Biopsy / excision general description diagnostic description (macroscopic & microsc)radical hysterectomyGenaral description Identity condition of cervix type of procedure topography number of specimen

Diagnostic description : Macroscopic Specimen condition (fresh, fixative) Specimen shape Measurements of each piece of specimen Macroscopic appearance (before & after the specimen were cut) : color, consistencyBIOPSYDiagnostic description : MicroscopicType WHO classification :Epithelial TumoursMesenchymalMixed epithelial and mesenchymalOthers2. Grade Degree of differentiation 3 grade : - well differentiated / grade 1 - moderate differentiated/ grade 2 - poorly differentiated / grade 3 not assessable : very early lesions

3. Distribution of invasive component (unifocal/multifocal) Multifocal disease : - Each focus of invasion is measured separately - Staging : dimensions of largest focus

4. Tumour Dimension - two dimensions : depth and width a. Depth of invasion: from the base of the epithelium (surface or glandular) from which the carcinoma arises 3 mm T1a1 >3 - 5 mm T1a2 > 5 mm >T1B

b. Tumour Width (maximum horizontal dimension) Measure from one lateral edge of the tumour to the other 7 mm > 7 mm

5. Excision Status - distance to closest excision margin in mm

6. Presence of intraepithelial neoplasia

CIN 1CIN 2CIN 3-Ca insitu7. Lymphovascular invasion

Tumour : - Present or not - Maximum dimensions of tumour - Position - Macroscopic involvement of vagina - Macroscopic involvement of parametrial/paracervical tissues
