    INTRODUCTION As an industry leader, providing business services for health care providers, Health

    Care Cooperative (HCC) is committed to conducting their business affairs ethically and

    in compliance with all of the laws that govern their operations. Investing in excellent

    annual employee training that is relevant, collaborative and highly engaging is one way

    HCC accomplishes this.

    However, with the increase in costs associated with conducting the annual off-site

    meeting and training events, HCC is looking for alternatives that are more affordable

    and still effective to train employees. The solution(s) must simulate the training events

    at the annual meeting in terms of collaboration and discussion forums. The experience

    must prepare employees so that on average, 99% of the time customer surveys indicate

    that employees deal with every-day situations effectively by doing the right thing.

    The goal of this project is to achieve these standards for HCC with the deliverables outlined

    below and within the framework laid out by this document. High Five Learning plans to

    complete this project with our five-step process.

    PROJECT MANAGEMENT APPROACH The Project Manager, Shana Schwarz, has the overall authority and responsibility for managing

    and executing this project according to this Project Plan. The project team will consist of

    personnel from the HCC IT and Human Resources staff and High Five! Learning instructional

    design team. The project manager will work with all resources to perform project planning. All

    project management plans will be reviewed and approved by the project sponsor. All funding

    decisions will also be made by the project sponsor. Any delegation of approval authority to the

    project manager should be done in writing and be signed by both the project sponsor and project

    manager.

    The project team includes members from the HCC staff and High Five! Learning. The High

    Five! Learning staff assigned to this effort will be 100% dedicated to this project. However, the

    HCC employees assigned to this project will contribute no more than 20% of their work time at

    the peak of this project. The project manager is responsible for communicating with

    organizational managers on the progress and performance of each project resource.

    PROJECT SCOPE Through this Employee Ethics Training project, Health Care Cooperative (HCC) informs and

    defines what is expected from employees during the daily work routine in the following two

    areas:

    1. Ethical and personal conduct with clients and coworkers 2. Appropriate use of company assets

    To accomplish the successful delivery and integration of these two courses, High Five! Learning

    will accomplish the following:

    Deliver two self-paced online training modules with one hour of seat time each, replacing the current on-premise training with a more efficient mode of delivery. These

    two courses will be developed in Articulate Storyline and will include the following:

    New content with scenarios specific to HCC

    Approximately 60 screens of instruction each

    A minimum of 10 quiz questions each

    Audio throughout the entire course

    Interactions at minimum every third screen

    A three minute introduction video each and additional videos throughout the course where appropriate

    HCC will receive the Articulate Storyline source code, media source files, and any additional source files used to create the course.

    Implement Litmos, a hosted web-based learning management system. This solution has an intuitive interface for maintaining course materials and easy reporting and tracking

    capabilities for course completions, assessments and surveys.

    Provide project management throughout the entire effort. The project management function will connect HCC and High Five! with formal communications. This role will

    oversee daily work efforts, mitigate risks, keep the budget in check, and manage

    regularly scheduled meetings. Ultimately, this role will keep the project on track.

    Provide two dedicated instructional design resources to design develop and deliver the courses based on industry best practices. While collaborating with your subject matter

    experts (SMEs) these resources will write new content for both courses.

    Train one of your staff on the Litmos solution. The skills your staff will attain from this training include uploading and assigning new online courses, creating and delivering

    aggregate responses to surveys. Training will be limited to the existing canned reporting

    available through Litmos.

    This project will not include:

    Ongoing support of the Litmos solution.

    Articulate Storyline licenses.

    Updates to the courses once the project has been closed out.

    Audio retakes or updates once the project has been closed out.

    MILESTONE LIST The below chart lists the major milestones for the Employee Ethics Training Project. This chart

    is comprised only of major project milestones such as completion of a project phase or gate

    review. There may be smaller milestones, which are not included on this chart but are included

    in the project schedule and WBS. If there are any scheduling delays which may impact a

    milestone or delivery date, the project manager must be notified immediately so proactive

    measures may be taken to mitigate slips in dates. Any approved changes to these milestones or

    dates will be communicated to the project team by the project manager.

    SCHEDULE BASELINE AND WORK BREAKDOWN STRUCTURE The Work Breakdown Structure (WBS) for the Employee Ethics Training Project is comprised

    of work packages, which do not exceed 40 hours of work but are at least 4 hours of work. Work

    packages were developed through close collaboration among project team members and

    stakeholders with input from functional managers and research from past projects.

