails - A common connection detail in non-hurricane prone areas is the toe-nail, where approximately 3 nails are driven at an oblique angle through the rafter and into the top plate.
Clips - Clips are defined as pieces of metal that are nailed into the side if the rafter/truss and into the side of the top plate or wall stud. The metal does not wrap around the top of the rafter/truss and the clip is only located on one side of the connection.
Single wraps - The wrap style straps are attached to the side and/or bottom of the top plate and are nailed to the rafter/truss.
Double wraps - Straps that are wrapped on both sides of the top plate. Double wraps have twice the capacity of a single wrap.
Reinforced Concrete Roof Deck - Although not very common in residential roof construction in Florida, there are homes constructed with concrete roof decks. When these homes are equipped with wind-borne debris impact resistant opening protection, they are extremely resistant to building failures. To qualify for this type of construction, the roof deck must be designed and constructed in accordance with ACI (American Concrete Institute) 318, including integral construction with a masonry wall system.
Opening Protection – Openings in the wall and roof include windows, doors, sliding glass doors, skylights and garage doors. Gable end vents and other roof vents are not considered openings for purposes of this program. Openings are vulnerable to wind-borne debris impacts in hurricanes and other windstorms. Typical single and double strength glazing are easily broken by impact from lightweight debris that is generated from roof covering failures during high winds. In addition, heavier debris such as roof tiles, 2X4 wood members and plywood will easily penetrate openings that are not protected by impact resistant products.
The protection of openings is perhaps the greatest single loss mitigation strategy for a building. The reason for this is that once a window or door fails, the pressure inside the structure increases due to the breach in the building envelope. The positive pressure inside the building produces an additive load on the building envelope which can be up to twice the load the building would experience without the breach.
Hurricane Impact (Class A) – Either all exterior wall and roof openings, or only all glazed exterior wall and roof openings, in buildings (doors, windows, skylights and vents, other than roof ridge, gable, soffit and plumbing vents) must be fully protected with impact resistant coverings (e.g. shutters), impact resistant doors, and/or impact resistant glazing that meet the requirements of one of the following:
a) SSTD12;
b) ASTM E 1886 and ASTM E 1996 (Missile Level C – 9 lb);
c) Miami-Dade PA 201, 202 and 203; or
d) Florida Building Code TAS 201, 202 and 203.
Basic Impact (Class B) – Either all exterior wall and roof openings, or only all glazed exterior wall and roof openings, in buildings (doors, windows, skylights and vents, other than roof ridge, gable, soffit and plumbing vents) must be fully protected with impact resistant coverings (e.g. shutters), impact resistant doors, and/or impact resistant glazing that meet the requirements of ASTM E 1886 and ASTM E 1996 (missile Level B – 4.5 lb).
Roof Shape - Roof shape refers to the geometry of the roof and not the type of roof covering. There are many common roof shapes in residential construction. Gable and hip are the most common although flat, Dutch hip, gambrel, mono slope and many shape combinations are possible.

Gable roofs have vertical walls that extend all the way to the top of the inverted V and are very common throughout Florida.
A hip roof has sloping ends and sloping sides down to the roof eaves line.
Roof shape determines the aerodynamic pressure loads experienced by the roof due to wind flow and wind direction. Gable and hip shapes and their combinations comprise more than 90% of the residential buildings.

