Health Homes is a care coordination program that is performed by care coordinators in the member community & homes. Six services are offered in monthly visits. We offer help with social determinants of health, provider connections & transitions from inpatient settings. This program offers integrated services & is in populated areas as well as rural settings. Often the care coordinators work together with home services caregivers.