    The Employee Ethics Training Project schedule was derived from the WBS and Project Charter

    with input from all project team members. The schedule was completed, reviewed by the Project

    Sponsor, and approved and base-lined. The schedule will be maintained as a MS Project Gantt

    Chart by the Employee Ethics Training Project Manager. Any proposed changes to the schedule

    will follow High Five! Learning change control process. If established boundary controls may

    be exceeded, a change request will be submitted to the Project Manager. The Project Manager

    and team will determine the impact of the change on the schedule, cost, resources, scope, and

    risks. If it is determined that the impacts will exceed the boundary conditions then the change

    will be forwarded to the Project Sponsor for review and approval. The boundary conditions are:

    Cost Performance Index (CPI) less than 0.8 or greater than 1.2

    Schedule Performance Index (SPI) less than 0.8 or greater than 1.2

    Note: Below 0.8 could mean the project is falling behind schedule. Above 1.2 could indicate the project is ahead

    of schedule.

    If the change is approved by the Project Sponsor then it will be implemented by the Project

    Manager who will update the schedule and all documentation and communicate the change to all

    stakeholders in accordance with the Change Control Process.

    The Project Schedule Baseline and Work Breakdown Structure are provided below. Tasks may

    be worked on in sequence or at the same time.

    CHANGE MANAGEMENT PLAN The following steps comprise High Five! Learnings organization change control process for all projects and will be utilized on the Employee Ethics Training project:

    Step #1: Identify the need for a change (Any Stakeholder)

    Step #5: Change Control Board decision (CCB)

    The CCB will discuss the proposed change with HCC key stakeholders and decide

    whether or not it will be approved based on all submitted information

    Step #6: Implement change (Project Manager)

    If a change is approved by the CCB, the project manager will update and re-baseline

    project documentation as necessary as well as ensure any changes are communicated to

    the team and stakeholders

    Any team member or stakeholder may submit a change request for the Employee Ethics Training

    Project. The Employee Ethics Training Project Sponsor will chair the CCB and any changes to

    project scope, cost, or schedule must meet his approval. All change requests will be logged in

    the change control register by the Project Manager and tracked through to completion whether

    approved or not.

    A denial by the project sponsor for the major change request must be provided to the project

    manager in writing, and should include an explanation such as:

    Extends timelines beyond contingency schedule

    Exceeds cost beyond agreed upon threshold

    Request is beyond original scope

    COMMUNICATIONS MANAGEMENT PLAN This Communications Management Plan sets the communications framework for this project. It

    will serve as a guide for communications throughout the life of the project and will be updated as

    communication requirements change. This plan identifies and defines the roles of all project

    team members as they pertain to communications. It also includes a communications matrix,

    which maps the communication requirements of this project, and communication conduct for

    meetings and other forms of communication. A project team directory is also included to

    provide contact information for all stakeholders directly involved in the project.

    The Project Manager will take the lead role in ensuring effective communications on this project.

    The communications requirements are documented in the Communications Matrix below. The

    Communications Matrix will be used as the guide for what information to communicate, who is

    to do the communicating, when to communicate it, and to whom to communicate.

    Meeting Format for Formal Meetings:

    The meetings conducted between High Five! Learning and HCC will be conducted in Adobe

    Connect. Adobe Connect accommodates a larger audience and offers the ability to share files,

    use webcams, take notes, poll attendees, etc. All meetings will include the following topics:

    Progress since previous meeting (or quick introductions in the first meeting)

    Plans for work until the next meeting

    Feedback on work completed

    Change suggestions (see Change Management section)

    Questions/Answers

    The meetings will be moderated by the Project Manager for timeliness (time limit shall be 1

    hour, unless noted), with longer discussions or items not applicable to the group at large being

    tabled or diverted into smaller session meetings so that these meetings can be kept to an hour.

    The Project Manager will record all meetings, archive them in Adobe Connect, take notes during

    the meeting (or delegate them as necessary), and deliver a summary of the minutes to all

    stakeholders for review.

    Meeting Format for Informal Meetings:

    For quick questions and answers between High Five! Learning and HCC, phone calls are the

    preferred method of communication, as long as the topics do not involve changes or major issues

    for the project/product. If something of a sensitive or urgent nature does arise between meetings,

    a formal meeting request and can should be made to the Project Manager who can call a meeting

    to address the topic at hand.

