How Our Program Works Our therapists work in conjunction with your doctor to develop a treatment program geared toward your individual needs. It’s designed to put you on the path to recovery and will help you to maintain the skills and strength you have gained after you’re finished with the program. Cardiac Care Our program combines skilled nursing care and medical treatments to help prevent complications and improve the rehabilitative outcomes of patients with cardiac conditions. Individuals with heart disease, congestive heart failure and those who have finished inpatient subacute cardiac rehabilitation are perfect candidates for this program. Orthopedic Care Whether you have had a total knee replacement, total hip replacement, fracture or other orthopedic procedure, Brookhaven Health Care Facility can help you through the rehabilitation process.We work in concert with your physician to ensure comprehensive and complete care based upon your personal recovery plan. If you’ve stayed with us for your subacute care, we’ll be able to pick right up where you left off, making your transition seamless. Pulmonary Rehabilitation Our program is tailored to address your pulmonary concerns. Diagnoses treated include: COPD, emphysema and pulmonary fibrosis.We have supplemental oxygen available if needed. Our Pulmonary Rehabilitation program runs 2-3 times per week for a total of 8 weeks depending on your specific needs and goals. Our Services As a patient in our outpatient rehabilitation program, you will be cared for by our skilled physical therapists or occupational therapists, as your diagnosis requires. Our registered nurses are also always available for consultation regarding your medications if needed. Many of our local physicians have their protocols on file for reference. Under their guidance, you will find that the time you invest in rehab after surgery has a direct impact on the quality of your recovery. Our outpatient rehab program will give you a head start on your healing and prepare you to resume your normal daily activities as quickly and safely as possible.
