WHY TECHNOLOGY MAY NOT WORK

The two most relevant technical innovations of our lifetimes that can help us improve

clinical care for patients in Africa are: 1) the advent of the internet and 2) the

growth of the mobile telephone industry and its growing networks.

These two innovations can allow every sector of society to benefit through their use—

including health care.

Unfortunately, two factors exclude many using these innovations in places like Kenya

and across Africa: 1) cost and 2) technical complexity.

In 2010, some colleagues and I decided to tackle these two issues in hopes of

helping to improve health outcomes in Africa, Asia, Latin America and the

Caribbean. We created an international, social good, medical software company

called E-Health Records International.

We studied systems in the U.S. and in Europe.

They were perfect examples of what not to do: Too costly and too complicated.

CAN TECHNOLOGY BE AFFORDABLE?

Several large companies like Epic and Cerner regularly sell billion dollar systems to hospitals. Often takes 3 years to implement and train staff. When that is finished it is time for a $5 million upgrade and cycle starts all over again

They were doing it all wrong

And were using outdated technology. 35 year old software platforms that use in-house server based systems , which means higher costs for the hospitals—equipment and training. And means more complicated training, often for people who have never used a computer. Sound familiar?

There was a study done in 2011 on the average cost of EMR systems in the U.S. by the CIO consortium. They discovered that the average costs for server based systems, even in very small hospitals, was$684,000 per year for the hardware and $1,100,000 per year for the software. And that cost goes on forever, year after year!!. Can you afford$1,800,000 per year for an EMR system? Probably not.

KHAMA ROGO) We decided to go a different way—using cloud based solutions that we could fix remotely(no

$25,000 trips halfway around the world to fix a system) and hospitals could operate on very low cost mobile devices like $100 Android tablets, using low bandwidth WiFi or a mobile phone chip with a data plan.

Essentially, any place you can operate a mobile phone you can use our system. That is technical innovation you can afford and your people can actually use. Your cost goes from $1.8 million per year to no more than the cost of paper and staff costs you are now using—and you know everything about your patients, never lose records…and you have their bill in real time, all the time….you have a transparent financial system at your facility all the time. It is your right to have this and at a cost you can afford. Insist on it like the Prophets of Old!!

Why do your hospitals need to acquire an EMR and cloud based billing systems: Two examples:

1) Drug Interactions or drug allergies either killed or caused serious long-term health consequences for 14% of patients, according to a recent study done in South Africa, and published by the British Journal of Clinical Pharmacology.

The likelihood that the percentage is higher where your hospital is located is almost certain. Is it 20% or 30% of your patients that are adversely affected because your clinicians do not know what your patients are allergic to or what other drugs they may be taking? Would you like to have a tool for your hospital that prevents thousands of patients from dying unnecessarily?

EFFICIENCY IS NOT ANTI-CHRIST

In countries where there is a high incidence of HIV and Tuberculosis, this risk is even greater because of the medications they are currently taking for these ailments.

Cloud based systems like ours have a drug and allergy data base included to warn the clinicians that certain drugs may not be right for this patient—and lives are saved.

2) Another recent study showed that worldwide, 33% of hospital employees steal from their employers. Probably even in church related hospitals

These thefts result in literally billions of dollars being removed from the health care infrastructure and from the hospitals themselves.

People take what they can resell. Pharmaceuticals are high on the list. And when the hospital runs out of a needed drug and the funds to restock, innocent lives are put in jeopardy.

Again, what is new is that cloud based systems like ours create transparency within hospitals that assure that most items can never be stolen for authentic patient use and lives are saved. This is true for inventory management as well as financial management.

HEALTH CARE THAT DOES NOT IMPOVERISH

Technology can help FOBs to lead the way to create transparent systems for managing our health care, especially for needy patients

And it should be all of our job to assure that when tools like this are created, we make it possible for use everywhere; they should be made affordable and accessible for every hospital and every clinic everywhere in the world. That is our biggest innovation. We make it our job to challenge the greed and avarice that says you only get this tool if you can afford $1.8 million per year. Justice for healthcare