    Project Meeting Schedule for Major Milestones:

    a change agreement for minor and/or mutually agreed upon changes within project scope.

    Email:

    All email pertaining to the Employee Ethics Training Project should be professional, free of

    errors, and provide brief communication. Email should be distributed to the correct project

    participants in accordance with the communication matrix above based on its content. All

    attachments should be in one of the organizations standard software suite programs and adhere to established company formats. If the email is to bring an issue forward then it should discuss

    what the issue is, provide a brief background on the issue, and provide a recommendation to

    correct the issue. The Project Manager should be included on any email pertaining to the

    Employee Ethics Training Project.

    COST MANAGEMENT PLAN The Project Manager will be responsible for managing and reporting on the projects cost throughout the duration of the project. The Project Manager will present and review the projects cost performance during the monthly project status meeting. Using earned value calculations, the

    Project Manager is responsible for accounting for cost deviations and presenting the Project

    Sponsor with options for getting the project back on budget. All budget authority and decisions,

    to include budget changes, reside with the Employee Ethics Training Project Sponsor.

    For the Employee Ethics Training Project, control accounts will be created at the fourth level of

    the WBS, which is where all costs and performance will be managed and tracked. Financial

    performance of the Employee Ethics Training Project will be measured through earned value

    calculations pertaining to the projects cost accounts. Work started on work packages will grant that work package with 50% credit; whereas, the remaining 50% is credited upon completion of

    all work defined in that work package. Costs may be rounded to the nearest dollar and work

    hours rounded to the nearest whole hour.

    Cost and Schedule Performance Index (CPI and SPI respectively) will be reported on a monthly

    basis by the Project Manager to the Project Sponsor. Variances of 10% or +/- 0.1 in the cost and

    schedule performance indexes will change the status of the cost to yellow or cautionary. These

    will be reported and if its determined that there is no or minimal impact on the projects cost or schedule baseline then there may be no action required. Cost variances of 20%, or +/- 0.2 in the

    cost and schedule performance indexes will change the status of the cost to red or critical. These

    will be reported and require corrective action from the Project Manager in order to bring the cost

    and/or schedule performance indexes back in line with the allowable variance. Any corrective

    actions will require a project change request and be must approved by the CCB before it can be

    implemented.

    Earned value calculations will be compiled by the Project Manager and reported at the monthly

    project status meeting. If there are indications that these values will approach or reach the

    critical stage before a subsequent meeting, the Project Manager will communicate this to the

    Project Sponsor immediately.

    PROJECT APPROVAL AND AUTHORIZATION TO PROCEED Project approval and Authorization to Proceed (ATP) is requested at the end of the first formal

    meeting, with a document signed by both parties being received to both companies by courier.

    All changes to the original agreement must be submitted in the form of a Change Request in a

    Formal Meeting (see next page).

    PROCUREMENT MANAGEMENT PLAN The Project Manager will provide oversight and management for all procurement activities under

    this project. The Project Manager is authorized to approve all procurement actions up to

    $10,000. Any procurement actions exceeding this amount must be approved by the Project

    Sponsor.

    While this project requires minimal or no procurement, in the event procurement is required, the

    Project Manager will work with the project team to identify all items or services to be procured

    for the successful completion of the project. The Project Manager will then ensure these

    procurements are reviewed by the Program Management Office (PMO) and presented to the

    contracts and purchasing groups. The contracts and purchasing groups will review the

    procurement actions, determine whether it is advantageous to make or buy the items or resource

    required services internally, and begin the vendor selection, purchasing and the contracting

    process.

    In the event a procurement becomes necessary, the Project Manager will be responsible for

    management any selected vendor or external resource. The Project Manager will also measure

    performance as it relates to the vendor providing necessary goods and/or services and

    communicate this to the purchasing and contracts groups.

    PROJECT SCOPE MANAGEMENT PLAN Scope management for the Employee Ethics Training Project will be the sole responsibility of

    the Project Manager. The scope for this project is defined by the Scope Statement, and Work

    Breakdown Structure (WBS). The Project Manager, Sponsor, and Stakeholders will establish and

    approve documentation for measuring project scope, which includes deliverable quality

    checklists, and work performance measurements.

