Credentials website. Be prepared to pay a one time access fee! (Credit card
or PayPal) Health requirements are determined by clinical
agencies which require compliance with their employee health standards and are specific to each program.
See your program requirements for specifics concerning required immunizations or titers, physical exam, drug screen, and CPR.

Preparing Documents Your Full Name is on all documents. CPR Card & required forms are signed. Scan your acceptable documentation to a pdf or jpg
file. Save the documents on a flash drive or in your
Moraine Park Student I drive. Upload required documents in a pdf or jpg format
onto your Verified Credentials account. Remember to include your Hepatitis B Vaccine
Documentation Form. Do not upload worksheet.

Remember to use your MPTC Student E-Mail address. Check your email on a regular basis and be alert for messages from Verified Credentials related to your health requirements. You will receive an alert if you need to correct a document or obtain additional documentation. When you enter your documents initially enter them under the My Profile tab and when you are updating your records you will be entering under the Home Tab.

Tuberculosis
A positive result for either a skin test or titer requires a chest x-ray. A completed Tuberculosis Symptom Questionnaire is also required annually.
Miscellaneous Information Student Physical Examination– use the program’s
form. A complete exam is required initially at the program’s start,
unless there’s a significant change in your health. The physical can be completed at one of the Occupational
Health Clinics sites or by your Primary Care Provider. Call and schedule an appointment. Record date physical exam was completed

Medical History and Physical must be documented on MPTC’s Form,
Miscellaneous Information continued Drug Test Results – 10 panel or greater. Must be done at one
of the designated Occupational Health Clinics. Final Report is sent to designated staff at Moraine Park. The results will be e-mailed to you via your Moraine Park student
e-mail. Upload the test result, enter the date it was done & submit
documentation.
Drug screen must be completed during your programs identified completion dates!

Professional Certifications CPR – Must Be AHA BLS Healthcare Provider

CPR is valid for 2 years. You need to remain certified until graduation. Take refresher course if taking before old CPR expires. If CPR expires must take full course.