Secondary Water Resistance (SWR) - Secondary water resistance is a layer of protection that protects the building if the roof covering fails. This mitigation technique is aimed at keeping rainwater out of the house once the roof covering fails. Generally, roof coverings begin to peel off in peak wind gusts ranging from about 70 to 100 mph. The underlayment (felt) also is easily torn and becomes separated from the roof deck, exposing the house interior to water damage. Water enters through the space between the pieces of the roof deck. SWR covers these seams and provides for redundant water proofing of the house.
The most economical way to achieve SWR is to apply Self-Adhering Modified Bitumen Tape to the plywood joints. This self adhering tape is generally known as Ice & Water Shield or Peel N Seal and is a rubber like product applied directly to a roof deck to prevent damage from ice dams in northern climates. Here, the product is applied to the outside of a clean plywood/OSB deck prior to the application of regular underlayments and roof covering. The application of SWR is done when a new roof covering is being put on the house.
Another way to achieve SWR is a foamed polyurethane structural adhesive applied from inside the attic to cover the joints between all plywood sheets. Note that this product is also used to reinforce the connection between the trusses and roof sheathing, qualifying for improved roof deck attachment. Structural adhesives that meet standard AFG-01 should not be confused with foamed insulating products.
The verification of SWR must be done at the time of application since, once covered, it is difficult to verify. The foamed structural adhesive applied from inside the attic however, is readily verified with an attic inspection. Roofing contractors should complete a form to provide certification for the owner in order to receive this credit. Note: underlayments or hot-mopped felts are not SWR.
FBC 2001 Construction - Dwellings built to the standards of Florida Building Code 2001 which became effective 3/1/2001.
Enclosed Building - An enclosed structure is designed assuming that all openings are closed and, therefore, the wind loads are determined using a small internal pressure inside the building.

Partially Enclosed - A partially enclosed building is designed assuming that one or more areas of the building are open to allow wind to enter the building and pressurize the interior. This pressurization means that individual parts of the building must be designed to withstand greater wind loads than an "enclosed" building and thus are stronger than the similar features of an "enclosed" house. However, the openings (windows, doors, etc) in partially enclosed designs are vulnerable to wind-borne debris impact failures and the resulting wind and rain water damage to the building interior and contents.
For insurance rating purposes, clearly the design option chosen for a house in the Wind-Borne Debris Region of the FBC is a key factor in hurricane loss mitigation. Enclosed designs in the Wind-Borne Debris Region will have all glazed openings protected for debris impact. These buildings will perform better that partially enclosed designs and will have lower losses.

G. PREMIUM CREDIT COMPUTATION
1. In determining which Windstorm Loss Reduction Credit Table to use, it is necessary to know when the dwelling was built and to which edition of the Florida Building Code the construction of dwelling was subject. Existing Construction tables apply to all residential construction built prior to the implementation of Florida Building Code 2001, which became effective March 1, 2002. Post Construction Tables apply to all residential construction built after the implementation of, and subject to, Florida Building Code 2001, which became effective March 1, 2002.
2. Referring to the Florida Uniform Mitigation Verification Inspection Form (OIR-B1-1802) provided by the insured, identify the construction material and features present in the structure.
3. Refer to Paragraph D. Definitions of Key Terms, of this rule to determine the applicable Terrain of the risk location.
4. Based on the above information, locate the applicable credit in the table.
Select the appropriate loss reduction factor from the Tables provided in Rule 670. Multiply the resulting Windstorm Loss Reduction Credit by the Wind Base Premium as determined in Rule 300 to determine the premium credit.

675. Evacuation Coverage Coverage is automatically provided at no charge under the Protector Homeowners Program if a mandatory evacuation is ordered by a civil authority, as follows: A. If a mandatory evacuation from the Described Location is ordered in anticipation of impending potential
damage from a Tropical Storm or Hurricane, we will cover costs (such as hotel and food costs) incurred by the Insured during the period of evacuation.
B. We will also cover expenses incurred by the Insured for labor costs if the Insured pays to have storm shutters or plywood installed on the openings of the Described Location.
C. Direct damage to the Described Location need not occur in order for this coverage to be activated. D. This coverage is additional insurance. E. The policy deductible does not apply to this coverage. F. Recovery for loss under this coverage is limited to a maximum of $250. Use Endorsement UPC 174– Evacuation Coverage.

