We are a faith-based outreach ministry serving all at risk
women of childbearing age, their children and families to achieve improved health, well-being and
self –sufficiency.

Healthy Moms & Babes founded in August 1986, celebrates its 25 anniversary of
‘Investing in the Future’ this year.
All services are provided at no cost to the client and are not income eligible status

In 2010 374 pregnancy tests 131 pregnancies identified 29 STD tests performed 1776 educational services offered annually 1942 clients served
Provide services in 12 neighborhoods in Greater Cincinnati & 2 Northern Kentucky
2 vans service the neighborhoods M-Th, year round, 216 days last year
MOBILE VANS How we do it…
♥ Proactive education, outreach and intervention for the neediest moms in their own neighborhoods and homes, using Healthy Moms and Babes mobile units
♥ Prenatal education and pregnancy testing in a supportive, safe, non-judgmental environment
♥ Advocacy for clients to help them overcome obstacles to healthy pregnancy, such as the need for food, shelter, safety, and education
VAN SERVICES Why we do it . . .
Shantell is six months pregnant when she comes to Healthy Moms and Babes.
She’s had no prenatal care and has stopped taking some vital blood pressure medications.
A Healthy Moms and Babes nurse sends her to the hospital immediately.
Without this intervention, she might have lost her baby.
She and her healthy baby continue to stop by the van for parenting tips.
HOME VISITING SERVICES
Home Visiting Services are available to:
All pregnant women
first time parent with
a child less than 6 months of age, or,
a child under age 3 with one parent in active military duty
HOME VISITING SCREENING Single parent Income less than 185% of federal poverty guidelines Prenatal complications/preterm labor Limited or no prenatal care No health insurance Family crisis Lack of stable residence History of child abuse or neglect Mental health issues Substance/alcohol abuse Lack of adequate social support
HOME VISITING How we do it . . . Minimum of Monthly Home Visits
Assistance in obtaining medical care and medical insurance
Assistance in obtaining social services, support and education for needs during pregnancy and through the child’s first 2-3 years of life
Developmental screenings and fun learning activities Parenting education
Support in achieving goals toward self sufficiency, such as finishing school and employment

What is a Pathway
• Systematic and Logical progression of care that recognizes and reinforces benefits of each milestone in pregnancy, birth and follow-up

• Incentives are provided to maintain motivation for Pregnancy Confirmation from a health care professional

Healthy Moms & Babes Clientele rate 2009 = 6 before age 1 (.3% of all client births)
• Amber is young, pregnant, and scared. A Healthy Moms and Babes Outreach specialist accompanies her to her prenatal appointment and ensures that she follows through on prenatal care.
HELP ME GROW How we do it . . .
Provide home based services recognizing the parent / primary caregiver role as the child’s first teacher
Provide in-home evaluations and developmental / social screenings, ie: Take a Look (vision) ASQ & ASQ-SE (Ages & Stages Questionnaire, Social Emotional), if indicated Battelle, Edinburgh depression screening
Teach and model effective parenting techniques Provide linkage to additional community services Provide accurate information on child developmental

HELP ME GROW Why we do it . . .
A new mom, new to our country, delivered premature twins with a diagnosis of extremely low birth weight! The mom’s barriers at referral included language, lack of knowledge to available resources, and inability to function productively within her community. All that on top of the fact that she was a new mom caring for her special needs children!
Our role with this family was extremely important for her obtaining the knowledge, courage, and dedication to withstand this new stage of life for her family….
Today the family is doing spectacular! Mom has learned to drive; She is receiving her viable resources such as medical, WIC, and of course Developmental Disability Services for her children!

…and the twins, they are now sitting up alone, observant and aware of sounds & their surroundings, even starting to walk!
This mom has been so inspired and humbly grateful to her experience that she has been a leader for her own country. Sharing and modeling via webcam all of her new knowledge, self-sufficiency, empowerment, parental skills, and development awareness
HOME VISITING - CAPTA
Client referral received from DJFS Must meet criteria of

Child is assumed Part C eligible - qualifies for home based services through Department of Disability Services– until screenings identify otherwise
CAPTA how we do it . . .
Child is in home visiting program with services provided to child, primary care giver and /or parent (if plan of JFS is to reunify the family & parent maintains custody)
When child is initially referred the Home visitor obtains Release of Information to communicate with JFS- JFS should already have the Release of Information to share information with a HMG staff provider
Continue CAPTAHow we do it . . .
HV places call to JFS worker to confirm their assignment to referral and contact information AND invitation to next planned visit
A minimum of quarterly progress reports are written by HV and e-mailed or faxed to JFS worker on each family in the HMG program. (A copy should become part of the client HMG folder.) The Individualized Family Plan is available IF a release is signed and JFS requests a copy
