CareATC Personal Health Assessment (PHA) A Personal Health Assessment consist of a blood draw and biometric measurements. To schedule a PHA at the clinic please call 800.993.8244, or visit https://careatc.com/patients, or log in to the CareATC app.

Patients may choose to receive their PHA results electronically or by U.S. Mail. If you opt for text, within 48 hours you will receive a notification indicating your results are available by logging into the mobile app or careatc.com/patients with the access code provided. If you opt for a paper booklet, your results will arrive at your home via U.S. Mail within 2 weeks. PHAs must be completed no later than June 30, 2020.

If you are unable to participate, CareATC will work with you and your doctor to create a Reasonable Alternative and Waiver form to earn the reward. Please contact the Health Coach for more information and for access to the appropriate form.

Wellness Activities

Lampeter-Strasburg School District will post activities in HealthPassport and communicate those activities to all staff as they become available. Please watch for emails from CATC at [email protected], the Wellness Committee, and postings in your buildings.

The following competitions are available on the HealthPassport homepage. To earn incentive credit, join the competition and start tracking your healthy activities!

Walk America (Nov-Dec 2019)From a lobster dive on the East Coast, to Yosemite National Park, clear to salmon fishing in the Pacific, this virtual journey has it all. Experience some of the splendor the beautiful USA has to offer in this activity-based/steps challenge.

Eat the Rainbow (Jan-Feb 2020)The Eat the Rainbow Challenge is all about getting the right amount of fruits and veggies! Drag your fruits and veggies on to your plate and watch as your rainbow fills up with color!

Crime Mystery Challenge (Mar-Apr 2020)A piece of sporting equipment has been stolen! Can you figure out what was stolen before the end of the challenge? Participants track their daily activity to receive clues in this fun scavenger hunt. Along the way, you’ll learn fun facts about Crime Solver extraordinary, Sherlock Holmes.

No Time like the Pleasant (May-July 2020)Work-life balance and stress have been identified as two of the most predominant concerns in the workplace today. In this challenge, participants earn points for engaging in simple relaxation techniques throughout the day. Taking just a few ‘me’ minutes to stretch, breathe deeply. . . or even laugh has proven to recharge the batteries, and improve overall work performance.
