What is the purpose of this workbook? The information that you record in this book will allow you to demonstrate your knowledge and understanding of health and safety within your workplace. It is a record of your learning and can be used as evidence of your professional development and may contribute towards your SVQ in Health and Social Care This completed workbook should be kept in your Green CPD Portfolio. Please remember to return the front page to the Training and Learning Centre.

How do I use this workbook? This book is designed to be able to be used regardless of your place of work. If there is information that your supervisor thinks that you need to know but there is not a place in the book to record then just add it in. Try to begin to complete each section of this workbook during your first day in your workplace. You may also want to add to it as you begin to find out how this information specifically relates to you, your role and your workplace. Always make sure that you relate the notes you record to your own place of work and the person or people you support or are working with. Remember this is your own workbook and can be used to evidence your knowledge for SVQ level 2 DK8W 04 (HSC22) Support the Health and Safety of yourself and individuals. Please remember not to mention people by name (initials only) in this book. The columns on the right hand side of the workbook provide space to claim evidence knowledge points for SVQ level 2. Your supervisor will arrange for you to attend further health and safety training as appropriate. Please bring this workbook along when you attend your health and safety training. Please take responsibility to ask your supervisor if you are unclear about health and safety in your workplace. When you have completed the book and can demonstrate your practical competence to your supervisor, you should arrange to meet with a health and safety facilitator or health and safety rep and discuss your progress. Following a successful demonstration of your skills, knowledge and understanding of health and safety you should all complete the statement of competence at the end of this book and transfer this statement to your CPD portfolio. If you are working towards your SVQ level 2 Health and Social then the completed workbook along with the signed Competency Statement should be presented to your SVQ Assessor to validate and authenticate your knowledge and claim the evidence within the knowledge part of your SVQ Unit. Once your Assessor has validated your knowledge they will provide a statement which will acknowledge your evidence and complete a report stating this within the last page of your workbook. Your completed workbook should then be presented along your SVQ Portfolio when you have completed your SVQ.