680. Building Code Effectiveness Grading A. General Information
1. The Building Code Effectiveness Grading Schedule develops a grade of 1 to 10 for a community based on the adequacy of its building code and the effectiveness of its enforcement of that code. Policies which cover the perils of Windstorm or Hail are eligible for special rating treatment, subject to the criteria in the following paragraphs. The Building Code Effectiveness Grading factor applies, where applicable, in addition to the Public Protection Classification factors.
2. In some communities, two Building Code Effectiveness Grades may be assigned. One grade will apply to 1 and 2 family dwelling buildings and/or personal property contained in such buildings. The other grade will apply to all other buildings occupied for residential, commercial and/or manufacturing purposes including personal and business property contained therein. The Public Protection Classification Manual will indicate the application of each grade.
3. The Building Code Effectivess Grades for a community, and their effective dates, are provided in the Public Protection Classification Manual published by Insurance Services Office, Inc.
4. The credit/surcharge does not apply when the peril of windstorm or hail is excluded from the policy. B. Community Grading
1. The Building Code Effectiveness Grade applies to any building that has an original certificate of occupancy dated in the year of the effective date of the community grading, or later. A rating factor is provided for each community grade.
2. If a community is re-graded subsequent to its initial grading, the factor for the revised grade applies to buildings that have an original certificate of occupancy dated the year of the effective date of the revised grading, or later.
3. Where certificates of occupancy are not issued, equivalent documentation must be used. 4. If, due to an addition or alteration, the original building is changed to comply with the latest building code,
the factor for the community grading applicable at the time the reconstruction is completed will apply to such building.
5. The Building Code Effectiveness Grade applies to Windstorm/Hail. Specific information is provided in the Public Protection Classification Manual. If the grade in the manual does not apply to Windstorm/Hail, the factor should not be applied.
6. Communities that decline to participate in the Building Code Effectiveness Grading Program will be identified in the Public Protection Classification manual as not participating. Buildings in these communities will receive a premium surcharge, unless they qualify for individual grading under C. below. This surcharge will only apply to buildings that have an original certificate of occupancy dated in or after the year of the effective date of the community evaluation that identifies the community as not participating.
C. Individual Grading Where buildings have been built in full conformance with the natural hazard mitigation elements of one of the nationally recognized building codes even though the community grade is greater than 1, or the community is not participating in the program, exception rating procedures may apply. Any building may be classified as Grade 1 for Windstorm/Hail upon certification by a registered or licensed design professional, based on an in-site inspection, that such building is in compliance with one of the three nationally recognized building codes with respect to mitigation of the Windstorm and/or Hail hazard. This classification is effective only from the date of the certification.
D. Ungraded and Non-Participating Risks Buildings which do not meet the criteria in B. and/or C. above for Grade assignment are rated and coded as either ungraded or non-participating risks. Do not classify as Grade 10.
E. Premium Credit Computation – Community Grading and Individual Grading 1. For graded and ungraded risks, calculate the premium credit as follows:
a. Multiply the previous Wind rating step in the rating algorithm by the appropriate factor found in Rate Table 680 –Building Code Effectiveness Grading Credits; and
b. Multiply the result from step a. above by the Key Factor for the desired amount of insurance. Note: For any building classified as Grade 1 based upon certification as set forth in Paragraph C. of this rule, use Grade 1 in determining the factor from Rate Table 680.
c. For a risk that is eligible for a premium credit under Rule 670. Windstorm Resistive Features of Residential Construction, the combined credit factors from credits obtained from Rule 680. Building Code Effectiveness Grading and Rule 670. may not exceed 90%.
2. For risks identified as non-participating, calculate the premium surcharge as follows:
a. Multiply the previous Wind rating step in the rating algorithm by the surcharge factor of 0.01; and
b. Multiply the result from step a. by the Key Factor for the desired amount of insurance.
c. Use Classification Code 98.

