Sweeny ISD is proud to announce the continuation of our Power On Initiative for grades 6 - 12. The initiative fuels curricular innovation, creates classrooms without walls, and prepares students for the technology-driven careers that await them. To achieve this goal, students will need the appropriate tools. In grades 6-12, each student will receive an iPad on loan for the school year. Prior to check-out, there are two steps to be completed:

1. Sign and return student and parent iPad user contracts2. Insurance waiver payment and acknowledgement (online at www.sweenyisd.org)

The District has opted to require insurance on the iPads. The damage waiver for the insurance is $35.00. Students on the Free and Reduced lunch program may apply for a reduced fee of $10.00. All agreements must be signed and fees paid before an iPad will be issued. The damage waiver and agreement can be found online at www.sweenyisd.org.

Sweeny ISD iPad Loan Agreement and Insurance CoverageAugust 2016 - June 2017

Loan Agreement

Sweeny Independent School District has initiated an iPad loan program for students in grades 6 -12. Using instructional technology, we enhance our mission to prepare and empower students to utilize current and emerging technologies as tools for life-long learning. In an effort to increase access to 21st Century skills and prepare our students to be responsible members of the global community, Sweeny ISD will offer 24/7 accessibility to learning.

Terms of Agreement

Students are expected to have their iPad with them every school day – fully charged.

All students and parent/guardians must adhere to the loan and insurance agreement policies, Student Code of Conduct, Student Handbook, Responsible Use Guidelines, Board policies, and federal/state laws.

Sweeny ISD will monitor use of the iPad through management software. Anyone found to be violating acceptable use standards will be disciplined. An iPad issued to a student as part of this program is owned by the District, and contents of the device can be viewed at any time.

Internet Use

The iPad will have the same filtering and firewall policies as other devices or computers in the District. Users should not attempt to bypass the filtering system, as doing so will cause the user to be in violation of school policy. When iPads are away from the District, and WiFi is accessible, the Sweeny ISD profile will direct Internet traffic through the Sweeny ISD content filter. Parents will also be responsible for monitoring the appropriateness of material viewed in accordance with all District policies.

Device Specifics

The technology device covered by this agreement is an iPad mini, charger, charging cable, and case. iPads may NOT be serviced or replaced at an Apple Store or service shop. Any technical or mechanical issues with the iPad must be reported to the school and repairs will be handled by the Sweeny ISD Technology Department. This equipment is, and at all times remains, the property of Sweeny Independent School District. The iPad is subject to inspection, damage assessment, and can be confiscated at any time. It will need to be returned in an acceptable condition if a student withdraws or leaves school for any reason. iPads will be turned in for inventory and maintenance at the end of each school year, with the plan that students will receive the same iPad in the fall.

Student agreement (Please read before signing):1. I understand the iPad and its accessory equipment are the property of Sweeny ISD.2. I agree to abide by Sweeny ISD’s Responsible Use Technology Agreement and all applicable policies and procedures.3. I understand that the iPad is subject to inspection at any time without notice and that it remains the property of the Sweeny

Independent School District.4. Sweeny ISD has the right to set the iPad back to factory default setting, without notice.5. I will use an unobstructed, clear picture of myself for the lock screen so my iPad can be quickly identified.6. I will never leave the iPad unattended. I will make sure Location Services is turned on and remains on.7. I will never loan my iPad to other individuals.8. I will arrive at school every day with a fully charged battery.9. I will use care when handling the iPad and when plugging and unplugging accessories.10. I will protect the screen from scratches, damage, and any harmful agents.11. I will only use appropriate, pre-approved methods to clean the iPad.12. I will keep food and beverages away from my iPad since they may cause damage to it.13. I will not disassemble or try to repair any part of my iPad or attempt any repairs possibly voiding the warranty.14. I will protect my iPad by storing it in the protective case provided by Sweeny ISD.15. I will care for the equipment on a daily basis and ensure it is kept in a safe environment.16. I will not place decorations (stickers, drawings, writing, etc.) on the iPad.17. I will use the camera and audio recording features of the iPad responsibly and appropriately and will not video or record

anyone without their permission. I will not post recordings without permission.18. I will only install appropriate apps, videos, music and photos on the iPad.19. I will secure my iPad between classes in a backpack or bookbag.20. I will manage my memory resources and keep a sufficient amount of space available for apps, ebooks, and educational

resources provided by Sweeny ISD.21. I will not use the iPad to engage in academic dishonesty.22. I will not use my iPad to access, submit, post, publish, or display any defamatory, inaccurate, abusive, obscene, profane,

sexually oriented, threatening, racially offensive, harassing, or illegal material.23. I will always mute my sound on the iPad unless permission has been granted by the teacher for instructional purposes.24. I will report damage or loss to Campus Administration within one school day.25. I will file a police report in case of theft or vandalism.26. I agree to pay the insurance deductible ($60) should the iPad be damaged, stolen, or lost.27. I agree to pay for the replacement of lost/damaged power cords, power bricks, cases.28. I understand the insurance covers loss, damage, and theft after a $60 deductible. A police report must be filed in case of

theft.29. I will return the iPad and accessories issued to me at the end of each school year or upon request by the District. If I

withdraw, get assigned to DAEP, or terminate enrollment at Sweeny ISD for any reason, I will return the school-issued iPad and accessories on the date of withdrawal.

30. I understand that failure to return the device subjects me to criminal prosecution or civil liability and may result in a theft report being filed with the Police Department.

I have read, understand, and will abide by the terms of the above iPad User Contract. I further understand that the failure to follow these expectations may result in privileges being denied, revoked, or suspended at any time and may also result in disciplinary action.

1. I have read and understand the terms of the student agreement.2. I understand the terms and conditions of the insurance plan.3. I assume financial responsibility for waivers, deductibles, and any other replacement or repair costs not

covered by the plan.4. I understand the iPads are intended for educational use.5. I understand that students must have their iPad at school every day, fully charged.6. I will file a police report immediately in the case of theft or vandalism.7. I understand that the iPad is subject to inspection at any time without notice and that it remains the property of

the Sweeny Independent School District.8. I agree to return the District iPad, Apple power cords, and case in good working condition at the end of the

school year or immediately upon request by the District.9. I understand the insurance covers loss, damage, and theft after a $60 deductible. A police report must be filed

in case of theft.10. I understand I am responsible for the replacement cost of the iPad if it is stolen or lost a second time during the

life of the device (approximately four years.) While there is no limit on claims for repairs, I am limited to ONE lost/stolen claim during the life of the device.

11. I agree to pay for the replacement cost of the District-issued iPad and Apple accessories if I fail to return it at the end of the school year or upon termination of enrollment.

12. I understand that failure to return the device subjects me to criminal prosecution or civil liability and may result in a theft report being filed with the police.