    Proposed scope changes may be initiated by the Project Manager, Stakeholders or any member

    of the project team. All change requests will be submitted to the Project Manager who will then

    evaluate the requested scope change. Upon acceptance of the scope change request the Project

    Manager will submit the scope change request to the Change Control Board and Project Sponsor

    for acceptance. Upon approval of scope changes by the Change Control Board and Project

    Sponsor the Project Manager will update all project documents and communicate the scope

    change to all stakeholders. Based on feedback and input from the Project Manager and

    Stakeholders, the Project Sponsor is responsible for the acceptance of the final project

    deliverables and project scope.

    The Project Sponsor is responsible for formally accepting the projects final deliverable. This acceptance will be based on a review of all project documentation, testing results, beta trial

    results, and completion of all tasks/work packages and product functionality.

    SCHEDULE MANAGEMENT PLAN Project schedules for the Employee Ethics Training Project will be created using MS Project

    starting with the deliverables identified in the projects Work Breakdown Structure (WBS). Activity definition will identify the specific work packages, which must be performed to

    complete each deliverable. Activity sequencing will be used to determine the order of work

    packages and assign relationships between project activities. Activity duration estimating will be

    used to calculate the number of work periods required to complete work packages. Resource

    estimating will be used to assign resources to work packages in order to complete schedule

    development.

    Once a preliminary schedule has been developed, it will be reviewed by the project team and any

    resources tentatively assigned to project tasks. The project team and resources must agree to the

    proposed work package assignments, durations, and schedule. Once this is achieved the project

    sponsor will review and approve the schedule and it will then be base lined.

    In accordance with High Five! Learning organizational standard, the following will be

    designated as milestones for all project schedules:

    Completion of Initiation

    Project implementation Roles and responsibilities for schedule development are as follows:

    The project manager will be responsible for facilitating work package definition, sequencing, and

    estimating duration and resources with the project team. The project manager will also create the

    project schedule using MS Project and validate the schedule with the project team, stakeholders,

    and the project sponsor. The project manager will obtain schedule approval from the project

    sponsor and baseline the schedule.

    The project team is responsible for participating in work package definition, sequencing,

    duration, and resource estimating. The project team will also review and validate the proposed

    schedule and perform assigned activities once the schedule is approved.

    The project sponsor will participate in reviews of the proposed schedule and approve the final

    schedule before it is base lined.

    The project stakeholders will participate in reviews of the proposed schedule and assist in its

    validation.

    QUALITY MANAGEMENT PLAN/EVALUATION The quality evaluation of the project and products is based on a Standard Survey and Cost

    Effectiveness Analysis.

    To ensure that stakeholders are satisfied with project progress, a survey using likert style test

    questions will be sent out via email on a bi-weekly basis, measuring satisfaction on a scale from

    1-10 (with 10 meaning complete satisfaction) in the following categories:

    Timeliness of project deliverables

    Communication of progress

    Ease of communication

    Results so far

    Overall project satisfaction

    Once the product has been delivered, High Five! Learning will complete and deliver a Cost

    Effectiveness Analysis to HCC based on the following possibilities:

    Decreased Cost, Increased Effectiveness

    The overall goal of the project is to deliver a product that rests squarely in the Decreased Cost,

    Increased Effectiveness quadrant, and the product will not be deemed successful without meeting

    that objective. By ensuring that staff no longer has to travel for the training in question and by

    teaching the material in a way that is both easy to understand and recyclable,

    High Five! Learning will demonstrate just how this product can achieve that result for HCC.

    As with the quality management section, both the project and product will undergo quality

    assurance checks throughout the process and after the product is delivered. These quality

    checks will be performed by each member of the project team, HCC staff involved with the

    project, and ultimately, the learners who will be interacting with the product.

    Here are the categories for QA, along with their assigned reviewers:

    Content: Will be reviewed by HCC staff that have been through training or are knowledgeable in the subject matter (SME) upon creation and after each revision.

    Design: Designers and the Project Manager will review the course design to check for consistency and effectiveness. Members of HCC staff will give approval of design to

    ensure branding is handled correctly.

    Usability: All stakeholders and members of project design team will be given the chance to review the course to check for seamlessness of usability and overall course flow.