730. Premier Mini-Farm Endorsement – HO 00 03 Only A. For an additional charge the Premier Mini-Farm Endorsement may be added to an HO 00 03 policy when
incidental farming operations are conducted on the residence premises. The prohibitions against “working farm” and “livestock and saddle animal” exposures contained in the “Ineligible Risks” section of the Underwriting Guidelines at the beginning of this manual are suspended when this coverage is added to the policy.
B. Eligible Exposures: 1. 20 acres or less; 2. No more than a total of six head of cattle, horses, hogs, pigs, sheep, or other livestock; 3. No more than a total of twelve chickens, roosters, or other poultry animals; 4. Annual gross receipts from incidental farming operations not exceeding $5,000; 5. No additional premises farmed by the insured.
C. Ineligible Exposures: 1. Any farm with either full or part-time farm employees; 2. Aquaculture farms; 3. Exotic animals; 4. Boarding of horses; 5. Dude ranches, resorts, and camps; 6. Horse training; 7. Horseback riding for a fee; 8. Hunting for a fee, fishing for a fee, hunting clubs, fishing clubs, hunting or fishing guide services, or
outfitting services; 9. Nursery operations; 10. Commercial feed lots, stockyards, or sale barns; 11. Commercial grain elevators or storage facilities; 12. Poultry farms (including egg production, hatcheries, and fryer production); 13. Tobacco production; 14. “U pick” or “pick your own” operations; 15. Farms located in a Protection Class 10 community; 16. Farms owned by the insured and rented to others.
D. Underwriting Requirements:
Pictures of all buildings being insured must be submitted with the application.
Replacement cost estimators must be completed and submitted for all buildings $50,000 or more.
The Company’s Premier Mini-Farm Application (UIM 417) must be completed and submitted.
A monitored central station burglar alarm is required for all Coverage C amounts over $75,000 and/or Scheduled Personal Property amounts over $25,000.
All dwellings must have smoke detectors in good working order located close to the kitchen and all sleeping areas.
No three wheel All Terrain Vehicles (ATVs) are allowed.
Four wheel off-road ATVs used only for servicing and maintenance of the residence premises are allowed, for an additional premium, when the Premier Mini-Farm Endorsement is added to the policy. All other usage is prohibited. Up to two four wheel off-road ATVs are allowed per policy.
No dwelling units or tenants allowed in barns or outbuildings not specifically designed as living quarters.
A contract for regularly scheduled garbage pickup must exist – on site trash disposal and trash burning are not allowed.
Adequate clear space for firebreaks must be maintained around the property perimeter.
All fencing must be properly maintained; wire fencing must have a minimum of four strands.
An updated renewal application is required every three years.

830. Golf Cart Liability

910. Emergency Management Preparedness and Assistance Trust Fund Surcharge
A. An Emergency Management Preparedness and Assistance Trust Fund (EMPATF) surcharge of two
dollars ($2) shall be added to every policy on new or renewal business. B. The EMPATF surcharge is a flat charge and is fully earned. C. The EMPATF surcharge is non-commissionable.

920. Florida Insurance Guarantee Association Assessment
A. No Florida Insurance Guarantee Association (FIGA) charges are in effect at this time. The following
paragraphs specify rating and underwriting guidelines to be used in the event a FIGA charge becomes necessary at some future date.
B. Multiply the premium Grand Subtotal by the factor displayed in the Premium Calculation Worksheet and round to the nearest whole dollar.
C. Additional premium endorsements will be subject to the applicable surcharge increase while return premium endorsements will effect a decrease in the applicable surcharge.
D. In the event of policy cancellation, return premium on this assessment shall be prorated. E. The FIGA charge is non-commissionable.

930. Citizens Assessment Recoupment
A. An additional charge may apply on policies before application of the $25 Policy Fee and the $2 EMPATF
Surcharge due to recoupment of an assessment from Citizens Property Insurance Corporation. There is no applicable Citizens recoupment factor in place at this time.
B. Multiply the premium Grand Subtotal by the factor displayed in the Premium Calculation Worksheet and round to the nearest whole dollar.