    Content/Design/Usability (Full Package): Will be reviewed by all stakeholders and members of the project team upon completion and after each review. Will also be reviewed

    by a small sample of test users.

    In addition, High Five! Learning will also conduct a post-delivery program evaluation plan and

    project review. While the project review results will be used internally for High Five! Learning,

    results will be shared upon request for future work or referrals. To evaluate the program

    efficiency, we will compare test results from before the project and after to gauge how well the

    deliverables have helped the staff retain the curriculum. The evaluation will be a straight

    percentage comparison (what percentage of learners did better on the tests after going through

    our training than before), while the project review will be based on the milestones and

    deliverables outlined in this document. With a reasonable window around deadlines, we will

    check to see how well we stayed on pace and how well the client responded based on survey

    responses and overall happiness with the project at each stage and upon completion.

    RISK MANAGEMENT PLAN The approach for managing risks for the Employee Ethics Training Project includes a methodical

    process by which the project team identifies, scores, and ranks the various risks. Every effort

    will be made to identify risks proactively, ahead of time in order to implement a mitigation

    strategy from the projects onset. The most likely and highest impact risks were added to the project schedule to ensure that the assigned risk managers take the necessary steps to implement

    the mitigation response at the appropriate time during the schedule. Risk managers will provide

    status updates on their assigned risks in the bi-weekly project team meetings, but only when the

    meetings include their risks planned timeframe.

    Upon the completion of the project, during the closing process, the project manager will analyze

    each risk as well as the risk management process. Based on this analysis, the project manager

    will identify any improvements that can be made to the risk management process for future

    projects. These improvements will be captured as part of the lessons learned knowledge base.

    STAFFING MANAGEMENT PLAN This project will be managed in a matrix style management structure. The project will be staffed

    by four full-time personnel employed by HCC: A project manager, two instructional designers,

    and an instructional technologist. These staff will be dedicated to this project, full time.

    Additionally, the project matrix staff will include at least two HCC staff to include: 1. An IT

    network security person, and 2. subject mater expert on HCCs ethics policies and procedures.

    The Employee Ethics Training Project will consist of a matrix structure with support from two

    HCC staff members. All work will be performed internally. Staffing requirements for the

    Employee Ethics Training Project include the following:

    Project Manager (1 position) responsible for all management for the Employee Ethics Training Project. The Project Manager is responsible for planning, creating, and/or managing all work

    activities, variances, tracking, reporting, communication, performance evaluations, staffing, and

    internal coordination with functional managers.

    Senior Instructional Designer (1 position) responsible for oversight of all instructional design and development tasks for the Employee Ethics Training Project as well as ensuring online

    course and learning management system (LMS) functionality is compliant with quality

    standards. Responsible for working with the Project Manager to create work packages, manage

    risk, manage schedule, identify requirements, and create reports. The Senior Instructional

    Designer will be managed by the Project Manager who will provide performance feedback to the

    functional manager at High Five! Learning.

    Instructional Designer (1 position) responsible for instructional design and development the Employee Ethics Training Project. All instructional design tasks will be reviewed by the Senior

    Instructional Designer prior to implementation. Responsibilities also include assisting with risk

    identification, determining impacts of change requests, and status reporting. The Instructional

    Designer will be managed by the Project Manager and feedback will be provided to the

    functional manager for performance evaluations by the Project Manager and Senior Instructional

    Designer.

    Instructional Technologist (1 position) responsible for assisting the Project Manager in creating quality control and assurance standards. The Senior Instructional Designer is also responsible

    for maintaining quality control and assurance logs throughout the project. The Instructional

    Technologist will be managed by the Project Manager who will also provide feedback to the

    functional manager for performance evaluations.

    The Project Manager will negotiate with all necessary High Five! Learning functional managers

    in order to identify and assign resources for the Employee Ethics Training Project. All resources

    must be approved by the appropriate functional manager before the resource may begin any

    project work. The project team will not be co-located for this project and all resources will

    remain in their current workspace.

    RESOURCE CALENDAR The Employee Ethics Training Project will require all High Five! Learning project team

    members for the entire duration of the project although levels of effort will vary as the project

    progresses. The Project is scheduled to last approximately three months with standard 40-hour

    workweeks. If a project team member is not required for a full 40-hour workweek at any point

    during the project, their efforts outside of the Employee Ethics Training Project will be at the

    discretion of their Functional Manager.
